Impact of alveolar corticotomy-assisted orthodontics on root resorption and treatment duration: A prospective study

The impact of alveolar corticotomy-assisted orthodontics (ACAO) on root resorption and treatment duration in orthodontic patients is of interest. Fifty patients were randomly divided into ACAO and control groups, with root resorption assessed via CBCT and treatment duration recorded. The ACAO group exhibited significantly shorter treatment duration (10.2 ą 1.8 months) compared to the control group (16.5 ą 2.1 months, p<0.05). A slight increase in root resorption was observed in the ACAO group (0.8 ą 0.3 mm) versus the control (0.5 ą 0.2 mm, p<0.05), but no severe resorption was detected. ACAO effectively accelerates orthodontic treatment without clinically significant root resorption.

The Efficacy and Modes of Accelerating Orthodontic Tooth Movement

Orthodontic treatment, while highly advantageous for correcting dental misalignments and enhancing oral health, typically demands a substantial time commitment. To reduce treatment duration and improve various aspects of the orthodontic process, accelerated methods for tooth movement have been explored. These methods aim to offer benefits such as decreased discomfort, enhanced patient compliance, and overall treatment efficiency. This study undertakes a review of existing literature, aiming to comprehensively assess the current evidence on accelerated orthodontic tooth movement. The review encompasses various acceleration techniques, including high-frequency vibration, microosteoperforations, Photobiomodulation, corticotomy-assisted orthodontics, and advancements in appliance technology. While some interventions demonstrate accelerated tooth movement and enhanced patient comfort, the field calls for further research, particularly in the form of large-scale clinical trials, to establish long-term efficacy, safety, and applicability across diverse orthodontic cases. This review offers valuable insights into the evolving landscape of orthodontics, emphasizing the need for evidencebased approaches and ongoing innovation to optimize patient outcomes and experiences in orthodontic care.

Periodontal effect of augmented corticotomy-assisted orthodontics versus conventional orthodontics in treatment of adult patients with bialveolar protrusion

BackgroundThe patients of bialveolar protrusion always demonstrate thin anterior alveoli which may aggravate subsequent gingival recession and bone loss during retraction. This study aimed to investigate the periodontal changes, including alveolar height, thickness, and area, and the width of keratinized gingiva, in mandibular anterior teeth after augmented corticotomy-assisted orthodontics (ACAO) compared with traditional orthodontics.MethodsTwenty adult patients with skeletal class I bialveolar protrusion were selected from two groups: ACAO group (augmented corticotomy on the labial side of the anterior mandibular teeth, n = 10) and control group (conventional orthodontics, n = 10). In all patients, four first premolars were extracted and the incisors were retracted under the maximum anchorage. The measurements included the labial alveolar bone area, vertical alveolar bone height, alveolar bone thickness surrounding the mandibular anterior teeth, root length, gingival recession and width of keratinized gingiva after alignment (T0) and 3 months after space closure (T1).ResultsThe labial alveolar height, area, and thicknesses all decreased after space closure in the control group but significantly increased in the ACAO group. The decrease in the lingual alveolar height was statistically less in the ACAO group than that in the control group. Besides, the width of keratinized gingiva increased in the ACAO group but decreased in the control group. There was no significant difference in the changes of root length between groups. The dentoalveolar changes between anterior teeth were consistent but with different scales. The lateral incisors gained the most labial bone height and area.ConclusionCompared to conventional orthodontics, ACAO provided a more favorable effect of improving periodontal status surrounding the mandibular anterior teeth for Class I maxillary protrusion patients.

Trending topics in orthodontics research during the last three decades: A longitudinal bibliometric study on the top-cited articles.

To perform a bibliometric analysis on the top-cited articles over the last three decades from 1989 to 2018, using a longitudinal set-up, in order to analyse how articles' features have evolved over time. A sample of 312 articles divided into three subgroups of 104 top-cited articles for each decade were included. The h-index value was used to determine the sample size. The articles were identified through a search in the category "dentistry oral surgery and medicine" of the Web of Science database. The information relative to the articles was collected. Descriptive statistics was used to describe the articles and the journals; selected parameters were analysed with the Kruskal-Wallis and the Mann-Whitney U test at the 0.05 level of significance. The top-cited articles were published mainly by orthodontic journals. Along the last two decades, the scientific contribution of Asian and South American countries rose significantly. The trending topics varied in each decade. Skeletal anchorage-related articles dominated the production of the decade 1999-2008 while new technologies as CBCT displayed a higher prevalence in the last decade, where new topics such as corticotomy-assisted orthodontics started to appear. The number of multi-authored articles increased while the articles from no university institutions decreased. The publications in the three studied decades were significantly different in terms of numbers and characteristics. A longitudinal set-up allows to study the evolution of the scientific production and how the area of interest has developed over time.

Long-term Effects of Augmented Corticotomy-Assisted Orthodontics in Patients With Mandibular Anterior Alveolar Defect.

The purpose of this study was to quantitatively investigate the long-term effects of augmented corticotomy-assisted orthodontics in adult patients with alveolar defect in mandibular anterior region. Thirty lower incisors with alveolar defect on the labial sides were included in this study from 10 adult patients received augmented corticotomy-assisted orthodontics in mandibular anterior region. Cone-beam computed tomography images were obtained before orthodontic treatment (T0), 3 months after augmented corticotomy (T1) and 2 years after removal of orthodontic appliance (T2). Measurements of variables at different time points were compared using Student-Newman-Keuls test. Each patient completed augmented corticotomy-assisted orthodontics successfully. In the 30 subjects studied, root length and bone thickness on the labial side at 1/2 root length level were maintained from T0 to T2 (P >.05). Bone thickness at 3/4 root length level increased from T0 to T1 (P <.001), then decreased from T1 to T2 (P <.001), and which of T2 is greater than that of T0 (P <.001). Bone thickness at root apex level increased from T0 to T1 (P <.001), and remained steady from T1 to T2 (P >.05). Augmented corticotomy-assisted orthodontics is a promising approach for the treatment of adult patients with alveolar defect, which showed stable long-term effect of bone augmentation and minimal periodontal iatrogenic sequelae during treatment.

A piezo surgery with corticotomies and implant placement as part of a multidisciplinary approach to treat malocclusion disorder in an adult patient: clinical report.

This clinical report illustrates a multidisciplinary approach for the rehabilitation of a young adult patient affected by a bilateral edentulous space and an anterior deep bite. The patient required orthodontics and surgical corticotomy and implantology (both performed with a piezo device). A multidisciplinary planning approach, including orthodontics, oral and periodontic surgery, and restorative dentistry, has an important role in the final outcome of treatment. In fact a dental class I occlusion has been established only on the right side. The left side could not be restored to an ideal class I relationship due to the pontic prosthesis. The original collapsed right posterior occlusion was corrected. A stable posterior occlusion was established, and the balancing interference was eliminated. Centric relation and centric occlusion were established at the same vertical dimension of occlusion. The cephalometric analysis and clinical aspect at the end of treatment showed that the patient had improvements in overbite and overjet.Multidisciplinary management, including endodontic and restorative dentistry, periodontics, corticotomy-assisted orthodontics, implants, and prosthetics, was used for a young female patient with multiple missing teeth, anterior deep bite, and a malocclusion with cant of the occlusal plane. The interaction of interdisciplinary specialties and careful treatment planning were required. The patient also benefited esthetically from our effort.

Patients' acceptance of corticotomy-assisted orthodontics.

ObjectiveTo study patients’ acceptance of corticotomy-assisted orthodontics as a treatment option.MethodsAdult patients seeking orthodontic treatment were asked to complete two sets of questionnaires; the first set included questions about age, sex, and level of education and general questions about orthodontic treatment; and the second set was related to the corticotomy-assisted orthodontics. Before answering the corticotomy questions, a brief description of the clinical procedure was explained and photographs of an actual procedure were shown.ResultsA total of 150 subjects were approached and 129 (86%) agreed to answer the questionnaires (72 male and 57 female patients). Of these, only 3.1% did hear about corticotomy and 7.8% selected corticotomy instead of extraction. Fear from the surgery (53.2%) was the most frequent reason for not selecting corticotomy followed by fear from pain (36.9%). The acceptance of corticotomy between males and females was similar. No relationship was found between the level of education and prior knowledge of the procedure, P=0.857. Prior knowledge about corticotomy was not a factor in selecting it as a treatment option (P=0.556) to reduce the treatment time (P=0.427).ConclusionThe acceptance of corticotomy-assisted orthodontics as a treatment option was low. Fear from the surgery was the main reason for not selecting it. The acceptance of corticotomy-assisted orthodontics was not related to patient’s level of education or sex.

A Novel Surgical Technique for Augmented Corticotomy-Assisted Orthodontics: Bone Grafting With Periosteum

To introduce grafting fixed with the periosteum (dumpling technique) as an alternative surgical technique for augmented corticotomy-assisted orthodontics in the lower anterior region and evaluate the preliminary outcomes. Eleven patients (9 women, 2 men; mean age, 21.4yr) with a thin alveolus or alveolar defect in the lower anterior region by clinical and radiographic examination underwent an augmented corticotomy using the new dumpling technique. Cone-beam computerized tomography was used to evaluate morphologic changes of the lower anterior ridge before treatment (T0) and 1week (T1) and 6months (T2) after the bone-augmentation procedure. Repeated-measures analysis of variance with Bonferroni multiple-comparison test was used to compare variables at each time point. No severe postsurgical complications occurred in any patient. The mean alveolar bone thickness of the labial plate increased from T0 to T1 (P < .001) and decreased from T1 to T2 (P < .001). However, compared with T0, there was still a significant increase in horizontal bone thickness at T2 (P < .05). The vertical alveolar bone level increased from T0 to T1 (P < .001) and was maintained from T1 to T2 (P > .05). No significant differences were found in root length of the lower anterior teeth at these 3 time points (P>.05). In this preliminary study, the dumpling technique for augmented corticotomy-assisted surgical orthodontics showed alveolar bone augmentation by increasing the vertical alveolar height and the horizontal bone thickness in the labial aspect of the anterior mandibular area. However, long-term follow-up is necessary.

The effect of corticotomy on tooth movements during canine retraction

Objectives: The objective was to evaluate the effect of corticotomy/corticectomy surgery combined with fixed appliances on tooth movement during the canine retraction phase. Materials and Methods: We assessed 13 postpubertal patients (3 males and 10 females) requiring extraction of both maxillary first premolars. One side of the upper dental arch was treated with a PG retraction spring as a control, while the other side was treated with combined corticotomy/corticectomy surgery and a PG spring. A force of 100 g was used for canine retraction on both sides, with activation of the spring monthly on the control side and once every 15 days on the combined treatment side. A paired t-test was used to assess the difference between the two sides. Results: There was a small but statistically significant reduction in the duration of canine retraction (P < 0.05) and an increase in the rate of canine retraction (P < 0.05) (mm/month) in the combined treatment group. No difference in anchorage loss was observed between the groups. Tipping per millimeter of retraction was significantly different between the groups (P < 0.05). Conclusion: Corticotomy-assisted orthodontics increased the rate of tooth movement during canine retraction in 20% ratio. Relative bodily tooth movement was achieved with the corticotomy combined treatment.

Corticotomy-assisted orthodontics: a review of surgical technique with case report

There has been a significant increase in the number of adult patients requesting orthodontic treatment. Demands of these patients have also changed; patients are often only requesting aesthetic correction in the shortest possible time. The development of corticotomy-assisted orthodontics (CAO) has offered potential solutions to many limitations in the management of these patients. A corticotomy is defined as a surgical procedure where only the cortical plate is cut, perforated or modified. CAO claims to have several advantages including reduced treatment time, increased traction of impacted teeth and added post-orthodontic stability. This article highlights the potential of corticotomy procedures in improving orthodontic outcomes and includes a case report.

Corticotomy-assisted orthodontics

The use of orthodontic treatment in adult patients is becoming more common and these patients have different requirements specially regarding duration of treatment and facial and dental aesthetics. Alveolar corticotomy is an effective means of accelerating orthodontic treatment. This literature revision include an historical background, biological and orthodontic fundamentals and the most significant clinical applications of this technique. Orthodontic treatment time is reduced with this technique to one-third of that in conventional orthodontics. Alveolar bone grafting of labial and palatal/lingual surfaces ensures root coverage as the dental arch is expanded. Corticotomy-assisted orthodontics has been reported in a few clinical cases, and seems to be a promising adjuvant technique, indicated for many situations in the orthodontic treatment of adults without active periodontal pathology. Its main advantages are reduction of treatment time and postorthodontic stability. Further controlled prospective and histological studies are needed to study tooth movement, post-retention stability, and microstructural features of teeth, periodontium, and regenerated bone after using this procedure. Nonextraction Treatment of Severe Crowding with the Aid of Corticotomy-Assisted Orthodontics

This paper illustrates the combined nonextraction orthodontic treatment with the corticotomy technique in an adult patient (age: 25 years and 3 months) with severely crowded arches to accelerate tooth movement and shorten the treatment time. Both her upper lateral incisors were congenitally absent and both upper central incisors' roots were short. Initial fixed orthodontic appliances (bidimensional) were bonded and one week later buccal and lingual corticotomy with alveolar augmentation procedure in the maxilla and mandible was performed. Orthodontic activation to level and align and unravel the crowding was performed every two weeks. The total treatment time was 8 months with no adverse effects observed at the end of active treatment. The addition of the decortication procedure to the conventional orthodontic therapy decreased the duration of treatment significantly. Successful alignment of both arches with ideal overbite and overjet as well as adequate occlusion was achieved.
