The Effect of Intravitreal Bevacizumab Injection in Wet Age-Related Macular Degeneration Patients with Cataract Surgery

This study was design to evaluate the efficacy of cataract surgery with simultaneous intravitreal bevacizumab injection for wet age-related macular degeneration. Of the patients who underwent anti-vascular endothelial growth factor injections, cataract surgery was performed in 24 eyes (control group), and simultaneous intravitreal bevacizumab injection was performed in 27 eyes (bevacizumab group). Best corrected visual acuity, ocular tonometry, anterior chamber inflammation, and central subfield macular thickness were measured at baseline and 1 week, 1 month, 3 months, and 6 months after surgery. The mean central subfield macular thickness increased 1 month postoperatively in the control group (<i>p</i> = 0.02), whereas there was no significant change in the bevacizumab group. When the preoperative and postoperative differences between the two groups were compared at each time point, the mean best corrected visual acuity improved in the bevacizumab group at 3 months after surgery (<i>p</i> = 0.045), and the mean central subfield macular thickness was more decreased in the bevacizumab group at 1 month after surgery compared to that of the control group (<i>p</i> = 0.032). Additional anti-vascular endothelial growth factor injection was performed after surgery at 1.56 ± 2.05 months and 2.52 ± 2.01 months, in the control and bevacizumab groups, respectively (<i>p</i> = 0.047). Simultaneous cataract surgery and intravitreal bevacizumab injection is considered an effective treatment since it reduces central subfield macular thickness in the short term, and it can delay the timing of additional anti-vascular endothelial growth factor injections after surgery.

Changes in the Choroidal Thickness following Intravitreal Bevacizumab Injection in Chronic Central Serous Chorioretinopathy.

We evaluated the effect of intravitreal bevacizumab injection (IVB) on choroidal thickness, and studied its association with the therapeutic response in chronic central serous chorioretinopathy (CSC). The clinical features of 78 eyes with chronic CSC treated with IVB from October 2014 to June 2020 were retrospectively evaluated. Visual acuity (VA), central retinal thickness (CRT), and sub-foveal choroidal thickness (SFCT) were analyzed at baseline, 1 month following initial IVB, and the last follow-up examination. Cases showing complete recovery (resolved eyes; n = 60) were compared with those with persistent subretinal fluid (refractory eyes; n = 18). The relationship between the potential risk factors and subretinal fluid resolution was examined using logistic regression. SFCT was significantly decreased along with the CRT following IVB at the resolved state. SFCT reduction following 1 month of IVB was notably greater in the resolved eyes. The association of refractory eyes with hypertension (p = 0.003) and a thinner baseline SFCT (p = 0.024) was significant. In most of the patients with chronic CSC, VA and CRT remarkably improved following treatment with IVB. Early changes in the SFCT following IVB were associated with the therapeutic response. Patients with hypertension and a thinner baseline SFCT could be unresponsive to IVB.

Evaluation of Infants with Retinopathy of Prematurity Treated with Intravitreal Bevacizumab

Objective: To evaluate the systemic and local effects and side effects of intravitreal bevacizumab injection in preterm infants with aggressive posterior retinopathy of prematurity. Material and Methods: A total of 16 patients were included in this single-center observational cohort study between June 2020 and March 2021. The clinical, laboratory, and radiological findings before and after intravitreal injection of 0.3 mg bevacizumab were recorded for a total of 32 eyes. Results: The patients’ mean gestational age at birth was 25±0.38 weeks and their mean birth weight was 695±119.2 g. None of the patients developed complications such as local vitreous hemorrhage or endophthalmitis secondary to intravitreal injection administered at a postmenstrual age of 34 weeks, and none required repeated injections. There was no increase in the rate of intraventricular hemorrhage after injection. There was no statistically significant difference in the patients’ laboratory parameters, blood pressure values, or systemic findings before and after treatment. Conclusion: Although no systemic or local side effects were observed in patients treated with intravitreal bevacizumab injection in this study, patients should be monitored closely for systemic hypertension, intraventricular hemorrhage, and hypotension. More detailed studies are needed to evaluate later neurodevelopmental outcomes compared to other treatment options.

Combined Intravitreal Bevacizumab and Laser Photocoagulation to Treat Retinal Arterial Macroaneurysms

Purpose: To investigate the effects of intravitreal bevacizumab injection combined with laser photocoagulation in patients with retinal microaneurysms.Methods: We retrospectively reviewed the medical records of 21 eyes of 21 patients who underwent intravitreal bevacizumab injection and/or laser photocoagulation to treat retinal macroaneurysms. Eleven eyes received the combination therapy (CT) and 10 eyes monotherapy (MT) (either bevacizumab injection or laser photocoagulation). Changes in visual acuity, central macular thickness, macroaneurysm size and location, blood pressure, and the lipid level were compared between the two groups.Results: The mean patient age was 74.0 ± 10.5 years and the mean study period 7.0 ± 5.3 months. The mean macroaneurysm diameter of the CT group was 480.00 ± 292.30 μm and that of the MT group 328.75 ± 87.09 μm. The diameter was significantly larger in the CT group (p = 0.002). The initial visual acuities were 0.91 ± 0.66 and 0.88 ± 0.83 in the CT and MT groups, respectively. At the 4-month follow-up, the visual acuities were 0.33 ± 0.26 and 0.17 ± 0.29 in the CT and MT groups, respectively, and had significantly improved only in the CT group (p = 0.042). The initial central macular thicknesses were 441.82 ± 226.81 and 541.63 ± 401.97 μm in the CT and MT groups, respectively. At the 4-month follow-up, the figures were 293.60 ± 46.10 and 269.00 ± 48.34 μm in the CT and MT groups, respectively, and had significantly decreased only in the CT group (p = 0.043). Compared to the initial findings, the proportion of patients whose final visual acuities improved by more than two lines were 73% and 40%, respectively, thus significantly higher in the CT group (p < 0.001).Conclusions: Combined intravitreal bevacizumab injection and laser photocoagulation treatment of retinal macroaneurysms improve visual acuity and decrease macular thickness.

Comparison of Intravitreal Bevacizumab and Aflibercept Injections for Central Serous Chorioretinopathy

Purpose: We examined differences in the treatment effects of intravitreal bevacizumab injections and intravitreal aflibercept injections in patients with central serous chorioretinopathy. Methods: This retrospective analysis included 51 eyes of 49 patients who received intravitreal anti-vascular endothelial growth factor agent injections after initial diagnosis with central serous chorioretinopathy. The patients were divided into two groups: one received an intravitreal bevacizumab injection, and another one received an intravitreal aflibercept injection. Patients with no reaction to treatment or a worsened condition, received repeat treatment with the same therapy. After treatment, patients were monitored for >3 months. Data were collected regarding best- corrected visual acuity (BCVA), subfoveal choroidal thickness, injection number, and treatment duration. Results: Both intravitreal bevacizumab injections and intravitreal aflibercept injections led to significant differences in BCVA (p < 0.0001, p = 0.001) and subfoveal choroidal thickness (p < 0.0001, p = 0.011), compared between before and after treatment. However, no differences between groups were observed in mean change of BCVA or subfoveal choroidal thickness. In addition, there were no differences between groups in injection number and treatment duration. Conclusions: In patients with central serous chorioretinopathy, both intravitreal bevacizumab injections and intravitreal aflibercept injections are effective treatment methods. There were no differences between the two medicines in terms of functional and anatomical recovery, or the injection number and treatment duration.

Effect of intravitreal Bevacizumab injection at the time of phacoemulsification on the development of macular edema in diabetic patients with and without preoperative retinopathy

Aim The aim of this study was to evaluate the effect of intravitreal Bevacizumab injection at the time of phacoemulsification on the incidence of postoperative diabetic macular edema (DME) in diabetic patients without preexisting DME.Patients and methods The study included 40 diabetic patients with no preexisting DME who underwent phacoemulsification. They were divided into two groups: group A (controls) included 20 eyes in which phacoemulsification was performed without Bevacizumab injection and group B (Bevacizumab): included 20 eyes in which 1.25 mg Bevacizumab was injected at the conclusion of surgery. Macular optical coherence tomography and visual acuity were assessed at 2 weeks, 2 months, and 6 months postoperatively.Results The incidence of DME in the noninjected patients was 5, 20, and 30% at 2 weeks, 2 months, and 6 months, respectively, compared with 0, 5, and 5% in the Bevacizumab injected patients. This difference in incidence was only statistically significant at 6 months (P=0.15 at 2 months and 0.04 at 6 months). On comparing the baseline central macular thickness in the control and Bevacizumab groups (229.4±26.8 and 230.8±26.7 μm, respectively) to the 6-month follow-up values (278.3±52.3 and 254.7±26.2 μm, respectively), a higher statistically significant difference was found in the control group (P=0.0001) versus the injected group (P=0.002) denoting more progression of DME in the control group.Conclusion Prophylactic intravitreal injection of Bevacizumab at the time of phacoemulsification is potentially effective in the midterm prevention of DME in cases with and without preoperative diabetic retinopathy.

Effect of Intravitreal Bevacizumab Injection before Panretinal Photocoagulation on the Prevention of Macular Edema Aggravation in Proliferative Diabetic Retinopathy.

Objective: To investigate the effects of intravitreal bevacizumab (IVB) injection before PRP on the prevention of macular edema (ME) in patients with PDR. Methods: This retrospective observational study included patients diagnosed with PDR treated by PRP with (combination group) or without (PRP alone group) preoperative IVB injection (1.25 mg/0.05 mL). The primary outcome measure was the change in the central macular thickness (CMT), while the secondary outcome measure was the change in visual acuity. Measurements were made before and at one, two, and three months after treatment. Results: In the PRP alone group, the mean baseline CMT of 277.8 μm increased to 290.4 μm at one month (p = 0.201) and 308.8 μm at two months (p = 0.002), followed by a decrease to 271.2 μm at three months (p = 0.437). In the combination group, the values at baseline and one, two, and three months after PRP were 322.9 μm, 295.4 μm (p = 0.002), 330.1 μm (p = 0.906), and 274.5 μm (p = 0.030), respectively. Visual acuity changes were comparable between the two groups at all time points. Conclusion: IVB injection before PRP leads to decreased CMT in comparison to CMT in patients with PRP alone.

Immediate Effect of Intravitreal Bevacizumab Injection on Intraocular Pressure

Purpose: To determine the immediate effect of intravitreal Bevacizumab on intraocular pressure (IOP) in eyes with retinal vascular disorders.
 Study Design: Interventional case series.
 Place and Duration of Study: Ophthalmology Department, Fauji Foundation Hospital, Rawalpindi, from January 2019to July 2019.
 Methods: One hundred and thirty one patients were included in this study using non probability consecutive sampling. IOP was measured before intravitreal injection (baseline) and at 5 and 30 minutes post-injection. Descriptive statistics were obtained using SPSS version 21.0.
 Results: One hundred and thirty one eyes of 131 patients were observed for IOP change after intra vitreal injection of Bevacizumab. Among them 23 (18%) were males and 108 (82.4%) were females. Mean age was 57.57 ± 13.09 years. Mean IOP at baseline was 16.16 ± 2.52 mmHg. At 5 minutes after injection, 108 eyes (82.4%) had raised IOP, maximum IOP recorded was 44 mmHg. There was a statistically significant correlation between IOP at baseline and 5 minutes with p = 0.005 (≤0.05). At 30 minutes, 94 eyes (71.7%) had IOP fallen back to normal. Statistically, an insignificant correlation was present between IOP at baseline and 30 minutes (p = 0.081) post-injection. IOP rise was not significantly correlated to gender, age, hypertension and asthma at any interval (p value > 0.05). However, IOP rise was significantly correlated in diabetic patients at 30 minutes.
 Conclusion: IOP elevation has been observed after intravitreal bevacizumab in immediate post-injection period which warrants the monitoring of IOP in this critical period to avoid serious blinding complications.

Effect of intravitreal bevacizumab injection in central serous chorioretinopathy

AbstractPurposeTo investigate the effect of intravitreal bevacizumab injection on the type of central serous chorioretinopathy.MethodsWe performed a retrospective analysis of 43 patients (43 eyes) who were diagnosed with central serous chorioretinopathy and underwent intravitreal bevacizumab injection from March 2014 to March 2018. The best corrected visual acuity (BCVA) and central macular thickness (CMT) before and 3, 6, 9, and 12 months after injection were compared according to the type of central serous chorioretinopathy. The types of central chorioretinopathy were classified as acute (onest < 6 months), chronic (onest ≥ 6 months), and recurrent group.Results43 eyes of 43 patients, 10 eyes of acute group, 21 eyes of chronic group, and 12 eyes of recurrent group. Age, sex, initial BCVA, CMT, duration of follow‐up, and the number of intravitreal bevacizumab injection were not significantly different between the three groups (p > 0.05). The BCVA (logMAR) was not significantly different between the three groups before, 3, 6, 9, and 12 months after the injection. But, at 3, 6, 9, and 12 months after injection, the BCVA of the chronic group tended to be worse than that of the acute group and the recurrent group. The CMT was not significantly different between the three groups before, 3, 6, 9, and 12 months after injection. But, at 12 months, the CMT of the chronic group tended to be larger than that of the acute group and the recurrent group. In each group, a comparison of the initial BCVA and BCVA at 12 months, initial BCVA and final BCVA, and initial CMT and CMT at 12 months showed a significant decrease in the CMT at 12 months compared to the initial CMT in the acute group(p = 0.005). There was no statistical significance in the chronic group. In the recurrent group, both the BCVA at 12 months (p = 0.007) and final BCVA (p = 0.011) improved significantly compared with the initial BCVA. The CMT was also significantly decreased at 12 months compared to the initial CMT (p = 0.006).ConclusionsWe concluded the effect of intravitreal bevacizumab injection on the improvement of BCVA and CMT in acute and recurrent central serous chorioretinopathy.

Treatment response to intravitreal bevacizumab in small pigmented choroidal lesions with subretinal fluid

BackgroundTo describe the effects of intravitreal bevacizumab injection (IVB) and/or transpupillary thermotherapy (TTT) in the treatment of small pigmented choroidal lesions with subfoveal fluid (SFF), and to investigate prognostic value of the therapeutic response in future tumor growth.MethodsRetrospective chart review of 19 patients, who were diagnosed with choroidal neovascularization (CNV)-free small pigmented choroidal lesions and treated with IVB and/or TTT, was performed.ResultsComplete resolution of SFF was achieved in two eyes (2/14; 14.3%) after IVB, and in three eyes (3/4; 75%) after TTT. Best corrected visual acuity was improved in two eyes (2/9; 22%) after IVB, and in three eyes (3/4; 75%) after TTT. Among five patients who underwent TTT after IVB, four patients (4/5; 80%) demonstrated additional advantage. All IVBs could not reduce tumor sizes. Rather, tumor growth was detected in seven out of 14 eyes (7/14; 50%) that underwent IVB. None of the patients who underwent TTT showed tumor growth. The lack of treatment response to IVB was suggestive of malignancy, as most small pigmented lesions that had no response to IVB showed tumor growth (86%, p = 0.010).ConclusionIVB was not effective in reducing tumor size and subfoveal fluid in small pigmented choroidal lesions. Therapeutic response to IVB can be used as an indicator between melanoma and nevus in small pigmented choroidal lesion.

Effect of Intravitreal Bevacizumab Injection on Corneal in vivo Biomechanics: A Pilot Study.

Purpose:To evaluate the effect of intravitreal bevacizumab (IVB) injection on corneal biomechanical parameters as measured by the ocular response analyzer (ORA) and Corneal Visualization Scheimpflug Technology (CorVis).Methods:In this prospective pilot study, ORA and CorVis parameters were recorded before and after a three-month course of IVB injection therapy in 16 patients in the injected and the contralateral non-injected control eyes. The changes in the recorded parameters in each group and the differences between the two groups were evaluated and compared.Results:None of the changes in ORA parameters were statistically significant in the injected and non-injected groups before and three months after injection, except for corneal resistance factor (CRF) in injected eyes (paired t-test, P = 0.039). The differences in corneal hysteresis (CH) and CRF were not statistically significant between the two groups (P = 0.441 and 0.236, respectively), but significant differences were noted between corneal compensated IOP (IOPcc) and Goldmann-correlated IOP (IOPg) (P = 0.045 and 0.047, respectively). None of the changes in CorVis parameters were statistically significant in the groups before and at the end of study, except for the time of first corneal applanation (TAp1 ms) in the injected group (P = 0.040, paired t-test). Differences in TAp1, length of the second corneal applanation (LAp2 mm), velocity of the second corneal applanation (VAp2 m/s), intraocular pressure (IOP), and central corneal thickness (CCT) also showed borderline significance between the two groups.Conclusion:In this pilot study IVB injection could change CRF, IOPcc, IOPg, and TAP1 as measured by ORA and CorVis.

The long‐term effect of intravitreal bevacizumab injection in central serous chorioretinopathy

PurposeTo study the effect of intravitreal bevacizumab injection in patients with idiopathic central serous chorioretinopathy.MethodsWe had performed retrospective review on the patients who had been diagnosed as idiopathic central serous chorioretinopathy and had regular follow‐up for at least 12 months from the first injection, either undergone intravitreal bevacizumab injection (107 eyes of 100 patients) or observed without injection (46 eyes of 40 patients). Changes in the visual acuity and the serous neurosensory retinal and retinal pigment epithelium detachment were evaluated through the results of best corrected visual acuity and optical coherence tomography measured at initial presentation, after 1, 3, 6, 9 and 12 months of the first injection.Results87 males and 13 females with mean age of 45.4 ± 7.6 years were recruited in the intravitreal bevacizumab injection (IVB) group, and 34 males and 6 females with mean age of 47.2 ± 9.8 years were recruited in the observation group. There was significant difference in the mean duration of retinal detachment to disappear between the IVB group (2.5 ± 0.2 months) and the observation group (4.3 ± 0.7 months). There was no significant difference in the best corrected visual acuity and the central macular thickness after 12 months follow‐up between the IVB group and the observation group.ConclusionsIntravitreal bevacizumab injection was more effective and faster treatment for resolution of sensory retinal detachment than observation during the 12 months of consecutive follow‐up. Therefore, intravitreal bevacizumab injection can be considered as a significant and safe treatment modality for central serous chorioretinopathy when the patients need prompt visual improvement, such as depending on the morbid eye or requiring binocular vision for occupational cause.

Effects of intravitreal bevacizumab injection on the clinical manifestations of nonproliferative diabetic retinopathy in patients with macular edema: a systematic review

Introduction: Bevacizumab (Avastin) is considered as an effective strategy in the treatment of various ocular diseases. As a vascular endothelial growth factor (VEGF) inhibitor, Avastin is used to control macular edema in patients with nonproliferative diabetic retinopathy (NPDR). Therefore, in this study, we systematically reviewed the effects of intravitreal bevacizumab injections on nonproliferative stage of diabetic retinopathy. Methods: Scopus and PubMed were systematically searched to find articles in which the effect of Avastin (bevacizumab) had been evaluated in nonproliferative stage of diabetic retinopathy. Literature search was performed using “Avastin OR bevacizumab”, “nonproliferative stage” and “diabetic retinopathy” as keyterms in the title, keywords, and abstract.Result: All 47 articles were found in all databases, two additional records were found through reference list screening, and only 10 articles were relevant to the purpose of this study. According to the inclusion/exclusion criteria, 39 articles were excluded in several step processes of article selection. The results revealed that intravitreal injection of bevacizumab could be safely used to treat various ocular disease, particularly NPDR stage of diabetic retinopathy with macular edema.Discussion: Bevacizumab is considered as a novel and effective modality in the treatment of various ocular diseases such as retinal neovascularization, neovascular glaucoma, macular edema, and other ocular complications. Findings also suggested that bevacizumab is a suitable therapeutic approach in clinical use.Conclusion: According to the results of included documents, intravitreal injection of bevacizumab could be considered as a promising treatment modality in the clinical management of NPDR stage of diabetic retinopathy.

The effect of intravitreal bevacizumab injection on the corneal endothelial cells

Introduction:Bevacizumab (Avastin), as an effectiveness treatment modality, is currently used in patients with various ocular disease. However the results have been promising, the use of bevacizumab in the treatment of ocular disease is an off-label application. Hence, the aim of this study was to systematically review the effectiveness of intravitreal injection of bevacizumab on various ocular tissues, especially corneal endothelial cells. Methods: The articles related to the effect of application of Avastin in the treatment of ophthalmic diseases and especially its effect on corneal endothelial cells were collected and reviewed. We searched PubMed, Google scholar, and Scopus databases and used Avastin, ocular diseases and corneal endothelial cells as search keywords.Result: Of all 55 articles found in all databases, only 10 were relevant to the purpose of this study, and 45 articles were excluded in several step by step process of article selection according to the inclusion/exclusion criteria. The results revealed that intracameral bevacizumab injection caused no changes in specular microscopy and corneal pachymetry. Moreover, it had no significant toxicity on corneal endothelial cells.Discussion: Effectiveness of bevacizumab as a new modality in the treatment of different ophthalmic diseases have been suggested. Recent data on both human and animal models showed that intravitreal injection of bevacizumab resulted in no significant toxicity on various ocular cells, and it could be considered as a suitable therapeutic approach in clinical use.Conclusion: According to the results of included documents, bevacizumab was not toxic to corneal endothelial cells at various clinically relevant doses.

Intravitreal Bevacizumab for Nonsubfoveal Choroidal Neovascularization Associated With Angioid Streaks: 3-Year Follow-up Study

To assess the effects of intravitreal bevacizumab injections in the treatment of nonsubfoveal choroidal neovascularization (CNV) associated with angioid streaks (AS) in a 3-year follow-up study. Noncomparative, interventional, prospective case series. Eighteen patients (18 eyes) with juxtafoveal/extrafoveal CNV secondary to AS were recruited. All patients underwent a complete ophthalmologic examination, including best-corrected visual acuity (BCVA) measurement on ETDRS chart, optical coherence tomography (OCT), and fluorescein angiography (FA). The protocol treatment included a first injection, followed by repeated injections on the basis of detection of new hemorrhage, any type of fluid on OCT, and/or presence of FA leakage. Primary outcome measures were final mean changes in BCVA and proportion of eyes with 10 ETDRS letters improvement. Secondary outcomes were mean changes of central macular thickness (CMT) and foveal involvement. After a mean BCVA stabilization over the first year, a statistically significant BCVA worsening was registered at the 24-month (72.8 ± 10.0 ETDRS letters, P= .03) and 36-month examinations (65.8 ± 15.0 ETDRS letters, P= .02) in comparison with the 1-year visual outcomes (80.1 ± 5.4 ETDRS letters); lastly, a substantial stabilization in the BCVA was observed at 36months in comparison with the baseline value (77.9 ± 10.0 ETDRS letters, P= .22). Two eyes (25%) with juxtafoveal CNV and no eye with extrafoveal CNV experienced a 10-letter improvement at the 3-year examination. Mean CMT at baseline was 220 ± 15μm and 235 ± 66μm at 36months (P= 1.00). During the first and second years of follow-up, 5 juxtafoveal CNVs and 3 extrafoveal CNVs showed foveal involvement. Intravitreal bevacizumab can be effective in the management of nonsubfoveal CNV secondary to AS, although monthly monitoring is required to control CNV recurrence or progression.

Predictors of short-term outcomes related to central subfield foveal thickness after intravitreal bevacizumab for macular edema due to central retinal vein occlusion.

To investigate the predictive factors for short-term effects of intravitreal bevacizumab injections on central subfield foveal thickness (CSFT) in patients with macular edema (ME) secondary to central retinal vein occlusion (CRVO). This was a retrospective study in 60 eyes treated with intravitreal bevacizumab injections for ME due to CRVO. Follow-up was three months. The Early Treatment Diabetic Retinopathy Study (ETDRS) score and CSFT measured by spectral-domain optical coherence tomography (SD-OCT) were used to observe the changes in best-corrected visual acuity (BCVA). Baseline BCVA, CSFT, age, CRVO duration and the presence of cystoid macular edema (CME) or subretinal fluid (SRF) were analyzed as potential predictive factors of the effects of intravitreal bevacizumab injections. BCVA improved from 0.9 logMAR at baseline to 0.6 logMAR at 3mo, which was associated with a significant reduction in CSFT from 721 µm to 392 µm 3mo after injection. About 50% of CME cases and more than 90% of SRF cases responded to treatment with a complete resolution at 3mo. Age (P=0.036) and low baseline CSFT (P=0.037) were associated with a good 3-month prognosis. Patients >60 years old achieved better CME resolution (P=0.031) and lower CSFT at 3mo (305 µm vs 474 µm, P=0.003). Intravitreal bevacizumab significantly improved visual acuity and CSFT in patients with CRVO after 3mo. Older age and lower baseline CSFT were good predictors of short-term CSFT outcomes. The retinal thickness response to bevacizumab might depend on the resolution of CME rather than SRF.

Effects of Intravitreal Injection of Bevacizumab or Ranibizumab on Systemic Circulation

Purpose: The purpose of this study was to evaluate the systemic effects of ranibizumab and bevacizumab by examining the plasma levels of anti-vascular endothelial growth factor (anti-VEGF) and VEGF before and after a single intravitreal injection. Methods: Twenty-eight eyes of 28 patients with various retinal diseases were enrolled. Seventeen eyes received an injection of intravitreal bevacizumab, and 11 eyes received an injection of ranibizumab. Blood samples were collected just before and 1 day, 1 week, and 1 month after injection. Concentrations of anti-VEGF and VEGF in plasma were measured using enzyme-linked immunosorbent assay (ELISA). Results: In the bevacizumab group, anti-VEGF concentration before the injection was 91.0 ng/mL, while those at 1 day, 1 week, and 1 month post-injection increased to 153.6, 196.3, and 140.3 ng/mL, respectively (p 0.05 for all). VEGF level also showed no significant change. VEGF concentration before the injection was 80.9 pg/mL, while those at 1 day, 1 week and 1 month post-injection were 96.7, 106.3, 106.1 pg/mL, respectively (p > 0.05 for all). Conclusions: Contrary to ranibizumab, intravitreal bevacizumab significantly lowers plasma VEGF level in patients with various retinal diseases. and 12 months after PRP was 0.24 (0.575), 0.27 (0.537), 0.28 (0.525), 0.28 (0.525), and 0.30 (0.501) (p = 0.13, 0.15, 0.56 and 0.79) in the regular PRP group and 0.32 (0.479), 0.25 (0.562), 0.26 (0.549), 0.27 (0.537), and 0.36 (0.436) (p = 0.02, 0.04, 0.02 and 0.13) in the combination group, respectively. CMT (µm) at pretreatment and 1, 3, 6, and 12 months after PRP was 257.66, 285.16, 282.21, 289.65, and 309.85 (p = 0.00, 0.00, 0.00 and 0.00) in the regular PRP group and 349.39, 312.17, 331.15. 353.30, and 333.55 (p = 0.04, 0.94, 0.79 and 0.06) in the combination group, respectively. Conclusions: Pretreatment of anti-vascular endothelial growth factor injection before PRP affected the decrease of macular thickness for 3 months after PRP and improved visual acuity for 6 months after PRP when compared with PRP alone in patients with CSME.
