Effect of operative interval on efficacy of patients with open ocular trauma

Objective To investigate the effect of operative interval on efficacy of patients with open ocular trauma. Methods The clinical data of 102 patients with open ocular trauma from Jan. 2014 to Jan. 2018 were analyzed retrospectively.According to the timing of operation, all patients were divided into two groups, group A of 79 eyes in 52 cases and group B of 74 eyes in 50 cases.The operative interval between onset and operation in group A was 2-4 days, and in group B was 5-14 days.Vitrectomy was performed in both groups, and the efficacy were compared. Results The total cure rate in group A was 93.67%, which was higher than that in group B (82.43%)(P=0.031). The levels of serum IL-4, IL-6, IL-12, IL-17 and IFN-gamma in the two groups at 1 week after operation were lower than those before operation, and those in group A were lower than those in group B(P 0.05). Conclusion For the patients with open ocular trauma the sooner the operation is performed, the better the efficacy is. Key words: Trauma, ocular, open; Vitrectomy; Efficacy

Management of stylohyoid syndrome: A systematic review following PRISMA guidelines

ObjectivesA systematic review of the literature on stylohyoid syndrome treatment was performed according to PRISMA guidelines. Material and methodsThree hundred and forty-nine articles were retrieved in the PubMed and Cochrane databases using the search-terms “stylohyoid syndrome” and synonyms. Articles documenting treatment and outcome with more than 1 month's follow-up were selected. Treatment-related complications and rate of cure, defined as disappearance of symptoms and/or of revelatory complication, were analyzed. Overall analysis was performed for series and a mixed logistic regression model for case reports. ResultsHundred and two articles (12 series, 90 case reports) were selected. The 12 series included 482 patients with pain syndrome managed by styloidectomy, with 84.2% and 73.7% cure rates for cervical and transoral approaches, respectively. There were no complications with the transoral approach, versus 1.2% transient facial paresis with the cervical approach. In the 90 case reports, 112 patients had pain syndrome (Group I) and 16 neurological deficit (Group II). Cure rate in Group I varied significantly (P=0.005; OR 8.33, 95% CI [2.12–32.81]) from 64.3% following medical treatment (antiepileptics, muscle relaxants, analgesics, per os and/or locally injected anti-inflammatory drugs) to 91.8% following styloidectomy, without any significant impact of surgical approach (P=0.1; OR 0.17, 95% CI [0.02–1.60]). In Group I, no complications occurred after medical treatment, versus 4.3% and 16.3% after transoral and cervical styloidectomy, respectively. In Group II, cure and complication rates were 87.5% and 6.2%, respectively. Due to the small sample size and heterogeneity of Group II, no statistical assessment of the contribution of styloidectomy to medical treatment (antiplatelet drugs, with or without stenting) was performed. ConclusionStyloidectomy appears to be the treatment of choice for stylohyoid syndrome. The surgical approach does not significantly influence the cure or complications rate.

Efficacy of pterygium excision combined with pedicled conjunctival flap transplantation

Objective To evaluate the clinical efficacy of pterygium excision combined with pedicled conjunctival flap transplantation. Methods The clinical data of 126 eyes of 107 patients of pterygium from Dec.2015 to Nov.2016 were analyzed retrospectively and all cases were divided into 2 groups randomly. Group A, 60 eyes of 50 patients, were treated with pterygium excision. Group B, 66 eyes of 57 cases, were treated with pterygium excision combining pedicled conjunctival flap transplantation. The follow-up time was 1 year. Results The healing time of group B was shorter than that of group A, the cure rate of group B was higher than that of group A, the recurrence rate and complication rate of group B were lower than those of group A, the difference were statistically significant between the two groups (P=0.000, 0.032, 0.014, 0.017). The visual acuity of both groups after surgery were better than that before surgery (P=0.000). The difference of visual acuity at any time after surgery were not significant between the two groups (P=0.246, 0.113, 1.000, 0.164). At 6 months after surgery, there was no significant difference in BUT between the two groups (P=0.071). BUT of group B at 1 year after surgery were longer than that of group A (P=0.000). Conclusion Pterygium excision combined with pedicled conjunctival flap transplantation is benefit to improve the cure rate, maintain stable tear film, and prevent recurrence after operation. Effect of 0.9-ms 1064-nm Nd:YAG laser combined with itraconazole for treatment of toenail onychomycosis

To evaluate the efficacy and safety of 0.9-ms 1064-nm Nd:YAG laser alone or combined with itraconazole for treatment of toenail onychomycosis. A total of 37 patients with onychomycosis (178 toenails) were randomly assigned to groups A and B, and each group was further divided into different subgroups according to the Scoring Clinical Index of Onychomycosis (SCIO) and Onychomycosis Severity Index (OSI) scoring. All the patients were treated with 0.9-ms Nd:YAG laser once a week for 8 times. The patients in group A were treated with laser alone, and those in group B were treated with laser combined with itraconazole. The clinical effect, clinical scores, appearance of the toenails and adverse reactions in the two groups were analyzed, and the patients' satisfaction rate was also investigated. At the 12th months of follow-up, the clinical response rate and mycological cure rate in group A were 31.33% and 30.00%, respectively, similar to the rates in group B (35.79% and 41.18%, respectively) (P>0.05). After the treatments, the SCIO and OSI scores showed no significant changes in group A (P>0.05) but both increased significantly in group B (P<0.05). The response rates did not differ significantly among the subgroups with SCIO<12 or with OSI<16 (P>0.05), but showed significant differences among the subgroups with SCIO≥12 or with OSI≥16 (P<0.05). Of the total of 178 toenails, 33.71%, 74.72% and 70.79% toenails showed improvements in terms of clear nail growth, shape and color, respectively. The overall patients' satisfaction rate was 62.16%, and no adverse reactions related with the therapy were recorded in these patients. For treatment of toenail onychomycosis, 0.9-ms 1064-nm Nd:YAG laser can effectively improve the aesthetic appearance of the toenails, and a combined treatment with Nd:YAG laser and itraconazole can be better option in severe cases of onychomycosis.

Clinical observation on intracorneal injection of different anti-fungal drugs for the treatment of fungal keratitis caused by injury due to chestnut

Objective To observe the clinical efficacy of injection of different anti-fungal drugs in corneal stroma for the treatment of fungal keratitis caused by injury due to chestnut stab. Methods Eighty-three eyes of 83 cases of fungal keratitis caused by chestnut were randomly divided into three groups. The etiological results of all cases were obtained by fungal culture. After corneal debridement, patients in group A, group B and group C received the injection of fluconazole, amphotericin B and voriconazole in corneal stroma respectively on the basis of using 5% natamycin eye drops. The effect of the treatments for the three groups were observed. Results The first pathogenic bacteria was the Alternaria of the pathogen accounting for 34.9% (29 cases), followed by Fusarium 30.1% (25 cases) and Aspergillus 15.7% (13 cases). The cure rates of three groups were 70.3% (19/27), 76% (18/25) and 90.3% (28/31) respectively. The average cure rate of group C was higher than that of group A and B, and the differences were statistically significant (χ2=5.349, 4.895; P=0.021, 0.027). There was no statistically significant difference in the cure rate between group A and B (χ2=0.027, P=0.869). The cure time of group C was (22.48±8.36)d which was shorter than that of group A (27.40±3.97)d and group B (28.76±5.12)d and the differences were statistically significant (t=2.544, 3.006; P=0.014, 0.004). There was no statistically significant difference in the cure time between group A and B (t=1.069, P=0.290). Two cases in group B suffered eye irritation symptoms, and the symptom disappeared after the use of Diclofenac Sodium eye drops for two days. No adverse reactions during the treatment occurred in the remaining cases. All cases were followed up for 1-3 months and no puncture tract scar formed or no recurrent case happened. Conclusion The main pathogenic bacteria of fungal keratitis by chestnut stab are Alternaria and Fusarium. The injection of fluconazole, amphotericin B and voriconazole respectively in corneal stroma can bring better curative effect, especially the voriconazole and for severe cases the curative effect of voriconazole is better than that of fluconazole and amphotericin B. Effect of medical skin collagen regeneration functional dressing on facial dermatitis

Objective To observe and evaluate the healing effect of medical skin collagen regeneration functional dressing on facial dermatitis. Methods 40 facial dermatitis patients who met the inclusion criteria were randomly divided into the observation group(group A)and control group(group B)averagely. Patients of group A were administrated with ketotifen tablets orally and with medical skin collagen regeneration functional dressing externally, while patients of group B were only orally administrated with ketotifen tablets. The total observation period last 15 days with a follow-up once after 2 days for both of the groups. Results The cure rates of group A after 12 day and 15 day were 63.2%and 65%, higher than the cure rates of group B(5.0%and 5.0%). The 12 day and 15 day cure rates of group A were significantly higher than those of group B(χ2=14.83,15.82,all P<0.01). Conclusion Medical skin collagen regeneration dressing in combination with fexofenadine tablets provide a safe and effective healing function to facial dermatitis. Clinical characterization and treatment of acute spinal cord decompression sickness after repeated dives

To analyze and summarize the characteristics and treatment of acute spinal cord decompression sickness. To explore the factors that influence the treatment effect. 77 cases of acute spinal cord decompression sickness patients should be divided into 4 groups according to the pressurized treatment and drug treatment options. They were group I, group II, group III and group IV. At the same time they were get hyperbaric oxygen therapy and other treatment. The evaluation index, were Frankel function classification and paraplegia index. There were 17 factors that affected the treatment effect. The rate of early cure was 57.14% (44/77). The rate of late cure was 74.03% (57/77). Their difference was statistically significant (P<0.05). In 3 months and 1 year the cure rate of group IV and group B were the highest. But there was no difference between them(P>0.05). They were higher than group ii and group I (P<0.05). The Frankel function classification in 3months and 1 year in each group was higher than before treatment (P<0.05). The paraplegia index in 3 months and 1 year in each group was lower than before treatment (P<0.05). In 3 months and 1 year the Frankel function classification was increased gradually and the paraplegia index was decreased gradually in group I , group II, group III (P<0.05). In group IV and group III the Frankel function and the paraplegia index had not significant difference (P>0.05). Among the 17 factors that affect the treatment effect there are 9 factors that affect the proportion of the large. The first choice of treatment method for the patients with acute spinal cord decompression sickness would be group III. Drug therapy was also imporpant. At the same time the hyperbaric oxygen therapy and other treatments were taken. Although the cure rate was not high in this article. But most of the cure is within 3 months. Within 1 year.the cure rate still could be improved. 9 factors that affect the efficacy of acute spinal cord decompression sickness was more noteworthy.

Endoscopic dilatation combined with submucosal injection of triamcinolone acetonide for benign esophageal stricture

Objective To explore the safety and efficacy of endoscopic dilatation combined with submucosal injection of tfiamcinolone acetonide in treatment of benign esophageal stricture. Methods The patients of benign esophageal stricture proved by biopsy were randomly divided into three groups. Group A were treated by expansion only, group B were injected with triamcinolone acetonide submucosal combined with expansion, and we treated group C by another injection a week later on the basis of treatment in group B. A 12 months＇ follow-up was conducted, the dysphagia score, the cure rate. time to sustained symptom re- lief, re-expansion time interval and adverse reactions were recorded. Results Some patients experienced chest pain and regurgitation after the procedure and all improved by corresponding managements. No compli- cations including massive bleeding, infection, perforation or local tissue atrophy were observed. The dysphagia scores in 3 groups after the procedure were all improved （ P 〈 0. 05 ） , and no significant difference was noticed between 3 groups （ P 〉 0. 05 ）. At the end of the follow-up, the duration of dysphagia remission and the interval between 2 consecutive dilatation in Group A were 14.4 ± 3.2 weeks and 18.2 ± 3.7 weeks, respectively, which were significant shorter than those in Group B （19. 3 ± 3.9 weeks and 24. 6 ± 4. 2 weeks, respectively） and those in Group C （20. 2 ± 4. 2 weeks and 26. 1 ± 4. 5 weeks, respectively） , while there was no difference between Groups B and C. The cure rate in Group C （43.5% , 10/23 ） was significantlyhigher than that of Group A （29.2%, 7/24） or Group B （27.3% , 6/22） , while there was no difference between Groups A and B. Conclusion Endoscopic dilatation combined with submucosal injection of triamcinolone acetonide is safe and may improve the symptoms of dysphagia and delay the re-stenosis for patients with benign esophageal stricture, meanwhile, multi-frequency treatment may improve the cure rate. Improving islet yield and function by pancreatic ductal preservation

Objective To determine whether pancreatic ductal preservation（pancreatic ductal injection of sparing TIUW solution at the time of pancreas procurement）can improve islet yield and function after cold preservation of pancreas.Methods Pancreases were classified into five groups：fresh pancreases（group1 ,n= 10）;preserved for 6 hr in TIUW solution without and with pancreatic ductal preservation by TIUW solution（group2, n=10 and group3, n = 10）;preserved for 24 hr in TIUW solution without and with pancreatic ductal preservation by TIUW solution（group4 ,n= 10 and group5,n= 10）.Dithizon（DTZ）staining was used to identify islet morphous and yield, trypan blue uptake（TBU）was used to assess the nonviability of islets, the insulin secretory response to glucose was used to evaluate islet function in vitro, islet transplantation was used to determine islet function in vivo.Results Islet yields per pancreas after purification in group 1 to 5 were 590± 127, 272 ± 50,454 ± 65, 253± 56, 447 ± 66（islet equivalent ± SD）, respectively.Percentage of nonviable islets in grouplto5were（5.7±4.2）%,（18.3±6.5）%,（11.7±4.2）%,（26.3±5.6）%,（15.0±5.3）%.Stimulation index in group 1 to 5 were 7.32±2.32, 4.81±1.17, 7.56±2.44, 2.88±1.00, 5.71±1.90.Cure rates in group 1 to 5 were 100%, 0%, 100%, 0%, 70%.In this study, the differences were significant between control groups and experimental groups（P〈0.05, group 2 vs.group3 and group4 vs.group5）.Conclusion The pancreatic ductal preservation can improve islet yield and function. Observation on the effect of three species of drug treatment on vulvovaginal candidiasis in the mid trimester of pregnancy

Objective To observe the clinical therapeutic effect of the treatment on vulvovaginal candidiasis (VVC) in the mid trimester of pregnancy with three species of drug (Clotrimazole,Macmirorcomplex, Baofukang Shuan). Methods 83 women diagnosed with VVC in the mid trimester of pregnancy were divided into three groups: Group A with 30 cases, treated with Clotrimazole for twice; Group B with 28 cases, treated with Macmirorcomplex for 6 days; Group C with 25 cases, treated with Baofukang Shuan for 8 days. Observed the clinical effect, the microbes therapeutic effect and the adverse effect of the treatment with three species of drug. Results (1)The clinical cure rate of Group A was 86.67% and the fungal negative rate was 93.33%. The clinical cure rate of Group B was 57.14% and the fungal negative rate was 75%. The clinical cure rate of Group C was 60% and the fungal negative rate was 40%. There were no adverse effect to be found in the three groups. (2) The clinical cure rate and curative rate of Group A were significantly higher than those of Group B and Group C( P＜0,05 ),but there were no difference in Group B and Group C ( P＞ 0.05 ). The V VC scores of the three groups were significantly descended compared with those of pretherapy ( P＜ 0.01 ), but there were no difference of the curative rate about microbes therapeutic effect in the three groups ( P＞ 0.05 ).Conclusion The three drugs for VVC in pregnancy are all effective and no serious adverse effect. Clotrimazole has better clinical therapeutic effect than Macmirorcomplex and Baofukang Shuan.
