Genetic basis of endometriosis comorbidity

Introduction. The results of a number of studies indicate the comorbidity of endometriosis with various diseases, such as uterine fibroids, endometrial cancer, migraine, depression, infertility, bronchial asthma, and stomach diseases.Aim. Present literature data on the comorbidity of endometriosis and various diseases.Materials and methods. A review of the literature for 2021–2023 in the databases PubMed (National Library of Medicine), Cochrane Library, CyberLeninka, Medline, as well as in scientific citation databases (Scopus, Web of Science).Results and discussion. This paper analyzes data on the comorbidity of endometriosis and various diseases such as uterine myoma, endometrial cancer, migraine, depression, infertility, bronchial asthma, and gastric diseases. It was found that the comorbidity of endometriosis and the described diseases is based on common risk factors (early age of menarche, late menopause, absence of childbirth in the anamnesis, chronic inflammatory diseases of the pelvic organs, smoking, chronic stress, exposure to phthalate esters), molecular pathogenetic mechanisms (changes in gene expression, sex hormone levels, aberrant immune response, subclinical inflammatory response, hyperestrogenism, characteristic of both endometriosis and comorbid diseases), as well as genes involved in the development of these diseases. According to the results of full genomic studies, more than 170 polymorphic loci associated with the development of endometriosis and various comorbid diseases have been identified, and a positive correlation between endometriosis and these diseases has been revealed.Conclusion. These studies dictate the need to take into account in clinical practice the syntropy of endometriosis with other diseases, which, in turn, requires a multidisciplinary approach to the management of patients with endometriosis.

Association of Age at Menarche with General and Abdominal Obesity in Young Women.

Background and Objectives: Age at menarche is related to various biological and socioeconomic factors in childhood. The aim of the study was to examine the association of age at menarche with general and abdominal obesity in young women. Materials and Methods: A transversal anthropometric survey was conducted with 102 females from 21 to 25 years of age. The surveyed traits included height, weight, waist circumference (WC) and hip circumference (HC). General obesity was assessed using the body mass index (BMI) and abdominal obesity by WC, waist-to-hip ratio (WHR) and waist-to-height ratio (WHtR). A retrospective method was used for collecting age at menarche data. Results: The average age at menarche is 12.80 years. Early menarcheal age (<12 years) is detected in 25.5% of young females, while late onset of menarche (>14 years) is recorded for 20.6% of subjects. Early menarche age subjects exhibit significantly higher BMI, WC and WHtR in comparison with their late menarche age peers. There is a significant negative correlation between BMI, WC and WHtR values and menarcheal age. Late age at menarche is associated with higher probability of underweight status (BMI < 18.5 and/or WHtR < 0.4). Conclusions: Age at menarche has a negative correlation with general and abdominal obesity. Young women with early age at menarche show statistically higher values of BMI, WC and WHtR, while those with late menarcheal age show greater susceptibility to becoming underweight.

Effects of early age of menarche on the health of Indian middle-aged women: A survey study

The age of menarche, which is a sign that a female has reached reproductive maturity, has a significant impact on the health of women. Numerous components, including nutritional status, socioeconomic status, diet, environment, sibling relationship, hereditary and genetic factors, religious group, ethnicity, psychological stress, migration, and chronic illness, have been postulated to influence the age of menarche. Opinions on these theories have varied, both for and against the theory. In a survey it was reported that women complaints joint pains /stiff shoulder and LBA. Risk of musculoskeletal problems and the perception of pain could increase with obesity.To find the prevalence of physiological and psychological health problems in middle-aged females due to early menarche.Women matching the study criteria and willing to participate in the survey were approached and informed consent was taken. An interview was conducted on either a physical or virtual mode by a trained therapist as per the check list designed for this study.The content of the checklist was validated with the help of experts in this study domain and subjected to statistical tests which yielded a Cronbach’s score of 0.967. The participants response on this checklist was analyzed and a strong association of physical and psychological health issue was documented particularly among working women, multiparous women, and with type of delivery women had undergone during 1 pregnancy.The present study documented significant findings with the early age of menarche and its association with physical and psychological health of the women.

Age at Menarche and Its Association With Adult-Onset Metabolic Syndrome and Related Disorders in Women: A Cross-Sectional Study of a Nationally Representative Sample Over 10 Years.

The prevalence of metabolic syndrome (MetS) as well as related social costs and efforts is increasing. The purpose of this study was to investigate the association between age at menarche and the risk of MetS, obesity, diabetes, and cardiovascular disease (CVD) in women over 30 years, using data from the Korean National Health and Nutrition Examination Survey (2010-2020). The analysis of 30 916 participants showed that early menarche (before 11 years) significantly increased the risk of obesity, diabetes, and MetS compared with the median age of 14 years. Late menarche (after 17 years) was also linked to MetS and related disorders. The relationship between age at menarche (at <10 years and >19 years) and the risk of MetS and related disorders exhibited a reversed J-shaped (し) pattern characterized by a pronounced increased risk among those who experience early menarche, whereas the increased risk associated with late menarche was less consistent. These results will help to decrease the risk of MetS and related disorders by enabling early intervention in early and late menarche age groups.

CYP21A2 Intron 2 Genetic Variants Might Be Associated with the Clinical Characteristics of Women with PCOS.

Pathogenic variants in the CYP21A2 gene are related to the classic and non-classic forms of congenital adrenal hyperplasia (CAH). However, the role of CAH carrier status in the clinical presentation of polycystic ovarian syndrome (PCOS) is still unclear. Moreover, the possible associations of different CYP21A2 gene polymorphisms with metabolic and reproductive abnormalities in PCOS have not been investigated. Therefore, the present study aims to examine the prevalence of the most common CYP21A2 pathogenic variant IVS2-13A/C>G (c.293-13A/C>G) in Eastern European women with PCOS and to evaluate the associations between common intron 2 genetic polymorphisms and the clinical symptoms of the patients. Sixty consecutively recruited women with PCOS were genotyped for the CYP21A2 intron 2 IVS2-13A/C>G genetic variant. Additionally, CYP21A2 intron 2 polymorphic variants rs6453 (c.293-44G>T), rs6451 (c.293-67C>A/G), rs369651496 (c.293-104del), and rs6474 (c.308G>A/p.R103L) were tested and described. The clinical and hormonal characteristics were compared in women with PCOS and with polymorphic and wild-type genotypes. The heterozygous CYP21A2 pathogenic variant IVS2-13A/C>G was found in one of the investigated PCOS patients (1.67%) with a non-hyperandrogenic type of PCOS. The presence of the rs6453 (c.293-44G>T) T-allele was associated with increased levels of DHEAS (15.18 vs. 9.14 µmol/L, p = 0.003) compared to the wild-type genotype in the investigated group. The rs6451 (c.293-67C>A/G) minor alleles were associated with an earlier age of menarche in the patients (12.0 vs. 13.0 years, p = 0.007). The polymorphic rs369651496 minor 6G allele was related to a better lipid profile in the women with PCOS, while the rs6474 variant modulated the blood pressure of the patients. The presence of CYP21A2 genetic minor alleles of rs6467 (IVS2-13A/C, c.293-13A/C), rs6453 (c.293-44G>T), rs6451 (c.293-67C>A/G), rs369651496 (c.293-104del), and rs6474 (c.308G>A/p.R103L) might modulate the adrenal androgens, age of menarche, and metabolic features in women with PCOS. Further studies on 21-hydroxylase genetic variants (pathogenic and polymorphisms) in different ethnic groups might help reveal the influence of adrenal steroidogenesis on PCOS development, clinical manifestations, and lifelong cardiovascular risks.

Association of early menarche with breast tumor molecular features and recurrence

BackgroundEarly menarche is an established risk factor for breast cancer but its molecular contribution to tumor biology and prognosis remains unclear.MethodsWe profiled transcriptome-wide gene expression in breast tumors (N = 846) and tumor-adjacent normal tissues (N = 666) from women in the Nurses’ Health Studies (NHS) to investigate whether early menarche (age < 12) is associated with tumor molecular and prognostic features in women with breast cancer. Multivariable linear regression and pathway analyses using competitive gene set enrichment analysis were conducted in both tumor and adjacent-normal tissue and externally validated in TCGA (N = 116). Subgroup analyses stratified on ER-status based on the tumor were also performed. PAM50 signatures were used for tumor molecular subtyping and to generate proliferation and risk of recurrence scores. We created a gene expression score using LASSO regression to capture early menarche based on 28 genes from FDR-significant pathways in breast tumor tissue in NHS and tested its association with 10-year disease-free survival in both NHS (N = 836) and METABRIC (N = 952).ResultsEarly menarche was significantly associated with 369 individual genes in adjacent-normal tissues implicated in extracellular matrix, cell adhesion, and invasion (FDR ≤ 0.1). Early menarche was associated with upregulation of cancer hallmark pathways (18 significant pathways in tumor, 23 in tumor-adjacent normal, FDR ≤ 0.1) related to proliferation (e.g. Myc, PI3K/AKT/mTOR, cell cycle), oxidative stress (e.g. oxidative phosphorylation, unfolded protein response), and inflammation (e.g. pro-inflammatory cytokines IFNα\\documentclass[12pt]{minimal} \\usepackage{amsmath} \\usepackage{wasysym} \\usepackage{amsfonts} \\usepackage{amssymb} \\usepackage{amsbsy} \\usepackage{mathrsfs} \\usepackage{upgreek} \\setlength{\\oddsidemargin}{-69pt} \\begin{document}$$\\alpha$$\\end{document} and IFNγ\\documentclass[12pt]{minimal} \\usepackage{amsmath} \\usepackage{wasysym} \\usepackage{amsfonts} \\usepackage{amssymb} \\usepackage{amsbsy} \\usepackage{mathrsfs} \\usepackage{upgreek} \\setlength{\\oddsidemargin}{-69pt} \\begin{document}$$\\gamma$$\\end{document}). Replication in TCGA confirmed these trends. Early menarche was associated with significantly higher PAM50 proliferation scores (β = 0.082 [0.02–0.14]), odds of aggressive molecular tumor subtypes (basal-like, OR = 1.84 [1.18–2.85] and HER2-enriched, OR = 2.32 [1.46–3.69]), and PAM50 risk of recurrence score (β = 4.81 [1.71–7.92]). Our NHS-derived early menarche gene expression signature was significantly associated with worse 10-year disease-free survival in METABRIC (N = 952, HR = 1.58 [1.10–2.25]).ConclusionsEarly menarche is associated with more aggressive molecular tumor characteristics and its gene expression signature within tumors is associated with worse 10-year disease-free survival among women with breast cancer. As the age of onset of menarche continues to decline, understanding its relationship to breast tumor characteristics and prognosis may lead to novel secondary prevention strategies.

«Perinatal portrait», reproductive health parameters and risk factors for fat metabolism disorders in young overweight women

Overweight and obesity are one of the most serious social and medical problems today. It is appropriate to single out a special category of young women aged 18–35 years with excessive body weight, that is, the age which is capable of ensuring the reproductive potential of the future.The objective: to determine the probable factors of metabolic imbalance and the main parameters of reproductive health disorders in young overweight women.Materials and methods. Reproductive health parameters and the main predictors of its impairment were evaluated in two groups of patients – the main group (890 women with BMI &gt; 25.0 kg/m2) and a control group (40 patients with BMI of 18.5–24.9 kg/m2).The main group was divided into two subgroups taking into account the age of formation of overweight and obesity: I subgroup – 596 women with pubertal obesity (66.9%) and II subgroup – 294 women with overweight in the postpubertal period (33.1%).Results. A high rate of pregnancy complications (86.9%), gestational diabetes (25.8%), preeclampsia (34.1%), premature birth and low birth weight fetus (20.9%), as well as the birth of a girl with a large body weight (24.4%), can become modified factors in the formation of the reproductive system according to the characteristics of the “perinatal portrait” and form the basis of screening programs. The relationship between the early introduction of complementary foods and the absence of breastfeeding and the presence of obesity in the girl in the future was established (OR=7.25; 95% CI: 2.99–17.59; χ2=23.53, p&lt;0.0001).Attention should be paid to reproductive function disorders: amenorrhea was found 1.7 times more often in the case of postpubertal obesity, as well as abnormal uterine bleeding or metrorrhagia, infertility (24.4%), dyshormonal breast diseases (34.7%), polycystic ovary syndrome (59.8%), proliferative processes of the endometrium and myometrium (34.7%), as well as the formation of endometrial hyperplasia and polyposis with age (25.8%).A high percentage of stressogenic factors was determined in the examined cohort of patients, which could initiate the manifestation of metabolic and endocrine disorders in 569 cases – 63.9% (OR=4.14; 95% CI: 2.07–8.25; χ2=17.38, p&lt;0.0001).Conclusions. Probable predictors of impaired fat metabolism in young women were statistically identified: heredity from the mothers of such patients regarding metabolic and endocrine disorders, body weight at birth more than 4000 g and artificial feeding, early age of menarche, pubertal paratrophy, stress factors, infectious diseases during menarche, hypodynamia and a high level of intellectual differentiation.The reproductive health of women with overweight and obesity is characterized by a high frequency of menstrual cycle disorders, increased manifestations of changes in the vaginal microbiome and dysuric symptoms, as well as infertility, hormone-dependent diseases, and impaired reproductive function.

Risk Factors Of Ovarian Cancer Among The Postmenopausal Women

Background: Ovarian cancer ranks 5thin cancer deaths among women. A woman’s risk of getting ovarian cancer during her lifetime is about 1 in 75. Cancer mainly develops in older women.&#x0D; Methods: The case control study was carried out among 154 respondents in medical oncology department, NIRCH, Dhaka from July 2016 to June 2017. Among the participants, 77 were cases who had ovarian cancer and another 77 were controls who had no ovarian cancer. Convenient sampling technique was used and data were collected by face-to-face interview using a pretested semi-structured questionnaire. Data were entered on to the template of SPSS and analysis was done.&#x0D; Results: The mean age of case and control group were 60.06±7.00 and 59.89±6.96 years respectively. Highest number of respondents were illiterate. Majority came from rural area and had no personal monthly income. Most of them were taken high fiber diet, 83.1% in case and 77.9% in control group. About 80.5% in case and 66.2% in control group had BMI ≥25 kg/m2. Sedentary workers 40.3% in case and 13.0% in control group; moderate workers were 31.2% in case and 20.8% in control group and hard workers were 28.5% and 66.2% in case and control group respectively. The mean age of menarche 12.02±1.19 years and 12.87±1.30 years in case and control group respectively and mean age of menopause in case and in control groups respectively 51.54±2.39 and 49.61±1.29 years. In case group 49.3% had early term pregnancy whereas in control group 9.5% had early term pregnancy; in case group 82.1% and in control group 86.5% had breast feeding history. Around 16.9% in case group and 27.3% in control group used to take OCP. Significantly more respondents 6.5% in case group had personal H/O breast cancer than control group. Positive family history of breast and ovarian cancer was found 16.9% in case group and 2.6% in control group. In binary logistic regression analysis for risk factors of OC was found that sedentary activity was associated with increased risk, increased BMI (≥25 kg/m2), early age of menarche (≤12 years), late age of menopause (&gt;51 years), nullipara and early term pregnancy. Family H/O breast or ovarian cancer significantly associated with development of ovarian cancer.&#x0D; Conclusion: Ovarian cancer observed more in older age group (≥60 years) among sedentary worker with high body mass index (≥25 kg/m2). Early age of menarche (≤12 years), late menopause (&gt;50 years), never having given birth to a child in life time, early term of pregnancy and family history of breast and ovarian cancer were significantly associated with ovarian cancer among post-menopausal women.&#x0D; JOPSOM 2021; 41(2):28-35

Exposure to Particulate Matter Air Pollution and Age of Menarche in a Nationwide Cohort of U.S. Girls.

It remains unclear whether in utero and childhood exposure to air pollution affects pubertal development, particularly age of menarche in girls. The aim of this study was to determine whether residential ambient particulate matter (PM) exposure in utero and during childhood is associated with age of menarche. We studied 5,201 girls in the Growing Up Today Study 2 (2004-present) who were 10-17 y of age at enrollment (47.7% premenarchal; 52.3% postmenarchal). Exposure to three size fractions of PM [fine PM with aerodynamic diameter (), PM with aerodynamic diameters (), and PM with aerodynamic diameter ()] was assigned based on maternal residential address, updated every 2 y, using nationwide spatiotemporal models. We estimated average PM exposure in utero, and time-varying windows: annual average exposure in the prior 1 and 2 y and cumulative average from birth. Age of menarche was self-reported on three surveys administered in 2004, 2006, and 2008. We calculated hazard ratios (HR) for menarche for an interquartile range (IQR) increase in PM exposure using Cox proportional hazard models adjusting for potential confounders. Girls attained menarche at 12.3 y of age on average. In the adjusted model, higher residential exposure to ambient during all time windows was associated with earlier age of menarche. The HRs of menarche for each IQR () increase in exposure to during the in utero period, 1 y prior to menarche, and throughout childhood were 1.03 [95% confidence interval (CI): 1.00, 1.06], 1.06 (95% CI: 1.02, 1.10) and 1.06 (95% CI: 1.02, 1.10), respectively. Effect estimates for exposure were similar, albeit attenuated, for all time windows. exposure was not associated with age of menarche. Among a large, nationwide, prospective cohort of U.S. girls, higher exposure to and in utero and throughout childhood was associated with an earlier age of menarche. Mother's age at menarche is associated with odds of preterm delivery: A case-control study.

There is a secular trend towards earlier age of menarche in the US and globally. Earlier age at menarche (AAM) has been associated with metabolic disorders that increase risk for preterm delivery (PTD), yet no studies in the US have investigated whether AAM influences risk of PTD. This study tested the hypothesis that AAM is associated with PTD. A case-control study. The Boston Medical Center (BMC) in Boston, Massachusetts. 8264 mother-newborn dyads enrolled at birth at BMC between 1998 and 2019, of which 2242 mothers had PTD (cases) and 6022 did not have PTD (controls). Multivariable-adjusted logistic regression models and restricted cubic splines were used to examine the association between AAM and risk of PTD. The combined impact of AAM and age at delivery on the risk of PTD was also examined. Preterm delivery and gestational age (GA) was defined by maternal last menstrual period and early ultrasound documented in medical records. Maternal age at delivery was 28.1 ± 6.5 years and AAM was 12.85 ± 1.86 years. Multivariable-adjusted cubic spline suggested an inverse dose-response association of AAM with odds of PTD and, consistently, a positive association with GA. A 1-year earlier AAM was associated with 5% (95% CI 2%-8%) higher odds of PTD, after adjustment for maternal year of birth, parity, maternal place of birth, education, smoking status and Mediterranean-style diet score. The association between AAM and PTD was stronger among older mothers whose age at delivery was ≥35 years. Earlier AAM is associated with higher odds for PTD, and this association is stronger among women at advanced reproductive age.

Adolescence, as individuals in the age group 10-19 years, is marked by physical, emotional, cognitive, and behavioural changes. Behaviours characteristic of adolescence, such as alcoholism, use of narcotics, smoking, and high-risk sexual behaviour, are associated with risk to sexual and reproductive health. Unwanted pregnancies and sexually transmitted diseases can lead to serious common reproductive health concerns for adolescents. Lack of information is a significant factor behind poor adolescent knowledge, attitudes, and risk behaviours. This activity aims to discover adolescents' knowledge of Monopoly game-based learning on reproductive health. Activity in 42 adolescents undergoing health education monopoly game-based learning on reproductive health in July 2022 at Panti Asuhan Putri Muhammadiyah Tanjung Purwokerto. The questionnaire consisted of characteristic data filled by adolescent students such as age, body mass index, early age of menarche, cycle menstruation, and knowledge of adolescents about sexual and reproductive health before intervention measured. The pre-test results found that 42.86% of knowledge on reproductive health was poor. Innovative health education can be used to improve the sexual health education of adolescents. The methods can potentially contribute socially, particularly in improving sexual health behaviour and adolescent knowledge of sexual and reproductive health problems.

Age at menarche and asthma onset among US girls and women: findings from NHANES, 2001–2018

PurposeTo quantitatively estimate the association of age at menarche with the risk of childhood- and adult-onset asthma separately. MethodsA retrospective cohort study of 24,282 US girls and women was conducted using continuous National Health and Nutrition Examination Survey (NHANES) data from 2001 to 2018, and Cox proportional hazards regression models with censoring ages of 19 and 79 years were employed to separately estimate hazard ratios of childhood- and adult-onset asthma associated with age at menarche. ResultsEach one-year increase in age at menarche was significantly associated with a 16% (hazard ratio [HR] = 0.84; 95% confidence interval [CI]: 0.77-0.91) decrease in the risk of childhood-onset asthma. Compared with age at menarche of 12-14 years, we observed a 56% (HR = 1.56; 95% CI: 1.19-2.04) increased risk of childhood-onset asthma for early menarche (age at menarche < 12 years) and a 40% (HR = 0.60; 95% CI: 0.32-1.10) decreased risk for late menarche (age at menarche ≥ 15 years). No significant association was noted between age at menarche and adult-onset asthma. ConclusionsEarly menarche may represent a risk factor for childhood-onset asthma, which indicates the need for timely and effective management of individuals with early menarche to prevent asthma.

Awareness of symptoms, risk factors, and barriers to seeking help related to breast cancer among university students in Mexico City.

e18652 Background: In Mexico, about 15% of young women (age ≤ 40 years) have breast cancer (BC). However, only 40% of women in this age group know that they should practice self-examination, and only 30% know the proper technique. Additionally, studies carried out in this group show reasons for delaying diagnosis like emotional barriers when a sign is identified, and lack of knowledge of risk factors. The purpose of the present study is to identify knowledge about self-examination techniques, lifestyle, and risk factors that influence the development of BC in university students. Methods: In this exploratory study, we used a 10-section and 55-question questionnaire based on validated international tools such as the Cancer Awareness Measure (CAM) questionnaire (CAM), the National Survey of Trends in Health Information, and the Mediterranean Diet Score (MDS), as a healthy eating pattern. Results: A sample of 53 health sciences school students was analyzed, 93% were women, and the average age was 21 years old. 83% of the students did not feel capable of recognizing signs of BC. Regarding risk factors knowledge, 78.9% recognized using hormone replacement for more than 5 years, 32.7% early menarche age, 98.1% having a closer relative with BC, 63.4% drinking more than 1 unit of alcohol by day, 65.4% being overweight; as risk factors for BC. Interestingly, 32.7% consider being sedentary as a risk factor, however, 43.8% disagree or strongly disagree. The same happens with being overweight, the 34.6% did not recognize this condition as a BC risk. Higher meat consumption was present in 54.9% of the participants. Related to barriers to receiving medical attention if they had symptoms, 21.2% feel embarrassed and 30.8% are scared to see a doctor, 30.8% are too busy, 44.3% are worried about other things, 46.2% are worried about medical findings. Conclusions: Although there was a high recognition of non-modifiable risk, the students dismiss the lifestyle as a risk factor for BC and are not capable to recognize BC signs. It is important to develop interventions to increase knowledge of these risk factors and promote healthy lifestyles from a young age to improve BC prevention. The present study gave us information to design educational strategies, based on the identified needs, that can be implemented as a cancer prevention strategy in this age group. It is also necessary to implement BC cancer prevention education strategies in health sciences school students to improve their knowledge.

Association of Social Determinants of Health, Race and Ethnicity, and Age of Menarche among US Women Over 2 Decades

Age of menarche (AOM), or the first menstrual cycle, is one indicator of female puberty. The timing of AOM can be influenced by social determinants of health (SDOH). This study examined associations between SDOH and AOM over the past 2 decades in the United States. US National Health and Nutrition Examination Survey data (1999 to early 2020) were analyzed. Multinomial logistic regression analyses examined associations between AOM (early [11.99 years and younger], typical [12-13 years], and late [13.01 years and older]) and race/ethnicity, insurance coverage, education, family income-to-poverty ratio, money management, and home status. AOM remained consistent over the past 2 decades (mean 12.50 years, SE 0.02) for the aggregate sample. Females identifying as Hispanic (excluding Mexican Americans) were 63% more likely (adjusted odds ratio [aOR] 1.63, 95% confidence interval [CI] 1.13-2.36) to report early menarche. Those identifying as other/multiracial were 46% more likely to report late menarche (aOR 1.46, 95% CI 1.13-1.89) vs non-Hispanic Whites. Financial and home status instability was associated with early menarche (aOR 1.46, 95% CI 1.17-1.83; aOR 1.25, 95% CI 1.05-1.48). Less than 9th-grade education was associated with late menarche (aOR 1.47, 95% CI 1.14-1.89). Although the average AOM has remained stable in the United States over the past 20 years, identifying as Hispanic (excluding Mexican Americans) and financial/home instability are associated with early AOM, and lower education levels are associated with late AOM. Identifying programming and policy options targeting SDOH may help improve current and future reproductive health.

Premenopausal women with breast cancer in the early post-partum period show molecular profiles of invasion and are associated with poor prognosis

PurposeYoung premenopausal women develop breast cancer (BC) within 5–10 years of the last childbirth, known as post-partum breast cancers (PPBC), often present with aggressive disease. The exact mechanisms that lead to poor prognosis in these patients are largely unknown.MethodsWe have evaluated the association of clinical and reproductive factors with BC in a cohort of women ≤ 45 years (N = 155) with long-term follow-up. Based on duration since last childbirth (LCB), grouped patients into PPBC1 (LCB ≤ 5 years), PPBC2 (LCB between 6 and 10 years), PPBC3 (LCB > 10 years), and NPBC (age-matched nulliparous BC patients). We compared disease-free survival and hazard associated with recurrence/metastasis between the groups. RNA sequencing of tumor samples was performed from three parous groups (n = 10), and transcriptomic data were analyzed for differentially expressed genes and altered pathways.ResultsWomen in the PPBC1 group had an early menarche and late age at first and last childbirth compared to other groups. Survival analysis within lymph node-positive tumors showed that PPBC1 tumors had a worse prognosis than PPBC2 and NPBC tumors (p = 0.015 and p = 0.026, respectively). Clustering of the differentially expressed genes between the groups showed distinct expression in early PPBC (E-PPBC) tumors. Pathway analysis revealed upregulation of invasive-related pathways along with T cell exhaustion, extracellular matrix remodeling, angiogenesis, and epithelial-to-mesenchymal transition in E-PPBC tumors.ConclusionEarly PPBC is a unique subtype with aggressive clinical features and distinct biology. Further research is needed to accurately project the risk of recurrence and optimal treatment strategies in these young patients.

The genetics of autism and steroid-related traits in prenatal and postnatal life.

Autism likelihood is a largely heritable trait. Autism prevalence has a skewed sex ratio, with males being diagnosed more often than females. Steroid hormones play a mediating role in this, as indicated by studies of both prenatal biology and postnatal medical conditions in autistic men and women. It is currently unclear if the genetics of steroid regulation or production interact with the genetic liability for autism. To address this, two studies were conducted using publicly available datasets, which focused respectively on rare genetic variants linked to autism and neurodevelopmental conditions (study 1) and common genetic variants (study 2) for autism. In Study 1 an enrichment analysis was conducted, between autism-related genes (SFARI database) and genes that are differentially expressed (FDR<0.1) between male and female placentas, in 1st trimester chorionic villi samples of viable pregnancies (n=39). In Study 2 summary statistics of genome wide association studies (GWAS) were used to investigate the genetic correlation between autism and bioactive testosterone, estradiol and postnatal PlGF levels, as well as steroid-related conditions such as polycystic ovaries syndrome (PCOS), age of menarche, and androgenic alopecia. Genetic correlation was calculated based on LD Score regression and results were corrected for multiple testing with FDR. In Study 1, there was significant enrichment of X-linked autism genes in male-biased placental genes, independently of gene length (n=5 genes, p<0.001). In Study 2, common genetic variance associated with autism did not correlate to the genetics for the postnatal levels of testosterone, estradiol or PlGF, but was associated with the genotypes associated with early age of menarche in females (b=-0.109, FDR-q=0.004) and protection from androgenic alopecia for males (b=-0.135, FDR-q=0.007). The rare genetic variants associated with autism appear to interact with placental sex differences, while the common genetic variants associated with autism appear to be involved in the regulation of steroid-related traits. These lines of evidence indicate that the likelihood for autism is partly linked to factors mediating physiological sex differences throughout development.

Analysis of factors associated with early menarche in adolescents

The incidence of menarche praecox is 5.2% of adolescents in Indonesia. Indonesia ranks 15th out of 67 countries with a decline in menarche age of up to 0.145 years per decade. The study aimed to analyze factors associated with early menarche (praecox menarche) in adolescents. This type of analytic research with quantitative methods and crosssectional research design. This used primary data, the instruments used microtoise, weight scales, and checklists. The population was 145 adolescents. The sample size was 124 respondents using simple random sampling. The research was tested using the Chi Square test. It was conducted in Kediri on October to December 2022. The research results were found that adolescents who experienced praecox menarche were 41.9%, nutritional status was not obese by 75.2%, exposed to pornography by 65.6%, genetics (early menarche age of mother) by 51.2%, not active in sports by 69.6%. From the results of data analyze it can be concluded that there was no correlation between nutritional status and praecox menarche (P-value 0.100), there was correlation between exposure to pornography and praecox menarche (P-value 0.000), there was correlation between genetics and praecox menarche (P-value 0.002), there was correlation between physical activity (exercise) and praecox menarche (P-value 0.001).

The relationship of reproductive factors with adiposity and body shape indices changes overtime: findings from a community-based study

BackgroundStudies focusing on the relationships of adiposity and body shape indices with reproductive factors have reported conflicting results. This study aimed to investigate the influence of reproductive factors on adiposity and body shape indices changes overtime.Materials and methodsIn this community-based prospective study, 1636 postmenopausal women were selected from Tehran Lipid and Glucose Study (TLGS). The unadjusted and adjusted Generalized Estimating Equation models (GEE) were applied to investigate secular longitudinal trends of adiposity and body shape indices.ResultsAccording to the adjusted GEE models, mean changes in body mass index (BMI) in women with early menarche was 1.18 kg/m2 higher than those with normal menarche age (P = 0.030). Moreover, the mean changes in BMI overtime were 0.11 kg/m2 higher in women with premature/early menopausal age than those with normal menopausal age (P = 0.012). Mean changes of waist circumference (WC) in women with late menopause were 2.27 cm higher than those with normal menopausal age (P = 0.036). We also observed higher mean changes in a body shape index (ABSI) in women with late menopause (P = 0.037), compared to those with normal menopausal age. We found a marginal effect of parity on BMI and WC as well.ConclusionsThis study demonstrated higher BMI in females with earlier menarche age. We also showed higher values of BMI overtime in women with premature/ early menopause, whereas women with late menopausal age had higher WC and ABSI values. However, more longitudinal studies investigating body composition indices by adjusting all potential confounders are still required to confirm our study findings.

The influence of the trajectory of obesity indicators on the age of pubertal onset and pubertal tempo in girls: A longitudinal study in Chongqing, China.

This study aims to explore the influence of the trajectory of obesity indicators on the onset age of different pubertal development characteristics and pubertal tempo among girls. Our longitudinal cohort study recruited 734 girls at baseline in May 2014 from a district of Chongqing and followed them at 6-month intervals. Data were available from baseline to the 14th follow-up with a full record of height, weight, waist circumference (WC), breast development, pubic hair, and armpit hair development, as well as the age of menarche. The Group-Based Trajectory Model (GBTM) was fitted for the optimum trajectory of the body mass index (BMI), WC, and waist-to-height ratio (WHtR) of girls before the pubertal onset and menarche. The ANOVA and multiple linear regression model were conducted to analyze the influence of the trajectory of obesity indicators on the onset age of different pubertal development characteristics and pubertal tempo in girls. Compared with the healthy (gradual BMI increase) group before pubertal onset, the overweight (persistent BMI increase) group has an earlier onset age of breast development (B: -0.331, 95%CI: -0.515, -0.147) and pubic hair development (B: -0.341, 95%CI: -0.546, -0.136). The B2-B5 development time was shorter in girls in the overweight (persistent BMI increase) group (B: -0.568, 95%CI: -0.831, -0.305) and the obese (rapid BMI increase) group (B: -0.328, 95%CI: -0.524, -0.132). The age of menarche was earlier, and the B2-B5 development time was shorter in girls in the overweight (persistent BMI increase) group than in girls in the healthy (gradual BMI increase) group before menarche (B: -0.276, 95%CI: -0.406, -0.146; B: -0.263, 95%CI: -0.403, -0.123). Girls with high WC (rapid WC increase) before menarche had an earlier age of menarche than normal WC (gradual WC increase) (B: -0.154, 95%CI: -0.301, -0.006), and the B2-B5 development time was shorter in girls in the overweight (gradual WHtR increase) group than in girls in the healthy (persistent WHtR increase) (B: -0.278, 95%CI: -0.529, -0.027) group. Among girls, overweight and obesity (BMI scale) before pubertal onset can not only influence pubertal onset age but also accelerate B2-B5 pubertal tempo. Overweight (BMI scale) and high WC before menarche also have an impact on the age of menarche. Overweight (WHtR scale) before menarche is significantly associated with B2-B5 pubertal tempo.
