ASSESSMENT OF EFFICACY AND SAFETY OF ORAL FOSFOMYCIN SINGLE DOSE IN UNCOMPLICATED URINARY TRACT INFECTION AT A TERTIARY CARE HOSPITAL IN SOUTH INDIA

Objectives: Urinary tract infection (UTI) affects 150 million people worldwide each year. The rise in the UTIs is attributed to multidrug-resistant pathogens for which there are minimal treatment options available. This has facilitated the reemergence of certain old antimicrobials such as fosfomycin trometamol (FT). It seems an alternative, but the evidence towards its therapeutic efficiency is scanty. The objective of the study is to evaluate the safety and efficacy of single dose of FT in treating uncomplicated UTI and the resultant variations in the intensity of symptoms after the treatment. Methods: The study is a prospective, observational, and open-label study in the outpatient unit of the Department of General Medicine, RVM institute of Medical Sciences, for 6 months. The study comprises 50 patients among the age group of 18–70 years. Urinary Tract Infection Symptom Assessment questionnaire was used for the evaluation of symptoms pre and post treatment. After the diagnosis of uncomplicated UTI, patients were treated with single-dose of FT. Results: The study comprises 22 males and 28 females. After the treatment, there was a drastic improvement in the condition of patients. The severity level reduced and the quality of life improved post treatment and the results were statistically significant. Among 50 patients treated with the drug, 11 patients reported the side effect of diarrhea. Conclusion: Single dose oral Fosfomycin (3 g) regimen is effective in managing uncomplicated UTI with minimal side effects.

The use of oral fosfomycin-trometamol in patients with catheter-associated urinary tract infections (CAUTI): new indications for an old antibiotic?

We present the results of a multicenter retrospective study of 35 difficult-to-treat patients with urinary tract infections associated with indwelling urinary catheters (CAUTIs). All patients received oral administration of 3 g fosfomycin trometamol once a day for two days and then with a dose of 3 g every 48 h for two weeks. The most commonly isolated strains were: Escherichia coli (65.7%) and Enterococcus spp. (25.7%); prevalence of Extended-Spectrum Beta-Lactamase strains was 48.5%. Six patients (17.1%) had a clinical response after a single dose of fosfomycin trometamol, 12 (34.2%) after two doses and 13 (37.1%) patients had a clinical response after three or more doses. Four patients (11.6%) failed prolonged antibiotic treatment with fosfomycin trometamol. During the follow-up period, 30 out of 35 (85.7%) patients were without symptomatic infections. No significant side effects were reported. In conclusion, fosfomycin trometamol seems to be a valid treatment option in patients with CAUTIs.

Short term therapy for uncomplicated urinary tract infection today. Clinical outcome upholds the theories

The clinical and bacteriological activity of fosfomycin trometamol (FT) has been compared with several other antibiotics in the treatment of uncomplicated urinary tract infections. A single dose of FT had activity comparable with a 5-day course of trimethoprim in a trial where the causative organism and its sensitivity were unknown. In another trial FT showed better long term eradication compared with a 5-day course of cephalexin and other studies suggested a single dose of FT was comparable with a 7-day course of nitrofurantoin or norfloxacin. There were few important side effects with fosfomycin therapy and it was considered a safe and effective first line treatment in uncomplicated urinary tract infection.

A general practitioner multicenter study: Fosfomycin trometamol single dose versus pipemidic acid multiple dose

In order to evaluate the efficacy and safety of fosfomycin trometamol as single dose oral treatment for acute cystitis in women, an open, multicenter comparative study was carried out in general practices in France, 386 women, aged 16 to 75 years, with clinical symptoms of acute cystitis were enrolled in the study to receive either a single 3 g oral dose of fosfomycin trometamol or a five-day course of 400 mg pipemidic acid twice daily. The diagnosis of cystitis was based on clinical symptoms and significant bacteriuria (greater than or equal to 10(5) cfu/ml midstream urine). Follow-up examinations were carried out five to ten and 28 days after the end of treatment, 289 and 244 patients, respectively, were available for clinical and bacteriological evaluation at short-term (five to ten days) and medium-term (28 days) post-treatment follow-up. Both regimens were comparable for clinical and bacteriological efficacy with short-term eradication rates of 122/146 in the fosfomycin trometamol group and 130/143 in the pipemidic acid group. The results of medium-term follow-up were 113/122 and 114/122 for the eradication rates of the respective groups. Both drugs were well tolerated. Side effects were mild and of significantly shorter duration in the fosfomycin trometamol group.

General Practitioner Study: Fosfomycin Trometamol versus Amoxycillin Clavulanate in Acute Urinary Tract Infections

A clinical trial comparing 5 days' treatment with amoxycillin/clavulanate (group A) and a single dose of fosfomycin trometamol (group B) is presented. The study was done in symptomatic patients presenting to their family practitioner, with the microbiological testing being carried out in a university hospital laboratory. Of 62 patients with significant bacteriuria, 29 were given amoxycillin/clavulanate and 33 fosfomycin trometamol, in a randomized fashion. Cure rates 1 week and 5 weeks after the end of treatment were 72 and 65% in group A and 85 and 81% in group B. Adverse events assessed in 141 patients were unusual (10.1% in group A and 8.3% in group B) and were mild in nature. The results of this study suggest that single-dose treatment with fosfomycin trometamol is effective and acceptable as a conventional course of amoxycillin clavulanate for the treatment of simple acute dysuria and/or frequency with infection.
