The effect of narrow band imaging in CO2 laser therapy in early-stage glottic cancer

Objective:To explore efficacy of narrow band imaging（NBI） technique in CO2laser therapy in Early-Stage Glottic cancer. Methods:The clinical data of patients with Early-Stage Glottic cancer who underwent CO2laser vocal cord resection from June 2011 to August 2022 were retrospectively analyzed. Among these, 27 patients who underwent surgery assisted by NBI were assigned to the observation group, while 25 patients who underwent conventional CO2 laser microsurgery with a suspension laryngoscope were assigned to the control group. The differences between the two groups were analyzed in terms of intraoperative frozen pathology results, postoperative recurrence rates, 5-year cumulative disease-free survival rates, complications, and voice recovery. Results:All 52 patients were operated successfully. Temporary tracheostomy and serious complications did not occur during the operation. The postoperative patient's pronunciation was satisfactory. One patient experienced vocal cord adhesion, but there were no severe complications such as breathing difficulties or bleeding, with an overall complication rate of 1.92%. Postoperative follow-up was 1-5 years. The 5 years recurrence free survival in the general group was 77.90％, and the 5 years recurrence free survival in the NBI group was 100％, the difference was statistically significant（P<0.05）. NBI endoscopy is safer and more accurate than the general group in determining the safe margin of tumor mucosal resection（P<0.05）. Among the patients who accepted the voice analysis, the difference was no statistically significant（P>0.05）. Conclusion:Compared with conventional CO2laser surgery under microscope, NBI guided laser resection of Early-Stage Glottic cancer is more accurate. NBI guided laser resection could improve 5 years recurrence free survival rate. In a word, narrow-band imaging endoscopy can has very high value in clinical application.

Comparison of Simultaneous Swallowing Endoscopy and Videofluoroscopy in Neurogenic Dysphagia.

In the evaluation of oropharyngeal dysphagia, instrumental procedures, for example, flexible endoscopic evaluation of swallowing or videofluoroscopic swallowing study, are essential to improve diagnostic accuracy for salient findings such as penetration, aspiration, or pharyngeal residue. To date, it is unclear which of the 2 methods represents the diagnostic gold standard. The aim of this study, therefore, was to compare videofluoroscopy and swallowing endoscopy during a simultaneous swallowing examination in a large cohort of patients with oropharyngeal dysphagia. Prospective observational study. In this study, 49 patients with oropharyngeal dysphagia (mean age 70.0 ± 10.8years) were evaluated using simultaneous swallowing endoscopy and videofluoroscopy. Furthermore, the effect of narrow-band imaging in swallowing endoscopy on the assessment of penetration and aspiration was investigated in a subgroup of 19 patients. The Penetration-Aspiration Scale and the Yale Pharyngeal Residue Severity Rating Scale were rated independently based on both modalities. Both modalities showed a high correlation between penetration, aspiration, and pharyngeal residue. Causes for a higher score on the Penetration-Aspiration Scale in videofluoroscopy were intradeglutitive events that were not visible in swallowing endoscopy or false-positive events because of the loss of the lateral dimension in videofluoroscopy. A typical reason for a higher score on this scale in swallowing endoscopy was the better visualization of the anatomical structures. Narrow-band imaging in swallowing endoscopy resulted in a higher score on the Penetration-Aspiration Scale for liquids and semisolids in individual cases, although overall there was no statistically significant difference between scores using white light or narrow-band imaging. Videofluoroscopy and swallowing endoscopy may equally be considered as a diagnostic gold standard for oropharyngeal dysphagia regarding penetration, aspiration, and pharyngeal residue. Narrow-band imaging may increase the sensitivity for penetration and aspiration in individual cases.

Evaluation of narrow band imaging for diagnosis of unilateral nasal lesions.

This study aimed to investigate the effect of narrow band imaging (NBI) examination on differentiating diagnosis between benign and malignant neoplasms involving nasal cavity. A retrospective study was conducted to analyse cases from January 2018 to December 2019 at a single centre. A total of 188 consecutive patients who were newly diagnosed with lesions in unilateral nasal cavity underwent complete examination with white light endoscopy (WLE) and NBI endoscopy. Biopsy specimens were harvested from the target lesions and sent to the pathologist for definite diagnosis. Participants with a history of congenital malformation, trauma and surgery in nasal cavity were excluded from the study. Endoscopic diagnosis was assessed using sensitivity, specificity, accuracy, positive and negative predictive values (PPV and NPV, respectively). In identifying benign and malignant lesions of nasal cavity, NBI had a significant higher sensitivity (92.7% vs 70.7%, P=.020) and NPV (98% vs 92.3%, P=.032) than WLE, but there were no significant differences between NBI and WLE in specificity (98.6% vs 97.3%, P=.684), accuracy (97.3% vs 91.5%, P=.416) and PPV (95% vs 87.9%, P=.400). NBI as an emerging technique can improve the diagnostic accuracy by distinguishing benign and malignant lesions in nasal cavity and remains a promising and helpful adjunct to the endoscopy techniques.

Effect of narrow-band imaging (NBI) combined with magnifying endoscopy in early diagnosis of gastric cancer

Objective To explore the effect of narrow-band imaging (NBI) combined with magnifying endoscopy (ME) in early diagnosis of gastric cancer, and to observe whether it is helpful for inexperienced endoscopists to diagnose. Methods From April 2015 to October 2017, a total of 480 lesions were selected as research objects which were detected in the normal white light endoscopy (WLE). Pathological examination was used as the gold standard for diagnosis. 3 experienced doctors were selected to read the endoscopy films. 40 intern endoscopy doctors were selected to read the endoscopy filins at the same time after simple training. 80 WLE images and narrowband imaging technology combined with magnifying endoscopy (ME-NBI) images were randomly selected to hold a film reading meeting. Real-time voting was used to judge the nature of lesions. Results The accuracy, sensitivity, specificity false positive rate and false negative rate of ME-NBI were 97.9%(470/480), 96.8%(120/124), 98.3%(350/356), 1.7%(6/356), 3.2%(4/124). And WLE values were 91.7%(440/480), 80.6%(100/124), 89.9%(320/356), 10.1%(36/356), 19.4%(24/124), respectively. The diagnostic accuracy, sensitivity and specificity of ME-NBI were significantly higher than those of WLE (χ2=18.989, 16.104, 15.859, P<0.05), and the false positive rate and false negative rate were significantly lower than those of WLE (χ2=22.772, 16.104, P<0.05). The diagnostic results of ME-NBI of 40 endoscopy interns were significantly better than those of WLE (t=11.467, 16.188, P<0.05). The diagnostic results of NBI by 40 interns were significantly better than that of WLE (t=11.467, 16.188, P<0.05). Conclusions The accuracy of ME-NBI in screening early gastric cancer is satisfactory, and it has certain diagnostic value for inexperienced doctors. A study on the intrapapillary capillary loop detected by narrow band imaging system in early oral squamous cell carcinoma

ObjectiveThe purpose of this study is to determine the effectiveness of narrow band imaging (NBI) by analyzing three-dimensional (3D) images of intrapapillary capillary loop (IPCL) and recording changes over time. MethodsThis study was conducted between April 2007 and October 2011 at the Department of Oral and Maxillofacial Surgery of Tokyo Dental College Chiba Hospital. A total of 119 cases and 40 healthy volunteers were observed with NBI. Within a total of 119 cases oral squamous cell carcinoma (OSCC) was diagnosed in 48 cases (40%), leukoplakia in 26 cases (22%), oral lichen planus in 18 cases (15%), gingivitis in 5 cases (4%), aphtha in 4 cases (3.5%), ulcer in 4 cases (3.5%), erythroplakia in 3 cases (2.5%), pigmentation in 3 cases (2.5%), and other diseases in 8 cases (7%). The 40 healthy volunteers were medically examined at the regions of the gingiva, lower lip, buccal mucosa, oral floor, lateral border of the tongue, and dorsal surface of the tongue. ResultsMorphological changes of IPCL were analyzed through the construction of 3D images. The dimension, volume and density of vascular vessel were compared between OSCC, dysplasia and normal mucosa. Abnormal vascular vessels were apparent in cases of dysplasia and OSCC when examined through magnifying endoscopy with NBI. Morphological changes of IPCL such as dilation, aggregation and thickness increasing observed by NBI were certified through 3D imaging. ConclusionsThese results suggest that NBI is an effective method for the early detection of OSCC.

Effectiveness of narrow band imaging in the detection of premalignant and malignant lesions of the larynx: Validation of a new endoscopic clinical classification

The purpose of this study was to assess the value of narrow band imaging (NBI) endoscopy in the diagnosis of pharyngolaryngeal lesions and to demonstrate the validity of a new NBI-based classification of their vascular pattern. From 2009 to 2011, 248 patients with pharyngolaryngeal lesions underwent NBI-endoscopic evaluation. NBI findings were classified into 5 types according to the Ni classification and were compared with histopathological reports. Sensitivity, specificity, accuracy, and positive predictive value (PPV) and negative predictive value (NPV) were calculated. Sensitivity, specificity, accuracy, PPV, and NPV were 97.4%, 84.6%, 92.7%, 91.6%, and 95.1%, respectively. Ninety-eight percent of histologically malignant lesions corresponded to type V endoscopic pattern, whereas 84.8% of the non-neoplastic lesions corresponded to a type I to IV pattern. The NBI ability to detect changes in the mucosal microvasculature can be useful for distinguishing nonmalignant from malignant lesions. An internationally accepted NBI-endoscopic classification may contribute to the further definition of the diagnostic validity of this technique.

Image-enhanced, chromo, and cap-assisted colonoscopy for improving adenoma/neoplasia detection rate: a systematic review and meta-analysis

Objective. The effectiveness of narrow band imaging (NBI), chromoendoscopy (CE), and cap-assisted colonoscopy (CAC) on adenoma detection rate (ADR) has been investigated in previous meta-analyses; however, there have been no meta-analyses of autofluorescence imaging (AFI) or flexible spectral imaging color enhancement (FICE) or i-scan. The aim of this study was to determine whether AFI and FICE/i-scan was more effective than standard/high-definition white light endoscopy to improve ADR and to update previous meta-analyses of NBI, CE, and CAC. Design. A systematic review and meta-analysis was conducted. Four investigators selected appropriate randomized controlled trials (RCT) using the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) guideline. All RCTs in which colonoscopy were performed with AFI, FICE, i-scan, NBI, CE, and CAC were included. The risk ratios (RRs) calculated from adenoma/neoplasia detection rate were used as the main outcome measurement. Results. A total of 42 studies were included in the analysis. Pooled estimates of RR (95%confidence interval [CI]) using AFI, FICE/i-scan, NBI, CE, and CAC were 1.04 (95% CI: 0.87–1.24) (I2 = 0%) (fixed effects model [FEM]); 1.09 (95% CI: 0.97–1.23) (I2 = 5%) (FEM); 1.03 (95% CI: 0.96–1.11) (I2 = 0%) (FEM); 1.36 (95% CI: 1.23–1.51) (I2 = 16%) (FEM); and 1.03 (95% CI: 0.93–1.14) (I2 = 48%) (random effects model [REM]), respectively. The pooled estimate of RR (95%CI) using indigo carmine in non-ulcerative colitis (UC) patients and methylene blue in UC patients was 1.33 (95% CI: 1.20–1.48) (I2 = 14%) (FEM) and 2.39 (95% CI: 1.18–4.84) (I2 = 0%) (FEM), respectively. Conclusion. In contrast to AFI, FICE/i-scan, NBI, and CAC, only CE improves ADR. CE with methylene blue, though not NBI, is effective for surveillance of neoplasia in chronic UC patients.

Narrow band imaging diagnosis of bladder cancer: systematic review and meta‐analysis

What's known on the subject? and What does the study add? In recent years, more attention has focused on the role of narrow band imaging (NBI) in bladder cancer detection and NBI technology has spread rapidly. It is an important method for diagnosing new or recurrent bladder cancer. But its diagnostic accuracy is still uncertain. This paper summarizes the diagnostic accuracy of NBI in bladder cancer and compares NBI with white light imaging. The results show that NBI cystoscopy significantly improves the detection accuracy in bladder cancer, compared with white light imaging. However, some limitations still exist. Multicentre randomized studies are recommended to determine whether the visual advantages of NBI can translate into real therapeutic benefit for individual patients. • To assess the test performance and clinical effectiveness of narrow band imaging (NBI) cystoscopy compared with white light imaging (WLI) cystoscopy in people suspected of new or recurrent bladder cancer. • Literature on NBI cystoscopy in the diagnosis of bladder cancer was searched in PubMed, EMBASE, Cochrane Library, MEDLINE and CNKI, with hand searching of relevant congress abstracts and journals. • The literature was selected according to inclusion and exclusion criteria. The Meta-DiSc1.4 software was used to review management and analysis. • Eight studies including 1022 patients assessed test performance. • On a per-person analysis, the pooled sensitivity, specificity, positive likelihood ratio, negative likelihood ratio and diagnostic odds ratio of NBI and WLI were respectively 0.943 (95% CI 0.914-0.964) and 0.848 (95% CI 0.803-0.885), 0.847 (95% CI 0.812-0.878) and 0.870 (95% CI 0.831-0.903), 7.038 (95% CI 3.357-14.754) and 6.938 (95% CI 2.052-23.465), 0.054 (95% CI 0.012-0.237) and 0.181 (95% CI 0.091-0.361), and 185.32 (95% CI 45.714-751.26) and 42.931 (95% CI 8.088-227.88). • The area under the curve and Q* of NBI and WLI were respectively 0.9781 and 0.8944, and 0.9337 and 0.8253. • For the characterization of carcinoma in situ, the pooled sensitivity, specificity, positive likelihood ratio, negative likelihood ratio and diagnostic odds ratio of NBI were 0.927 (95% CI 0.878-0.960), 0.768 (95% CI 0.730-0.802), 4.545 (95% CI 2.820-7.325), 0.125 (95% CI 0.051-0.304) and 48.884 (95% CI 15.642-152.77) on a per-person analysis. • The area under the curve and Q* were 0.9391 and 0.8763. • NBI is an effective method for the identification of abnormal lesions including carcinoma in situ and can provide higher diagnostic precision of bladder cancer than WLI.

Effectiveness of narrow band imaging in patients with oral squamous cell carcinoma after treatment

We evaluated the effectiveness of narrow band imaging (NBI) in patients with oral squamous cell carcinoma (OSCC) after treatment. In all, 101 consecutive OSCC patients underwent NBI examination for posttreatment follow-up. Four patients had local recurrence. Twenty-six second primary malignancies were found in 18 patients; 6 patients (33%) had more than 1 lesion. Seventeen lesions (65%) were carcinoma in situ or severe dysplasia. Most of them occurred in the oral cavity (77%). A higher incidence (18% vs 9%, p = .037) and less-advanced stage (4% vs 37%, p = .0005) of second primary malignancies were found among the NBI group compared with a previous cohort without NBI examination, and fewer patients needed postoperative adjuvant therapy (12% vs 50%, p = .0005). NBI is an effective method to identify early lesions in the head and neck area, especially the oral cavity, among patients with OSCC after treatment.

S195 – Laryngeal Observation of Early Cancer with NBI System

Objectives The Narrow Band Imaging (NBI) is an illumination method for medical endoscopes that can visualize the micro-vascular structure of the organ surface. The effectiveness of NBI has been reported in detecting the oropharyngeal and hypopharyngeal neoplasm lesion. The aim of this study is to identify the usefulness of NBI in laryngeal lesion, especially in observing cases of post-operated or post-irradiated early laryngeal cancers. Methods 16 patients of early laryngeal cancer after treatment for at least 6 months were entered in this study: 1 case of T1s, 8 cases of T1a, 3 cases of T1b, and 4 cases of T2. 3 cases of T1a underwent cordectomy under direct laryngoscope using KTP laser. 13 cases were treated with irradiation. Results The cases for which NBI was suggested to be useful were: T1b, undergoing laser surgery, supra-glottic carcinoma, appearing hyper-adduction of the false vocal fold during phonating, and causing severe mucosal edema after irradiation. Conclusions NBI system may play an important role in the observation of post-therapy of early laryngeal cancer.

Endoscopic Observation of Tissue by Narrowband Illumination

We propose a new illumination method for a medical endoscope: narrow band imaging (NBI), in which the spectral bandwidth of the filtered light is narrowed. To confirm how the spectral specifications of the filtered light influence a reproduced image, an experiment was conducted observing the endoscopic images of the back mucosa of a human tongue. In addition, the effect of NBI on endoscopic images was investigated through preliminary clinical tests in colonoscopy and upper gastrointestinal endoscopy. It has been shown that NBI can enhance the capillary pattern and the crypt pattern on the mucosa. These patterns are useful features for diagnosing an early cancer.
