Laboratory Quality Assurance in the Department of Restorative Dentistry at the University of Illinois at Chicago, College of Dentistry

The purpose of this study was to evaluate data collected in University of Illinois at Chicago College of Dentistry (UIC COD) laboratory quality assurance (QA) forms, analyze the collected data, and create a report of the findings. The goals of the study were to (1) identify the most common mistakes occurring during laboratory and clinical procedures when performing prosthodontic treatment, (2) note the incidence and trends of mistakes made by D3, D4, and IDDP2 students, and (3) observe any differences in the types of mistakes made by D3, D4, and IDDP2. QA data from June 1, 2007 to May 31, 2009 were evaluated based on forms gathered from the QA dental laboratory from all D3, D4, and IDDP2 students' submissions. All students had graduated from the UIC COD at the time of collection. Data were recorded for type of errors made in submission of laboratory work (Indirect Restorations [IR], Removable Partial Dentures [RPD], Complete Dentures [CD]), year of student in dental school (D3, D4, IDDP2), and frequency of rejection for each respective student. The frequency of common mistakes were pooled, evaluated, and reported by respective class year. The five most common laboratory submission errors for D3, D4, and IDDP2 students were nearly the same among student years for IR, RPD, and CD. D4 students had disproportionately higher numbers of work rejections compared to D3 and IDDP2 students. D4 students had a higher percentage of laboratory submission errors compared to D3 students for all laboratory procedures. There were similar types of errors noted between foreign-trained students (IDDP2) and domestically trained students (D3, D4).

A Comparative Evaluation of DIAGNOdent and Caries Detector Dye in Detection of Residual Caries in Prepared Cavities

The objective of this study was to determine the association between DIAGNOdent laser and caries detector dye in detection of the remaining caries in restorative cavities. The sample consisted of 100 cavities prepared in patients referring to the Department of Restorative Dentistry of Mashhad Dental School. After confirming caries absence by tactile examination, the presence of any residual caries was determined by a laser fluorescence (LF) device (DIAGNOdent Pen) and then by caries detector dye. The data were analyzed through McNemar test. When the cut off value was considered as ≥13, both DIAGNOdent Pen and caries detector dye found 54 cavities as without caries and 12 cavities as carious. There were 32 teeth diagnosed as decayed only by the dye and two cases that were diagnosed as having residual caries only by the DIAGNOdent. The McNemar test revealed a significant difference in the diagnosis of residual caries between the two methods (p < 0.05), as well as significant differences between each method and tactile examination (p < 0.05). When the cut off value was set at ≥25, no significant difference was found between laser fluorescence and tactile examination in residual caries detection (p > 0.05). Both DIAGNOdent Pen and caries detector dye can be considered as adjuncts for detecting residual caries in prepared cavities. However, the use of laser fluorescence device can provide results that are more consistent with tactile examination, while relying on caries detector dye may result in excessive removal of tooth tissue, and thus increase the risk of pulpal exposure. Incomparision with caries detector dye, Residual caries detection by DIAGNOdent Pen is more consistent with tactile examination.

Clinical educators’ views on the quality of undergraduate clinical restorative dentistry in the UK and ROI

Following concerns expressed regarding recently expanded dental education, a survey was conducted on United Kingdom and Republic of Ireland clinical educators' perceptions of their work and the educational experiences they provide in clinical restorative dentistry. Many of the challenges facing dental education are common to all countries, and it would be of interest to note if the views reported are representative of the greater European perspective. The results of this survey represent a European regional picture. This is a survey of clinical staff in restorative dentistry departments across all of the fully established UK and ROI dental schools. The survey covered factors affecting the quality of undergraduate learning in restorative dentistry and the implications of these for programme delivery. Thematic analysis was applied to qualitative responses. One hundred and eighty-eight responses were received across the gamut of clinical educators' roles and across 15 of the 16 schools. Themes identified were personal workload, teaching effectiveness, students' clinical experiences, provision of teaching, departmental organisation and job satisfaction. The survey findings had many parallels with other recent reports but also suggested some developments. The survey provided an accurate snapshot of staff views on the quality of undergraduate clinical training in restorative dentistry. Despite experiencing difficulties in many areas of their work resulting in poor morale, staff considered teaching rewarding although undervalued by others. A number of recommendations are made for the development of undergraduate teaching.

Technical quality of root canal fillings performed in a dental school and the associated retention of root‐filled teeth: a clinical follow‐up study over a 5‐year period

The aim of this study was to examine the technical quality of root canal fillings performed in a dental school and to investigate the associated effect on the survival/retention of root-filled teeth. A review of case notes of patients who had root canal treatment performed in the department of Restorative Dentistry, University Dental School and Hospital, Cork, Ireland was carried out. The technical quality of the root canal filling was described according to its relationship with the radiographic apex on a post-treatment radiograph. Tooth status at review was defined as 'tooth present' or 'tooth absent' based on the presence or absence of the root-filled tooth recorded in the treatment records at a review appointment following placement of the root canal filling. One hundred and forty-eight teeth (129 patients) were considered. Of these, 69.6% (n = 103) were of acceptable technical quality, 23.6% (n = 35) were under-extended, and 6.8% (n = 10) were overextended. An increased number of intra-treatment radiographs to confirm the relationship of the canal preparation to the radiographic apex and operator experience were significant predictors of adequate root canal fillings (P < 0.05). Eighty-three per cent (n = 123) of teeth were present at a review appointment held an average of 40 months following completion of treatment (12-60 months). The technical quality of the root canal filling was the only significant factor in predicting tooth survival (P < 0.05), while the presence of pre-treatment periapical pathology had no significant effect on survival of the root-filled tooth. Determination and maintenance of the working length of the canal system is an important feature in producing good quality root canal fillings, which in turn, is associated with increased likelihood of survival/retention of root-filled teeth.

Endodontic treatment of the teeth with different root canal curvatures: Two case reports

Endodontic treatment of the teeth with severe root canal curvatures has become one of the greatest problems in clinical practice. Recently, new instruments and techniques have been introduced in the endodontic procedure. The aim of this study was, on the basis of clinical practice, to show the possibilities of the endodontic treatment in the teeth with severely curved root canals, after dependable decision about endodontic instruments and techniques. This article presents a complete endodontic procedure in two teeth with different curvature shape of the root canal in patients that were treated at the Department of Restorative Dentistry and Endodontics, School of Dentistry, Belgrade. Coronal to apical 'crown down' technique was used for the root canal preparation with hand NiTi ProTaper instruments with progressive multicone design. The main reasons for choosing manual technique were complexity, shape of presented curvatures and better tactile sensation control of canal instruments in order to avoid possible mistakes and complications. Clinical X-rays of the teeth with permanent obturation clearly showed that extremes of the root canal morphology could be successfully solved by the right choice of all factors during endodontic procedure.

Frequency of endodontic treatment in a Nigerian Teaching Hospital

Objective: To determine the frequency of endodontic treatment in dental patients treated in a Nigerian Teaching Hospital. Method: A survey of 470 patients undergoing endodontic treatment at the conservative clinic of the department of restorative dentistry of Lagos University Teaching Hospital was conducted to determine the frequency of endodontic treatment for each tooth, number of males versus females that needed endodontic treatment and the frequency of root canal therapy in the maxilla compared to mandible. The data on age, sex of patients, date of treatment and tooth treated were retrieved from patients\' dental records. Results: Endodontic treatment was carried out on a total of 490 teeth. The frequency of endodontic treatment was more in the females (56.4%) compared to males (43.6%). 58% of the treated teeth were maxillary teeth and 42% were mandibular teeth. There was predominance of endodontic treatment in the molars (49.8%) with the lower first molar being the most frequently treated and accounted for 18.8% of teeth treated. Conclusion: A high proportion of endodontic treatment was now frequently performed on molars in patients treated at Lagos University Teaching Hospital dental centre which suggested the availability of more dentists with better facilities to attend to patients that needed endodontic treatment. The use of digital photographs to locate implant abutment screws for implant-supported cement-retained restorations

(J Prosthet Dent 2008;100:238-239) Cement-retained implant-supported restorations are commonly used, and have advantages and disadvantages. Retention of implantsupported crowns by using cement eliminates unesthetic screw access openings. Another advantage is the potential to compensate for minor dimensional discrepancies in the fit of multiunit restorations by using cement and the cement space.1 However, cemented implant-supported crowns are more difficult to retrieve for subsequent maintenance.1 If the location of the access screw opening is not known, the removal of a cement-retained implant-supported restoration with a loose abutment screw or fractured porcelain may result in difficulties and perhaps irreversible damage to the restoration or the abutment. Several methods have been described in the literature to provide retrievability of cemented implant-supported restorations. One approach is the use of provisional cement. The argument that cements of differing retentive capacities can be titrated to somehow function as effectively as screws for either removal or retention has been challenged.1 A second method is the use of set screws that allow a retrieval screw to be placed in a position where a displacing force can be applied in the direction of the abutment to break the cement seal and allow removal of the restoration.2 The disadvantage of this technique is the access opening in the occlusal surface for the extra screw needed to remove the crown. A third approach is the placement of a well-defined small ceramic stain on the occlusal surface of a posterior implant-supported restoration in conjunction with a periapical radiograph for evaluating the implant angulation. This method provides a reliable landmark for locating the screw-access opening of the abutment.3 However, this technique is limited to posterior implant-supported restorations only, and the ceramic stain can only be placed in nonesthetic areas. A fourth approach is the use of a secondary lingual locking screw in the restoration for retention and retrievability.4 A fifth approach is the use of cylindrical guide holes in the lingual aspect of implant-supported restorations for the removal of cemented superstructures.5 A removing driver placed in these guide holes, when turned, creates a shear force and thereby raises and unseats the restoration. A sixth method is the use of a vacuum-formed clear retaining screw location guide, similar to a surgical guide, made on a cast. The location guide assists in locating the implant abutment access chamber and abutment retaining screw to facilitate the removal of a cemented implant-supported restoration.6 The disadvantage of this technique is the need to retain the original implant and abutment level casts. This article describes an uncomplicated technique to facilitate the removal of a cemented implant-supported restoration. This technique has 2 advantages: it will not damage the implant abutment, and the patient’s visit will be relatively brief. After seating the implant abutment(s) (Cast-to Gold Abutment, engaging; Zimmer Dental, Carlsbad, Calif ) and the implant-supported restoration(s), a series of digital photographs are made with a digital camera (Dine Pentax Digital Solution; Lester A. Dine, Inc, Palm Beach Gardens, Fla) with the use of the intraoral macro setting.

Psychosocial characteristics of adults who experience difficulties with retching

Objectives The current study aimed to reach a greater understanding of psychosocial variables in patients who gag, and is a recognised gap within current literature. Obtaining a profile of patients who gag could provide valuable information in relation to future treatment planning and management of this type of difficulty. Methods All new patients aged ≥18 years, identified as having a difficulty with retching, and who attended the Anxiety Management Clinic, Department of Restorative Dentistry, Birmingham Dental Hospital, were invited to complete a brief questionnaire with questions related to general demographic data; difficulties with retching; and an oral health status measurement. This study has reported on 48 patients who attended the dental anxiety management clinic at Birmingham Dental Hospital and who presented with retching difficulties. One patient did not wish to take part in the study, leaving a final sample size of 47. The sample comprised 57% male ( n = 27) and 43% female ( n = 20) patients. Results Retching was more commonly seen in the 40–49-year age group. Onset was frequently related to an earlier dental experience and over half the sample studied had experienced previous psychological difficulties. A fear of choking or suffocation was frequently expressed, supporting a cognitive model in terms of assessment and treatment. Conclusion A number of themes were identified which would be worthy of further exploration. For example, the impact of previous or existing physical health problems such as respiratory complaints, family history of retching, and prevalence of other psychological difficulties. Through participation in this study, patients were able to reach a greater awareness of this commonly encountered difficulty within dentistry.

Evaluation of a system for grading the complexity of root canal treatment

To evaluate the reproducibility of the Restorative Index of Treatment Need (RIOTN) system for grading the complexity of root canal treatment. The RIOTN system of grading the complexity of root canal treatment was applied to all endodontic referrals to a department of restorative dentistry in a district general hospital within a period of one year. Grading was repeated in 60 randomly selected teeth and weighted kappa analysis was used to test for intra-observer as well as inter-observer agreement with a consultant in restorative dentistry and a vocational trainee (VT). In all, 152 patients were referred for root canal treatment of 186 teeth within the period of study. Weighted kappa for intra-observer agreement when categorising complexity was 0.636. Weighted kappa for inter-observer agreement with the consultant was 0.570 and that for agreement with the VT was 0.223. The RIOTN system of grading the complexity of root canal treatment was incomplete; with moderate to poor reproducibility.
