The healing process of diabetic ulcers correlates with changes in the cutaneous microbiota

Skin microbiota plays an essential role in the development and function of the cutaneous immune system, in the maintenance of the skin barrier through the release of antimicrobial peptides, and in the metabolism of some natural products. With the aim of characterizing changes in the cutaneous microbiota specifically associated with wound healing in the diabetic condition, we performed a 16 S rRNA gene Next Generation Sequencing of skin swabs taken within the ulcer bed of ten diabetic patients before (t0) and after 20 days of therapy (t20) with a fluorescein-based galenic treatment. Considering the twenty most representative genera, we found at t20 an increase of Corynebacterium, Peptostreptococcus, and Streptococcus, and a decrease of Enterococcus, Finegoldia, and Peptoniphilus genera. However, differences were not significant due to the high variability among samples and the small patient cohort. S. aureus was the most abundant species at t0 and was reduced by therapy in four patients. Comparing the microbiome in the ulcer bed and in the perilesional tissue of the same patient at t0, no major differences were observed. Taken together, our data indicate that in the absence of antibiotic-based therapy the healing process of diabetic ulcers is accompanied by changes in the microbiome composition.

Pengaruh Zinc Chitosan Cream terhadap Proses Penyembuhan Ulkus Diabetikum pada Pasien Diabetes Mellitus Tipe II

The aim of this study was to analyze the healing process of diabetic ulcers in Type II Diabetes Mellitus patients on day 1, day 7 and day 14 at the Asri Wound Care Center Medan clinic. The research method used is Quasi Experimental research using a one group pretest-posttest design. The intervention used is Zinc Chitosan Cream. The tests used are the Kruskal Wallis Test and the Wilcoxon Test. The research results show that the Sig. (2-tailed) day 1 to day 7 for an Asymp Sig value of 0.000 < 0.05 there is a significant difference, day 7 to day 14 for an Asymp Sig value of 0.157 < 0.05 there is no significant difference, day 1 to day for the 14th Asymp Sig value of 0.000 < 0.05, there is a significant difference, it can be concluded that the effect of Zinc Chitosan Cream on healing diabetic ulcers in Type II Diabetes Mellitus patients only affects days 1 - 7 and day 1 - 14, whereas for days 7 – day 14 in healing diabetic ulcers in patients with Type II Diabetes Mellitus does not affect the wound healing process. In conclusion, Zinc Chitosan Cream in healing diabetic ulcers in Type II Diabetes Mellitus patients only has an effect on days 1 - 7 and days 1 - 14. Healing of diabetic ulcers using jamaican cherry leaf extract cream (Muntingia calabura l.) based on collagen density in white rats (Rattus norvegicus)

Abstract This research aims to determine the potential of jamaican cherry leaf extract cream (JCLEC) to accelerate the healing process of diabetic ulcers in white rat based on collagen density. The research used 4 month old male white rats (Rattus norvegicus) which were divided into K0 (negative control), KP (silver sulfadiazine), K1 (5% JCLEC), K2 (10% JCLEC), and K3 (15% JCLEC). An incision was made on the back of the rat then given JCLEC. Days 3, 7, and 14 after treatment, the wound tissue was collected after the rat were euthanized. Skin tissue was processed and stained with picrosirious red. The rate of wound healing was observed. Collagen density was presented in scoring. Based on the treatment group and observation time, there was a significant difference in collagen density between the treatment groups compared to the control (P<0.05). Based on the treatment group, the scoring of the collagen density was 1.78±0.6 (K0); 2.56±0.88 (KP); 1.78±0.4 (K1); 2.56±0.88 (K2); and 3.1±0.78 (K3). Based on observation time, the average collagen density scoring was 1.8±0.3 (D-3); 2.6±0.4 (D-7); and 2.8±0.6 (D-14). The conclusion is the 15% JCLEC has the potential to accelerate the healing of diabetic ulcers in white rats by stimulate collagen density.

Effectiveness of Modern Dressing on Healing Diabetic Foot Ulcer: A Literature Review

Background: Diabetes mellitus is a group of heterogeneous disorders that have indications in the form of increased content of high blood sugar levels. WHO also confirmed DM to be the 7th trigger of death. IDF (International Diabet Federation) data estimates from survey results after North America in 2017 the 3rd place was occupied by Southeast Asia and people with diabetes mellitus with a percentage of 8.5% at the age of 20-29 years in North Africa. One of the treatments for patients with diabetes mellitus can be wound care using Modern Dressing with the Moist Wound Healing method. Methods: The method used in this wound treatment is the Modern Dressing technique with the Moist Wound Healing method. This procedure can clinically grow tissue 30-50%, increase collagen synthesis by 50%, and the rate of re-epithelialization with moist conditions is 2-5 times faster and can reduce the amount of fluid from above the wound surface. Results: evaluation shows that Modern Dressing wound care with the Moist Wound Healing method is in fact able to maintain moist conditions in the wound, reduce inflammation in the wound so that it can improve and accelerate the healing process of diabetic ulcers. Conclusion: The results showed that the Modern Dressing technique with the Moist Wound Healing method was able to improve the healing process of diabetic ulcers. The results also showed that there were significant changes between modern and conventional dressings. The modern dressing group had a much better development than the conventional dressing. The application of the Moist Wound Healing method in patients with diabetic ulcers can provide a much faster wound recovery reaction.

Healing Diabetic Ulcers with Pure Honey dressing: Literature Review

Poor peripheral circulation in patients with diabetes mellitus causes wounds to be difficult to heal. Therefore, patients with diabetes mellitus with diabetic ulcers require wound care to reduce the risk of amputation. One alternative material that can be used for wound care is the topical application of pure honey. The purpose of the study was to determine the effect of honey dressing on the healing process of diabetic ulcers through a literature review. The databases used include PubMed, ScienceDirect, Google Scholar, and Garuda. Keywords using (((honey) OR honey dressing) AND wound healing) AND diabetic ulcer) OR diabetic foot ulcer. The library sources used are from 2010-2020. The search results obtained 9,507 articles. Furthermore, articles were selected based on inclusion and exclusion criteria, obtained 11 articles. The assessment of articles uses a checklist on the JBI (Joanna Briggs Institute) assessment list. Eleven articles that met the assessment criteria were then analyzed one by one. The results showed that eleven articles showed the effect of giving honey dressing on the healing process of diabetic ulcers. A total of 3 articles showed wound healing based on changes in score, 2 articles based on changes in grade, 5 articles based on changes in wound characteristics, and 2 articles based on healing time. Of the eleven journals analyzed, honey dressing on diabetic ulcers were most effectively used in wound Grade I and II (Wagner Classification) ulcer patients who received drug therapy with a GDS <200 mg/dL and received daily wound care for 1 month. Healthcare professionals may consider using honey dressings for the treatment of diabetic ulcers. In addition to a more affordable price, honey has good benefits for wound healing. Further research is needed on the type of honey that is more effective as an ingredient in wound care.

PENGARUH DUKUNGAN KELUARGA TERHADAP KEPATUHAN PASIEN MENJALANI PERAWATAN LUKA ULKUS DIABETIK

Diabetic ulcers are open sores on the skin layer to the dermis, which usually occur on the soles of the feet. Wounds that arise in DM patients due to complications of microangiopathy and macroangiopathy. Peripheral neuropathy will cause loss of sensation in the distal area of ​​the foot. The length of time a person suffers from DM will cause microangiopathic complications so diabetic neuropathy will cause ulcers on the feet. This study aims to determine the relationship between family support in the healing process of diabetic ulcers with patient compliance in undergoing treatment at the Asri Wound Care Center clinic in Medan using a descriptive correlation research design with a cross-sectional design. The sample in this study was 36 respondents using an accidental sampling technique. The results of the Spearman rank test (rho) with a p-value = 0.004 with a coefficient value of 0.467 then p-value <a (0.000 <0.05), meaning that there is a relationship between family support in the healing process of diabetic ulcers with patient compliance in undergoing treatment at the Asri Wound clinic. Care Center Medan. For education, it is expected that educational institutions can increase the knowledge of students at the institution regarding the importance of family support in the process of healing diabetic ulcers with patient compliance in undergoing treatment.

Literature Review: Efektifitas Metode Moist Wound Healing pada Ulkus Diabetik

One of the complications of diabetes mellitus is the presence of ulcer wounds which cause 50% to 75% of amputation. This literature review aims to determine the effectiveness of the Moist Wound Healing method in diabetic wounds. The design of this scientific paper is a literature review search using an electronic database, namely google scholar, semantic scholar, and pubmed. The keywords used in the search were: diabetes mellitus AND moist AND diabetic ulcer. Based on a literature review of 4 articles, significant results were obtained in wound healing, namely 2-3 times faster healing. it can be concluded that the moist wound healing method can affect the healing process of diabetic ulcers and has effectiveness for wound healing.

Modern Wound Treatment with Moist Wound Healing Effective for Healing Diabetic Ulcus

Diabetes Mellitus is a metabolic disease that is chronic and requires ongoing medical care in a variety of ways that can reduce the risk of multi-factors beyond glycemic control. Uncontrolled diabetes mellitus will cause an increase in complications, one of which is diabetic ulcers. Achieving better wound healing is the main goal of diabetic ulcer management. Moisturizing dressings can provide a supportive environment for cells to carry out the wound healing process and prevent further damage or trauma. This study was to determine the effect of modern dressing with moist healing on diabetic ulcers. This type of research is a quantitative research with a quasi-experimental research design with an accidental sampling approach. Data was collected using demographic data and the Wocare For Indonesian Nurses (WINNERS) Scale assessment sheet which is a modified Bates-Jasen score to predict and evaluate the average score of wound healing. WINNERS scale consists of 10 assessments. The results of the intervention that have been carried out have shown a significant improvement in diabetic ulcers as assessed using the Bates Jensen Wound Assessment Tools. Based on the results and discussion, it was concluded that there was a significant effect of modern wound care using the moist wound healing method on the healing process of diabetic ulcers.

Comparison of insulin soaked dressing with the conventional dressing in diabetic ulcers.

Objective: This study aims at demonstrating the benefits of insulin soaked dressing for the treatment and accelerating the healing process of diabetic ulcers thus helping the faster wound healing, reducing morbidity and also decrease the cost of management in patients with diabetic ulcers. Study Design: Randomized Controlled Trial. Setting: Surgical Unit 1, Holy Family Hospital, Rawalpindi Medical University, Rawalpindi. Period: November 2016 to May 2017. Material & Methods: The study was initiated after approval from the institutional research forum of Rawalpindi medical college. Patients fulfilling the selection criteria reporting at surgical unit – I, Holy Family Hospital, Rawalpindi were included in the study. Written informed consent was taken. Random allocation of the study participants to either study group was done by using lottery method, 60 patients were randomly allocated to either group A(insulin soaked dressing) or Group B (conventional dressing), each with 30 patients. Results: In Group A, insulin soaked dressing was applied whereas patients in Group B were applied with conventional dressing. Mean age (years) of patients was 48.04+13.45 whereas there were 32 (53.3) male and 28 (46.7) female patients. In the study, mean duration (day) of complete wound healing in patients with diabetic ulcers receiving insulin soaked dressing and conventional dressing was 39.80 + 8.04 and 47.60+6.52 respectively which was statistically significant (p-value 0.000). Conclusion: The study concluded that in patients with diabetic foot ulcer, average duration of complete wound healing was significantly less in patients with insulin soaked dressing as compared to conventional dressing.

Modern Dressing Applications Use Hydrogel as Autolysis Debridement in Patients Diabetic Ulcer

Objective: This study was conducted to present a wound care intervention using hydrogel dressing in diabetic ulcer patients with the principle of modern dressing to speed up the wound recovery process. Method: This study used the nursing care method, including assessment, formulating nursing diagnoses, interventions, implementation, and evaluation, which were carried out for 6 days with 3 meetings at the patient's house located at Kp Benteng Kel Sukamenak, Purbaratu District. Enforcement of nursing diagnoses refers to the Indonesian Nursing Diagnosis Standards (SDKI). Indonesian Nursing Intervention Standards (SIKI), Indonesian Nursing Outcome Standards (SLKI), and nursing evaluations are documented using the SOAPIER method. Result: The provision of wound care interventions using hydrogel with the principle of modern dressing has proven to be useful for improving the wound healing process of diabetic ulcers. The benefits of this application will be maximized if done regularly and gradually. Conclusion: Based on the entire series of nursing care processes that the author carried out on Ny. H with the problem of infection risk associated with diabetic ulcers, the authors can draw the conclusion that hydrogel administration as debrilytic autolysis has significant results in 6 days with 3 meetings.

EFFECTIVENESS OF THE BARK DREDGING OF THE JAVA WOOD TREE (LANNEA COROMANDELICA) ON THE DIABETIC ULCER HEALING PROCESS

Background: Diabetic ulcer is a micro vascular complication that can be experienced by all DM patients. This condition necessitates appropriate wound care management to prevent infection, reduce necrotic tissue, and enhance the wound healing process. This study aims to determine the effect of using Javanese tree bark scraping on the healing process of diabetic ulcers, namely changes in the size of diabetic ulcers.
 Methods: This type of research is a Quasy Experiment with a Pre-test Post-test approach with control group. The populations in this study were all diabetic ulcer patients who came for wound care at Puskesmas Puuwatu totaling 16 respondents.
 Result: The results of statistical tests show that the p-value for the intervention group is 0.317 and the control group is 1,000, which means that there is no difference in wound size before and after the intervention in the intervention group and the control group. In the Mann Whitney statistical test, it was found that the p-value in the intervention group was 0.189, which is greater than the value of ? = 0.05.
 Conclusion: The dredging of the bark of the Java tree is not effective against changes in wound size in patients with diabetic ulcers.

Potensi Ekstrak Daun Bandotan (Ageratum conyzoides L.) Sebagai Spray Untuk Pemulihan Luka Mencit Diabetik Yang Terinfeksi Staphylococcus aureus

Ageratum conyzoides L. leaf has the main compound of flavonoids and alkaloids that contribute to accelerate the healing process of diabetic ulcers with Staphylococcus aureus infection that can affect the quality of life of patients. This study aims to test the leaves of Ageratum conyzoides L. on the recovery of diabetic ulcers of mice infected with Staphylococcus aureus. The method used is the extraction of Ageratum conyzoides L leaves as the material of spray 15%, induction of diabetic compound (Alloxan monohydrate), length wound (1cm) and bacterial infections, and spray application with 1 times daily frequency, 2 times daily, 3 times a day for 9 days. The results showed that the lenght of wounds in group A, B, C, D, and E were 0,5; 0,8; 0,4; 0,2; 0,0. The administration of Ageratum conyzoides L. spray which has active flavonoide substances, alkaloid substance, and saponin substances has an effect on decreased length of diabetic wounds and restoration of dermis tissue structure (increases epithelial cells proliferation, initiates fibroblast cells, and stimulates type 1 collagen). Spray treatment 3 times a day (group E) was more influential than the other treatments, which experienced wound closure in 9 days. Thus, Ageratum conyzoides L. leaf spray is potential for therapy of diabetic ulcers infected with Staphylococcus aureus.

This study aimed to analyze the effect of hydrocolloid turmeric against diabetic ulcer healing process. This research was conducted by case study on 7 respondents, wound care with hydrocolloid tumeric at 21 days. At 7 patient with diabetic foot ulcers were intervention wound care with hydrocolloid tumeric at 21 days, showed score decrease in degree of wound healing, based Betes Jensen Scale.Â

The purpose of this research is to describe the characteristics of respondents, knowing Diabetic Ulcer healing process without lower extremity elevation, knowing Diabetic Ulcer healing process with lower extremity elevation and knowing the effectiveness of Diabetic Ulcer healing without lower extremity elevation and with lower extremity elevation. This research is to design a queasy experiment non-equivalent control group design and analysis data used Chi-Square test. Results of this research are the elevation of lower extremities more effectively to increased Ulcers Diabetic healing process. It’s evidenced by the Chi-Square test using Pearson Chi-Square analysis and obtained ρ = 0,027 and α = 0,05. Elevation of lower extremities more effectively to increased Diabetic Ulcer healing in patients with Diabetic Ulcer in Melati I RSUD Dr. Moewardi. Lower extremity elevation expects can be applied in patients with Diabetic Ulcer.

Combined Use of Acid Fibroblast Growth Factor, Granulocyte Colony-stimulating Factor and Zinc Sulphate Accelerates Diabetic Ulcer Healing

Our previous studies demonstrated that topic application of recombinant human acid fibroblast growth factor (aFGF) significantly, but still not completely, improved in diabetic ulcer healing. To obtain a maximal therapy for diabetic ulcer healing, a combined protocol containing aFGF, anti-oxidative reagent zinc (Zn) and stem cell stimulator granulocyte colony-stimulating factor (G-CSF), i.e.: aFGF/G-CSF/zinc sulphate (ZnSO4), was explored in the present study. Diabetes was induced by a single dose of streptozotocin (STZ, 55 mg/kg) in Sprague Dawley rats, and full thickness skin wound was made in diabetic rats at 2 months after diabetes onset. Diabetic ulcer rats were treated with aFGF, G-CSF, ZnSO4, aFGF/G-CSF/ZnSO4 or vehicle control, respectively and 3, 6, 9, 12, 15, 18, 21 and 28 days later, therapeutic effects were evaluated by calculating ulcer area. On day 7, 14, 21 after treatment, rats were sacrificed to collect 2 pieces of dorsal skin from 3 different sites (wound center, edge and healed area) to perform histopathological examination. Results showed that treatment with aFGF/G-CSF/ZnSO4 significantly enhanced ulcer healing compared with single drug treatment groups at different time points. Healing times for 100% of the wound in the aFGF/G-CSF/ZnSO4 group was 20.00 ± 1.15 days, and significantly shorter than those in single treatment groups (p < 0.05). Histopathological and immunohistochemical analysis disclosed significant increases in capillary density, proliferating cells, the expression of tissue inhibitor of metalloproteinase 1 (TIMP-1) and the ratios of TIMP-1 to matrix metalloproteinase 1 (MMP-1) in aFGF/G-CSF/ZnSO4 group as compared to other single treatments. Collectively, the combinative protocol of aFGF/G-CSF/ZnSO4 significantly enhanced the therapeutic effect on diabetic ulcer wound, probably through the promotion of fibroblast proliferation and differentiation, enhancement of blood vessel regeneration, and up-regulation of TIMP-1 and down-regulation of MMP-1 expression in diabetic ulcer healing process.
