Long-Term Musculoskeletal Disorders and Work Career in 55-Year-Old Workers

The purpose of the present study was to describe the associations between long-term disorders of musculoskeletal system and work career in elderly workers. The data were obtained by a postal questionnaire and clinical examinations. The study group consisted of 778 elderly urban persons, who were 55 years old and who lived in the city of Oulu in northern Finland. Long-term musculoskeletal disorders were reported by 62% of the women and 57% of the men. The high prevalence of musculoskeletal disorders was significantly associated with heavy physical work. However, job mobility and declining work career were correlated with the occurrence of long-term musculoskeletal disorders in women whereas advanced work career was connected with a low prevalence of long-term musculoskeletal disorders.

Clinical safety of becaplermin (rhPDGF-BB) gel

This overview of the safety of becaplermin gel is based on six well-controlled clinical studies that included patients with lower extremity diabetic neuropathic ulcers. Patients receiving becaplermin gel (n = 538), placebo gel (n = 278), or good ulcer care alone (n = 190) had a similar incidence of ulcer-related adverse events, such as infection, cellulitis, or osteomyelitis. Erythematous rash occurred in 2% of patients with suspected wound infections treated with becaplermin gel and 1% of patients treated with placebo gel. No rashes were observed in patients treated with good ulcer care alone. The incidence of cardiovascular, respiratory, musculoskeletal, and central and peripheral nervous system disorders were similar across all treatment groups. Mortality rates were also similar across all treatment groups. Patients treated with becaplermin gel did not develop neutralizing antibodies against becaplermin. Therefore, becaplermin gel appears to be a safe therapy for the treatment of lower extremity diabetic ulcers.

Impairment Tutorial: Rating Pain After a Musculoskeletal Injury

Abstract The AMA Guides to the Evaluation of Permanent Impairment (AMA Guides), Fourth Edition, notes that pain is a subjective perception, and usually no exact relationships exist among the degree of pain, extent of pathologic change, and extent of impairment. Chapters 3, 14, and 15 regarding the musculoskeletal system, mental and behavioral disorders, and pain, respectively, may be relevant in evaluating an individual's pain following musculoskeletal injury. The eight diagnostic characteristics of chronic pain are duration, dramatization, diagnostic dilemma, drugs, dependence, depression, disuse, and dysfunction; if four of the eight are present, a presumptive diagnosis is established. The fourth edition of AMA Guides distinguishes chronic pain syndrome and psychogenic pain (the former is not considered a mental disorder), but diagnosis of pain should consider psychological factors that are judged to play a significant role in the onset, severity, exacerbation, or maintenance of the pain. Thus, two individuals may have identical musculoskeletal histories, physical exams, and pain out of proportion to their injuries, but when one of the patients has psychological factors that are known and understood, the evaluating physician may diagnose a pain disorder associated with psychological factors. Because formally associating psychological factors and diagnosis usually requires legal discovery, physicians may keep the psychological factors confidential, diagnose chronic pain syndrome, and evaluate using Chapter 15 of the fourth edition of the AMA Guides.

A long-term study to evaluate the safety and efficacy of meloxicam therapy in patients with rheumatoid arthritis.

Meloxicam is a new non-steroidal anti-inflammatory drug (NSAID), which has a higher activity against cyclooxygenase-2 (COX-2) than against cyclooxygenase-1 (COX-1), with potentially high anti-inflammatory and analgesic action. This study was designed to assess the long-term safety and efficacy of meloxicam 15 mg daily. Three hundred and fifty-seven patients (aged 19-84 yr, mean 56 yr) with rheumatoid arthritis (RA) received meloxicam 15 mg orally once daily, for up to 18 months. Sixty-six per cent of patients remained on therapy for 18 months. Mean global efficacy, assessed by each patient on a visual analogue scale (0 cm = excellent, 10 cm = useless), was 3.32 +/- 3.1 cm at the last study visit (all patients included) and 2.33 +/- 2.25 cm after 18 months. Health status, general condition, morning stiffness, grip strength of right hand, Ritchie joint index, pain in the morning and pain at night all improved significantly. Efficacy was maintained through the study. Only 11.4% of patients discontinued prematurely due to lack of efficacy. Mean global tolerance was good. Twenty-eight per cent of patients experienced gastrointestinal (GI) adverse events, 21% musculoskeletal system disorders, 18% skin disorders and 15% respiratory disorders. Only 13.7% of patients discontinued due to adverse events. Severe GI effects, such as perforation, ulcer and bleeding, occurred in only three patients (0.8%). Withdrawals due to GI adverse events occurred in 3.9% of patients. Meloxicam 15 mg once daily was effective and compared favourably with standard NSAIDs regarding tolerance when administered to patients with RA over an 18 month period.

A questionnaire study on health of taxi drivers--relations to work conditions and daily life

A questionnaire survey was conducted on 5,523 taxi drivers in Osaka Prefecture to investigate their working conditions and daily life as well as characteristics of their health condition and various effects on health, especially on their cardiovascular system. The major results of this survey were as follows: 1) The rate of subjective symptoms, morbidity rate and rate of poor physical condition over the past year in the taxi drivers were higher than those of the control group. Among the subjective symptoms, the rate of gastrointestinal disorders, fatigue, musculoskeletal system disorders, sensory system disorders, hemorrhoids, etc. was especially high. 2) Work shift, density of work, years of experience in taxi-driving, frequency of fright while driving, pattern of taking meals, way of recuperation on rest days or holidays, obesity, smoking, and intake of coffee and alcohol were found to be factors affecting the health of taxi drivers. 3) Nearly half of the respondents said that they would like to quit or change their job with as much as 62% giving "condition of health" as reason. On the other hand, the rate of heart-related symptoms such as palpitation, and breathlessness did not differ from that of the control group. The possible reason for this deduced from the foregoing results, is that there were some who had changed or quit the job at an early stage for health reasons such as heart trouble and severe physical and mental burden resulting from taxi driving. 4) Many taxi driver are obese and the rate of those with heart-related symptoms was considerably high among those classified as obese. In addition, the results showed that those with longer driving experience tended to be obese.

Effect of acupuncture on disorders of musculoskeletal system in Nigerians.

Acupuncture was offered to patients with lesions affecting the locomotor system. These patients had received conventional physiotherapy treatment with limited success. The lesions treated by acupuncture were hemiplegia, low back pain, frozen shoulder, dropped foot, Sciatica, and arthritis of the knee and hip. Response to acupuncture was excellent in the more acute conditions of low back pain and frozen shoulder, where six treatment sessions were required for complete recovery. The more chronic conditions required many more treatment sessions and the improvement recorded was significant, though not complete. It was concluded that acupuncture has a valuable role to play in a physiotherapy department.

Pattern of Musculoskeletal Disorders among Diabetic Patients Attending a Tertiary Care Hospital in Dhaka

Disorders of musculo-skeletal (MSK) system are common conditions seen in diabetic patients. The present study investigated the pattern of MSK disorders among the Bangladeshi diabetic patients attending the Department of Physical Medicine and Rehabilitation of a tertiary care hospital in Dhaka over a period of one year. A total of 2062 diabetic patients with MSK disorders were included in the study. Degenerative joint diseases were more common (53.2%) than inflammatory arthropathies (20.1%). Ibrahim Med. Coll.
