Masking the Discolored Enamel Surface with Opaquers before Direct Composite Veneering

Conservative and aesthetic treatment of a single discolored tooth in the anterior zone is a restorative challenge. Intra-pulpal hemorrhage, dental trauma and endodontic treatment are common reasons for tooth discoloration. If a bleaching procedure cannot be implemented or has been tried without pleasing results, other conservative treatment plans should be considered under the scope of minimal intervention. A relevant application would be the construction of a direct composite veneer. The problem is that composite resins have inherent limitations in their opacification ability, especially when increments are very thin. Thus, in order to match the chromaticity of the adjacent teeth, the use of opaquers is suggested. Opaquers are highly pigmented resinous materials, containing metal oxides that are responsible for their potent opacification ability, characteristic tint and saturation. These materials can effectively mask a dark enamel surface, after minimal tooth preparation. Different composite layers can be stratified on the masked substrate according to the layering technique, emulating the natural dentine and enamel tissues. The aim of this study is to describe the use of opaquers in masking discolored enamel surfaces, prior to direct veneering with resin composites.

Integrated approach of ceramic and composite veneers in tetracycline stained teeth: A case report

Abstract Tooth discoloration is one of the most frequent reasons that one seeks dental care. The common causes of tooth discoloration are fluorosis, enamel hypoplasia, tetracycline staining etc. Based on the severity of discoloration, treatment may vary from more conservative treatment like bleaching to more extensive treatments involving full crowns and veneers. The purpose of this article was to report the advanced treatment options and their integrated use for generalized intrinsic discoloration caused by Tetracycline encompassing bleaching to lighten the discoloration followed by Direct and Indirect veneer restorations using Composite resin and Ceramic respectively. Direct composite laminate veneers: three case reports.

Re-establishing a patient’s lost dental esthetic appearance is one of the most important topics for contemporary dentistry. New treatment materials and methods have been coming on the scene, day by day, in order to achieve such an aim. Most dentists prefer more conservative and aesthetic approaches, such as direct and indirect laminate veneer restorations, instead of full-ceramic crowns for anteriors where aesthetics is really important. Laminate veneers are restorations which are envisioned to correct existing abnormalities, esthetic deficiencies and discolo-rations. Laminate veneer restorations may be processed in two different ways: direct or indirect. Direct laminate veneers have no need to be prepared in the laboratory and are based on the principle of application of a composite material directly to the prepared tooth surface in the dental clinic. Indirect laminate veneers may be produced from composite materials or ceramics, which are cemented to the tooth with an adhesive resin. In this case report, direct composite laminate veneer technique used for three patients with esthetic problems related to fractures, discolorations and an old prolapsed restoration, is described and six-month follow-ups are discussed. As a conclusion, direct laminate veneer restorations may be a treatment option for patients with the esthetic problems of anterior teeth in cases similar to those reported here.

Retreatment and surgical repair of the apical third perforation and osseous defect using mineral trioxide aggregate

One of the causes of non-healing periapical pathosis in endodontically treated tooth is root perforation. This can occur pathologically by resorption and caries, iatrogenically during endodontic therapy (zip, strip, furcal perforations). Root perforation results in bacterial contamination, periradicular tissue injury, inflammation, and bone resorption. The purpose of this case report is to describe endodontic retreatment and surgical management of a longstanding periapical lesion on maxillary lateral incisor, associated with perforation and osseous defects using mineral trioxide aggregate (MTA). Although the majority of bone support and root dentin was damaged, an attempt was made to repair the defect and restore the tooth. After the surgical intervention and root canal treatment, the perforation was subsequently sealed with MTA. Later, the root was reinforced with composites and the tooth was restored with direct veneer. Conclusion: Four-and-a-half year (54 months) recall examination showed no evidence of periodontal breakdown, no symptoms of further deterioration, and complete healing of periradicular lesions when examined by radiography. This case report presents a treatment strategy that could improve the healing process and beneficial outcomes for patients with perforation and osseous defect.

The Clinical Microscope and Direct Composite Veneer

This paper presents the advantages and limitations related to the use of a clinical microscope in restorative dentistry, and it demonstrates the aid of magnification during preparation and restoration of a direct composite veneer. Good illumination and visibility is important to adequately viewing the adjacent dental tissues so that the resin composite buildup can mimic natural teeth. The reproduction of details results in a naturally esthetic direct veneer.

An indirect veneer technique for simple and esthetic treatment of anterior hypoplastic teeth

This study describes a technique for treating anterior hypoplastic teeth using indirect nanocomposite veneer restoration. The prime advantage of an indirect veneer technique is that it provides an esthetic and conservative result. One of the most frequent reasons that patients seek dental care is discolored anterior teeth. Although treatment options such as removal of surface stains, bleaching, microabrasion or macroabrasion, veneering, and placement of porcelain crowns are available, conservative approach such as veneer preserves the natural tooth as much as possible. Full veneers are recommended for the restoration of localized defects or areas of intrinsic discoloration, which are caused by deeper internal stains or enamel defects. Indirectly fabricated veneers are much less sensitive compared to a operator's technique and if multiple teeth are to be veneered, indirect veneers can be usually placed much more expeditiously. Indirect veneers last much longer than the direct veneers. Therefore, indirectly fabricated veneers are more advantageous than directly fabricated veneers in many cases.

Direct veneer restoration using a nano ceramic composite material: a clinical report

Direct composite veneers can be used to mask tooth discolorations and/or to correct unaesthetic tooth forms and/or positions. However, these type of restorations are often regarded as one of the most challenging in aesthetic dentistry presumably due to the extent of natural tooth structure that must be recreated. This paper discusses easy application techniques and tips for Ceram.X Duo?, a nano ceramic composite restorative material. Its natural shading system allows the restoration of tooth with both dentine and enamel shade and transforms it into a final direct veneer restoration that mimics a natural tooth.

Finishing composite veneer restorations: The rainbow technique

Background and Objective: Direct veneer restorations, as a definitive treatment plan in esthetic therapies, have only been a second choice to porcelain veneers when it comes to the final esthetic result and longevity. Achieving a highly polished surface on the composite resin has always been a clinical challenge and numerous techniques and systems have been introduced to accomplish this. The aim of this article is to discuss the various tools and techniques available and to demonstrate the use and results of a recommended multi-step disk based polishing technique on various clinical cases restored with resin veneer build-ups. Materials , Methods and Conclusion: Three different clinical situations requiring esthetic anterior veneer restoration were restored with composite resin veneers. The completed resin veneers were finished and polished using a multi-step disk-based polishing system (Super-Snap® Rainbow Technique Kit, Shofu Inc.). The system was found to be very cost effective, easy to handle, hassle free, faster and therefore it definitely guarantees an excellent finish without a compromise on quality.

Endodontic treatment of an unusual case of fusion

This case report describes the endodontic and restorative treatment of two anterior permanent teeth with unusual external and internal anatomy due to lusion. A maxillary right central incisor had fused with a supernumerary and the maxillary left central incisor had fused with the left lateral incisor. The complexity of the root canal systems in each tooth required that different endodontic obturation techniques be used. The restorative treatment involved large complex chamber-retained composite buildups and direct veneers.
