True Isolated Deep Femoral Artery Aneurysm Treated with Endovascular Technique: Case Report and Literature Review

In this paper, we aimed to report an alternative endovascular treatment in a patient with an isolated deep femoral artery aneurysm. A 49-year-old male patient, with no comorbidities, was admitted with right limb pain, mainly in the thigh, associated with a weakness and difficulty walking. The patient was submitted to a duplex ultrasound, that suggested a deep femoral artery aneurysm, with 6 cm diameter, confirmed with CT angiogram. The patient was scheduled to undergo endovascular treatment of the deep femoral artery aneurysm with a covered self-expandable 9x100 Solaris and embolization of the first branch of the Deep femoral artery with Azur coils 2 4x5, 6x10, 6x15 and 6x20. The patient was discharged from the hospital two days after the surgery with no complications, palpable pedal pulses on Aspirin 100mg and Clopidogrel 75mg. At year follow-up the patient was asymptomatic and duplex showed patent stent without evidence of endoleak.

Embolization of Deep Femoral Artery Aneurysm with a Ligated Proximal Artery Using the Direct Percutaneous Puncture Technique.

We report a case of a deep femoral artery aneurysm with a ligated proximal artery that was successfully managed with endovascular therapy. An 84-year-old male was referred to our institute with a history of surgical resection of a left ruptured deep femoral artery aneurysm wherein another aneurysm was found on the peripheral side. Proximal artery ligation of the peripheral lesion was performed. The residual aneurysm had gradually enlarged after surgery, and contrast-enhanced computed tomography showed contrast effects in the aneurysm that extended to the distal artery. The aneurysm was successfully treated by direct percutaneous puncture embolization with N-butyl-cyanoacrylate.

患者は80歳代，男性．急に発症した左鼠径部および左大腿部痛と，同部位に径6 cmのドーム状有痛性皮下腫瘤を主訴に救急車で来院した．造影CTで左大腿深動脈瘤破裂と判明し，左大腿深動脈瘤切除＋左大腿深動脈再建術を施行され，術後合併症なく退院した．大腿深動脈瘤は非常に頻度が低く，日常の救急外来で大腿深動脈瘤破裂を診療することは極めて稀である．この1例について経験したので，文献的考察を加えて報告する．

A case of deep femoral artery aneurysm found with superficial femoral artery aneurysm.

Objective: Deep femoral artery (DFA) aneurysms are extremely rare cases of aneurysms that are difficult to diagnose. The objective of this report was to discuss the timing and method of surgery for this disease.Patient: We encountered an asymptomatic left DFA aneurysm that was discovered along with a symptomatic aneurysm of the right superficial femoral artery (SFA). Both sides of the aneurysm were resected with Dacron knitted artificial vascular grafts (Gelsoft™ Plus, Vasctek, UK) simultaneously.Result: After the operation, the right SFA had good blood flow, but the graft of the left DFA was occluded. The occlusion was considered to be caused by insufficient blood flow in the graft. The patient was discharged without any complications.Conclusion: The coexistence of DFA aneurysms should be examined if other aneurysms are found. DFA aneurysms are at a high risk of rupture. Careful follow-up is required, and intervention is recommended when the diameter exceeds 35 mm.

Surgical Management of a Deep Femoral Artery Aneurysm

Standard surgical methods for managing true aneurysms of the deep femoral artery have not been established because these aneurysms are rare. An 85-year-old man, who had a ruptured aneurysm of the deep femoral artery, underwent aneurysmectomy and distal reconstruction with a contralateral autologous vein graft. Three-dimensional computed tomography is a valuable diagnostic modality to evaluate synchronous aneurysms and peripheral arterial circulation for treatment planning. Because of their etiology, aneurysms of the deep femoral artery should be treated with revascularization when technically feasible, even if the superficial femoral artery is patent. A contralateral saphenous vein may be the preferred conduit because ipsilateral venous stasis is likely.

Deep femoral artery aneurysms are a rarity among arterial aneurysms. Based on a selected case of a patient who was treated at our clinic, clinical images are presented and treatment options are discussed.

Synchronous spontaneous arteriovenous fistula and aneurysm of the deep femoral artery incidentally found in a patient with aortic aneurysmal dissection

The patient was a 77-year-old man with sudden-onset chest and back pain. Computed tomography angiography (CTA) from the abdomen to the proximal thigh showed an aortic aneurysmal dissection (AAD) and a left deep femoral artery (DFA) aneurysm. The AAD was conservatively managed as there was no sign of increase. The left DFA aneurysm was surgically resected to avoid complications such as rupture, thromboembolism, and limb ischemia. On follow-up CTA obtained 3 weeks later, a spontaneous AVF in a varicose vein of the right DFA was noted. There were no symptoms associated with the AVF, and he was conservatively managed. However, the varicose vein gradually increased, and he underwent successful transarterial catheter embolization (TAE) with metal coils without any complications. After vascular repair of the aneurysm and the AVF of the right DFA, there was no sign of recurrence.

Diagnosis of an Aneurysm by Musculoskeletal Ultrasonography: A casereport

High resolution musculoskeletal ultrasonography has been a comprehensive tool for differentiation of soft tissue mass. However, it is not commonly used as a standard examination in the diagnosis of an aneurysm. We present a case of a growing mass in the medial aspect of the left thigh status post internal fixation of periprosthetic fracture. Initial examination with a 10 MHz linear array probe revealed a blurred hypoechoic lesion at a depth of 5 cm. After switching to a 7.5 MHz curve-linear array probe, we found a well-defined, ovoid, hypoechoic cystic lesion with posterior enhancement. In color Doppler mode, a half-blue/half-red swirling signal inside the lesion with pulsation was noted. Angiography showed an aneurysm of the left deep femoral artery with extravasation. After trans-artery embolization and debridement, the symptom of left thigh swelling improved. We conclude that the key to accurate diagnosis of this case is proper application of probes of various array and frequency, and Doppler mode.
