Objectives: To assess the efficacy of mineral trioxide aggregate (MTA)pulpotomy in mature permanent teeth with irreversible pulpitis due to carious exposure. Placeand Duration: The study was conducted in the Department of Operative Dentistry, AltamashInstitute of Dental Medicine from 05-01-2016 to 05-07-2016. Methodology: A total of 70 teethof 70 patients with irreversible pulpitis were treated with mineral trioxide aggregate (MTA)pulpotomy. The patients were re-evaluated after 2 months. All patients were asked whether theyhad experienced spontaneous pain, pain on chewing or pain stimulated by hot/cold and thetreatment was counted as successful in case of no post-operative symptoms. Results: Efficacyof pulpotomy treatment with mineral trioxide aggregate in symptomatic mature permanentteeth with irreversible pulpitis and normal periapical periodontium was 88.57%. Conclusion:MTA pulputomy is a good alternative treatment for cases of irreversible pulpitis in permanentteeth with mature roots and normal apical periodontium. However, long term clinical trials andhistological assessment is desirable.

One-year outcomes of MTA and modified Portland cement pulpotomy in primary teeth: a randomized clinical trial

Purpose: To compare the efficacy of Mineral Trioxide Aggregate (MTA) and Modified Portland Cement (MPC) as pulpotomy medicaments in primary molars. Methods: A sample of 54 children 4 to 6 years old of age, who had at least one primary mandibular second molar that needed pulpotomy were randomly placed in MTA (n = 28) or MPC (n = 26) groups. After completing the pulpotomy procedures, the teeth received a stainless-steel crown. Clinical and radiographic successes/failures were blindly evaluated at 6 and 12 months, and Fisher's exact test was used to analyze the differences. Results: At 6- and 12-month follow-ups, MTA and MPC had 100% clinical success rate. Radiographic success rates of MTA were 92.9% at 6 months and 89.3% at 12 months. While the rate for MPC group was 88.5% at both intervals. There was no statistically significant difference between the two groups. Conclusion: The results of this investigation showed that treatment success rate with MPC was comparable to MTA pulpotomy. However, additional clinical research that considers long-term follow-ups is required to test the usefulness of MPC in the pulpotomy treatment of primary teeth.

Evaluation and comparison of mineral trioxide aggregate and biodentine in primary tooth pulpotomy: Clinical and radiographic study.

Pulpotomy is the common therapy for cariously exposed pulps in symptom-free primary molar teeth. For many years, researchers have searched for an ideal material that allows regeneration of the residual pulp. The purpose of this study was to evaluate the efficacy of mineral trioxide aggregate (MTA), Biodentine as a pulpotomy medicament in primary teeth, both clinically and radiographically. A total of 25 children (50 human primary molar teeth) aged between 5 and 9 years were selected in this randomized clinical study. The patients were randomly assigned to receive the pulpotomy medicaments in either [Group 1]: MTA or [Group 2]: Biodentine. All pulpotomized teeth were restored with stainless steel crowns and evaluated clinically and radiologically at 1, 3, 6 and 12 months. Statistical analysis using Mann-Whitney U test and Fisher's exact test, and chi-square test was performed to determine the significant differences between the groups. Clinical and radiological success rates were 96 and 80% in Group 1 and 96 and 60% in Group 2, respectively. There were no significant differences between the groups (P > 0.05). The radiographic success rates decreased in the controls, but there were no significant differences. Biodentine showed similar clinical and radiographic results as MTA in the 12-month evaluation and can be safely used as a pulpotomy medicament.

Antimicrobial Efficacy of Mineral Trioxide Aggregate against Therapy Resistant Endodontic Microorganisms

Microorganisms, usually from the dental caries, are the main sources of diseases in dental pulp (root canals) and periapical region. Facultative bacteria and fungi have been identified in therapy resistant persistent endodontic infection. The objectives of this study was to evaluate the antimicrobial efficacy of Mineral Tri Oxide Aggregate (MTA) against therapy resistant endodontic microorganisms. The efficacy of MTA was also compared with that of calcium hydroxide. Six standard bacterial stains were used: Staphylococcus aureus, Pseudomonas aeruginosa, Escherichia coli, bacillus subtilis, Candida albicans and Enterococcus faecalis. The agar diffusion method on Muller- Hilton media was employed. The plates containing media were inoculated with the specified bacterial suspensions. Two standard holes were prepared on each microorganism inoculated plate with a copper puncher and one hole was completely filled with MTA & the other with Ca (OH)2 . The plates were then kept at environmental temperature for one hour to ensure prediffusion and then incubated at 370C for 24 hours. After 24 hours, the diameters of inhibition zones were measured. Tests were replicated for thirty times for each sample and mean values were taken. Zone of inhibition as measured for MTA and Ca (OH)2 were statistically analyzed with Student’s t-Test and Post Hoc Games Howell Test and were presented as mean ± SD to compare of efficacy of MTA and calcium hydroxide on different microorganisms. Both MTA and Ca(OH)2 were found to produce zone of inhibition against Staphylococcus aureus (ATCC 25923), Pseudomonas aeruginosa (ATCC 27853), , Bacillus subtilis (BTCC 17 ), and Candida albicans (BTCC 493). MTA showed highest activity against S. aureus and lowest activity against P. aeruginosa which was similar to the activity range of Ca (OH)2 against the mentioned organisms. But both of them failed to produce any activity against E. coli and. E. faecalis. MTA was found to produce a lower efficacy than Ca (OH)2 while comparing the zone of inhibition between them and statistically it was significant. Mineral Tri Oxide Aggregate (MTA) showed antimicrobial efficacy against some therapy resistant microorganisms but it did not show antimicrobial efficacy against Escherichia coli and Enterococcus faecalis. Sealing efficacy of mineral trioxide aggregate with and without nanosilver for root end filling: An in-vitro bacterial leakage study

BackgroundVarious materials have been added to mineral trioxide aggregate to enhance its properties. This study was aimed to compare the sealing efficacy of MTA with and without nanosilver using bacterial leakage approach.Material and MethodsSeventy canine teeth were prepared and obturated. Then, after apical resection, the root-end cavities were prepared by ultrasonic retrotips. Teeth were randomly divided into 4 groups containing two experimental groups (n=30) and two negative and positive controls (n=5). In group 1 and 2, root-end cavities were respectively filled with MTA and MTA with nanosilver (by 1% weight). Leakage assessment was carried out by bacterial leakage apparatus with Enterococcus faecalis species. Leakage comparison between experimental groups was done using Mann-Whitney test by Spss 16 software at significancy level of 0.05.ResultsThe median bacterial leakages for MTA and MTA with nanosilver were 19 and 2, respectively. The mean bacterial leakages for MTA and MTA with nanosilver were 30.06±28.67 and 9.66±14.25, respectively. Mann-Whitney test indicated that there was a significant difference in bacterial leakage day between two experimental groups (P=0.002).ConclusionsBased on the findings of this in-vitro bacterial leakage study, adding nanosilver to MTA decreased its sealing ability. The Effect of Mineral Trioxide Aggregate Mixed with Chlorhexidine as Direct Pulp Capping Agent in Dogs Teeth: A Histologic Study.

Introduction:The aim of the present investigation was to compare the efficacy of mineral trioxide aggregate (MTA) and 0.2% chlorhexidine (CHX) mixture to pure MTA, as a pulp capping material.Methods and Materials:The pulp of 24 lateral incisors and canines from four dogs were exposed and capped either with MTA or MTA+0.2% CHX. After 2 months the animals were sacrificed and the teeth were prepared for histological evaluation in terms of calcified bridge formation, the degree of inflammation and presence of necrosis. The Fisher’s exact test was used for data analysis. Results: The results showed that formation of complete calcified bridge in MTA specimens was significantly more than MTA+CHX (P<0.05). No significant difference was found in the degree of inflammation and necrosis between MTA and MTA+CHX groups (P>0.05). Conclusion:Mixing MTA with CHX as pulp capping agent had a significant negative impact on formation of calcified bridge on directly capped dog’s teeth.

MTA apical barrier: In vitro study of the use of ultrasonic vibration

BackgroundThe apexification is the first alternative treatment on a permanent tooth when, after a tooth trauma and in the presence of immature apex trauma, pulp necrosis occurs. Many studies have demonstrated the efficacy of mineral trioxide aggregate (MTA) as apical sealing material of choice in these cases, but has a degree of filtration as all other materials. The objective of this study was to analyze the seal ability of MTA on the duct walls in immature teeth unirradicular apexes, using indirect vibration.Material and MethodsThe study was conducted on 45 teeth divided into 3 groups: Group A or control group in which no vibration for placing the MTA was used, Group B and C or groups where indirect vibration analysis was used. All samples were immersed in methylene blue to assess filtration. After performing longitudinal cuts millimetric measuring were made of the degree of filtration, divided into 3 degrees (2mm each grade filtration).ResultsResults obtained confirm our hypothesis, obtaining lesser degree of filtration those groups in which indirect vibration (Groups B and C) was performed. It was shown that the degree of filtration is closely linked to the degree of adaptation.ConclusionsMTA vibration offers better results in its adaptation to the canal walls, significantly reducing the degree of filtration. Clinical and radiographic assessment of mineral trioxide aggregate and calcium hydroxide as apexification agents in traumatized young permanent anterior teeth: A comparative study

To evaluate and compare the efficacy of mineral trioxide aggregate (MTA) and traditionally used calcium hydroxide (Ca(OH)2) in inducing root end formation of immature roots of traumatized young permanent anterior teeth. The study was carried out on 22 nonvital, immature permanent maxillary incisors. Samples were allotted into two groups - Group I MTA and Group II Ca(OH)2 Success rate was determined based upon the time duration required for apical barrier formation. The canals were obturated using gutta-percha points in MTA group, after 24 h, whereas in Ca(OH)2 group, obturation was carried out after radiographic confirmation of the apical barrier. The clinical and radiographic evaluation was carried out at a follow-up periods of 3, 6, and 9 months and statistical analysis was carried out by SPSS version 15.0 statistical analysis software (Chi-square test and fisher exact test). In MTA Group, barrier formation was observed in 90.90% of the patients after 9 months whereas in Ca(OH)2 Group, the same was observed in 81.81%. The mean time required for barrier formation in MTA group was 4.90 months and 5.33 months in Ca(OH)2 group. MTA and Ca(OH)2, as medicaments for apexification, were comparable in terms of the evaluation parameters. However, MTA was beneficial in terms of immediate obturation of immature roots with wide open apices.

Evaluation of Antibacterial Efficacy of MTA with and without Additives Like Silver Zeolite and Chlorhexidine.

Microorganisms, predominantly Enterococcus faecalis are found responsible in the progression of pulpal, periradicular diseases and in endodontic failures. Unsuccessful conventional treatment might necessitate the need for a surgical approach; where in retrograde restorative materials are used to seal the apex. Among the root end filling materials, Mineral Trioxide Aggregate (MTA) is considered biocompatible and is most commonly used in clinical applications but it has limited antibacterial activity. Metallic silver and chlorhexidine have been added into various dental materials to enhance the antibacterial activity. This study aimed to compare the antibacterial effect of MTA, MTA mixed with silver zeolite and MTA mixed with chlorhexidine against Enterococcus faecalis. Test materials used in the study were divided into three groups namely Group 1- MTA, Group 2-MTA + Silver Zeolite, Group 3-MTA + Chlorhexidine. Direct contact test was done by placing a standardized suspension of Enterococcus faecalis on the test materials in a 96 well microtiter plate. The bacterial growth was measured spectrophotometrically using ELISA reader at intervals of one, three and seven days. Data was collected by recording the optical density and analyzed using two-way ANOVA and Tukey's multiple post hoc test followed by paired-t test. All test groups showed antibacterial activity against Enterococcus faecalis at day one, three and seven. MTA with silver zeolite showed the maximum antibacterial activity followed by MTA with 2% chlorhexidine. The least antibacterial effect was shown by MTA mixed with sterile water.

Patient and Clinical Characteristics Associated with Primary Healing of Iatrogenic Perforations after Root Canal Treatment: Results of a Long-term Italian Study

IntroductionThere are few data on the long-term efficacy of mineral trioxide aggregate (MTA) in treating root canal perforations. We investigated the extent of primary healing after perforation repair with MTA and non-healing as a result of repair. We also investigated patient/clinical characteristics affecting treatment outcomes and long-term prognosis. MethodsThis was a prospective cohort study that enrolled consecutive patients with a single dental perforation treated with MTA (January 1999–June 2009). Patients were followed up until December 2012 for a maximum of 13 years after treatment, with analyses carried out at 8 years. ResultsOf the 110 patients (median age, 36 years; 54.5% male) eligible for inclusion, 101 were judged to have started to heal at the first (n = 98, 89%) or second (n = 3, 3%) annual post-treatment checkup, and 9 (8%, 4 women and 5 men, aged between 18 and 65 years) did not show any sign of healing. Patients >50 years had a higher percentage of non-healing perforations compared with those ≤50 years (12% versus 7%). The percentages of perforations at post-treatment analysis that failed to heal were 13% (intermediate/middle), 4% (coronal), and 0% (apical). The percentages of non-healing perforations according to size were 16% for >3 mm, 6% for 2–3 mm, and 0% for smaller perforations. Characteristics associated with probability of progressing after initial healing were gender, positive probing, size, and site of perforation. ConclusionsOur results show that having obtained primary healing with MTA, the likelihood of progressing is very low. They provide good evidence of the combined effectiveness of experienced operators and use of state-of-the-art materials.

Antibacterial and Odontogenesis Efficacy of Mineral Trioxide Aggregate Combined with CO2 Laser Treatment

IntroductionMineral trioxide aggregate (MTA) has been successfully used in clinical applications in endodontics. Studies show that the antibacterial effects of CO2 laser irradiation are highly efficient when bacteria are embedded in biofilm because of a photothermal mechanism. The aim of this study was to confirm the effects of CO2 laser irradiation on MTA with regard to both material characterization and cell viability. MethodsMTA was irradiated with a dental CO2 laser using directly mounted fiber optics in the wound healing mode with a spot area of 0.25 cm2 and then stored in an incubator at 100% relative humidity and 37°C for 1 day to set. The human dental pulp cells cultured on MTA were analyzed along with their proliferation and odontogenic differentiation behaviors. ResultsThe results indicate that the setting time of MTA after irradiation by the CO2 laser was significantly reduced to 118 minutes rather than the usual 143 minutes. The maximum diametral tensile strength and x-ray diffraction patterns were similar to those obtained without CO2 laser irradiation. However, the CO2 laser irradiation increased the amount of Ca and Si ions released from the MTA and regulated cell behavior. CO2 laser–irradiated MTA promoted odontogenic differentiation of hDPCs, with the increased formation of mineralized nodules on the substrate's surface. It also up-regulated the protein expression of multiple markers of odontogenic and the expression of dentin sialophosphoprotein protein. ConclusionsThe current study provides new and important data about the effects of CO2 laser irradiation on MTA with regard to the decreased setting time and increased ion release. Taking cell functions into account, the Si concentration released from MTA with laser irradiation may be lower than a critical value, and this information could lead to the development of new regenerative therapies for dentin and periodontal tissue.

A comparative evaluation of ideal apical sealing material for open apex single rooted permanent tooth: Anin vitrostudy

Introduction: Traumatic injury and dental caries in developing tooth can cause irreversible damage to the pulpal tissue, which hinders the normal physiologic root development. Aims and Objectives: The aim was to find out the sealing efficacy of mineral trioxide aggregate (MTA) as an ideal apexification material of a permanent tooth with an open apex by comparative evaluation of grey and white MTA of different thickness and time interval in vitro. Materials and Methods: A total of 144 teeth were randomly distributed into 6 equal test groups containing 24 teeth each and then filled with white and gray MTA to thickness of 1, 2, and 4 mm. Microleakage of these samples was studied by bacterial leakage model at definite intervals (After 24 h,7 th day,15 th day, and 30 th day). Results: After 24 h, for 1 mm thickness, grey MTA showed less leakage than white MTA, but the difference is not statistically significant at 5% level (Z = 0.32, P = 0.75). After 7 days, in 1 and 2 mm thickness, White MTA showed 64.71% and 16.67% leakage, while Grey MTA revealed 50% and 8.33% leakage in the same category. No leakage was found among the samples of 4 mm in both MTA. After 15 days in 1 mm, 100% leakage was recorded in both MTA. In 2 mm, presence of leakage is 15% for White and 13.64% leakage were present in Grey MTA. However, this difference is not significant (Z = 0.126, P = 0.9). In 4 mm MTA, only 1 leakage was found in White and none for Grey. This difference is not significant (Z = 1.02, P = 0.31). After 30 days, in 2 mm, 9% more leakage was observed in White MTA samples (23.53%) than that of Grey MTA (14.29%). In the category of 4 mm, 0.37% more leakage was observed in White MTA samples (8.70%) than that of Grey MTA (8.33%). However, this difference was not statistically significant. Conclusion: The present study showed that apical barrier of 4 mm demonstrated less leakage than 1 and 2 mm barrier in both White and grey MTA after 30 days of study and grey MTA demonstrated significantly less leakage than white MTA.

Clinical and radiographical evaluation of mineral trioxide aggregate, biodentine and propolis as pulpotomy medicaments in primary teeth.

ObjectivesThe purpose of this study was to evaluate the efficacy of mineral trioxide aggregate (MTA), Biodentine and Propolis as pulpotomy medicaments in primary dentition, both clinically and radiographically.Materials and MethodsA total of 75 healthy 3 to 10 yr old children each having at least one carious primary molar tooth were selected. Random assignment of the pulpotomy medicaments was done as follows: Group I, MTA; Group II, Biodentine; Group III, Propolis. All the pulpotomized teeth were evaluated at 3, 6, and 9 mon clinically and radiographically, based on the scoring criteria system.ResultsThe clinical success rates were found to be similar among the three groups at 3 and 6 mon where as a significant decrease in success rate was observed in Group III (84%) compared to both Group I (100%) and Group II (100%) at 9 mon. Radiographic success rates over a period of 9 mon in Groups I, II, and III were 92, 80, and 72%, respectively.ConclusionsTeeth treated with MTA and Biodentine showed more favorable clinical and radiographic success as compared to Propolis at 9 mon follow-up.

Clinical Evaluation of Success of Primary Teeth Pulpotomy Using Mineral Trioxide Aggregate(®), Laser and Biodentine(TM)- an In Vivo Study.

Pulpotomy technique basically consists of removing the coronal pulp and fixing the radicular pulp with a medicament. It is the most widely accepted clinical procedure for treating primary teeth with coronal pulp inflammation caused by caries with no involvement of the radicular pulp. To evaluate the success and efficacy of Mineral Trioxide Aggregate (MTA), Lasers and Biodentine as pulpotomy agents both clinically and radiographically. In the present study, 60 primary molars in children whose pulpal status warranted pulpotomy were selected and randomly assigned into three groups that included MTA, Laser and Biodentine allocating 20 teeth to each group. The pulpotomy procedure was then performed on all selected teeth followed by restoration with stainless steel crowns. Later the patients were recalled for 3 months and 6 months for clinical and radiographic evaluation. Statistical analysis was done using Fisher exact test to determine pair wise comparison of three agents with respect to clinical and radiographic criteria. Kruskal-Wallis ANOVA, Mc Nemars test was applied to evaluate the efficacy of each agent between 3 months and 6 months. The results showed that maximum success rate was found in MTA group. However, the comparison between three groups was statistically not significant (p<0.05). Pulpotomies performed with either MTA, Laser or Biodentine are equally efficient with similar clinical/radiographic success and hence can be considered as alternatives to Formocresol.

Antimicrobial Efficacy of Mineral Trioxide Aggregate with and without Silver Nanoparticles.

Most current root-end filling materials do not provide a perfect seal. Thus, a microscopic space is likely to exist in the interface between walls of the root-end cavity and filling material, which allows microorganisms and their products to penetrate. In addition to good sealing ability and biocompatibility, root-end filling materials should ideally have some antimicrobial activity. Therefore, this in vitro study aimed to evaluate the antimicrobial properties of Angelus white mineral trioxide aggregate (MTA) and the mixture of MTA with silver nanoparticles (1% weight; MTA/SN). Antimicrobial properties of MTA and MTA/SN were tested by agar diffusion technique against Enterococcus faecalis, Pseudomonas aeruginosa, Staphylococcus aureus, and Candida albicans. The microbial inhibition zones around the materials were measured by a caliper with 0.1-mm accuracy. Student's t-test was used for comparison between the two groups in normal data distribution and Man-Whitney U test for non-normal distribution. Student's t-test revealed that for E. faecalis, C. albicans, and P. aeruginosa, microbial inhibition zone of MTA/SN was significantly greater than that of MTA (P = 0.000). Mann-Whitney U test indicated no significant difference between the effect of MTA and MTA/SN on S. aureus (P > 0.05). Based on the results of this study, adding silver nanoparticles to MTA improved its antimicrobial efficacy.

Is there sufficient evidence to support the long‐term efficacy of mineral trioxide aggregate (MTA) for endodontic therapy in primary teeth?

Several papers have been published to illustrate the effectiveness of mineral trioxide aggregate (MTA) as a pulpotomy medicament. Most of these reports do not offer a critical assessment on the data quality. Therefore, this review evaluated whether the currently available evidence is of an appropriate quality to support the long-term effectiveness of MTA as a pulpotomy medicament in primary molars using a standardized assessment criterion. A comprehensive literature search of human clinical outcome studies, which employed MTA as a pulpotomy medicament in primary teeth, was conducted using the MEDLINE database. Two independent observers rated these articles using the standardized assessment criteria. Furthermore, based on the initial sample mentioned in the individual studies and the sample included for the final analysis, the drop-out rates were calculated. Twenty-two studies were included for quality assessment with an excellent interobserver agreement. None of the 22 studies obtained grade A, four studies attained grade B1, five were graded B2 and 13 received grade C. Based on the assessment criteria employed, there was no evidence that MTA was better than present materials and techniques as a pulpotomy medicament. Furthermore, given the low quality of data, it is highly desirable to establish standard requisites for conducting and reporting on pulp therapy studies in primary teeth so as to benefit both researchers and clinicians to produce high-quality studies that are comparable and to prevent the misuse of clinical material and resources.

Remineralization of artificial dentinal caries lesions by biomimetically modified mineral trioxide aggregate

Fluoride-releasing restorative materials are available for remineralization of enamel and root caries. However, remineralization of dentin is more difficult than remineralization of enamel due to the paucity of apatite seed crystallites along the lesion surface for heterogeneous crystal growth. Extracellular matrix proteins play critical roles in controlling apatite nucleation/growth in collagenous tissues. This study examined the remineralization efficacy of mineral trioxide aggregate (MTA) in phosphate-containing simulated body fluid (SBF) by incorporating polyacrylic acid and sodium tripolyphosphate as biomimetic analogs of matrix proteins for remineralizing caries-like dentin. Artificial caries-like dentin lesions incubated in SBF were remineralized over a 6 week period using MTA alone or MTA containing biomimetic analogs in the absence or presence of dentin adhesive application. Lesion depths and integrated mineral loss were monitored with microcomputed tomography. The ultrastructure of baseline and remineralized lesions was examined by transmission electron microscopy. Dentin remineralization was best achieved using MTA containing biomimetic analogs regardless of whether an adhesive was applied; dentinal tubules within the remineralized dentin were occluded by apatite. It is concluded that the version of MTA employed in this study may be doped with biomimetic analogs for remineralization of unbonded and bonded artificial caries-like lesions in the presence of SBF.

Efficacy of Mineral Trioxide Aggregate as an Apical Plug in Non-Vital Young Permanent Teeth: Preliminary Results

The purpose of this study was to evaluate the efficacy of Mineral trioxide aggregate (MTA) clinically and radiographically as material used to induce root end closure in nonvital permanent teeth with immature apices (apexification) in children. The study included 30 non vital young permanent, single rooted teeth of 22 children between 8 and 13 years of age. Treatment followed a standard non-surgical root canal treatment protocol and the root canal was filled with a apical plug of 4-5 mm of MTA (white MTA--Angelus, Brazil), followed by gutta-percha obturation. The children were reviewed for 1 year at 3 month interval and the teeth were assessed clinically and radiographically. MTA showed success rate of 100% both clinically and radiographically at the 12th months follow up and root end closure was seen in 86.6% of cases and root growth in 30% of cases. MTA showed clinical and radiographic success as an apexification material by inducing root end closure and root growth in non-vital young permanent teeth.
