The role of informatics in patient-centered care and personalized medicine.

The practice of cytopathology has dramatically changed due to advances in genomics and information technology. Cytology laboratories have accordingly become increasingly dependent on pathology informatics support to meet the emerging demands of precision medicine. Pathology informatics deals with information technology in the laboratory, and the impact of this technology on workflow processes and staff who interact with these tools. This article covers the critical role that laboratory information systems, electronic medical records, and digital imaging plays in patient-centered personalized medicine. The value of integrated diagnostic reports, clinical decision support, and the use of whole-slide imaging to better evaluate cytology samples destined for molecular testing is discussed. Image analysis that offers more precise and quantitative measurements in cytology is addressed, as well as the role of bioinformatics tools to cope with Big Data from next-generation sequencing. This article also highlights the barriers to the widespread adoption of these disruptive technologies due to regulatory obstacles, limited commercial solutions, poor interoperability, and lack of standardization. Utilization of cytocentrifugation slides in flow cytometry laboratory: a tool for correlation of morphology and immunonophenotype

Cytocentrifuge slide (“cytospin”) is a simple and rapid technique that is mainly used in the cytology laboratory. We have routinely used the technique in flow cytometry (FCM) laboratory at Montefiore Medical Center for more than 15 years. Our aim is to illustrate the contribution of cytospin in direct cellular morphologic visualization in FCM immunophenotyping, in result interpretation, and additionally as a quality control tool. Four thousand two hundred forty-eight cases sent to our FCM Laboratory in the past 21 months were studied retrospectively. The clinical history, cytospins, immunophenotyping panels, FCM contour plots, final FCM reports, and pathology reports were reviewed. Morphologic examination of the cytospin is a cost-effective tool for sample quality screening. Of the 4248 immunophenotyping cases, 359 (8.4%) cases were canceled, according to laboratory protocol based on cytospin review. Morphologic analysis of the cytospin provides extra information that guides both selection of the FCM immunophenotype panel and gating the cells of interest. Additionally, morphologic analysis of cytospin can yield early diagnosis when cohesive benign or malignant non-hematologic cells are present or even infectious organisms such as parasites are identified. Finally, the cytospin is a convenient tool for morphology-phenotype correlation. Cytospin is cost-effective and provides important morphologic information that facilitates FCM immunophenotyping panel selection and result interpretation.

CellDetect is a unique histochemical stain which enables color discrimination, in addition to morphological examination, for the differentiation between benign and malignant cells in urine specimens. A multi-institutional blinded study has recently shown that this color feature significantly improves the sensitivity for LG tumors when compared to standard urine cytology. The objective of the present study was to confirm this performance in an independent cytology laboratory. METHODS Voided urine samples were collected from a first cohort of patients undergoing routine cystoscopic surveillance. To enrich the study with positive cases, a second cohort of patients scheduled for transurethral resection (TURBT) was also enrolled. The patients from both cohorts had a documented history of bladder cancer. Urine samples were processed into two cytocentrifuge smears and each slide was stained with either CellDetect or standard cytology stain. Both specimens were observed by a cytopathologist blinded to the final diagnosis. The results were then compared to the gold standard (biopsy for positive cases and biopsy or cystoscopy for negative cases). RESULTS 73 patients were enrolled in this study, among which 51 were UCC-negative and 22 UCC-positive. The sensitivity of CellDetect was 82% compared to 59% for standard cytology (p<0.05) while the specificity was not significantly different (86% versus 94%). Moreover, the urine-based biomarker was able to detect 73% of the LG tumors compared to 45% by standard cytology. In addition, it correctly diagnosed 91% of the HG tumors compared to 73% for standard stain. CONCLUSIONS This study validates the usability of CellDetect in clinical settings. Particularly, it confirms its ability to accurately identify UCC recurrence throughout all cancer grades. This article is a biographical essay about a prominent scientist - the cytogeneticist G. A. Levitsky, N. I. Vavilov’s closest associate, who devoted more than 15 years (1925-1941) to his work at VIR, who organized and headed the Laboratory of Cytology there. His scientific achievements and tragic fate, very similar to Vavilov’s, are highlighted. Key words: genetics, cytology.

Biochemical Markers and Genetic Improvement in some Tomato Genotypes under Drought Stress Conditions

This study was conducted at the Experimental Farm of Maryout Research Station; Desert Research Center,Alexandria Governorate, in addition to the Genetic and Cytological Laboratory of Desert Research Center, Matarya,Cairo, Egypt; during summer growing seasons from 2012/2013 and 2013/2014, in order to evaluate some tomatogenotypes under drought stress conditions. The tolerant parent UC82B, susceptible parents, Peto82, Super Marmande andSuper Strain B with their crosses and reciprocal crosses were used. The results showed that there was unique band 35KDawas found in hybrid UC82B×Super Strain B due to it represent specialized mechanism in the tolerant genotypes underdrought stress conditions as well as a unique band 42 KDa which was represented in hybrid Super Marmande×UC82B. Inaddition, the results of peroxidaseis ozymes indicated that the unique band associated with line of UC82B. This bandcould be considered as a distinguished marker for the tolerant line of UC82B under drought stress conditions. Also, theresults of polyphenoloxidase isozymes indicated that there are four common monomorphic bands were mentioned at thebands of 1, 2, 5 and 6 with molecular weight. These common bands could consider as markers for positive identificationof previously mentioned genotypes under drought stress conditions. On the other hand, a unique band 4 associated withline LA3772) of UC82B. This band could be considered as a distinguished marker for the tolerant line of UC82B underdrought stress conditions. Moreover, the phylogenetic tree indicated that one cluster included the hybrids SuperMarmande×UC82B and reciprocal hybrid Super Marmande×UC82B and UC82B in sup cluster as a tolerant parentalgenotype.

Introduction of liquid‐based cytology and human papillomavirus testing in cervical cancer screening in Luxembourg

In 2014, liquid-based cytology with HPV triage replaced conventional cytology. The aim of our study was to compare conventional and liquid-based cytology (LBC), estimate the prevalence of abnormal cervical cytology and high risk HPV (hrHPV) infection and their correlation, among screened women in Luxembourg. Between the first January 2013 and 31st December 2015, 315,868 cervical samples from 150,815 women (mean age 42.2years) were investigated by the national cytology laboratory. Slides were prepared and screened according to European Guidelines. All cytological results were classified according to the Bethesda 2001 system terminology. The prevalence of abnormal cervical lesions was as follows: atypical squamous cells of undetermined significance (ASC-US), 1.3%; low-grade squamous intraepithelial lesion (LSIL), 1.9%; high-grade squamous intraepithelial lesion (HSIL), 0.4%. The detection rate of cytological lesions was significantly higher with LBC than with conventional cytology. Based on 11,838 samples with concomitant cytology and HPV testing, hrHPV was detected in 9.5, 45.3, 70.0, and 92.6% of women with negative cytology, ASC-US, LSIL, and HSIL, respectively. More cervical lesions were identified using LBC compared to conventional cytology. HrHPV infection was correlated with the severity of intraepithelial lesions. The current findings provide important information to evaluate the prevention of cervical cancer in Luxembourg and for monitoring the future impact of HPV vaccination. Diagn. FEATURES OF THE ORGANIZATION OF ANTICANCER MEASURES IN TERRITORIES WITH LOW POPULATION DENSITY ON THE EXAMPLE OF THE TOMSK REGION

The poor performance of rural health in remote from the center territories of the Tomsk region, determined the need for the optimization of cancer control measures. In 2013-2015 in the oncology service of the Tomsk region there were occurred structural and qualitative changes: there was formed a three-level system of the provision of specialized health care. For workers of the first level of health care system - feldsher obstetric units (FOUs) there were approved methodical approaches to the organization and implementation of screening for external location cancer, the formation of cancer risk groups for further examination; routing and observation of cases with risk factors, precancerous and background processes; documenting of the execution of the examination for cancer; routing and tactics for the observation of cancer patients. There are introduced motivational measures for the improvement of the quality of the work of medical staff. There are actively practiced mobile forms of the activity, with taking into account seasonality, the state of the transport accessibility (the use of mobile mammographs based on KAMAZ chassis, the regional project «The floating clinic», curation of districts by oncologists). There was introduced the mechanism of «individual road map», including telemedical technologies for distance observation of patient documents (the number of unnecessary visits of rural residents decreased by 30%). There were founded primary oncological units (POU), their base is enhanced by the diagnostic equipment, which reduced the number of underexamined patients by 47.5%. There was created the network of primary cytological laboratories completed with microscopes with digital processing and transmission of images over the Internet, there were performed more than 20,000 telemedical consultations, resulting in 70 diagnosed cases of cervical cancer for 2 years. The execution of POU-based courses of drug antitumor therapy increased the number of patients received the treatment at site of residing by 2.2 times. There was organized beneficiary drug coverage, including drugs for the treatment of chronic pain. Thanks to the work of POUs in the new status there were increased both the coverage of the population by cancer preventive examinations and indices of detection rates of precancerous diseases and chronic external localization diseases as in men as well in women.

Liquid-based and conventional cytology for bronchial washings/bronchoalveolar lavages in the diagnosis of malignancy - An institutional experience.

Background:Liquid-based cytology (LBC) has been developed as an alternative for conventional cytology (CC) in cervical smears. It is now increasingly being used all over the world for cervical cancer screening. However, its role and diagnostic accuracy in bronchial wash (BW)/bronchoalveolar lavage (BAL) specimens remains undetermined.Aims:To assess and compare the diagnostic performance and accuracy of LBC with CC for detecting malignancy in bronchial specimens.Settings and Design:This was a retrospective analytical hospital-based study.Materials and Methods:Bronchial specimens (BW/BAL) received over a period of 4.5 years were reviewed. The samples were processed by CC from June 2010 to September 2012 (2.25 years) and by LBC from October 2012 to December 2014 (2.25 years). Data were retrieved from the records of cytology laboratory and compared among both the groups. Detection rate for histologically or cytologically verified samples was calculated.Results:A total of 559 cases verified by histological and cytological follow-up were evaluated. These included 247 CC cases and 312 LBC cases. The positive diagnostic rate for malignancy in CC was 28.6% whereas that for LBC was 32.9%. The negative diagnostic rates were 66.5% and 66.3% for CC and LBC, respectively. However, unsatisfactory rates had shown a good reduction from 4.4% in CC to 0.6% after LBC introduction. The smears showed more homogeneous distribution of cells with elimination of obscuring factors such as blood, inflammation, and mucus.Conclusions:The diagnostic accuracy of LBC was slightly better than CC. The unsatisfactory rates showed reduction in LBC preparation. Thus, LBC is a viable alternative to CC and has the advantages of standardization of preparation with decrease in unsatisfactory rates.

The effect of the small amount of formaldehyde in the SurePath liquid when establishing protocols for immunocytochemistry.

Background:SurePath® is an ethanol-based liquid fixative. In addition to ethanol, it also contains a small amount of formaldehyde (<0.2%). The aim of this study was to investigate the immunoreactivity of cells stored for different lengths of time in the SurePath liquid.Materials and Methods:Rest material from one malignant and three benign effusions were fixed in SurePath for 1–12 days. Cytospins were incubated with cytokeratin 7 antibody (AB) to evaluate the staining intensity of carcinoma cells and benign, reactive mesothelial cells. Protocols varied as to pretreatment and AB incubation time.Results:Reduced immunostaining intensity was seen within 5 days of storage in the SurePath liquid. It was restored when the pretreatment time was prolonged.Conclusions:The small amount of formaldehyde in the SurePath liquid seems to affect the immunoreactivity. Local immunocytochemistry protocols in the cytology laboratories should consider this when optimizing their procedures. Postfixation with formalin should be omitted.

Improving Adequacy of Small Biopsy and Fine-Needle Aspiration Specimens for Molecular Testing by Next-Generation Sequencing in Patients With Lung Cancer: A Quality Improvement Study at Dartmouth-Hitchcock Medical Center.

- At our medical center, cytopathologists perform rapid on-site evaluation for specimen adequacy of fine-needle aspiration and touch imprint of needle core biopsy lung cancer samples. Two years ago the molecular diagnostics laboratory at our institution changed to next-generation sequencing using the Ion Torrent PGM and the 50-gene AmpliSeq Cancer Hotspot Panel v2 for analyzing mutations in a 50-gene cancer hot spot panel. This was associated with a dramatic fall in adequacy rate (68%). - To improve the adequacy rate to at least 90% for molecular testing using next-generation sequencing for all specimens collected by rapid on-site evaluation by the cytology laboratory. - After baseline data on adequacy rate of cytology specimens with rapid on-site evaluation for molecular testing had been collected, 2 changes were implemented. Change 1 concentrated all the material in one block but did not produce desired results; change 2, in addition, faced the block only once with unstained slides cut up front for molecular testing. Data were collected in an Excel spreadsheet and adequacy rate was assessed. - Following process changes 1 and 2 we reached our goal of at least 90% adequacy rate for molecular testing by next-generation sequencing on samples collected by rapid on-site evaluation including computed tomography-guided needle core biopsies (94%; 17 of 18) and fine-needle aspiration samples (94%; 30 of 32). - This study focused on factors that are controllable in a pathology department and on maximizing use of scant tissue. Optimizing the adequacy of the specimen available for molecular tests avoids the need for a second procedure to obtain additional tissue.

Sexually Transmitted Infection in Correlation with Cervical Precancerous Lesion

Objective: To identify the correlation between sexually transmitted&#x0D; infection and cervical precancerous lesion.&#x0D; &#x0D; Method: The study design is cross‐sectional. Samples were collected&#x0D; by consecutive sampling method until the minimal amount was fulfilled.&#x0D; This study was conducted in the Colposcopy Outpatient Clinic&#x0D; and Cytology Laboratory, Division of Specialistic Gynecology, Department&#x0D; of Obstetrics and Gynecology, Dr. Cipto Mangunkusumo&#x0D; Hospital (RSCM), Jakarta, from September 2008 to March 2009.&#x0D; Laboratory examination for sexually transmitted infection (STI) was&#x0D; performed in Department of Dermatovenereology, RSCM, Jakarta.&#x0D; Patients were grouped into cases and control group. The case group&#x0D; consisted of patients diagnosed with cervical intraepithelial neoplasia&#x0D; (CIN) and the control group consisted of patients without CIN.&#x0D; Sexually transmitted infection was identified and its relationship to&#x0D; CIN was analyzed.&#x0D; &#x0D; Result: We included 130 patients into this study, 25.38% without&#x0D; CIN and 74.62% with CIN. We found that one patient can be infected&#x0D; by up to five types of infection at a time. We also discovered a statistically&#x0D; significant relation between CIN 1 and STI (p=0.028), CIN 2&#x0D; and STI (p=0.007), and CIN 3 and STI (p=0.013).&#x0D; &#x0D; Conclusion: Based on our study, we discovered a significant relationship&#x0D; between the incidence of STI and CIN.&#x0D; &#x0D; Keywords: cervical intraepithelial neoplasia, cervical precancerous&#x0D; lesion, sexually transmitted infection

PAROKSIZMALNA NOČNA HEMOGLOBINURIJA - PRIPOROČILA ZA ODKRIVANJE BOLEZNI TER PREGLED POPULACIJE BOLNIKOV V SLOVENIJI

Background: Paroxysmal nocturnal hemoglobinuria (PNH) is a rare, acquired disease associated with hemolytic anemia, bone marrow failure, thrombosis, and, frequently, poor quality of life. It is caused by defects in the membrane of blood cells, where there is a lack of protein on the cell surface, which inhibits complement activation. We wanted to know the recognition of the disease in Slovenia and the incidence. We prepared the recommendations for discovering of the disease. Patients and methods: We collected data of 68 patients with prospective analysis of one – year period from 1.10.2013 to 30.9.2014 whose blood was sent to the laboratory of immunology and cytology because of suspected presence of PNH clone. The analysis of peripheral blood was performed with multiparametric high specific flow cytometry in a specialized laboratory KO of Haematology University Medical Centre in Ljubljana. Results: PNH clone was positive in 13/68 (19%) patients, 55/68 (81%) patients had a negative PNH clone, most positive samples were sent from the University Medical Centre Ljubljana (7/13). 4/13 positive patients were newly discovered. In average the incidence through 10-years was 1,3 / 1,000,000 population/year. The most common cause of PNH patient referral to a specialist hematologist was unexplained cytopenia - 12/13 (92.3%), the most common symptoms were fatigue and dyspnea (100%), in 2/13 patients was present dark urine with hemoglobinuria, 2/13 patients had transient renal insufficiency. 11/13 patients with positive PNH clone had associated a bone marrow failure (aplastic anemia or myelodysplastic syndrome). The size of PNH clone varied from patient to patient. Conclusions: Early identification of PNH is a key to effective treatment and survival of patients. We recommend determining PNH clone at Coombs negative hemolytic anemia, hemoglobinuria, an unexplained cytopenia, aplastic anemia, myelodysplastic syndrome with laboratory evidence of haemolysis and unexplained thrombosis.

Utility of flow cytometry analysis for pleural and peritoneal fluids

Pleural and peritoneal/ascites fluid samples with many lymphocytes are commonly received in the cytology laboratory. It is often difficult to distinguish reactive lymphocytes from hematopoietic malignancy based on morphology alone, however. Flow cytometry can be a useful adjunct in body fluids, although literature on this subject is limited. This study is a single-institution 5-year retrospective review of 377 fluid samples from 341 patients with corresponding flow cytometry analysis. The cytologic findings were correlated with the flow cytometry results and clinical data, as available. Of 4158 pleural fluids received over 5 years, 325 (7.8%) had corresponding flow cytometry analysis. Of these 325 samples, 57 (17.5%) were positive for hematopoietic malignancy by flow cytometry. Of the positive cases, only 24 (8.7%) represented a new diagnosis of hematopoietic malignancy (ie, did not have a known history). Of 3020 peritoneal/ascites fluids received over 5 years, 52 (2%) had corresponding flow cytometry. Of these, 8 were positive for hematopoietic malignancy, and only 2 represented a presumed new diagnosis. Routine flow cytometry analysis for pleural and peritoneal/ascitic fluids is of limited utility, with only rare cases positive for hematopoietic malignancy without a known history. Of these cases, many had atypical cells that suggested a positive diagnosis. Conversely, in cases with a known history, about 75% were positive for hematopoietic malignancy. Our study suggests that the utility of flow cytometry for pleural and peritoneal/ascitic fluids is limited, and should be used sparingly in cases without atypical cytologic features, high clinical suspicion, or known history.

Impact of Internet on Cytology Information Management.

Internet technologies and services impose global information standards in the sphere of healthcare as a whole, which are then implied and applied in the domain of cytology laboratories. Web-based operations form a significant operating segment of any contemporary cytology laboratory as they enable operations by the use of technology, which is usually free of the restrictions imposed by the traditional way of business (geographic area and narrow localisation of activities). In their operations, almost all healthcare organisations currently create and use electronic data anddocuments, which can originate both inside and outside the organisation. An enormous amount of information thus used and exchanged may be processed timely and in a high-quality way only by integrated information systems, given three basic safety requirements: data confidentiality, integrity and availability. In the Republic of Croatia, integration of private and public healthcare information systems has been ongoing for several years but the private healthcare does not yet operate as an integrated system. Instead, each office operates using its own separate information system, i.e. This paper elaborates the argument that the sample private cytology laboratory possesses an IT system that meets current market and stakeholder needs of the healthcare sector in Croatia, given that private doctors' offices/polyclinics use IT technologies in their operations but make only partial use of Internet capacities in the segment of communication with their business associates and patients, implying the need to continue the research on a statistically relevant sample of EU countries.

Pre-analytical issues in effusion cytology.

Effusions or body cavity fluids are amongst the most commonly submitted samples to the cytology laboratory. Knowledge of proper collection, storage, preservation and processing techniques is essential to ensure proper handling and successful analysis of the sample. This article describes how the effusions should be collected and proper conditions for submission. The different processing techniques to extract the cellular material and prepare slides satisfactory for microscopic evaluation are described such as direct smears, cytospins, liquid based preparations and cell blocks. The article further elaborates on handling the specimens for additional ancillary testing such as immunostaining and molecular tests, including predictive ones, as well as future research approaches.
