Low-Field Magnetic Resonance Imaging for the Detection of Medial Meniscal Lesions in Cranial Cruciate-Deficient Stifle Joints in Dogs

Background: The aim of this study was to substantiate the ability of noninvasive low-field magnetic resonance imaging (lfMRI) to accurately depict meniscal lesions, thereby establishing lfMRI as a viable option for the diagnosis of meniscal pathology. Study Design: This was a prospective, controlled study. Materials and Methods: In each of 57 stifle joints, 5 predefined lfMRI sequences using a 0.25 T lfMRI were used. The scans were evaluated by an experienced surgeon (DECVS) and through external assessment by a radiologist (DECVDI). The patients were divided into two groups depending on their meniscal status after lfMRI. Dogs (Group 1; n = 33) without lfMRI evidence of medial meniscal pathology underwent TPLO, whereas concurrent craniomedial mini-arthrotomy was performed in patients with suspected meniscal lesions (Group 2; n = 24). Gait assessment was performed 6 weeks later and 6 months postoperatively. Results: Cohen’s kappa value of 0.8571 corresponds to a high level of agreement between the observers. A significant correlation was found between preoperative lameness severity and intraoperative meniscal damage (p < 0.04). The results showed a highly significant correlation between meniscal injuries on MRI and intraoperative findings (p < 0.0001). Conclusions: LfMRI appears to be a noninvasive, reliable imaging tool for the detection of medial meniscal lesions in cranial cruciate-deficient stifle joints, aiming to avoid (minimally) invasive diagnostics in healthy menisci.

TTA-Rapid for treating the canine cruciate deficient stifle: Medium to long-term outcome and analysis of risk factors.

This study aimed to evaluate medium to long-term outcomes following treatment of lameness associated with cranial cruciate ligament insufficiency using the tibial tuberosity advancement (TTA)-Rapid technique and to investigate risk factors associated with outcome, including tibial plateau angle (TPA). Patient records for cases treated over an 80-month period were examined retrospectively. An owner postal questionnaire using a clinical metrology instrument (Liverpool Osteoarthritis in Dogs) evaluated the outcome at least 6 months after surgery. A total of 149 procedures were carried out in 120 dogs. Questionnaires were received for 64 dogs (53.3%), with a median follow-up time of 37.5 months. Outcome was judged to be satisfactory, based on frequency of lameness, in 63 (98.4%), while satisfactory outcomes were achieved in 61 (95.3%) based on severity of lameness and in 50 (78.1%) based on mobility score. Statistical analysis showed that age, bodyweight, TPA, meniscal injury and concurrent patellar surgery did not influence outcome, but time to follow-up positively correlated with mobility score. This study involved one surgeon in one centre with outcome data based on the completion of an owner questionnaire, although this used a validated clinical metrology instrument. TTA-Rapid offers a treatment option for cranial cruciate-deficient stifles with a good medium to long-term outcome, and no risk factors were identified to suggest any limitations to its use.

The Effect of Increasing Tibial Tuberosity Advancement and Quadriceps Muscle Force on Cranial Translation of the Tibia in the Cranial Cruciate Deficient Stifle Joint in Dogs.

BackgroundCranial cruciate ligament (CCL) disease is a well-known pathology that generates both rotational and translational instability of the stifle joint that leads to osteoarthritis in dogs. Tibial tuberosity advancement (TTA) is a common surgical technique used to dynamically neutralize the tibiofemoral shear forces to achieve stifle joint stability. However, significant persistent instability has been documented in clinical cases. The purpose of this study was to evaluate the effect of increasing quadriceps load, increasing tibial tuberosity advancement, and increasing joint flexion angle on the cranial translation of the tibia relative to the femur in the cranial cruciate ligament deficient stifle joint.Methods and ResultsSix cadaveric hind limbs were collected from six healthy mixed breed dogs of medium build and prepared for biomechanical testing. The specimen was placed into a custom-made joint testing machine, and translation of the tibia relative to the femur was measured at stifle angles of 135°, 120°, and 105°. Cranial tibial thrust was generated by applying a vertical load to the metatarsal pad and the quadriceps muscle was simulated with loads of 0, 5, and 10 kg applied to the patella via a system of weights and pulleys. All specimens were tested with the CCL intact and cut, both of which served as controls. The tibial tuberosity was then advanced using both 6 mm and 9 mm cages, and the specimen was tested using the identical technique. Each specimen was loaded to failure by increasing the load applied to the pes until the sudden marked cranial translation of the tibia. Tibial tuberosity advancement with an applied quadriceps load was successful in limiting cranial tibial translation in 54/62 tests. Under similar loading conditions, TTA failed to limit cranial translation in 8 tests. The failures occurred at all angles of flexion examined. In the cases that failed cranial translation could be limited by either increasing the quadriceps load or increasing the amount of tibial tuberosity advancement.ConclusionThis study showed that TTA with an applied quadriceps load is effective at decreasing cranial tibial translation at functional joint angles.

Three-dimensional kinematic evaluation of lateral suture stabilization in an in vitro canine cranial cruciate deficient stifle model.

The impact of surgical correction of cranial cruciate ligament rupture (CCLR) on 3D kinematics has not been thoroughly evaluated in dogs. The success of current techniques remains limited, as illustrated by suboptimal weightbearing and progression of osteoarthritis. The inability to restore the stifle’s 3D kinematics might be a key element in understanding these suboptimal outcomes. The objective of this study was to evaluate the impact of lateral suture stabilization (LSS) on the 3D kinematics of the canine stifle joint. We hypothesized that LSS would not restore 3D kinematics in our model. Ten cadaveric pelvic limbs collected from large dogs (25–40 kg) were tested using a previously validated apparatus that simulates gait. Three experimental conditions were compared: (a) intact stifle; (b) unstable stifle following cranial cruciate ligament transection (CCLt) and (c) CCLt stabilized by LSS. Three-dimensional kinematics were collected through 5 loading cycles simulating the stance phase of gait and curves were analyzed using a Wilcoxon signed-rank test. LSS restored baseline kinematics for the entire stance phase for cranial and lateromedial translation, flexion, and abduction. It restored distraction over 90% of the stance phase. Internal rotation was limited, but not restored. This in vitro study had limitations, as it used a simplified model of stifle motion and weight-bearing. The results of this study report that LSS can restore physiologic 3D kinematics largely comparable to those of healthy stifles. Suboptimal outcome in patients following CCLR stabilization by LSS may therefore result from causes other than immediate postoperative abnormal 3D kinematics.

Influence of tibial plateau leveling osteotomy and tibial tuberosity advancement on passive laxity of the cranial cruciate deficient stifle in dogs.

To evaluate the effect of tibial plateau leveling osteotomy (TPLO) and tibial tuberosity transposition (TTA) on craniocaudal stability of the cranial cruciate ligament (CCL) deficient stifle at 3 joint angles. Ex vivo biomechanical study. Eight stifles from 4 dogs. One of the hind limbs was randomly assigned to undergo TPLO or TTA, with the contralateral limb assigned to the alternate procedure. Each specimen was tested with the stifle at 150°, 135°, and 120° of flexion. Cranial translation of the tibia relative to the femur was measured under cranial loads ranging from 0.2 to 2.0 kg with a single motion tracking sensor (Nest of Birds) fixed to the tibia. Each stifle was tested before and after transection of the CCL and after performing the assigned procedure. Tibial plateau leveling osteotomy and TTA caused a decrease in the cranial translation of the tibia relative to the femur, especially at greater angles of flexion. Tibial plateau leveling osteotomy caused a greater decrease in tibial cranial translation and was associated with a degree of translation that did not differ from joints with intact CCL, regardless of the degree of stifle flexion. Tibial plateau leveling osteotomy and TTA caused an angle dependent decrease in craniocaudal translation of the tibia in the CCL deficient stifle. Only TPLO restored translations similar to those of the intact joint at all angles tested. Tibial plateau leveling osteotomy and TTA provide passive stability to the stifle. Tibial plateau leveling osteotomy seems more effective than TTA at restoring craniocaudal stability of the stifle.

TTA-Rapid in the treatment of the canine cruciate deficient stifle: short- and medium-term outcome.

To evaluate complications, short- and medium-term outcomes following treatment of lameness associated with cranial cruciate ligament insufficiency in dogs using the "TTA-Rapid" technique. Patient records from two veterinary clinics were retrospectively examined to collect data from cases treated over a period of 21 months. An owner postal questionnaire using a clinical metrology instrument (Liverpool Osteoarthritis in Dogs) evaluated medium-term outcome of at least six months. In total 152 procedures were carried out in 141 dogs by two surgeons. Major complications, requiring further intervention, were identified in 11 (7%). Nine of these involved "late" meniscal injuries and two sustained tibial fractures postoperatively, one requiring internal fixation and the other external coaptation. The 3-month outcome was considered satisfactory in 99% of 135 procedures whilst the medium-term outcome (mean 16 months) was considered satisfactory in 86% of 108 dogs. "TTA-Rapid" offers a treatment option for cranial cruciate-deficient stifles with good short- and medium-term outcomes. The complication rate was low in our hands.

Effect of tibial tuberosity advancement on cranial tibial subluxation in the feline cranial cruciate deficient stifle joint: An ex vivo experimental study

The effects of Tibial Tuberosity Advancement (TTA) on Cranial Tibial Subluxation (CTS) and Tibial Rotation Angle (TRA) were evaluated in a model of feline Cranial Cruciate Ligament (CrCL)-deficient stifle joint. Ten hindlimbs of adult cats were used. Quadriceps and gastrocnemius muscles were simulated using cables, turnbuckles and a spring. An axial load of 30% body weight was applied. The stifle and talocrural joint angles were adjusted to 120°. Patellar tendon angle (PTA), CTS and TRA were measured radiographically before and after CrCL section, after TTA and after additional advancement by 1 and 2mm. CrCL section resulted in a CTS of 8.1±1.5mm and a TRA of 18.4±5.7 °. After TTA, PTA was significantly decreased from 99.1±1.7° to 89.1±0.7°; CTS and TRA did not change significantly (7.8±1.0mm and 15.9±5.7° respectively). Additional advancement of the tibial tuberosity by 1mm did not significantly affect CTS and TRA. Additional advancement of the tibial tuberosity by 2mm significantly reduced the PTA to 82.9±0.9°. A significant decrease of CTS (6.9±1.3mm) and TRA (14.7±3.6°) was also observed. A lack of stabilization of the CrCL deficient stifle was observed after TTA in this model of the feline stifle. Even though the validity of the model can be questioned, simple transposition of the technique of TTA from the cat to the dog appeared hazardous.

Effect of tibial plateau levelling osteotomy on patellar tendon angle: a prospective clinical study.

To evaluate the effect of the tibial plateau levelling osteotomy (TPLO) procedure on the patellar tendon angle (PTA) in dogs with naturally occurring cranial cruciate ligament rupture. Prospective observational clinical study. Dogs with naturally occurring CrCL rupture that had a TPLO performed (n = 40). Preoperative tibial plateau angle (TPA) and PTA were measured on radiographic images of affected stifle joints at 90° and standing angle (135°) respectively. The TPA and PTA were measured after TPLO was performed. Regression analysis was performed to evaluate the effect of preoperative TPA and PTA and postoperative TPA on postoperative PTA. Linear regression analysis was performed to evaluate the correlation between postoperative TPA and PTA. The mean (± SD) preoperative TPA and PTA were 26.5 ± 3.8° and 105.7 ± 3.8° respectively. The postoperative TPA and PTA values were 7.6 ± 3.3° and 91.4 ± 5.5°, respectively. Regression analysis showed that higher preoperative PTA and postoperative TPA are associated with a larger difference between the postoperative PTA and 90°. There was a positive correlation between postoperative TPA and PTA. The TPLO procedure alters the PTA to 90° in dogs with cranial cruciate ligament rupture similar to tibial tuberosity advancement (TTA). The TPLO may provide dynamic stability to the cranial cruciate deficient stifle by altering the TPA relative to the patellar tendon and creating a PTA of 90°. The biomechanical principle and mechanism of action of TPLO may be similar to TTA.

Extra-articular stabilization of the cranial cruciate deficient stifle with anchor systems

Complete or partial rupture of the cranial cruciate ligament (CCL) is a common injury of the canine stifle. Most practicing veterinarians would agree that optimal outcome is best achieved with surgical intervention. A popular method of stabilization is an extra-articular suture stabilization. The objective of this manuscript is to describe suture placement in a more isometric position as compared to traditional suture placement. A second objective is to introduce the veterinary surgeon to novel anchor products used for stabilization.

Evaluation of a Transcondylar Toggle System for Stabilization of the Cranial Cruciate Deficient Stifle in Small Dogs and Cats

To evaluate use of a transcondylar toggle system (TCTS) for stabilization of the cranial cruciate ligament (CrCL) deficient stifle in small dogs and cats. Prospective clinical study. Small dogs (<7 kg; n=14) and cats (2) with CrCL-associated lameness of <3 months duration and a tibial plateau angle <32 degrees. Affected animals had an extracapsular CrCL repair using the TCTS. Lameness score, muscle atrophy, osteoarthritis (OA) score, and range of motion (ROM) were evaluated preoperatively, and at 6 weeks and 7-10 months postoperatively. Operative time was 75 +/- 16 minutes. Fifty-six percent required >1 bone tunnel attempts. One dog required revision at 2 weeks because of suture loosening. All stifles were stable at 6 weeks postoperatively. Fifteen animals were available for follow-up (7-10 months). Lameness improved significantly at 6 weeks (P<.0001), whereas muscle atrophy was worse at 6 weeks (P=.008) but improved at 7-10 months (P<.0001). OA scores were unchanged at 6 weeks (P=.08) but were significantly worse at 7-10 months (P<.0001). ROM remained unchanged at 6 weeks (P=1) and 7-10 months (P=.6). The medially placed toggle provides a reliable short-term proximal anchor for the extracapsular suture with outcomes similar to other extracapsular techniques. The aiming device and drill bit are not recommended in their current form. The TCTS appears to be a well-tolerated technique for proximal suture anchoring in extracapsular CrCL repair in small dogs and cats where instrumentation and anatomic constraints preclude other techniques.

Modified stabilization method for the tibial tuberosity advancement technique: a biomechanical study

The present study aimed to determine biomechanical alterations resultant from a modification in the fixation method of the tibial tuberosity advancement technique (TTA), originally described for stabilization of the cranial cruciate-deficient stifle. Ten adult mongrel dogs weighing 25-30kg were used. After euthanasia, performed for reasons unrelated to this study, the hind limbs were distributed into two groups: G1 operated (n=10) and G2 control (n=10), represented by the contralateral limb. The operated hind limbs were orthopedically, goniometrically and radiographically evaluated, sequentially at four moments: moment 1, in intact joints; moment 2, after cranial cruciate desmotomy; moment 3, after surgical stabilization of the stifle joint using modified TTA; and moment 4, after caudal cruciate ligament desmotomy. The tibial tuberosity was stabilized by one shaft screw craniocaudally and a titanium cage inserted at the osteotomy site. The position of the patellar tendon at 90° in relation to the tibial plateau allowed cranial tibial thrust force neutralization, despite cranial drawer motion maintenance in all dogs. The biomechanical tests confirm the viability of the tibial tuberosity fixation method and support future clinical trials to validate the technique.

Mechanical resistance of the modified stabilization method for the tibial tuberosity advancement technique: ex vivo experimental study in dogs

The present study aimed to evaluate a modification of the stabilization method of the tibial tuberosity advancement technique (TTA), originally described for stabilization of the cranial cruciate deficient stifle. Ten adult mongrel dogs with weights ranging from 25 to 30kg were used. After euthanasia, the hind-limbs were divided into two groups: G1 - test (n=10), and G2 - control (n=10) represented by the contra lateral limb. The test group was submitted to the modified TTA technique, stabilized by one shaft screw in craniocaudal position and a titanium cage inserted at the osteotomy site. The position of the patellar tendon, 90° in relation to the tibial plateau, and the correct position of all implants were confirmed radiographically after surgery. Posteriorly, in both groups, limbs were harvested and tibias collected with their respectively patellar tendon insertion preserved for the mechanical resistance test. The fixation of the tibial tuberosity with a shaft screw and titanium cage resulted in resistance compatible with the normal physiological forces transferred to the hind-limbs during locomotion. The biomechanical tests confirmed the viability of the method performed for the tibial tuberosity fixation and support future clinical trials to validate the technique.

Effect of Meniscal Release on Rate of Subsequent Meniscal Tears and Owner‐Assessed Outcome in Dogs with Cruciate Disease Treated with Tibial Plateau Leveling Osteotomy

To determine and compare rates of meniscal tears after tibial plateau leveling osteotomy (TPLO) among 3 groups of dogs based on treatment method: arthrotomy with meniscal release (openR), arthrotomy without meniscal release (openNR), arthroscopy without meniscal release (scopeNR), and compare long term owner-assessed outcomes for the same groups. Retrospective cohort study. Stifles (n=254) of dogs that had TPLO. The three groups were compared for significant (P<.05) differences in rate of subsequent tears using a chi(2) test. Odds ratios for likelihood of subsequent meniscal tears were determined. Data for signalment, outcome, time to peak function, and time to subsequent tear were compared for significant differences using ANOVA, t-test, or rank sum test. Subsequent meniscal tears were diagnosed in 16 cases (6.3%). Of dogs with subsequent meniscal tears, 9 had openNR, 4 had openR, and 3 had scopeNR; the proportion of subsequent meniscal tears was significantly different (P=.035) among groups. Odds ratio indicated that subsequent meniscal tear was 3.8 times more likely to occur for openNR than openR or scopeNR. No significant differences among groups were noted for measures of outcome. Meniscal release did not reduce the rate of subsequent meniscal tears when compared with cases treated arthroscopically or when compared with all cases combined, but may be advantageous when meniscal pathology cannot be comprehensively assessed in the cranial cruciate deficient stifle. Meniscal release had no effects on owner-assessed outcome as determined in this study. The low rates of subsequent meniscal tears in conjunction with the relatively high and equivocal levels of owner-assessed outcome and time to peak function for all 3 treatment groups suggest that any of these surgical management strategies can be considered acceptable. We suggest that a meniscal release be performed when complete and thorough exploration of the joint and meniscus cannot be, or are not, performed.

Proximal tibial osteotomy for the repair of cranial cruciate-deficient stifle joints in dogs

Factors Related to the Risk of Meniscal Injury in Dogs with Cranial Cruciate Ligament Rupture

Neaas A., J. Zatloukal: Factors Related to the Risk of Meniscal Injury in Dogs with Cranial Cruciate Ligament Rupture. Acta Vet. Brno 2002, 71: 77-84. Risk factors for damage of the medial meniscus in 160 cranial cruciate deficient stifles in 138 dogs have been evaluated. The medial meniscal damage was found in a total of 72 patients (45%) with CCL rupture. The risk of damage to the meniscus was significantly higher in dogs of 25-45 kg compared to those weighing less than 24 kg. Cases of more severe grades of arthritis and joint effusion were significantly more often accompanied by the medial meniscal damage. Severe osteoarthritis of grade 3, as compared to the grade 1, was considerably frequently (p < 0.01) associated with the medial meniscal lesion. The meniscal damage was most commonly accompanied by moderately severe stifle joint effusion. The medial joint cavity collapse was a very important radiographic sign expressing the medial meniscal damage (p < 0.01). Its sensitivity and specifity in the diagnostics of the medial meniscal damage is 52% and 71%, respectively. Duration of lameness, extent of the CCL rupture (partial versus total) and radiographically measured index of the stifle joint laxity do not belong to statistically significant risk factors of the medial meniscal damage.

Effect of tibial plateau leveling on cranial and caudal tibial thrusts in canine cranial cruciate-deficient stifles: an in vitro experimental study.

To investigate the effect of tibial plateau leveling (TPL) on tibial subluxation and tibial axial rotation; to determine the minimal tibial plateau rotation (MinTPR) angle that provides stifle stability; and to evaluate caudal cruciate ligament (CaCL) strain following tibial plateau rotation in cranial cruciate ligament (CrCL)-deficient stifles. Fifteen canine cadaver hind limbs. Tibial subluxation was measured from lateral radiographs in intact, loaded stifles and after sequential CrCL transection, MinTPR, TPL, and CaCL transection. The MinTPR angle was determined using a custom-made hinge plate and compared with the TPL angle. Tibial axial rotation was evaluated in CrCL-deficient stifles before and after TPL. Finally, CaCL strain was recorded in intact, loaded stifles, and following MinTPR, TPL, and tibial plateau over-rotation (MaxTPR) using a force probe. Cranial tibial subluxation in CrCL-deficient stifles was eliminated with TPL. Tibial plateau rotation, however, induced caudal tibial subluxation, which significantly increased from MinTPR to TPL before and after CaCL transection. The MinTPR angle was 6.5 degrees +/- 0.9 degrees less than the TPL angle (P <.05). Tibial internal rotation decreased significantly after TPL in CrCL-deficient stifles. Finally, CaCL strain increased with increasing tibial plateau rotation. This study suggests that, during stance phase, TPL transforms cranial tibial thrust into caudal tibial thrust, thereby stabilizing the stifle in the cranio-caudal plane via the constraint of the CaCL. The increase in CaCL stress, which results from tibial plateau rotation, could predispose the CaCL to fatigue failure and therefore would caution against tibial plateau over-rotation.

A Prospective Study Comparing Two Fascial Reconstruction Techniques to Stabilise the Cranial Cruciate Deficient Stifle in the Dog

SummaryThe standard intra-articular “underand-over” (U &amp; O) cranial cruciate ligament substitute technique, using autogenous fascia lata, was compared with a combined intra- and extracapsular U &amp; O fascial reconstruction procedure. Medium, large and giant dog breeds (body weights ≥ 15 kg), presented with unilateral rupture of the cranial cruciate ligament, were included in the study. Functional usage of the operated limb was evaluated according to a clinical grading system within the first six weeks and again at 12 and 26 weeks after the operation.Dogs treated with the standard intraarticular U &amp; O procedure were partly weight-bearing on the operated limb within three to five weeks postoperatively. Return to full use of the limb with no or only mild, occasional lameness was recorded in 20 of 24 dogs (83.3%) after 26 weeks. Those treated with the combined intra- and extracapsular U &amp; O procedure, returned to partial weight-bearing within four to seven weeks after the operation. These dogs exhibited slightly more cranial drawer motion (± 1.0 - 2.0 mm) at the six- and 12-week follow-up examinations, when compared to the first group, but stifle stability improved between 12 and 26 weeks. The results obtained from 45 of 55 dogs (81.9%) treated with the combined intra- and extracapsular U &amp; O technique were comparable with the standard intra-articular U &amp; O procedure with return to full use of the limb after 26 weeks. Radiological evidence of peri-articular osteophyte formation showed no remarkable difference between the two techniques after 26 weeks. Results obtained from the standard intra-articular U &amp; O technique indicated that it was an effective method for use in large dogs with body weights greater than 30 kg. This is in agreement with findings in other studies. The combined intra- and extracapsular U &amp;O fascial reconstruction was a reliable alternative for use in the medium to large dog breeds with body weights ranging between 15 and 26 kg.The standard intra-articular “under-and-over” (U &amp; O) replacement technique was effective for use in dogs with body weights (BW) &gt;30 kg. Dogs treated with the combined intra- and extracapsular U &amp; O method had slightly more cranial drawer motion at the six-week and 12-week examinations, but stifle stability improved between 12 and 26 weeks, especially in medium-sized to larger dogs with BW ranging between 15 and 26 kg.

The Use of Orthopedic Wire as a Lateral Suture for Stifle Stabilization

Extracapsular suture stabilization is a long-accepted technique for stabilization of cranial cruciate deficient stifles in small-breed dogs. Use of stainless steel wire as the suture material in the extracapsular technique extends the procedure to use in many large breed dogs and is currently the author's surgery of choice. The surgical technique and illustrative cases are described.

Tibial Plateau Leveling Osteotomy for Repair of Cranial Cruciate Ligament Rupture in the Canine

A new technique for correcting the cranial cruciate deficient stifle, the tibial plateau leveling osteotomy, is introduced by the author. The technique is described and illustrated. Clinical results of 394 cases are described.
