Central Anticholinergic Syndrome Following Dobutamine–Atropine Stress Echocardiography

We present a case of central anticholinergic syndrome following dobutamine-atropine stress echocardiography in an elderly female. Although atropine toxicity is a recognized complication of stress echocardiography, no case reports are currently available. The central nervous system side effects of atropine are varied in severity (mild cognitive impairment to severe coma) and spectrum (agitation or somnolence), and thus are often overlooked. Management includes prompt recognition, stopping the offending agent, providing supportive care, and consideration of physiostigmine. Atropine is used in up to 60% of dobutamine stress echocardiograms, and has the potential to cause morbidity.

Thoracoabdominal aortic aneurysm repair in high risk cardiac patients: A modified grafting technique

Severe cardiac disease is a major risk for early death following thoracoabdominal aortic aneurysm (TAAA) repair. Proximal aortic cross-clamping during TAAA repair dramatically increases left ventricular afterload risking myocardial ischemia. Although preoperative myocardial revascularization helps protect myocardium at risk during these periods of hemodynamic stress, in some patients myocardial revascularization is not feasible. Similarly, intraoperative shunting or bypass is not always practical. Under these circumstances we employ a modified multigraft technique during TAAA repair to reduce the risk of early death in high-risk cardiac patients. Case #1 is a 59-year-old male with end-stage ischemic cardiomyopathy (ejection fraction 15%), and recurrent admission for CHF, diagnosed with a 6 cm type III TAAA during evaluation for cardiac transplantation. Because of the potential need for intraaortic balloon support, he was not accepted for transplantation unless the TAAA could be repaired first. He underwent successful modified TAAA repair and subsequently had a successful cardiac transplant. He remains alive and well 3 years after TAAA repair. Patient #2 is a 70-year-old male who presented with an 8 cm type III TAAA. Cardiac evaluation revealed a history of prior myocardial infarction, severe nonreconstructable three-vessel coronary artery disease and inducible angina, left ventricular aneurysm, and ischemic wall motion abnormalities during dobutamine stress echocardiogram. Aneurysm size and multiple episodes of radiating central abdominal and back pain suspicious for aneurysm expansion precluded delays inherent to myocardial revascularization. He remains alive and well 10 months following successful modified TAAA repair. Patients with severe cardiac disease are at risk for early death following TAAA repair. Aortic cross-clamping contributes to this risk. The modified, multigraft technique of TAAA repair avoids aortic cross-clamping, minimizes myocardial risk, and may reduce early death.

Effects of delayed recanalization on left ventricular function in diabetic and non-diabetic patients with acute anterior wall myocardial infarction

To investigate the effects of delayed recanalization through percutaneous coronary intervention (PCI) on the left ventricular function after acute anterior wall myocardial infarction in diabetes (DM) and non-DM patients, a total of 125 consecutive subjects with acute anterior wall myocardial infarction were recruited and divided into DM group (n=43) and non-DM group (n=82) based on the presence of DM. All patients received successful PCI at about 2 weeks after symptom onset. Ischemic viable myocardium was detected with low-dose dobutamine stress echocardiogram test, and left ventricular function and wall motion were also assessed before PCI. Clinical manifestations and angiograms before and after PCI were analyzed. Levels of creatinine kinase-MB (CK-MB) and troponin T (TnT) before PCI, 6 and 24 h after PCI were determined. All patients received clinical and echocardiographic follow-up 6 months after PCI. Higher rate of TIMI flow grade 2 and lower rate of TIMI flow grade 3 were observed immediately after PCI, and the elevation of serum CK-MB and/or TnT levels were higher in DM group when compared with non-DM group (P 0.05). There were no significant differences in the LVEF, LVEDVI, LVESVI or wall motion score (WMS) between two groups before PCI (P>0.05). But, 6 months after PCI, WMS and LVEF were decreased and increased, respectively, in non-DM group, and the WMS and LVEF in DM group were not changed and the LVEDVI increased, when compared with before PCI. The LVEDVI, LVESVI, LVEF, and WMS differed significantly between two groups 6 months after PCI. Compared with non-diabetics, delayed revascularization with PCI in diabetics with acute myocardial infarction has less benefit on the improvement of left ventricular function, and it may be the insufficient reperfusion or reperfusion injury to myocardium but not the viable myocardium contribute to the poor result. The effect of coronary angiography on renal function in preemptive renal transplant candidates

Increasing numbers of patients undergo preemptive renal transplantation. Obtaining cardiac catheterizations prior to transplantation to screen for coronary artery disease is controversial because of the perceived risk of inducing contrast nephropathy and the need for dialysis in patients with marginal renal function. We sought to examine the true impact of cardiac catheterization on time to dialysis in a cohort of preemptive renal transplant candidates. From a cohort of 376 transplant candidates evaluated preemptively at our program between 2/2001 and 4/2005, 34 patients had positive dobutamine stress echocardiograms. We reviewed the subsequent need for dialysis in these patients. Among candidates undergoing angiography, 8.7% required dialysis within 14 d of contrast administration and 26% eventually needed dialysis prior to transplantation at 5.3 ± 3.7 months after their pre-transplant evaluation. Among patients who did not undergo angiography, 27% needed dialysis prior to transplantation at 2.4 ± 1.8 months after pre-transplant evaluation. Our results demonstrate a low risk of hastening the need for dialysis after coronary angiography in preemptive renal transplant candidates. Undergoing angiography had no effect on the ultimate need for or timing of dialysis initiation. These findings support completion of full cardiac evaluation as indicated for high-risk preemptive renal transplant candidates.

Increasing Adherence to Scheduled Outpatient Dobutamine Stress Echocardiograms

In a large, urban teaching hospital over a 10-month period, 45% of dobutamine stress tests were cancelled. The article discusses the results of a pilot study that was implemented to reduce appointment cancellations. Factors that impacted adherence to the testing schedule were proper documentation of cancellations, the importance of eliminating inappropriate referrals, and the use of an appointment reminder system. With the use of a checklist and scripted reminder tool, the cancellation rate of these tests was decreased to 32.4%, a 12.6% reduction.

Left ventricular outflow tract and mid-cavity obstruction may cause false-positive dobutamine stress echocardiograms

Left ventricular (LV) outflow tract obstruction (LVOTO) occurs in up to 20% of patients undergoing dobutamine stress echocardiography (DSE). Mid-cavity LV obstruction occurs less commonly during DSE. LV regional wall motion abnormalities during DSE may occur despite normal coronaries due to hypertensive blood pressure response and takotsubo stress cardiomyopathy. We describe herein two cases of LVOTO and one case of mid-cavity LV obstruction during DSE associated with transient apical hypokinesis.

Takotsubo (apical ballooning) syndrome in the recovery period following dobutamine stress echocardiography: a first report

We report a case of Takotsubo syndrome occurring in the recovery phase after a dobutamine stress echocardiogram. Takotsubo syndrome is a widely acknowledged cause of reversible left ventricular systolic dysfunction. It has garnered much attention from the cardiological community since its presentation frequently mimics that of ST-segment elevation myocardial infarction. The exact aetiology remains incompletely defined, although stress is recognized frequently as a precipitating factor. In recent years it has emerged that stress testing, as part of a patient's investigative assessment, can also induce Takotsubo's syndrome. All prior reports of dobutamine-induced Takotsubo's syndrome have described apical ballooning at peak stress. We describe the case of an 85-year-old lady who developed apical ballooning in the recovery period after a dobutamine stress echocardiogram, despite having normal left ventricular wall motion at rest and at peak stress. We believe this to be the first such case reported in the literature. Dobutamine stress testing can precipitate Takotsubo's syndrome not just at peak stress but also during the recovery period. All those performing dobutamine stress tests should be aware of this rare but potentially important complication.

Study of Left Ventricular Rotation and Torsion in the Acute Phase of ST‐Elevation Myocardial Infarction by Speckle Tracking Echocardiography

The mechanics of the complex left ventricular (LV) myocardial fiber architecture may accurately be assessed by speckle tracking echocardiography (STE). The role of STE to assess LV mechanical dysfunction in the setting of ST segment elevation myocardial infarction (AMI) is still poorly studied. 29 consecutive patients (55 +/- 13 years) presenting with AMI underwent STE within 72 hours of admission. Reperfusion was achieved with thrombolysis in 15 patients and with primary percutaneous coronary intervention in 14. LV rotational and torsion data were registered during peak systole. Standard Doppler data included LV ejection fraction (EF), mitral inflow deceleration time (DT), and conventional E/A ratio. E/E' ratio (mitral inflow E velocity/tissue Doppler E velocity) was calculated as a marker of LV filling pressure. Twelve subjects with clinically indicated but negative dobutamine stress echocardiogram served as Controls. Peak systolic torsion was not only significantly lower in AMI compared with Controls (13.3 +/- 7.6 vs. 21.8 +/- 6.1; P < 0.01), it was also lower in subjects with LVEF <40% (5.0 +/- 2.9) compared with those who had LVEF >40% (10.6 +/- 6.6; P < 0.02). Torsion had a modest but significantly positive linear relation (R = 0.6; P < 0.05) with DT, not with E/E' or LVEF. LV systolic torsion is decreased in AMI and more markedly decreased in patients with LVEF <40%. The most significant linear relationship between DT and torsion may possibly indicate that the LV mechanical dysfunction is also associated with altered filling dynamics.

Baseline Echocardiographic Predictors of Dynamic Intraventricular Obstruction of the Left Ventricle during Dobutamine Stress Echocardiogram

Intraventricular obstruction (IVO) during dobutamine stress echocardiogram (DSE) may be associated with or reproduce symptoms. Predictors of IVO are not well established. 149 patients were studied at rest and during DSE. The normal range of the left ventricular outflow tract (LVOT) velocities was investigated in 68 healthy patients. 19 patients (13%) developed IVO (peak LVOT velocity > 271 cm/sec). A significant linear correlation was observed between peak LVOT velocity during DSE and the following rest parameters: LV end-diastolic dimension (r =-0.20, P = 0.018), LV end-systolic dimension (r =- 0.27, P = 0.001), relative wall thickness (r = 0.23, P = 0.006), shortening fraction (r = 0.24, P = 0.004), LVOT diameter (r =- 0.20, P = 0.023) and LVOT velocity (r = 0.29, P < 0.0001). Only relative wall thickness (P = 0.012) and LVOT diameter (P = 0.027) were independent predictors of IVO. As a dichotomous variable, a relative wall thickness > or =0.44 was the only independent predictor of IVO (OR 5.7, 95% CI 1.6-20, P = 0.006), with sensitivity, specificity, negative predictive value, and positive predictive value of 77%, 62%, 95%, and 21%, respectively, and global accuracy of 63% (area under the ROC curve = 0.7). IVO was significantly associated with general cardiovascular symptoms (P = 0.0006) and with chest pain (P = 0.008). Relative wall thickness and LVOT diameter were independent predictors of obstruction. As a dichotomous variable, a relative wall thickness < or = 0.44 was the only independent predictor of dynamic IVO.

Direct injection of autologous mesenchymal stromal cells improves myocardial function

Cell-based therapies have been employed with conflicting results. Whether direct injection of ex-vivo expanded autologous marrow stromal cells (MSCs) would improve the function of ischemic myocardium and enhance angiogenesis is not well defined. In a porcine model of chronic ischemia, MSCs were isolated and cultured for 4 weeks. Sixteen animals were random divided into two groups to receive either direct intramyocardial injection of autologous MSCs, or equal volumes and injections sites of saline. Cine MRI and epicardial echocardiography were performed just prior to the injections and again 6 weeks later at the time of sacrifice at which point tissue was also analyzed. Myocardial function as assessed by regional wall thickening (as measured by dobutamine stress echocardiograms) demonstrated a 40.9% improvement after cell treatment of the ischemic zone ( p = 0.016) whereas the saline treated animals only had a 3.7% change ( p = 0.82) compared to baseline. The left ventricular ejection fractions of MSC group showed 19.5% improvement from baseline 35.9 ± 3.8% to 42.9 ± 5.8% ( p = 0.049). Increased vascularity was found in the MSC group compared to controls (0.80 ± 0.30 vs 0.50 ± 0.19 capillary/myocyte ratio, p = 0.018). Direct injection of autologous MSCs promotes angiogenesis and enhances the functional improvements following chronic myocardial ischemia. This suggests that the angiogenesis engendered by cell treatment may be physiologically meaningful by improving the contractility of ischemic myocardium.

Prognostic Significance of QRS Duration in Patients With Suspected Coronary Artery Disease Referred for Noninvasive Evaluation of Myocardial Ischemia

The purpose of this study was to evaluate the prognostic significance of QRS duration in patients with suspected coronary artery disease (CAD) referred for noninvasive evaluation of myocardial ischemia by dobutamine stress echocardiography. QRS duration is a prognostic marker in patients with previous myocardial infarction and/or heart failure. The relation between QRS duration and outcome of patients without known heart disease has not been evaluated. A total of 1,227 patients (707 men, mean age 61 +/- 14 years) with suspected CAD underwent dobutamine stress echocardiography for evaluation of myocardial ischemia. Patients were followed to determine predictors of cardiac events and to assess the incremental significance of QRS duration compared to clinical and dobutamine stress echocardiographic data. During a mean follow-up of 4.2 +/- 2.4 years, 280 patients (23%) died (129 cardiac deaths), and 60 (5%) had a nonfatal infarction. Annualized cardiac death rates were 2.0% in patients with QRS duration <120 ms and 4.4% in patients with QRS duration >or=120 ms, respectively (p <0.0001). Annualized event rates for cardiac death/nonfatal infarction were 2.8% in patients with QRS duration <120 ms and 4.8% in patients with QRS duration >or=120 ms (p = 0.0001). Multivariate models identified age, male gender, smoking, QRS duration >or=120 ms, and an abnormal dobutamine stress echocardiogram as independent predictors of cardiac death and the combined end point cardiac death/nonfatal infarction. In conclusion, QRS duration is an independent predictor of cardiac death and cardiac death/nonfatal infarction in patients with suspected CAD. This risk is persistent after adjustment for clinical variables, left ventricular function, and myocardial ischemia.

Incremental value of 2-dimensional speckle tracking strain imaging to wall motion analysis for detection of coronary artery disease in patients undergoing dobutamine stress echocardiography

Interpretation of dobutamine stress echocardiogram (DSE) is often subjective and requires expert training. The purposes of this study was to determine optimal cutoff values for longitudinal, circumferential, and radial strains at peak DSE for detection of significant stenoses on coronary angiography and to investigate incremental value of combining strain measurements to wall motion analysis. In this multicenter study, 102 patients underwent concomitant DSE and coronary angiography. Optimal cutoff values for mean global longitudinal (-20%), global circumferential (-26%), and mean radial (50%) strains at peak stress for detection of significant stenoses on coronary angiography were determined in a derivation group (n = 62) and tested in a prospectively recruited validation group (n = 40). Respective sensitivities for longitudinal, circumferential, radial strains, and expert wall motion score index (WMSI) were 84.2%, 73.9%, 78.3%, and 76%; respective specificities were 87.5%, 78.6%, 57.1%, and 92.9%; and respective accuracies were 85.2%, 75.7%, 70.3%, and 82.1%. Longitudinal strain analysis had comparable accuracy to WMSI (P = .70). However, combination longitudinal strain and WMSI had the highest sensitivity, specificity, and accuracy (100%, 87.5%, and 96.3% respectively), and its diagnostic accuracy was incremental to either longitudinal strain (P = .034) or WMSI alone (P = .008). Longitudinal strain analysis had higher diagnostic accuracy than circumferential and radial strains and was comparable to WMSI for detection of significant coronary artery disease. However, combination longitudinal strain and WMSI resulted in significant incremental increase in diagnostic accuracy.

Optimizing Screening and Management of Asymptomatic Coronary Artery Disease in Patients With Stroke and Patients With Transient Ischemic Attack

The feasibility of implementing an expert consensus guideline recommending use of a stroke patient's profile to manage undiagnosed coronary artery disease remains unclear. Following a guideline-based algorithm, we screened consecutive patients with ischemic stroke and patients with transient ischemic attack for asymptomatic coronary artery disease using the Framingham Heart Study Coronary Risk Score (FCRS) cutoff of high risk (> or = 20%) for experiencing a hard coronary artery disease event over a 10-year period. Patients with high FCRS received dobutamine stress echocardiogram outpatient screening, additional treatment (beta-blocker), or further management (cardiologist referral). From July 2004 to September 2007, among 693 patients, 501 (72%) met study criteria, of which 80 (16%) had FCRS > or = 20%. Elevated serum glucose, nonhigh-density lipoprotein, triglycerides, homocysteine, glycosylated hemoglobin as well as large vessel atherosclerotic stroke mechanism were more frequent in high versus low FCRS patients (P<0.05). Among high FCRS patients, 35 (44%) had dobutamine stress echocardiogram performed. Leading reasons for dobutamine stress echocardiogram nonperformance were patient noncompliance (42%) and primary care physician refusal (33%). Screening for coronary artery disease risk using FCRS is feasible in hospitalized patients with stroke, but outpatient adherence to stress testing is challenging largely due to patient and primary care physician-related factors.

Relation of Dyspnea in Patients Unable to Perform Exercise Stress Testing to Outcome and Myocardial Ischemia

Limited information exists regarding the significance of dyspnea in patients who are unable to exercise and the contribution of myocardial ischemia to this symptom. To assess this, we evaluated results of dobutamine stress echocardiography (DSE) and long-term outcome of patients with dyspnea referred for DSE. We studied 6,376 consecutive patients who were unable to perform an exercise test and were referred for DSE. Patients were classified according to presenting symptoms and followed for 5.5 +/- 2.8 years. End points were cardiac ischemic events (myocardial infarction or revascularization), hospitalization for heart failure (HF), and death. Dobutamine stress echocardiogram was positive for ischemia in 19% of patients with dyspnea versus 24% (p = 0.002) of those with typical angina and 17% (p = 0.2) of asymptomatic patients. In multivariate analysis, risk of death was increased in dyspneic patients versus asymptomatic patients (hazard ratio [HR] 1.14, p = 0.02) and patients with chest pain (HR 1.20, p <0.001). Hospitalization for HF occurred more often in patients with dyspnea (HR 1.26, p = 0.05 vs asymptomatic; HR 1.24, p = 0.06 vs chest pain), especially in the subset without previous HF (HR 1.45, p = 0.006 vs chest pain). Risk of cardiac ischemic events in patients with dyspnea was similar versus asymptomatic patients (HR 0.92, p = 0.39) and decreased versus patients with chest pain (HR 0.70, p <0.001). In conclusion, in patients referred for DSE, dyspnea was associated with a poor outcome. This increased hazard seems not to be linked to myocardial ischemia, but instead to HF and death.

Predictive value of normal dobutamine stress echocardiogram in patients with low-risk acute chest pain

Dobutamine stress echocardiography (DSE) has been proposed as a tool for risk stratification of patients with acute chest pain (ACP). In this single-center study, we evaluated the negative predictive value of DSE in 178 patients who presented with low-risk ACP (normal or inconclusive electrocardiogram and negative markers of myocardial damage) and were discharged following a maximal DSE that did not reveal ischemia. During the follow-up (median 321days), 2 of the 178 patients were admitted with an acute coronary syndrome and were diagnosed with obstructive coronary artery disease at angiography. In the time frame of the study, the negative predictive value of a normal and maximal DSE for an adverse cardiac event was 98.9% (95% CI: 96.0-99.8%). Thus, a normal DSE has a high negative predictive value and comprises a safe and effective tool for early risk stratification of patients who present with acute chest pain of low risk.

Dobutamine stress echo-induced apical ballooning (Takotsubo) syndrome

We report a case of dobutamine stress echocardiography (DSE) resulting in transient apical ballooning syndrome to highlight this rare condition as a potential complication of DSE. Takotsubo cardiomyopathy, or transient apical ballooning syndrome, is a recently described form of left ventricular (LV) dysfunction induced by stress. Clinically it can mimic acute coronary syndrome in its presentation. It is characterized by an atypical distribution of LV dysynergy with apical ballooning and compensatory basal hyperkinesis. Coronary angiography is normal. It has preponderance in females. Although the aetiology of Takotsubo syndrome remains obscure catecholamine release appears to be the principal trigger. We report a case of dobutamine-induced transient LV apical ballooning in a woman without coronary disease, during a dobutamine stress echocardiogram. There was evidence of ventricular recovery by 72 h. To our knowledge, only three other case reports describe dobutamine-induced Takotsubo cardiomyopathy. Dobutamine stress echocardiography is a widely performed diagnostic test, however, it can rarely result in presumed catecholamine-induced transient apical ballooning syndrome.

Ventricular Fibrillation in Late Recovery After Dobutamine Stress Echocardiography

A 62-year-old man with multiple cardiac risk factors, including diabetes mellitus type II, treated hypertension, and hyperlipidemia, had a dobutamine stress echocardiogram performed as part of a preoperative evaluation. At peak stress the patient developed an apical regional wall motion abnormality. Approximately 12 minutes into the recovery period, the patient developed ventricular tachycardia that degenerated into ventricular fibrillation. He was successfully resuscitated and underwent emergency coronary angiography that showed a 95% distal left anterior descending coronary artery stenosis.

Lack of subclinical myocardial ischaemia in Mexican patients with systemic lupus erythematosus without traditional risk factors for coronary artery disease

The objective of this study was to analyse whether patients with systemic lupus erythematosus (SLE) without traditional risk factors for coronary artery disease (CAD) develop subclinical myocardial ischaemia in the first years after diagnosis. A cross-sectional analysis of a cohort of 200 female SLE patients was conducted. We selected those patients who fulfilled the American College of Rheumatology (ACR) SLE criteria and had no traditional risk factors for CAD, including diabetes mellitus, hypertension, obesity, hyperlipidemia, and smoking. After an initial clinical and laboratory examination, patients were evaluated using a baseline echocardiogram and a dobutamine and atropine stress echocardiogram to search for subclinical myocardial ischaemia. Forty-one patients were included in the study. The mean age at the time of the study was 34.5 +/- 9.56 years (mean +/- SD). The mean age at diagnosis was 30.3 +/- 9.39 years. The mean time from diagnosis was 3.9 +/- 3.3 years. Baseline disease activity index (MEX-SLEDAI score) showed that 92.6% of patients had disease activity, although most patients had mild activity. A dobutamine and atropine stress echocardiogram was performed in 40 patients. All 40 patients had negative tests for subclinical myocardial ischaemia. Patients without traditional risk factors for CAD do not have an increased risk for subclinical myocardial ischaemia in the first years after diagnosis. A longitudinal follow-up study of these patients is needed to confirm our findings and assess if additional non-traditional risk factors for CAD increase the risk for myocardial ischaemia.
