 El linfangioma quístico es un tumor benigno infrecuente del sistema linfático que afecta habitualmente a los infantes, resulta de una malformación vascular congénita o adquirida de los vasos linfáticos, que impide el correcto drenaje de la linfa hacia el sistema venoso con la posterior proliferación y dilatación de estos.1 No constituye una entidad prevalente ya que su incidencia es de un 1 caso por cada 50 000 nacimientos, con frecuencia similar en niños y niñas, sin diferencias raciales. El 90 % se diagnostica en los dos primeros años de vida y representa el 6 % de los tumores en la infancia.1,2 La localización del linfangioma quístico en mediastino es muy raro y su frecuencia, es de 0.7-4.5% de todos los tumoresmediastinales,porellolarelevanciadel caso presentado en este reporte.
 
 

Chylopericardium In a Young Woman Revealing A Mediastinal Cystic Lymphangioma: A Case Report

Isolated chylopericardium due to mediastinal cystic lymphangioma is a rare entity. We report a case of asymptomatic chylopericardium in an 18 years old female with no history of trauma, thoracic surgery, malignancy, infection, or tuberculosis sand who presented with cardiomegaly noticed on chest x-ray before surgery of mandibular subluxation. Echocardiography showed a large number of pericardial effusions and pericardiocentesis yielded 1.25 L of milky-colored fluid which showed a triglyceride level of 4.82 g/L and cholesterol of 0.29 g/L with a cholesterol to triglyceride ratio of <1, characteristic of the chylous fluid. Chest computed tomography showed a huge mediastinal cystic lymphangioma. The patient successfully responded to continuous pericardial drainage and a medium-chain triglyceride diet then she was referred to the thoracic department for surgery. This type of case risks being misdiagnosed as tuberculosis, especially in countries where tuberculosis is endemic. Therefore, assessing pericardial fluid for chyle is crucial.

MASSIVE ISOLATED CYSTIC LYMPHANGIOMA OF ANTERIOR MEDIASTINUM IN CHILDREN: DIAGNOSIS AND TREATMENT DIFFICULTIES (clinical case presentation)

The authors report the case of a two-year-old patient, hospitalized urgently with respiratory distress, radiologically establishing the diagnosis of subtotal pneumonia on the right complicated by the exudative-fibrinous pleurisy. CT showed the presence of a massive closed fluid formation, located subpleurally in the right hemi thorax (closed pleurisy), and associated with subtotal passive atelectasis of the right lung with no signs of destruction of lung tissue. After the installation of the intrathoracic drain, the fluid collection persisted. After thoracoscopy, surgery was performed, the patient undergoing a latero-posterior thoracotomy on the right with radical resection of the lymphangioma, which came from the anterior mediastinum. Histopathological and immunohistochemical examination confirmed the diagnosis. Conclusion. Mediastinal cystic lymphangioma in children can mimic a pleural effusion, which causes certain errors in surgical conduct. The final diagnosis is confirmed only by histological examination

1008 Incidental Solitary Cystic Mediastinal Lymphangioma with Mass Effect Complications: A Case Report

Abstract Introduction Cystic mediastinal lymphangiomas (CML) are exceedingly rare, representing <1% of cystic lymphangiomas (CL), and typically present before 2 years of age. Few cases describe adult presentations of solitary CML and none describe symptomatic cases with ‘mass-effect’ complications. We describe an incidental finding of a symptomatic solitary CML causing mass effect complications and our definitive surgical management. Case Report A 44-year-old Caucasian male presented with reduced responsiveness due to mixed drug overdose of fluoxetine and ibuprofen. Plain chest radiograph (CXR) revealed incidental new mediastinal widening. Thoracic computed tomography (CT) showed a well-defined lesion (75x63mm) in the right paratracheal region, with associated compression of the superior vena cava, left brachiocephalic vein and displacement of the azygos vein. Further questioning revealed 6-months history of non-productive cough, worsening dyspnoea on exertion and chest pain radiating to the right of the chest. Complete surgical resection was performed via posterolateral thoracotomy. It was seen compressing the 2nd and 3rd intercostal nerves. Enlarged lymph nodes at station 4 and 10 were excised and sent for analysis with aspirate from the excised lesion. Pathological gross examination showed a multiloculated cystic lesion containing thin straw-coloured fluid. Microscopically, it was thin walled with smooth muscle and scattered clusters of lymphocytes; its lining was markedly attenuated comprising bland flat endothelial cells, positive for CD-31. Lymph node samples showed reactive changes. Conclusions Although CMLs are benign, their location and growth in the mediastinum can result in compression of surrounding structures. Early detection and complete surgical resection are paramount to prevent such complications and lesion recurrence.

Giant Cystic Mediastinal Lymphangioma.

Cystic lymphangioma mainly occurs in children and develops from the lymphatic vessels. The present study reports the case of a 36-year-old female patient with a giant cystic mediastinal lymphangioma. She was asymptomatic and the diagnosis was accidental. Imaging studies are important for referral to the Cardio-Thoracic Surgery Department for surgical intervention in order to remove the lesion. It is important that in the case of an enlargement of the mediastinum observed in a thoracic X-ray, the possible diagnosis takes into consideration various hypotheses. Early diagnosis may prevent further growth and infiltration of structures that hinder the surgical approach.LEARNING POINTSCystic lymphangioma corresponds to approximately 0.7 to 4% of all mediastinal tumours.They are usually asymptomatic and, in adults, the diagnosis is accidental.After an imaging study with computed tomography and nuclear magnetic resonance, patients are usually candidates for surgical excision.

Pathology of chest organs in the fetus: review of literature and own observations

The article presents various nomological forms of malformations of the chest organs and their population frequency. The ultrasound and differential diagnostic features are described, such as cystic adenomatoid malformations of the lung, pulmonary sequestration, bronchogenic cyst and such rare diseases as congenital high-airway obstruction syndrome and bronchial atresia, pulmonary hypoplasia and pulmonary agenesis, mediastinal cystic lymphangioma. The timing of detection and accuracy of diagnosis of various malformations of the chest is reported. Data on the nature of concomitant abnormalities and chromosomal pathology are given. Considerable attention has been paid to resolving the issue of the advisability of maintaining or terminating a pregnancy, the most rational tactics of managing patients with this pathology, the timing and methods of delivery, as well as the selection of optimal methods for treating newborns in the postnatal period and the prognosis of their further development.

A late onset solitary mediastinal cystic lymphangioma in a 66-year-old woman who underwent kidney transplantation

Lymphangioma is a congenital abnormality of the lymphatic system detected primarily in early childhood. There are rare reports of mediastinal lymphangioma in older adults. We hereby report on a 66-year-old female patient who underwent kidney transplantation 20 years previously and who developed pathologically confirmed solitary mediastinal lymphangioma 1 year ago. Chest radiography showed a mediastinal nodule, which was not observed 2 year previously, therefore she was referred to the pulmonary division. She had no symptoms, and chest computed tomography demonstrated a 25-mm, well-defined, low-density nodule located at the anterior mediastinum. The size of the nodule had increased from 25 mm to 34 mm 1 year later, and it was completely resected via video-assisted thoracic surgery. The histological diagnosis was cystic lymphangioma. Therefore, we recommend that clinicians consider cystic lymphangioma as a possible diagnosis even in older patients with a mediastinal cystic mass that shows progressive enlargement.

Giant primary cystic mediastinal lymphangioma: A case report.

Cystic lymphangioma mainly occurs in children. Cystic mediastinal lymphangioma (CML) originates from mediastinal tissues and is an extremely uncommon cystic lymphangioma that develops from the lymphatic vessels. The present study reports the case of 46-year-old male patient with a giant CML that was surgically resected by video-assisted thoracoscopy. The largest diameter of the CML was 18.0 cm, and ~400 ml of pale yellow fluid was removed from the cystic cavity during surgery. The postoperative pathological reports on the cystic wall showed that the neoplasm was a CML. At present, at the one-year postoperative follow-up, there are no signs of recurrence. In conclusion, complete surgical resection may prevent recurrence.

Acute onset of mediastinal cystic lymphangioma in the puerperium

Lymphangioma is a benign congenital malformation developing from the prenatal lymphatic vessels. Lymphangiomas are typically slow-growing and asymptomatic, but can occur throughout the body; there are few documented cases of either mediastinal or rapid growing lymphangiomas. Here, we report a case of a 27-year-old female who experienced sudden-onset nocturnal dyspnea 1month after delivery. Ultrasound and computed tomography revealed a cystic tumor (diameter, 8cm), which posteriorly displaced the superior vena cava and trachea. Ultrasound-guided aspiration was performed and the tumor disappeared on chest X-ray, only to return on subsequent chest X-ray performed the following day. Therefore, thoracoscopic excision of the mediastinal tumor was performed, and pathological diagnosis confirmed mediastinal cystic lymphangioma. In this report, we discuss the rare nature of this case and the treatment of lymphangioma in pregnancy.

Solitary cystic mediastinal lymphangioma

To the Editor: Solitary cystic mediastinal lymphangioma (CML) is a very uncommon benign vascular tumour developed from lymphatic vessels. Cystic lymphangioma is a cyst caused by a congenital malformation of the lymphatic vessels and can affect any site in the body, but <1% of lymphangiomas are mediastinal [1] and pulmonary lesions are even less common. They are most often located in the anterior mediastinum. We report a case of anterior CML in a 16-yr-old male, with a short review of the literature. A 16-yr-old male was admitted to the Thoracic Oncology unit (Hopital Nord, Marseilles, France) for recurrent left pleural effusion. The patient reported a 6-month history of chest pain and dyspnoea with hyperthermia. An initial diagnosis of left empyema was suspected. It was managed by iterative thoracentesis with saline pleural lavages, antibiotics and physiotherapy. On admission, physical examination was unremarkable except for left-sided dullness on percussion. The patient was afebrile. Chest radiography revealed a large basal opacity of the left lung silhouetting out the heart and diaphragm. Initially, this opacity was thought to represent a recurrent pleural effusion (fig. 1a). Computed tomography scans revealed a cystic lesion in the left basithoracic and anterolateral pleura with smooth borders measuring 63×90 mm (fig. 1b). No other intra- or extrapulmonary cystic lesions were found. With a presumptive diagnosis of a pleuropericardial cyst, video-assisted thoracic surgery was performed. Successful Excision of an Isolated Mediastinal Cystic Lymphangioma with Bilateral Thoracoscopic Surgery

Lymphangioma is a well-known benign tumor and its cystic abnormalities of the lymph vessels are predominantly congenital. Cystic lymphangioma usually occurs in the neck, axillary region, and rarely in the mediastinum, which frequently occurs in children and young adults. A 20-year-old woman had symptoms of palpitation, cough, and dyspnea during the recent 1 month. Both chest comuted tomography and magnetic resonance imaging of the chest revealed a well-defined, 13 × 10-cm cystic lesion in the anterior mediastinum. The patient underwent bilateral video-assisted thoracoscopic excision of the cyst and lymphangioma was confirmed based on histopathologic examination. Here, we report a rare case of isolated mediastinal cystic lymphangioma that was successfully excised using a minimally invasive technique.

Percutaneous sclerotherapy of a giant mediastinal lymphangioma

The present study reports a case of percutaneous sclerotherapy of a giant mediastinal cystic lymphangioma using Ethibloc (Ethicon, Norderstedt, Germany) and absolute ethanol in a 59-yr-old female. The tumour, situated predominantly in a retrocardiac location, caused dyspnoea and dysphagia by compression and was considered unresectable. Follow-up computed tomography 3 yrs after treatment showed a 90% volume reduction of the tumour. The patient is currently asymptomatic. To the best of the present authors' knowledge, percutaneous transthoracic sclerotherapy of a mediastinal lymphangioma has not previously been reported in the literature available in English.

Isolated mediastinal cystic lymphangioma in children: About two cases

Lymphangiomas are a heterogeneous group of benign vascular malformations of the lymphatic system composed of cystically dilated lymphatics. Most cystic lymphangiomas are found in the cervical region. Isolated mediastinal cystic lymphangiomas are very uncommon. Herein we present two pediatric cases of isolated mediastinal cystic lymphangioma. In the first case, a seven-year-old girl presented with chest pain and dyspnea revealing a giant anterior mediastinal cystic lymphangioma extending inferiorly into the upper part of the abdomen. In the second case, a four-week-old boy presented with an acute respiratory distress revealing a voluminous posterior mediastinal cyst lymphangioma. In the view of literature, these cystic lymphangiomas are accepted to be atypical because of their locations and large size.
