Evaluation of apical extrusion of debris and centering ability in different nickel-titanium files during curved root canal preparation

BackgroundCurved root canals lead to difficulties in cleaning, shaping and filling the root canal system. Apical extrusion of debris and root canal transportation are important factors causing postoperative complications. In clinical practice, commonly selected instruments include multifile NiTi systems, such as M3-Pro PLUS (M3-PRO), Orodeka Plex 2.0 (ODP), Rotate (ROT), and Protaper Gold (PTG), as well as single-file NiTi systems, such as M3-L Platinum 2019 (M3L), Waveone Gold (WOG), and Reciproc Blue (RCB). This study aimed to comprehensively evaluate the differences in the apical extrusion of debris and centering ability of the above NiTi files.MethodsSeventy 3D-printed resin teeth were used (n = 10). The apically extruded debris was collected in a preweighed centrifuge tube. The resin teeth with or without root canal preparation were cut into separate cross sections at 1 mm, 3 mm, 5 mm, and 7 mm away from the root apex, and then the root canal transportation and centering ratio of each cross section were calculated.ResultsApical extrusion of debris was highest in RCB but lowest in OD-P (P < 0.05). Root call deviation was lowest in ROT at the 3 mm level, in PTG at the 5 mm level, and in PTG and ROT at the 7 mm level (P < 0.05). The centering ratio of NiTi files was highest in the RCB group at the 3 mm level, in the PTG group at the 5 mm level, in the ROT group at the 7 mm level (P < 0.05).ConclusionsFor NiTi files with the same system, the cross-sectional design is the greatest factor affecting the extrusion of debris, and motion mode is the second. In addition, the multifile system could reduce the degree of root canal transportation.

The Efficacy of Three Rotary Systems (Reciproc Blue, WaveOne Gold, and AF Blue R3) in Preparing Simulated, Highly Curved Root Canals: An In Vitro Study.

Purpose This study aimed to evaluate the effectiveness of three rotary, single-file, and reciprocating systems in terms of preparation time and canal centralization in simulated highly curved root canal preparation. Materials and methods The study sample consisted of 45 simulated canals with a curvature of 40°. They were randomly distributed into three experimental groups: Group 1-Reciproc Blue; Group 2-WaveOne Gold; and Group 3-AF Blue R3. A glide path was established by #10 hand K-file, then red dye was colored on the simulated canals, and photographs of the sample were taken before preparation. Then, the canals of each group were prepared, and other photographs were taken after preparation. The images of the two phases were combined using AutoCAD, where the canal was divided into three parts and the preparation area was measured from the right and left sides of each part of the canal. The preparation time for each system was also measured. Results While there were no statistically significant differences in the rate of preserving canal centralization in the middle and apical thirds between preparation groups, a statistically significant difference was found in the coronal third, as the Reciproc Blue and WaveOne Gold systems have a greater ability to maintain the centrality of the canal compared to the AF Blue R3 system. While there were no statistically significant differences between the Reciproc Blue and WaveOne Gold systems in the coronal third, as for the preparation time, it was found that there were statistically significant differences in the preparation time between the groups in favor of the WaveOne Gold system. Conclusion Both preparation systems (Reciproc Blue and WaveOne Gold) maintained the anatomical shape and canal centrality, with more cons for WaveOne Gold compared to the Reciproc Blue system. Regarding the volumetric changes, AF Blue R3 had the greatest changes compared to the Reciproc Blue and WaveOne Gold systems. WaveOne Gold Group, in terms of canal preparation time, showed the least time among the investigated groups.

Effect of nickel-titanium alloys on root canal preparation and on mechanical properties of rotary instruments.

The aim of this study was to evaluate the quality of curved root canal preparation, torsional fatigue, and cyclic fatigue of rotary systems manufactured with different NiTi alloys. Ninety single-rooted canals with curvatures of 15° to 30o were scanned and divided into three groups according to the rotary system used: BT-Race (BTR) - 10.06, 35.00, 35.04; SequenceRotaryFile (SRF) - 15.04, 25.06, 35.04; and ProDesignLogic (PDL) - 25.01, 25.06, 35.05. Each system was used on three specimens. The teeth were prepared, scanned, and analyzed to assess increase in volume, transportation, and centering ability of the root canal. Torsional fatigue of glide path instruments (BTR 10.06, SRF 15.04 and PDL 25.01) and cyclic fatigue of the finishing instrument (BTR 35.04, SRF 35.04 and PDL 35.05) were obtained by analyzing completely new instruments (n = 10) and instruments after they had been used three times (n = 10). After the torsional and cyclic fatigue tests, the fractured surface of the new and used instruments were examined by scanning electron microscopy. Increase in volume, canal transportation, and centering ability showed no significant differences among the groups (p > 0.05). The torsional test showed that SRF 15.04 produced the highest torque values for both new and used instruments, followed by PDL 25.01 and BTR 10.06 (p < 0.05). PDL 25.01, both new and used, exhibited higher values of angular deflection followed by SRF 15.04 and BTR 10.06 (p < 0.05). As regards cyclic fatigue, use of PDL 35.05, both new and used, required a longer time and larger number of cycles than did SRF 35.04 and BTR 35.04 (p < 0.05). Clinical use affected the torsional fatigue of BTR; however, cyclic fatigue was not significantly affected (p < 0.05). All rotary systems were able to prepare the curved canals satisfactorily and were used safely on the three specimens. Relative to torsional fatigue, SRF 15.04 exhibited a higher torque, and PDL 25.01, higher angular deflection. BTR 10.06 was the most affected by clinical use. PDL 35.05 showed greater resistance to cyclic fatigue.

Influence of endodontic cavity access on curved root canal preparation with ProDesign Logic rotary instruments.

This study evaluates whether endodontic cavity access into molars influences the chemomechanical preparation of curved root canals. Twenty mesiobuccal canals of maxillary molars were randomly divided into two experimental groups (n = 10), according to the type of endodontic access analyzed (conventional and minimally invasive). The preparations were made by a single operator, and the canals, after access, were prepared with Logic system (0.25, 0.04 taper) along the working length. Samples were scanned before and after canal preparation (SMX-90CT Plus (Shimadzu® microtomograph operated with 70 kV, 100 uA, FOV (XY) 13.3 mm, FOV (Z) 7.0 mm, and voxel size 0.013 mm/Pix)) to match canal volumes. Images were analyzed to evaluate surface areas, transport, and centralization at 3 mm from the canal entrance and 3 mm from the apex (OnDemand3DTM Dental software). Student's t test with a significance level of 5% was used for the statistical analysis. Regardless of the position of the analyzed canals, there was no difference in the prepared area regarding the transport and centralization of the endodontic instrument between the two types of cavity accesses performed. The conventional and minimally invasive access did not influence the action of the 0.25 and 0.04 taper instrument during the curved root canal preparation. The type of endodontic access did not influence the action of the rotary instrument during the curved root canal preparation.

Cone-beam computed tomography evaluation of curved root canals prepared using reciprocal rotary files and rotational rotary files.

Aim:The aim of this study is to compare curved root canal preparation between reciprocal rotary files and rotational rotary files in extracted teeth using cone-beam computed tomography (CBCT). The independent variable was the file type (reciprocal vs. rotary), and the primary outcome variable was the centering ratio of distance change after canal preparation. Secondary outcomes were canal transportation in extension and direction and the preparation time.Materials and Methods:Forty mesial root canals from extracted permanent molars with curvatures ranging between 25° and 45° were randomly divided into 4 groups. Canals were prepared with two types of reciprocal rotary files (WaveOne and Reciproc) and two types of rotational rotary files (ProTaper and Mtwo), ten canals in each group. The root canals were scanned before and after preparation using CBCT. Group differences in outcome variables were tested using ANOVA and 5% type I error probability.Results:At the middle third of canals, reciprocal rotary files yielded the least amount of deviation from the center in inner–outer furcal direction (P < 0.001). Transportation was shown from the original canal shape in all directions in four groups. The reciprocal rotary files took the least amount of preparation time as well (P < 0.001).Conclusions:Reciprocal files result in less transportation and working time than the rotational files.

An assessment following root canal preparation by Er,Cr:YSGG laser irradiation in straight and curved roots, in vitro

In the present study, the effectiveness of Er,Cr:YSGG laser in straight and curved root canal preparation was compared with that of the conventional canal preparation technique, in vitro. The degree of root curvature of 40 root canals was determined, and then 20 canals were prepared by an Er,Cr:YSGG laser of 2 W by using the crown-down technique, while the other 20 canals were shaped by K-file (control). The achievement degree of root canal preparation and debris score was investigated morphologically. The results indicated that straight root canals could be successfully prepared by Er,Cr:YSGG laser irradiation; a significant decrease of smear layer or debris was also recognized (P<0.01). However, canal preparation by laser device in curve root often leads to a ledge or zipped formation, perforation or over-instrumentation. The results demonstrated that further development in laser device and technique are required to ensure its success in root canal preparation, especially in curve root.

A comparative study of root canal preparation with NiTi‐TEE and K3 rotary Ni‐Ti instruments

To evaluate and compare several parameters of curved root canal preparation using two different Ni-Ti systems: NiTi-TEE (Sjöding Sendoline, Kista, Sweden) and K3 (Sybron Endo, Orange County, CA, USA). Fifty extracted mandibular molars with mesial root canal curvatures ranging from 20 to 40 degrees were divided into two groups. In one group, 50 root canals were instrumented using NiTi-TEE files to an apical size 30; 0.04 taper (the largest available size at the time of this study). In the other group, 50 root canals were prepared with K3 instruments to an apical size 45; 02 taper. Both systems were used in a crowndown manner, with copious NaOCl (3%) irrigation and a chelating agent (Calcinase Slide, lege artis, Dettenhausen, Germany), employing torque-controlled motors. For assessment of shaping ability, pre- and postinstrumentation radiographs and cross-sectional photographs of canals were taken and changes in canal curvature and root canal diameter documented. Cleaning ability was evaluated by investigating specimens of the apical, medial and coronal third of the root canal wall under a scanning electron microscope using 5-score indices for debris and smear layer. Procedural errors (instrument separations, perforations, apical blockages, loss of working length) and working time were recorded. Nonparametric anova was used to compare straightening of canal curvatures, canal cross-sections and canal wall cleanliness (P < 0.05), whereas working time was analysed using the parametric anova (P < 0.05). Both Ni-Ti systems maintained curvature well: the mean degree of straightening was 0.2 degrees for NiTi-TEE and 0.4 degrees for K3 with no statistical significance between the groups. Post-instrumentation cross-sections of the root canals revealed an acceptable contour (round or oval) in 50.6% of cases for the NiTi-TEE group and in 65.3% of cases for the K3 group. The difference was not significant. The SEM investigation of canal walls showed equally good debris removal for both systems: NiTi-TEE prepared canal walls in 74.7% of cases with scores I and II; K3 achieved these scores in 78.7% of cases. For smear layer, NiTi-TEE and K3 only received good scores (I and II) in 38.7% and 40% of canal wall specimens, respectively. For both parameters, no significant differences were found between groups. File fractures did not occur, but loss of working length was observed in one case following the preparation with NiTi-TEE and in three cases during K3 instrumentation. Mean working time was significantly shorter for NiTi-TEE (170 s) than for K3 (208 s). Both systems maintained original canal curvature well and were safe to use. Whilst debridement of canals was considered satisfactory, both systems failed to remove smear layer sufficiently.

Clinical evaluation of ProTaper NiTi rotary instruments in management of curved root canals

To evaluate clinical effect of the new ProTaper Nickel-titanium rotary instruments in preparation of curved root canals. 68 teeth with curved root canals were instrumented by ProTaper rotary instruments using crown-down technique, and by K files using step-back technique for control. All teeth were obturated with lateral condensation method. The efficiency of preparation and obturation was analyzed with radiographs before, during and after operation. No transportation, apical blockage and ledge were found in test. The technique could keep the curvature and flow of curved canals. There were two ledge and more apical transportation (P < 0.01) in control than in test. The operative time was shorter and post treatment pain seldom occurred in ProTaper group. The ProTaper NiTi rotary instruments can prepare curved root canals effectively and safely. It is an efficient instrumentation method for curved canals and can be used widely.

A study of histomorphological change of curved root canal preparation using GT rotary file, profile and stainless steel K-file

The purpose of this study was to compare the histomorphological change of curved root canal preparation using GT rotary File, Profile .04 taper and stainless steel K-file. 45 mesial canals(over 20 degree) of extracted human mandibular first molars were mounted in resin using a modified Bramante muffle system and divided into three groups. The roots were cross-sectioned at 2.5mm 5mm and 8mm levels from apical foramen. Tracings of the canals were made from preinstrumentation pictures of the cross section. The canals were prepared using a step-back technique with stainless steel K file(group 1), Profile .04 taper rotary file(group 2) and GT rotary file(group 3). Tracings of the prepared canals were made from postinstrumentation picture. Canal centring ratio. amount of transportation, area of dentin removed and shape of canal were measured and statistically were evaluated with Student-Newman-Keuls test using Sigma Stat(Jandel Scientific Software, USA). The results were as followings : 1 Amount of transportation of group 2 was the lowest at apical part, but there was no statistical difference. The direction of transportation was the outside of curvature at apical part. 2. Centering ratio at the apical part of group 1 was the highest, and there was statistical differences between apical and middle part, apical and coronal part(p＜0.05). Centering ratio at the middle part of group 3 was the lowest, and there was statistical difference between apical and middle part(p＜0.05). Centering ratio of group 2 was the lowest at apical part, but there was no statistical difference. 3. Amount of dentin removed of group 1 was the highest at coronal, middle and apical part among three groups, and there was statistical difference(p＜0.05). 4. The majority of the cross-sectioned canal shape after instrumentation were irregular at coronal, middle and apical part. But there are more number of round shaped canals at group 3 than other group.

Comparative Analysis of Curved Root Canal Preparation Using Nickel-Titanium Instruments With or Without EDTA

The purpose of this study was to examine the effect of the use of EDTA as a root canal irrigant in curved root canals instrumented with nickel-titanium (Ni-Ti) instruments. Twenty extracted maxillary molars were selected. Mesiobuccal roots were used. Teeth were mounted in self-curing acrylic resin bases to allow standardized angulation of the initial and final radiographs. The preinstrumentation radiographs were made with a #15 file in the canal. The canals were instrumented with Ni-Ti instruments by the step-back technique, using or not using EDTA. The post-instrumentation radiographs were made with a #30 file in the canal. The films were projected (x10 magnification), drawn, and superimposed. Ni-Ti instruments used with EDTA were less effective in maintaining the original path of curved canals.
