Lipomas of the head and neck: presentation variability and diagnostic work-up

Head and neck lipomas have seldom drawn attention in the literature, except in isolated case reports. This study aimed to assess the presentation variability of head and neck lipomas as well as the relative importance and efficiency of pre-operative diagnostic methods used. A retrospective review was undertaken of medical records and imaging studies of 24 patients with histopathologically proven head and neck lipomas, over a three-year period. The 24 patients had 26 lipomas. Men predominated (62.5 per cent). The posterior subcutaneous neck was the most common site. Three patients had deep lipomas affecting the hypopharynx, larynx and parotid gland; all were correctly diagnosed pre-operatively. Computed tomography (CT) scan with specific radiodensity recording was the preferred pre-operative investigation. Lipomas should be considered in the differential diagnosis of soft-tissue head and neck masses even in rare locations. A CT or magnetic resonance imaging scan can correctly diagnose a lipoma pre-operatively, thereby allowing better treatment planning.

Superficial lipoma was easy to diagnose, but deep lipoma, especially intraosseus lipoma, was difficult to detect pre-operatively. Intraosseous lipoma is very rare and had been difficult to differentiate from other tumors before image diagnosis was developed. Recently, the detecting ratio has increased due to advanced diagnostic tools. Preoperative diagnosis is crucial for the treatment plan and influences the approach method for excision. We experienced one case of misdiagnosis which was diagnosed as intraosseous lipoma according to the preoperative MRI but turned out to be a parosteal lipoma after the surgery.

Giant lipoma of the breas

Lipomas of the breast are usually small, benign neoplasms which can be treated by simple excision. Diagnosis of these masses, however, can be difficult because of the normal fatty composition of the breast. A number of radiological manoeuvres have been described for diagnosing deep lipomas in the breast, yet the clinical and radiographic identification of these masses remains challenging. We present a case of giant lipoma of the breast which was not appreciated on initial clinical and radiographic evaluation, and which was later found to comprise most of the mass of the breast. Following resection, the deformed breast was reconstructed using folded dermoglandular flaps from the areas expanded by the lipoma with excellent results. This case is an excellent illustration of the difficulties associated with diagnosis of these tumours and the reconstructive options available following excision.

Massive cavernous lymphangiomatosis associated with a large lipoma and hemangiomas : a new syndrome?

A 3-year-old girl with massive cavernous lymphangiomatosis, a huge lipoma, and several types of hemangiomata is presented. A deep lipoma on the back was removed. The left margin of the lipoma intertwined with several cysts of cavernous lymphangiomas, resulting in continuous fluid accumulation postoperatively. At the first operation, a partial excision of cavernous lymphangiomas of the forearm was also performed. The forearm healed uneventfully, except for a moderate degree of hypertrophic scarring. As a result of a literature review, it can be stated that the case presented does not conform to any well-defined entity and would appear to represent a new syndrome to be categorized under congenital hamartomatous disorders. Everyone is interested in the fate of massive lymphangiomatoses such as this, and in the effects on several tissues or organs, such as breasts, muscles and bones.
