Desmoplastic ameloblastoma in maxilla mimicking fibro-osseous leison: A case report

Desmoplastic Ameloblastoma accounts for 4% to 13% of all ameloblastomas and is a rare variant with high rate of recurrence. The desmoplastic variant of ameloblastoma usually appears in the anterior and premolar regions and sometimes resembles a benign fibro-osseous lesion showing a mixed radiolucent and radiopacity in the radiographic examination. Malignant transformation with repeated postsurgical recurrences have also been reported. In this paper we present a case of a 22-year-old female with swelling in the left upper jaw which turned out to be desmoplastic ameloblastoma. The pathologist and the clinician should be aware of the concepts and the association with malignant transformation and spread of the lesion in order to deliver appropriate treatment and to avoid further recurrences of the leison.

Desmoplastic ameloblastoma of maxilla: Radiologic–Pathologic correlation

Desmoplastic ameloblastoma (DA) is a rare variant of ameloblastoma, and less than 170 patients have been reported in the literature. The present case deserves special importance because of its rare occurrence, unfamiliar appearance, potentially aggressive nature, and high chances of misdiagnosis. Moreover, the radiographic features of this lesion rarely point towards ameloblastoma. Desmoplastic variant of ameloblastoma tends to infiltrate between bone trabeculae. The risk of recurrence is high. This report is an attempt to help the dental community in developing familiarity with the clinical presentation and at the same time advocating to develop a high index of suspicion in recognizing such cases.

Desmoplastic Ameloblastoma —A Case Report

The desmoplastic ameloblastoma (DA) is a rare variant of the conventional ameloblastoma. It accounts for only 4% to 13% of all ameloblastomas. Desmoplastic ameloblastoma (DA) was included in the World Health Organization Classification of Head and Neck Tumors (WHO-2005) as a variant of ameloblastoma with specific clinical, imaging and histological features. The desmoplastic variant of ameloblastoma usually appears in the anterior and premolar regions as a mixed radiolucent and radiopaque lesion, sometimes resembling a benign fibro-osseous lesion [1]. Ameloblastoma is a locally aggressive tumor that may cause recurrence and in rare cases, malignant transformation with repeated postsurgical recurrences [2] [3]. In this paper we present a case of a 47-year-old female with swelling in the left upper jaw, bopsy of which turned out to be desmoplastic ameloblastoma.

Desmoplastic Ameloblastoma of the Mandible

The Desmoplastic Ameloblastoma exhibits important differences in anatomical distribution, histologic appearance and radiographic findings compared with the classic type of ameloblastoma. We describe a case of desmoplastic variant of ameloblastoma localized in the mandibular anterior portion of the jaw, extending posteriorly in the left side. The tumor was expansile, slowly growing and painless. The radiographic finding showed diffuse, radiolucent lesion with cortical bone expansion. Surgical excision was performed and follow-up examination done after two years disclosed no recurrence. The purpose of this article is to assist in better understanding of this variant of odontogenic tumor.

Recurrent desmoplastic ameloblastoma of mandible: A case report

Desmoplastic variant of ameloblastoma is rare. The surgical extirpation of ameloblastoma in mandible is challenging. Furthermore, the airway adequacy is crucial, both during and after mandibulectomy. The surgical reconstruction outcome has been satisfactory up to 12 months post surgery. Nonetheless, recurrence is possible and therefore frequent follow up is needed.

Peripheral desmoplastic ameloblastoma: report of a case and literature review

The desmoplastic variant of ameloblastoma is a rare, locally invasive, benign tumor of the jaws, typically found in the anterior regions of the maxilla and mandible. This ameloblastoma variant has been described as an intraosseous tumor; to our knowledge, there have been no published cases of a peripheral desmoplastic ameloblastoma. The purpose of this case report is to describe a patient with a desmoplastic ameloblastoma localized to the soft tissue of the palate with no osseous involvement.

Follow-up examination one and two years later disclosed no recurrence of tumor. The desmoplastic variant of ameloblastoma tends to infiltrate between bone trabeculae. Curettage leaves islands of tumor within bone, which later manifest as recurrences. Therefore, block excision is the most widely used treatment to avoid recurrence.

Desmoplastic ameloblastoma of the mandible: A case report and review of the literature

The desmoplastic variant of ameloblastoma was first described by Eversole et al1 in 1984. Since then, 80 cases have been reported in the literature.1-22 The histologic pattern of desmoplastic ameloblastoma (DA) is characterized by extensive stromal collagenization, or desmoplasia.13 Immunohistochemical studies suggest that the desmoplasia originates from de novo synthesis of extracellular matrix proteins. DAs do not present with radiographic or clinical features that are typical of other variants of ameloblastoma. The radiographic presentation is similar to that of many benign fibro-osseous lesions, so this newly recognized variant should be considered in the differential diagnosis of patients who have tumors that resemble fibro-osseous tumors of the mandible and maxilla. The purpose of this article is to present the case of a patient with DA and a comprehensive review of the literature and to discuss treatment options and outcomes.

Desmoplastic variant of ameloblastoma in Chinese patients

Desmoplastic ameloblastoma is a rare tumour, and we know of only 43 previously reported cases. We report seven Chinese patients (five men and two women) with the desmoplastic variant of ameloblastoma, which makes up 9% of all ameloblastomas diagnosed during the years 1981–1995. The age ranged from 18 to 68 years (mean 43). Five of the tumours were in the maxilla and two were in the mandible. Five of them were situated anteriorly, the remaining two cases involving both anterior and posterior maxilla. The features of the 42 cases previously reported were reviewed and were compared with those in the present study. Our results differ in that we found a male predominance, wider age range and more tumours in the maxilla. Histologically, this variant of ameloblastoma is characterized by abundant collagenous stroma. Because the epithelial clusters may show prominent squamous metaplasia or may be compressed into thin strands in most areas, the appearance may mimic a squamous odontogenic tumour or odontogenic fibroma. The behaviour of this variant of ameloblastoma is likely to be the same as that of the classic ameloblastoma.

Unusual ameloblastoma with extensive stromal desmoplasia

A clinicopathological study of 5 cases of unusual ameloblastoma was performed. Three cases were characterized by marked stromal desmoplasia. The lesions occurred in the tooth-bearing areas of the anterior or premolar regions of the jaw. They were relatively small and showed a honeycomb appearance radiographically. Histologically, the islands of tumour cells were small and discrete in the widely extended desmoplastic stroma. They lacked obvious ameloblastic polarity in many parts. The other 2 cases were hybrid lesions of desmoplastic and conventional types. The former appeared in the alveolar part and the latter in the lower part of the mandible. It was concluded that the desmoplastic variant of ameloblastoma could be categorized as a new distinct type histologically, clinically and radiographically.
