Practical advice for new diabetes specialist nurses

AbstractThe number of nurses specialising in the care of people with diabetes is increasing rapidly. On Merseyside in 1984 only four hospitals employed diabetes specialist nurses. However, six years later there are at least 16 specialist nurses working in the ten health districts and each district employs at least one specialist nurse.The British Diabetic Association (BDA) recommends that each district health authority should have, as an absolute minimum, one nurse whose full time work is with diabetic patients and their families. We are likely to see many more appointments made nationally in the next decade.Several useful articles have been written on the role of the diabetes specialist nurse1–5 but these contain limited practical guidelines for the newly appointed person.I have recorded my observations and experiences over my first two years as a diabetes specialist nurse and suggest guidelines for nurses joining this speciality.

Home blood glucose monitoring in children with insulin‐dependent diabetes mellitus

AbstractHome blood glucose monitoring (HBGM), if performed correctly, is potentially the most satisfactory way of monitoring diabetic control. This study describes the prevalence, pattern and performance of HBGM in 171 children, aged 10‐16 years, with insulin‐dependent diabetes mellitus (IDDM). All children had access to a diabetes specialist nurse (DSN) and all but two performed blood tests routinely, with 82% maintaining a record book. Blood tests were found to be adequate in number but failed to cover the ideal spectrum of test points over a one week period. The majority of children were responsible for their own HBGM and 79% altered their insulin without first consulting a doctor or nurse; 28% of those altering insulin did so without parental advice. Only 56% of children performed tests competently; skill was associated with age but not with duration of diabetes. Educational strategies are suggested.

Diabetes education and schools

AbstractA questionnaire was sent to schools, prisons and industries in the Trent region using addresses supplied by diabetes health visitors or specialist nurses to discover the level of diabetes education and whether it was thought to be adequate. Forty four (59%) of the 75 responded. Twenty four schools had received literature and all found it useful or adequate. However only 50% felt there was a need for educational facilities for children with diabetes within their school, but 56% thought there was a need for educational facilities for staff who deal with children with diabetes.

Psychological Care, Counselling and Diabetes Part 2: Medical Perspective

Future advances in the management of people with diabetes are becoming increasingly unlikely to take the form of new insulin preparations, delivery devices or drugs. The responsibility for diabetes management is increasingly being given back to patients themselves. Improvements in the management of diabetes may be more likely to come from an appreciation of how psychological well-being can have a major effect on an individual's ability to manage their diabetes. In this article I have described some examples where an awareness of these problems may allow for planning of their appropriate management. The ideal plan of action would be for the 'Diabetic Team' to meet with a psychologist in order to identify the needs of their clinic population. Plans can then be made for meeting those needs. Individuals can be identified for training in the appropriate skills. Counselling courses are widely available and some with diabetes particularly in mind already exist. Resources and time can then be set aside specifically for implementation of this care. Individual local resources and enthusiasm would decide what form this should take and whom, if anyone, from the 'Diabetes Team' should undergo training. There is already an awareness that Diabetes Nurse Specialists may require counselling skills and the opportunity to use these skills, and some 'Diabetes Teams' currently have a psychologist as a member. Overall, the most important change may be the recognition that specialist psychological advice and, hopefully, collaboration is required to provide a satisfactory standard of health care.

Diabetes Guidelines

A new folder has just been launched entitled 'Diabetes clinical guidelines for practice nurses', compiled by diabetes specialist nurses and practice nurses experienced in diabetes care. The folder is aimed to assist practice nurses who are about to or have recently become involved in the care and education of diabetic patients.

Community diabetic clinics and the diabetes specialist nurse

AbstractCommunity clinics run entirely by a diabetes specialist nurse have been established in Southall, in response to the high prevalence of diabetes within the Asian community. These have proved popular with patients and general practitioners. In an audit of 102 patients, the treatment had been altered by the nurse in 51 patients (11 on insulin and 40 on oral hypoglycaemic therapy), 13 had been referred for antihypertensive treatment, eight for chiropody, and six with abnormal fundi for further assessment. Two of the latter patients had severe maculopathy and were referred for consideration of laser therapy. Suitably trained diabetes specialist nurses may play an increasingly important role in diabetes care within the community.
