yas G> ~ E noted with interest your publication's recent editorial (1) which emphasizes the importance of injuries as W 5.T.T 2 causes of death, disability, and economic costs in the it W % United States today and proposes the establishment of a P c) "National Institute of Injury Prevention" to develop and coordinate research and control efforts directed at the problem of injuries. We strongly agree that injuries constitute a major public health problem that warrants greatly increased attention and support. Indeed, in the United States, injuries annually affect approximately 1 of 3 persons (2), lead to emergency room visits for 1 of 5 persons (3), and account for 9.5% of acute-care hospitalizations (4) and 5.6% of deaths (5). Injuries are the leading cause of death for persons aged 1-44 years (6) and the nation's leading cause of premature death (7). We disagree, however, with the statement that "injuries remain to this day a neglected and forgotten area of public health concern." The Centers for Disease Control (CDC) formally recognized the importance of selected injuries in the 1978 report of its ad hoc Programs and Policies Advisory Committee (8), and injury prevention is one of 15 health priority areas identified by the Public Health Service in 1980 for concerted national action (9). In line with these priorities, the CDC has established programmatic foci for the evaluation and control of injuries in the Special Studies Branch of the Center for Environmental Health (CEH), the Violence Epidemiology Branch of the Center for Health Promotion and Education, the Division of Safety Research of the National Institute for Occupational Safety and Health, and the Epidemiologic Studies Branch of

Descriptive epidemiology in job injury surveillance

Descriptive epidemiology is a well-defined set of methods for describing the distribution of diseases among human populations. It is less commonly used in describing accident and injury occurrences, particularly for occupational injuries. In particular, surveillance of occupational injuries in the National Institute for Occupational Safety and Health, Division of Safety Research, is currently carried out using traditional labor statistics and administrative statistics. By combining these data sources with employment figures, risk assessment using epidemiologic methods is carried out where possible. The data sources are described and their advantages and limitations discussed. The various purposes of surveillance activities are outlined, and the data sources are examined as to how well they serve these purposes. Recommendations are included for improving existing data sources and for planning new data collection efforts so that safety problems in the workplace can be more scientifically assessed and defined.

Determination of concentrations of selected radionuclides in surface soil in the U.S.

Background radionuclide concentrations in surface soil across the U.S. have been measured by the Remedial Action Survey and Certification Activities Group of the Health and Safety Research Division at Oak Ridge National Laboratory (ORNL). These measurements have been made as part of the ORNL program of radiological surveillance at inactive uranium mills and sites formerly utilized during Manhattan Engineer District and early Atomic Energy Commission projects. The background soil sampling program involved determination of 226Ra, 232Th and 238U concentrations in surface soil samples for comparative purposes to determine the extent of contamination present at the survey sites and surrounding off-site areas. The sampling program to date has provided background information at 356 locations in 33 states. The nationwide average concentrations of 226Ra, 232Th and 238U in surface soil were determined to be 1.1, 0.98 and 1.0 pCi/g, respectively. This paper summarizes the results of these background measurements and provides a brief analysis of regional differences and similarities in data values.

Injury surveillance. A review of data sources used by the Division of Safety Research.

Injury surveillance by the Division of Safety Research in the National Institute for Occupational Safety and Health has the following two major purposes: the detection of significant changes in the status of worker safety to help define areas for research and the assessment of importance of various safety problems for priority setting in the Division. Surveillance of occupational injuries in recent years has realized a quantum improvement due to the availability of several extensive work-injury case data bases. The Division of Safety Research utilizes three of these to provide essentially independent sources of evidence for the identification and definition of problems for safety research. The National Electronic Injury Surveillance System, based on daily reports from a representative sample of hospital emergency rooms across the nation, provides case data such as age and sex of the injured worker, injury diagnosis, and product involvement. The Supplementary Data System annually provides worker's compensation case files which include the occupation and industry of the injured worker from selected states. A third level of detail is provided by the work injury report surveys of injured workers, conducted annually, and focused on specific accident types of injury groups. Findings, include high-risk occupations, age groups, tools, and equipment.
