The Association between Ultrasound Results and the Hyaluronic Acid Treatment Response in Patients with Knee Osteoarthritis

Objective: Knee osteoarthritis (OA) is common in middle-aged and elderly people. Patients with knee OA can use hyaluronic acid (HA) injection therapy if they meet certain requirements. The purpose of this study was to delineate the effects of HA injection in patients with knee OA and to identify factors associating with the efficacy of the injections by using musculoskletal sonography (MUS). Methods: We included 100 patients with knee OA in this study from May 2012 to May 2013. The patients were evaluated using MUS 4 to 5 weeks after receiving a complete course of intraarticular HA injection. Radiographic and overall clinical assessments were conducted and a chi-square test was used to perform statistical analysis. Results: The mean ages of the men and women in the patient group were 72.6 years and 63.7 years, respectively. Most of the patients were women (86%). Approximately 61% of the patients (response group) exhibited favorable responses to the HA injections. The MUS findings indicated that only 21% of the patients exhibited Baker's cysts, osteophytes were present in 80% of the patients, and 69% had joint effusion. Fewer than half of the patients developed synovial proliferation (44%). The nonresponse group was highly correlated with joint effusion, Baker's cysts, and synovial proliferation (p ＜ 0.05). Conclusion: Our study indicated that joint effusion, Baker's cysts, and synovial proliferations should be treated before HA injection in order to obtain better treatment response.

Preliminary evaluation of histological changes found in a mechanical arthropatic temporomandibular joint (TMJ) exposed to an intra-articular Hyaluronic acid (HA) injection, in a rat model

Objective This study investigates the histological effects of Hyaluronic acid injections in the treatment of induced temporomandibular joint (TMJ) osteoarthritis in rats. Study design Twenty-four male Wister rats were subjected to induced mechanical osteoarthritis by manual hypermobility for 10 successive days. Animals were then divided into two groups; group I (control) and group II (experimental). Ten days after the induction of hypermobility, the right TMJ of the experimental animals was injected with a dose of 0.12 mg HA intra-articularly and 0.12 mg saline was injected into the left joint; while animals in the control group were left without any treatment. Two rats from group I were killed at one, two and six weeks; while 6 animals from group II were killed at one, two and four weeks post injection. Results The disk of the right joints in the experimental animals was of normal thickness and there was an increase in the thickness of the fibrocartilagenous layer. In the left joint; ulcerative changes in the disk were evident where the fibres were not well oriented and scalloped areas in the temporal bone area were present denoting osteoclastic activity. Conclusions Repeated intra-articular TMJ injection of Hyaluronic acid appears to be a safe and effective way of inhibiting the progression of osteoarthritic changes in the joint through development of articular cartilage and reducing fibrous tissue proliferation.

Medial knee joint loading increases in those who respond to hyaluronan injection for medial knee osteoarthritis

Knee osteoarthritis (OA) is a cause of decline in function and the medial compartment is often affected. Intraarticular injection of hyaluronic acid (HA) is indicated as a symptom modifying treatment with at least 6 months passing between consecutive injection series. The effects of HA injection on gait variables have not been extensively examined. Therefore, our objective was to investigate the effects of HA injection on gait in people with medial knee OA. Twenty-seven subjects were included; each was tested prior to treatment (baseline), no later than 3 weeks following the last injection (post-HA), and again 5 months after treatment ended (follow-up). Responder criteria were defined to identify responders and non-responders. Subjects underwent 3D gait analysis, muscle activity was sampled, and co-contraction indices were calculated. Responders experienced increased peak knee adduction moments post-HA, whereas non-responders did not. Improved self-report scores were associated with increased knee adduction moments and increased medial co-contraction. Pain relief may result in higher loading onto the already vulnerable medial compartment due to changes in lower limb mechanics and muscle activation patterns. Eventually this may result in a more rapid progression of joint deterioration.

Commentary

Hyaluronic acid (HA) injections are used to treat osteoarthritis of the hip but their efficacy has not been clearly established. The purpose of this meta-analysis was to determine the effect of HA injections on hip pain. There were twenty-three studies that met our criteria and the mean decrease in visual analog scores (VAS) was − 1.97 (95% CL, 2.83 to − 1.12, P < 0.0001). However, the clinical relevance of this change is difficult to determine since the decrease in VAS was only − 0.27 in the six randomized trials in the study and the duration of follow-up in most studies was less than six months. Multicenter randomized trials are needed to determine the true efficacy of HA injections in decreasing pain associated with hip osteoarthritis.
