Hydatid disease of spleen treated by cyst enucleation and splenic salvage

A 4-year-old boy with unilocular hydatid cysts of spleen and liver was successfully treated by enucleation of both the cysts and salvage of the spleen. The conventional surgical treatment of choice for hydatid cyst of the spleen is splenectomy. The authors demonstrate that preservation of a spleen afflicted by hydatid disease is technically feasible and is recommended as the choice of treatment to obviate the well-recognized postsplenectomy complications especially in children.

A CASE OF MUCIGENIC EPITHERIAL CYST IN THE SPLEEN

Though splenic cysts have been considered to be a relatively rare disease, a number of cases have recently been reported corresponding to a marked improvement in diagnostic technique. The clinico-pathological classification of these disease established in the 1940's to 1950's is used at present. However, as clinical experience increases, it has become obvious that there are many unclassifiable cases. A case reported was a eighty five year old woman whose chief complaint was diarrhea. Preoperative diagnosis by angiography, echography and computed tomography suggested a multiple cyst of spleen. At the operation when splenectomy was performed, the other organs seemed macroscopically tobe normal and the size of cystic spleen was 19×10×7cm. Pathological examination of the resected specimen was a mucigenic epithelial cyst of the spleen of a new type beyond the present classification.

Spelenomegaly in a child can present a difficult diagnostic problem. Portal hypertension, leukemia, infection, lipidosis, and lymphoma are the most frequent causes. 1 Epithelial cysts are rare; they compose approximately 10% of the nonparasitic benign cysts of the spleen. 2 The diagnosis seldom is confirmed preoperatively. Percutaneous aspiration of the cyst and selective abdominal angiography are reported to be of major diagnostic value. 3.9 In one case to be reported, splenic aspiration prior to splenoportography lead to the presumptive preoperative diagnosis.

Echinococcal cyst (hydatid) of spleen and liver: Report of two cases

1. 1. Two cases of echinococcal cyst are reported, one originating in the left lobe of the liver and the other in the spleen with recurrence in the omentum and left subdiaphragmatic area and upper surface of the urinary bladder. 2. 2. One patient was a native Hungarian, while the other was born in Italy. 3. 3. Both had symptoms greatly simulating peptic ulcer following operation but the diagnosis of ulcer could not be verified by x-ray or at operation.

About non-parasitic cysts of the spleen

On the basis of literature data and his own case, the author believes that non-parasitic spleen cysts are rare. Favourable conditions for the development of this disease are malaria, typhoid, syphilis, spleen dislocation and trauma.
