Early initiation of alcohol use during adolescence, and adolescent binge drinking are risk factors for the development of alcohol use disorder later in life. Adolescence is a time of rapid sex-dependent neural, physiological, and behavioral changes as well as a period of heightened vulnerability to many effects of alcohol. The goal of the present studies was to determine age-related changes in blood (leukocyte populations) and body composition across adolescence and early adulthood, and to investigate whether adolescent intermittent ethanol (AIE) exposure would alter the trajectory of adolescent development on these broad physiological parameters. We observed significant ontogenetic changes in leukocyte populations that were mirrored by an age-related increase in cytokine expression among mixed populations of circulating leukocytes. Despite these developmental changes, AIE did not significantly alter overall leukocyte numbers or cytokine gene expression. However, AIE led to sex-specific changes in body fat mass and fat percentage, with AIE-exposed male rats showing significantly decreased fat levels and female rats showing significantly increased fat levels relative to controls. These changes suggest that while AIE may not alter overall leukocyte levels, more complex phenotypic changes in leukocyte populations could underlie previously reported differences in cytokine expression. Coupled with long-term shifts in adipocyte levels, this could have long-lasting effects on innate immunity and the capacity of individuals to respond to later immunological and physiological threats.

The effect of a locally tailored intervention on the uptake of preconception care in the Netherlands: a stepped-wedge cluster randomized trial (APROPOS-II study)

BackgroundThe preconception period provides a window of opportunity for interventions aiming to reduce unhealthy lifestyle behaviours and their negative effect on pregnancy outcomes. This study aimed to assess the effectiveness of a locally tailored preconception care (PCC) intervention in a hybrid-II effectiveness implementation design.MethodsA stepped-wedge cluster randomized controlled trial was performed in four Dutch municipalities. The intervention contained a social marketing strategy aiming to improve the uptake (prospective parents) and the provision (healthcare providers) of PCC. Prospective parents participated by administering a questionnaire in early pregnancy recalling their preconceptional behaviours. Experiences of healthcare providers were also evaluated through questionnaires. The composite primary outcome was adherence to at least three out of four preconceptional lifestyle recommendations (early initiation of folic acid supplements, healthy nutrition, no smoking or alcohol use). Secondary outcomes were preconceptional lifestyle behaviour change, (online) reach of the intervention and improved knowledge among healthcare providers.ResultsA total of 850 women and 154 men participated in the control phase and 213 women and 39 men in the intervention phase. The composite primary outcome significantly improved among women participating in the municipality where the reach of the intervention was highest (Relative Risk (RR) 1.57 (95% Confidence Interval (CI) 1.11–2.22). Among women, vegetable intake had significantly improved in the intervention phase (RR 1.82 (95%CI 1.14–2.91)). The aimed online reach- and engagement rate of the intervention was achieved most of the time. Also, after the intervention, more healthcare providers were aware of PCC-risk factors (54.5% vs. 47.7%; p = 0.040) and more healthcare providers considered it easier to start a conversation about PCC (75.0% vs. 47.9%; p = 0.030).ConclusionThe intervention showed some tentative positive effects on lifestyle behaviours among prospective parents. Primarily on vegetable intake and the knowledge and competence of healthcare providers. The results of this study contribute to the evidence regarding successfully implementing PCC-interventions to optimize the health of prospective parents and future generations.Trial registrationDutch Trial Register: NL7784 (Registered 06/06/2019).

Prevalence of Alcohol Intake and Illegal Drugs among the Students at English Medium Private Schools of Tripura, India (North Eastern States of India).

Early initiation of alcohol use among adolescents can provide a useful indication of the potential future burden among adults including increased risk for academic failure, mental health problems, antisocial behavior, physical illness, risky sexual behavior, sexually transmitted diseases, early onset dementia, and the development of alcohol use disorders. The objective of this study is to measure the prevalence of alcohol consumption and illegal drug use among adolescent students studying in class ninth to eleventh standard in English medium private schools in Tripura, India. A cross sectional descriptive study was carried out in faith based English medium private schools of two selected subdivisions of Tripura, India. Students of class ninth to eleventh standard were selected for this current study. A validated questionnaire was used to find out the prevalence of alcohol and illegal drug intake. A total of 565 (boys = 308, girls = 257) students participated in this study from four schools. The prevalence of alcohol intake (ever) among adolescent students was 28.2% [95% confidence interval (CI) 24.6-32.1]. The adolescent students of 1.7% (95% CI 0.9-3.1) consumed alcohol once a week in the past 6 months. It was observed that 4.2% (95% CI 2.8-6.3) of adolescents use illegal drugs. The study participants of 12.7% (95% CI 10.2-15.8) reported having close friends who were using illegal drugs. Significant percentages of adolescent students were involved in alcohol intake (ever) and use of illegal drugs.

Pattern of Alcohol Use and its Effects in Relation to the Age of Initiation of Alcohol Use among the Residents of Jhaukhel Area of Bhaktapur District

Over the past few decades, alcohol consumption has increased in quantity and frequency. Several studies have suggested that people who start drinking at younger age have an increased risk of alcohol use disorder. This study was carried out to assess the pattern of alcohol consumption in relation to the age of initiation of alcohol use among the residents of Jhaukhel area of Bhaktapur district. A cross sectional descriptive survey was conducted in Jhaukhel area of Bhaktapur district among the permanent residents aged 15 years and above through a purposive sampling technique using a pre tested self-constructed proforma. Assessment of alcohol use, harmful/ hazardous drinking and alcohol dependence was done using an AUDIT questionnaire as a tool. Chi-square test and Fisher’s exact test were applied and ‘p’ value was calculated to see the association between age of onset of alcohol use and the alcohol use pattern and effects. The level of significance was set at 5%, and ‘p’ value of < 0.05 was considered statistically significant. A total of 190 respondents were interviewed. Mean age of the respondents was 41.25 ± 16.4 years while mean age of initiation of alcohol consumption was 20 ± 4.3 years and 31 (30.4 %) of drinkers initiated alcohol consumption before legal drinking age (18 years). This was more among males (36.5%) than females (14.3%), (p-value: 0.03, OR: 3.4, 95%CI: 1-11) Initiating alcohol consumption early in life before 18 years was strongly associated with binge drinking pattern (P value: <0.001, OR: 9.5, 95%CI: 2.7-33.3). Those who initiate drinking before 18 years were more likely to suffer from alcohol related medical problems compared to those who initiated drinking after 18 years (OR: 6.63, 95% CI: 2.3-19.3, P value: 0.001). Parental consumption of alcohol was found to be associated with the early initiation of alcohol use (OR: 2.7, 95%CI: 1.01-6.96, P value: 0.03). Early initiation of alcohol consumption before legal drinking age was not only a predictor for binge drinking and alcohol dependence, but also a risk factor for alcohol related medical problem, accidents and self-inflicted injuries. Hence this is a serious issue which needs to be addressed at the individual, family as well as community level.

Alcohol use at the intersection of sexual orientation and gender identity in a representative sample of youth in Colorado.

Lesbian, gay, bisexual, and questioning (LGBQ) youth are at greater risk of problematic alcohol use than their heterosexual peers, yet there is a dearth of research examining variability in alcohol use among youth that simultaneously accounts for sexual orientation and gender identity. This paper examines the relationship between alcohol use and intersecting identities of sexual orientation and gender while accounting for this population's disproportionate experiences of depression and dating and sexual violence. The study used a representative sample (n = 27,621) of high school students. Logistic regressions were used to determine if earlier age at first drink, 30-day alcohol use, and binge drinking were significantly related to intersectional sexual orientation and gender identity. Secondary models added depression, dating violence, and sexual violence to analyses to determine if they explained any of the variancein alcohol use variables in LGBQ and transgender youth. Transgender youth who identified as heterosexual or questioningtheir sexual orientation were at greatest risk for early initiation of alcohol use and binge drinking. These relationships between intersectional identity and alcohol use became nonsignificant when depression, dating violence, and sexual violence were added to the models. This is the first study to examine relationships between intersecting identities of sexual orientation and gender identity and alcohol use among youth. Risky alcohol use among transgender youth may be attributed to experiences of depression, dating violence, and sexual violence impacting these populations. Interventions should focus on reducing transgender youth exposure to violence.

Relationship Between the Early Initiation of Substance Use and Attempted Suicide Among in-School Adolescents in Seven Low- or Middle-Income African Countries: An Analysis of the Global School-Based Student Health Survey Data.

Background: Preventing suicide among adolescents is an urgent global public-health challenge, especially in Africa. Accordingly, the aim of this study was to examine the relationship between the early initiation (< 12 years old) of substance use (cigarette smoking, alcohol use, and drug use) and attempted suicide among in-school adolescents in seven African countries.Methods: Data on the early initiation of substance use and on attempted suicide among in-school adolescents over the previous 12 months in Benin, Liberia, Mauritius, Mozambique, Namibia, Seychelles, and the United Republic of Tanzania were collected from Global School-based Student Health Surveys and were pooled to determine the overall prevalence of these behaviors in adolescents. Univariate and multivariate logistic regressions were then performed to evaluate country-specific associations between the early initiation of substance use and attempted suicide in these adolescents, followed by meta-analyses to evaluate overall pooled associations.Results: In the abovementioned seven African low- or middle-income countries (LMICs), overall weighted prevalences of attempted suicide and early initiation of cigarette smoking, alcohol use, and drug use among in-school adolescents were 16.05, 7.76, 17.68, and 3.48%, respectively. Multivariate logistic regression analyses revealed that relative to non-smoking, the early initiation of smoking was significantly associated with attempted suicide in these adolescents [OR (95% CI) = 1.783 (1.219–2.348)]. Additionally, the relationship between early initiation of cigarette smoking and attempted suicide is mostly driven by a higher association in girls [OR (95% CI) = 1.867 (1.031–2.703)] than boys [OR (95% CI) = 1.392 (0.995−1.789)]. Moreover, relative to not using other drugs, the early and later initiation of other drug use were also significantly associated with attempted suicide in these adolescents [ORs (95% CIs) = 2.455 (1.701–3.208) and 1.548 (1.198–1.898)].Conclusion: Programs that can eliminate or decrease the early initiation of substance use among adolescents should be implemented in African LMICs to prevent subsequent suicide attempts, especially among adolescent girls.

Ventral striatal resting-state functional connectivity in adolescents is associated with earlier onset of binge drinking

BackgroundEarlier engagement in heavy drinking during adolescence is a risk factor for the development of alcohol use disorders later in life. Longitudinal studies in adolescents have linked brain structure and task-evoked function to future alcohol use; however, less is known about how intrinsic network-level interactions relate to future substance use during this developmental period. MethodsIn this prospective longitudinal study, resting-state functional connectivity of the ventral striatum, risky decision making, and sensation seeking were measured in 73 adolescents at baseline. Participants were between the ages of 14 and 15 and had no substantial history of substance use upon study entry. Follow-up interviews were conducted approximately every 3 months to assess the initiation of binge drinking (≥ 5 or ≥ 4 drinks per occasion for males or females, respectively). ResultsAdolescents who began binge drinking sooner exhibited greater connectivity of the ventral striatum to the left precuneus, left angular gyrus, and the left superior frontal gyrus. Greater connectivity of the ventral striatum to the right insula/putamen was associated with longer duration to the onset of binge drinking. Resting-state functional connectivity in these regions was not associated with baseline assessments of risky decision making or sensation seeking. ConclusionsFindings provide novel information about potential risk factors for early initiation of heavy alcohol use. Interventions that target relevant resting-state networks may enhance prevention efforts to decrease adolescent substance use by prolonging onset to heavier levels of alcohol consumption.

Age at first alcohol use and weapon carrying among adolescents: Findings from the 2019 Youth Risk Behavior Survey

BackgroundAlthough studies have investigated the association between alcohol use and violent behaviors such as weapon carrying, few studies have examined the association between age at first alcohol use and weapon-carrying among adolescents. The objective of this study was to investigate the association between age at first alcohol use and weapon carrying among adolescents. MethodsData for this study came from the 2019 Youth Risk Behavior Survey. An analytic sample of 13,442 adolescents aged 14–18 years old (51% female) was analyzed using binary logistic regression. The outcome variable investigated in this study was weapon carrying during the past 30 days, and the main explanatory variable investigated was age at first alcohol use. ResultsOf the 13,442 adolescents, 13.5% carried a weapon during the past 30 days, and 15.4% reported having their first alcoholic drink before age 13. In the multivariable logistic regression, adolescents who reported having alcohol before age 13 had more than double the odds of carrying a weapon when compared to those who never had alcohol before age 13 (AOR = 2.32, p < .001, 95% CI = 1.87-2.89). Other significant factors associated with weapon carrying include male gender, victim of bullying, teen dating violence, sexual violence, suicidal ideation, and history of substance use. Adolescents who self-identified as Black/African American or Hispanic were significantly less likely to carry a weapon when compared to adolescents who self-identified as non-Hispanic White. ConclusionThe findings of this study underscore the importance of developing age appropriate intervention strategies to curb early initiation of alcohol use and weapon carrying among adolescents.

Adverse childhood experiences and their Alcohol, and chat Consumption among school-going adolescents, Ethiopia: Cross-sectional study

Background: Alcohol and chat use during adolescence is associated with academic and health problems, including abuse or dependence in adulthood. The aim of the present study was to investigate associations between adverse childhood experiences (ACEs) and early initiation of alcohol and chat use among school-going adolescents. Methods: A cross-sectional study was performed with 546 school-going adolescents. The ACE International Questionnaire (ACE-IQ) was used to assess ACEs, Alcohol and chat use was assessed by questions prepared by the authors. Multiple logistic regression models were used to examine the associations between overall ACE score and alcohol use and the potential moderating effects of confounders. Results: prevalence of chat chewing behavior was 26.6% followed by alcohol use 20%. A total of 66.2% of participants reported at least one ACE, and 5.93% reported four or more ACEs. High/Multiple ACEs (ACEs=1-3 and above 3) were significantly associated with increased alcohol use behaviors (AOR=1.491(1.072-3.078) and (AOR=3.171(1.330-7.560) respectively and increased chat use behaviors by 4.92 times (AOR=4.92, 95%CI=2.640-8.432) and 11.022 times (AOR=11.022, 95%CI=1.230-25.560) respectively controlling other factors. Conclusion: ACEs were significantly associated with risk behaviors, alcohol and chat use may lead to poor health, and educational outcomes among adolescent students and numbers of ACEs have graded association. Social support, sex, residence, parent educational status, and current level of depressive symptoms were significant modulating factors, which parents, school teachers, psychologists, and adolescent health care providers should give the concern to decrease the effect on school-going adolescents.

The relationship between early risk-taking behavior and mental health problems among a nationally representative sample of Australian youth

BackgroundEarlier engagement in risk-taking behaviors has been associated with more severe mental health problems across development. However, sex differences in risk-outcome relationships remain underexplored and mental health outcomes spanning both ends of the internalizing and externalizing spectra are rarely considered within the same sample. The present study examined associations between age at initiation of alcohol use, illicit drug use, and sexual intercourse, and symptoms of internalizing, externalizing, depression and self-harm, for males and females. MethodsThe present study analyzed self-report survey data from a nationally representative sample of Australian adolescents (N=2,950). Logistic regression models were conducted separately for males and females to test the association of early (age 15 or younger) and concurrent (age 16–17) initiation of alcohol use, illicit drug use and sexual behavior with symptoms of internalizing, externalizing, depression and self-harm at age 16–17. ResultsRisk-taking behaviors were associated with all mental health outcomes. Generally, earlier initiation of risk-taking behaviors demonstrated stronger associations with mental health outcomes than concurrent initiation. Associations between risk-taking behaviors and mental health outcomes varied by participant sex and mental health outcome. LimitationsMechanisms underlying the relationship between risk-taking behaviors and mental health problems were not tested and the sample had not yet reached early adulthood. ConclusionsRisk-taking behaviors, particularly when initiated early, share links with multiple mental health problems in adolescence. Public health strategies to delay the onset of risk-taking behaviors may therefore incur multiple benefits as might an integrated approach to mental health prevention programs for youth.

Prevalence and risk factors of alcohol consumption behavior among late adolescents: Evidence from Puducherry, India

Introduction: Alcohol is a psychoactive substance with dependence-producing properties. Adolescents account for a quarter of current drinkers in the world. Early initiation of alcohol use is one of the most important predictors of future physical and social health. Materials and Methods: A cross-sectional descriptive study among a sample of 210 adolescent males (aged 18–19 years), was conducted to identify the prevalence of alcohol consumption and the factors associated with it. The data were statistically analyzed using frequencies, percentages, Chi-square tests of independence and binary logistic regression. Results: About 62.3% (n = 131) of the respondents have consumed alcohol and 54.3% (n = 114) were current drinkers. Alcohol consumption was significantly associated with monthly pocket money (N = 210, χ2 = 17.834, df = 5), parents-consuming alcohol (N = 210, χ2 = 6.926, df = 1), friends-consuming alcohol (N = 210, χ2 = 24.549, df = 1), smoking status (N = 210, χ2 = 54.189, df = 1), and awareness about health effects of alcohol (N = 210, χ2 = 193.482, df = 2). Social influences of “friends-consuming alcohol” and “parents-consuming alcohol” were strongly associated with alcohol consumption among adolescents with odds ratio (OR) of 4.048 (95% confidence interval [CI] = 1.485–11.032) and 2.172 (95% CI = 1.076–4.386), respectively. In addition, it was also observed that smoking status and high monthly pocket money of INR 8000–12000 was strongly associated with alcohol consumption status with OR of 15.161 (95% CI = 5.076–45.286) and 25.660 (95% CI = 1.323–498.489), respectively. Conclusion: The study highlights the possible role of the individual, economic and social dimensions in the initiation of alcohol consumption among adolescents. The alcohol prevention interventions should be delivered through a multi-pronged approach focusing on these dimensions to be more effective.

Effects of chronic intermittent ethanol exposure during early and late adolescence on anxiety-like behaviors and behavioral flexibility in adulthood

Although both humans and laboratory rodents demonstrate cognitive and affective alterations associated with adolescent alcohol exposure, it is still unknown whether the consequences of early initiation of alcohol use differ from those of later binge drinking within the adolescent developmental period. The present study was designed to assess the effects of early and late AIE on (1) anxiety-like behavior under social (modified social interaction test) and non-social test circumstances (modified light/dark box test, elevated plus maze), and (2) behavioral flexibility, indexed via set shifting in males and females. Early-mid adolescent intermittent exposure (early AIE) occurred between postnatal days (P) 25 and 45, whereas late adolescent intermittent exposure (late AIE) was conducted between P45 and P65, with behavioral testing initiated not earlier than 25 days after repeated exposure to ethanol (4.0 g/kg intragastrically, every other day for a total of 11 exposures). Anxiety-like behavior on the EPM was evident in males and females following early AIE, whereas only males demonstrated non-social anxiety on the EPM following late AIE. Social anxiety-like alterations and deficits in behavioral flexibility were evident only in males following early AIE. Taken together, the results of the present study demonstrate a particular vulnerability of young adolescent males to long-lasting detrimental effects of repeated ethanol and an insensitivity of older adolescent females to the intermittent ethanol exposure paradigm.

PATTERN OF SUBSTANCE USE IN A DRUG DEADDICTION CENTRE IN DIPHU- NORTHEAST INDIA-BASED STUDY.

India is a multicultural and multilingual democracy; the use of different substances is found to be regionally diversified. Ethnic variations have contributed to this diversification and so a single study on a particular region cannot be generalised to the whole Indian population. Alcohol is one of the most popular substances used in India which can be traced backed to the scriptures. The local homemade liquor is the most common form of liquor used by the Indian population. The traditional winemaking process is common among the different ethnic tribes of Northeast India and thus most of the alcohol use is unrecorded in this part of the country. An observational study was conducted in the Drug Deaddiction Centre of Diphu Civil Hospital, Assam where patients' recorded data over a four-year period was used. The data was reviewed retrospectively to understand the pattern of substance use in this region and the results were described using descriptive statistics. Alcohol was found to be the most used substance and significant gender difference was noted among the users. Both tribal and nontribal groups were almost equally seen to use alcohol while around one-fifth of the population belonged to the age group of less than 30 years. Early initiation of alcohol use due to cultural reasons was suspected. Various health issues ranging from gastrointestinal (GI) symptoms to death due to alcohol misuse were reported. Due to the retrospective review process, missing data was found to be a significant limitation to this study. Adequate health promotion focusing on effects of alcohol on health should be undertaken in this region and proper follow-up of the treated patients should be done to prevent relapse.

Internet-Based Universal Prevention for Students and Parents to Prevent Alcohol and Cannabis Use Among Adolescents: Protocol for the Randomized Controlled Trial of Climate Schools Plus

BackgroundEarly initiation of alcohol and cannabis use markedly increases the risk of harms associated with use, including the development of substance use and mental health disorders. To interrupt this trajectory, effective prevention during the adolescent period is critical. Despite evidence showing that parents can play a critical role in delaying substance use initiation, the majority of prevention programs focus on adolescents only. Accordingly, the Climate Schools Plus (CSP) program was developed to address this gap.ObjectiveThis paper outlines the protocol for a cluster randomized controlled trial (RCT) of the CSP program, a novel internet-based program for parents and students to prevent adolescent substance use and related harms. The CSP program builds on the success of the Climate Schools student programs, with the addition of a newly developed parenting component, which allows parents to access the internet-based content to equip them with knowledge and skills to help prevent substance use in their adolescents.MethodsA cluster RCT is being conducted with year 8 students (aged 12-14 years) and their parents from 12 Australian secondary schools between 2018 and 2020. Using blocked randomization, schools are assigned to one of the two groups to receive either the CSP program (intervention) or health education as usual (control). The primary outcomes of the trial will be any student alcohol use (≥1 standard alcoholic drink/s) and any student drinking to excess (≥5 standard alcoholic drinks). Secondary outcomes will include alcohol- and cannabis-related knowledge, alcohol use-related harms, frequency of alcohol consumption, frequency of drinking to excess, student cannabis use, parents’ self-efficacy to stop their children using alcohol, parental supply of alcohol, and parent-adolescent communication. All students and their parents will complete assessments on three occasions—baseline and 12 and 24 months postbaseline. In addition, students and parents in the intervention group will be asked to complete program evaluations on two occasions—immediately following the year 8 program and immediately following the year 9 program.ResultsAnalyses will be conducted using multilevel, mixed-effects models within an intention-to-treat framework. Associations of Father's Lifetime Cannabis Use Disorder with Child's Initiation of Cannabis Use, Alcohol Use, and Sexual Intercourse by Child Gender

ABSTRACTBackground: Early initiation of alcohol use, cannabis use, and sexual intercourse among adolescents is an important public health concern in the United States. Parents' history of substance use disorder is an important contributing factor for children's problem behaviors. Objectives: The associations of fathers' lifetime cannabis use disorder with children's initiation of cannabis use, alcohol use, and sexual intercourse were examined. In addition, child's gender was considered as a moderator of each association. Methods: Data from two companion longitudinal studies was utilized, the Rochester Youth Developmental Study and its intergenerational extension, the Rochester Intergenerational Study. Multiple linear regression analyses were conducted to examine the associations of the father's lifetime cannabis use disorder with the child's initiation of cannabis use, alcohol use, and sexual intercourse. To test gender differences in the associations, the study sample was stratified by child's gender. Results: The average age of first cannabis use (b = –3.71, p < .05), alcohol use (b = –3.65, p < .05), and sexual intercourse (b = –2.94, p < .05) among daughters of fathers with a lifetime cannabis use disorder was lower than that of their counterparts after adjusting for all other control variables, whereas no significant differences were detected in a father–son relationship. Conclusions: Homotypic continuity of cannabis use, as well as heterotypic continuity from the father's cannabis use to the child's alcohol use and sexual intercourse existed in a father–daughter relationship. These findings suggest that family-based interventions for female adolescents whose father has suffered from a cannabis use disorder be developed to prevent better adolescents' early substance use and sexual intercourse.

Adverse Childhood Experiences and Early Initiation of Marijuana and Alcohol Use: The Potential Moderating Effects of Internal Assets

ABSTRACTIntroduction: Early adolescence is a critical risk period for initiation of substance use. Internal assets (IAs), which are individual qualities guiding positive choices, and adverse childhood experiences (ACEs) are important protective and risk factors, respectively, against substance use. The purpose of this study is to investigate whether IAs modify associations between ACEs and early initiation of alcohol and marijuana use. Method: Data were from 9th and 11th graders who completed the 2013 Minnesota Student Survey (n = 79,339). Students reported on experiences of abuse, household dysfunction, and substance use. Multivariable logistic regressions examined associations between different types of ACEs and substance use. Interactions between IAs and ACEs were added to models to test effect modification. For significant interactions, main effects models were re-estimated at different percentiles of IAs. Result: IAs moderated associations of both abuse and household dysfunction with early initiation of marijuana (p <.003) and alcohol (p =.007) for females but not for males. For females with low IAs, odds of early initiation of marijuana were approximately twice as high as students without any ACEs. A similar pattern was detected for females' initiation of alcohol use. No effect modification was detected for IAs and experiencing only abuse or household dysfunction on initiation. Conclusion: Special attention should be paid to improving IAs among girls who have already experienced ACEs. Future research should examine protective factors that buffer the effects of ACEs for boys.

Expectations towards alcohol consumption in school children between 8 and 12 years of age: a descriptive study.

The World Health Organization (WHO) indicates that the consumption of alcoholic beverages at social gatherings is common in many parts of the world and can have negative health and social consequences. Early initiation of alcohol use is a major risk factor for the development of patterns of abusive consumption of alcohol. To analyze the expectations towards alcohol consumption and to identify the age of onset of alcohol use in school children between 8 and 12 years of age. The study was exploratory, descriptive and cross-sectional. Data were collected from February to March, 2016, at the elementary schools Prof. Claudio Cortés Castro and Manuel Prieto Diaz in the city of Villahermosa, Tabasco. The questionnaire of expectations towards alcohol for children was applied. Sample size was 203 children. Twenty eight percent of children had consumed alcohol once in life; mean age of consumption onset was between 8 and 9 years old. Most frequently, the drink was beer, mostly provided by the father in places of family life. Among alcohol consumption expectations, dimensions scoring higher were aggressiveness and sociability and the dimension scoring lower was that of negative states. With respect to expectations towards alcohol consumption, children think that consuming alcohol promotes aggressive behavior followed by swear words, disturbing other people and discussing with them. In contrast, there are situations produced by alcohol consumption that children fail to appreciate, such as sadness and shame.

Alcohol and other substance use among a sample of young people in the Solomon Islands

Objective:Investigate alcohol and other substance use, with a focus on harmful alcohol use patterns, among young people in the Solomon Islands. Methods:A structured, interviewer‐administered questionnaire was administered to respondents aged 15–24 years across four of the country's provinces in late 2015. Results:Four hundred young people completed the questionnaire across urban, peri‐urban and rural communities. The most common substances ever used by participants were betel nut (94%), licit/store‐bought and/or illicit alcohol (79%) and tobacco (76%). Lifetime and recent substance use was particularly common among male respondents; e.g. 89% of male participants reported ever using any alcohol versus 54% of females (p<0.001). Harmful alcohol use patterns were common. Conclusions:Our sample generally reported higher levels of substance use compared to previous research in the Solomon Islands, including in relation to the country's relatively recent (2012/13) Household Income and Expenditure Survey. Implications for public health:Our study made considerable advances in addressing key knowledge gaps regarding alcohol and other substance use among young people in the Solomon Islands. Evidence‐based initiatives to address early initiation of alcohol and other substance use and the progression to more problematic use patterns among young people in the Solomon Islands need to be explored.

Cumulative contextual risk at birth and adolescent substance initiation: Peer mediation tests

BackgroundChildren who experience multiple adversities, such as prenatal exposure to drugs and poverty, early in development are at increased risk for the early initiation of alcohol and cigarette use. However, studies that examine potentially malleable processes associated with substance use initiation in the context of exposure to cumulative stressors are scant. This study examined associations between cumulative contextual risk at birth and initiation of alcohol and cigarette use in adolescence, testing childhood peer marginalization and peer aggression and behavior problems as mediating mechanisms. Analyses further adjusted for fearfulness/inhibition and hyperactivity/distractibility to determine if the hypothesized mediating mechanisms were significant after accounting for temperamental characteristics associated with substance initiation. MethodsParticipants were 6190 adolescents from the Northern Finland Birth Cohort 1986 Study. Data were collected on cumulative contextual risk (parent reports), substance initiation (adolescent reports), childhood peer processes and behavior problems (teacher reports), and temperamental characteristics (teacher reports). Novel discrete-time survival mediation analysis was conducted to test the hypothesized mediating mechanisms. ResultsInitial analyses showed that the associations between cumulative contextual risk and both alcohol and cigarette initiation were mediated by childhood peer processes and behavior problems; however, the indirect effects became statistically non-significant after adding the temperament variables, which themselves predicted substance initiation. ConclusionsTargeting peer processes may not be an effective way to interrupt pathways leading from early contextual risk to substance initiation. Instead, early screening and intervention efforts to delay substance initiation may need to be tailored to the individual temperamental characteristics of targeted participants.
