Correlation of Central Corneal Thickness with Refractive Error, Corneal Curvature, and Axial Length: An Observational Study

Background: Various demographic and ocular factors influence central corneal thickness (CCT). Studies on the relationships between these parameters have shown conflicting results. We aim to investigate the correlation of CCT with different types of refractive error, axial length (AL), and corneal curvature. Materials and Methods: An observational study was undertaken in the Department of Ophthalmology, recruiting patients from 18 to 40 years of age with myopia (−0.5 D to −6 D) or hypermetropia (≤ +3 D) or emmetropia (−0.25 D to +0.25 D). All participants underwent comprehensive ophthalmic examination, including refraction, slit lamp examination and fundoscopy, followed by CCT, corneal curvature, and AL measurement. Result: A total of 1224 eyes of 612 patients (260 males and 352 females) were enrolled in the study. Out of them, 31.5% (n = 386) had emmetropia, 28.6% (n = 350) had hypermetropia, and 39.9% (n = 488) had myopia. The mean (±standard deviation [SD]) keratometry (K), CCT and AL were found to be 44.08 (±1.55) D, 528.46 (±29.78) μm, and 22.75 (±1.21) mm, respectively. With increasing age, CCT was found to be decreased (P < 0.001). Compared to myopia, a significantly higher CCT was noted in hypermetropia (P < 0.001). There was a negative correlation between CCT and corneal curvature among the entire study population, irrespective of their refractive status (P < 0.001). This correlation was more significant among emmetropes and hypermetropes as compared to myopes. However, we found no correlation between CCT and AL in any groups. Conclusion: CCT is found to be thicker in hypermetropia and thinner in myopia as compared to emmetropia. With increasing age, the cornea tends to become steeper and thinner.

Corneal Endothelial Morphology and Central Corneal Thickness Changes in Type 2 Diabetes Mellitus using Specular Microscopy and Ultrasonic Pachymetry: A Cross-sectional Comparative Study

Introduction: Diabetes Mellitus (DM) is a metabolic condition characterised by chronic hyperglycaemia, causing raised blood glucose levels which result in microvascular and macrovascular disorders and may introduce ocular manifestations including changes in corneal Endothelial Cell Density (ECD), corneal thickness, and intraocular pressure. It is clinically important to analyse the corneal endothelial status in patients with type II DM as preoperative corneal endothelial cell dysfunction may cause more corneal endothelial cell damage postoperatively leading to corneal decompensation. With the advent of precise and better measurement tool Central Corneal Thickness (CCT) and corneal endothelial morphology measurement has become more accurate. Aim: To compare corneal endothelium cell density, polymorphism, polymegathism and CCT in type 2 DM with age-matched, non diabetic control subjects using CEM-530 Specular microscope and ultrasonic pachymeter Tomey SP-100. Materials and Methods: This cross-sectional, comparative study was conducted at Eye Department at ESI Post Graduate Institute of Medical Sciences and Research, Basaidarapur, Delhi, India, from October 2018 to November 2019. Patients with ages between 40 to 70 years of either gender who were diagnosed to have DM were recruited in the study through non probability convenience sampling. A total of 150 subjects that met the inclusion criteria and exclusion criteria were included in the study. Seventy five known type 2 DM patients were enrolled in diabetic group (case group) and 75 non diabetic, age-matched subjects were enrolled as control group. Cases were classified under three major groups, namely on the basis of duration of DM and severity of Diabetic Retinopathy (DR) and glycaemic control {glycosylated haemoglobin (HbA1c) level}. All the findings were endorsed on a predesigned performa. Statistical Package for Social Sciences (SPSS) version 17.0 was used for analysis of data. Age and corneal parameters compared using unpaired t-test in diabetic and control group. Also, for comparison in between subgroup of diabetic patients on HbA1c level done by using Analysis of Variance (ANOVA) test. Chi-square test applied for categorical variables. Results: Among the 150 patients evaluated, type 2 DM patient’s corneas (540.51±32.578 µ) were thicker as compared to control group 517.51±22.155 µ (p-value<0.001). The mean ECD of control and diabetic group patients was 2723.75±287.253 cells/mm2 and 2716.11±296.081 cells/mm2 , respectively, found insignificant (p-value=0.821). The mean Coefficient of Variation (CV) of cell area of control and diabetic patients was 28.87±3.950 and 29.85±4.027, respectively, and was significant (p-value=0.034). The mean percentage of endothelial Hexagonal cells (HEX%) of control and diabetic patients were 67.39±6.419 and 67.41±5.493, respectively and was non significant (p-value=0.985). Thus, statistically significant difference was found with CCT (p-value<0.001) and CV (p-value=0.034) but not with ECD and hexagonality, between control and diabetic eyes. There was a correlation between CCT, CV, HEX% and ECD with duration of DM2 but it was statistically insignificant. There were higher CCT, CV and HEX% and lower ECD in >10 years of duration of diabetes mellitus than in patients with duration of diabetes mellitus ≤10 years. There was a correlation of CCT, CV, ECD and HEX with HbA1c level. There were significant higher CCT and CV values in >7% HbA1c level group than in group with ≤7% HbA1c level. There was also increased ECD in >7% group, but it was found to be insignificant. Percentage of hexagonality in >7% HbA1c level group was lower than in group with ≤7% HbA1c level, but found insignificant. There were higher values of CCT and CV in Non Proliferative Diabetic Retinopathy (NPDR) subgroup compared to Proliferative Diabetic Retinopathy (PDR) and no Diabetic Retinopathy group. CCT values were significant (p-value=0.007), so as CV values (p-value=0.001). There was increase in HEX% (p-value=0.005) which was significant, along with lower ECD in NPDR (p-value=0.143), group which was insignificant. Conclusion: The present study documented that DM has considerable effects on all the layers of the cornea especially endothelial layer, causes reduction of ECD and increased CV. Diabetic cornea has increased CCT and lower percentage of hexagonal cells than normal subjects. In addition, there is a correlation between the changes in corneal parameters like ECD, CV, HEX%, CCT with the duration of DM and severity of DR and glycaemic control {glycosylated haemoglobin (HbA1c) level}.

Determining the mean difference in central corneal thickness measured by applanation ultrasound and oculus Wavelight Occulyzer II

Abstract
 Objective: The objective is to determine the mean difference of central corneal thickness measurements by using ultrasound pachymeter and oculus Wavelight Occulyzer II.
 Patients & Methods: This prospective cross-sectional comparative study was conducted at Department of Ophthalmology, Liaquat National Hospital, Karachi for 6 months duration, after the approval from ethical committee. Patients were included in our study on the basis of inclusion & exclusion criteria. Central corneal thickness measurements was taken by using ultrasound pachymeter & oculus Wavelight Occulyzer II. All the data was collected by single researcher. The results were plotted, compared & analyzed.
 Results: 130 patients attending ophthalmology department and fulfilling the inclusion criteria were assessed. One eye of each patient was measured (65 were right eye and 65 were left eye). 73(56.2%) were male and 57(43.8%) were females. Mean age of these patients was 33.9 ±8.9 years. The mean ± SD thinnest oculus Wavelight Occulyzer II measurement was 538.61±23.46 ?m and ranged between 476.0 and 619.0 ?m whereas the mean thinnest ultrasound pachymeter measurement was 535.1±21.816?m and ranged between 482 and 601 ?m. There was highly significant correlation of central corneal thickness between both the instruments. (r=0.96, p<0.001) 
 Conclusion: According to the results of our study there is a highly correlation of central corneal thickness was found between the readings obtained from both UP and oculus Wavelight Occulyzer II. So we concluded that oculus Wavelight Occulyzer II can be used as an alternative technique to ultrasound pachymetry while assessing CCT in clinical settings to decrease the risk of procedure associated problems with UP like epithelial trauma and infection, to decrease the frequent use of topical anesthetic agents, for early detection and management of glaucoma & in anxious patients also.

Changes in corneal thickness after vitrectomy-Implications for glaucoma practice.

To evaluate changes in central corneal thickness (CCT) following vitrectomy. All consecutive old and new patients referred to glaucoma services for possible secondary glaucoma after vitrectomy and who had undergone corneal pachymetry between July 2013 to June 2020, were included. The eye that developed elevated intraocular pressure (IOP) and was diagnosed clinically as glaucoma after vitrectomy, was labelled as the "affected" eye. The contralateral eye of the patient with normal IOP and no history of vitrectomy was labelled as the "control" eye. The difference in CCT in the affected eye and the contralateral control eye (ΔCCT) and CCT were compared between different age groups. Correlation of CCT in the affected eye with age, diagnosis, type of surgery done, lens status and pre-existing glaucoma was done using multivariate regression analysis. Of 127 eyes of 120 patients (M:F = 85:35), the average CCT in the affected eye was significantly higher than the unaffected contralateral control eye (p<0.0001). The ΔCCT in eyes presenting at an age <25 years was higher (median 582, 497-840) than those that presented later (median 518, 384-755), p <0.0001, with maximum ΔCCT seen in eyes that had undergone vitrectomy at age<12 years. The CCT in the affected eye was significantly higher in aphakic eyes (588±81.6 microns) than in pseudophakic eyes (552±79.03 microns), p = 0.03. On multivariate analysis, age<25 years remained as a significant influencer of CCT in the affected eye (β = -1.7, p<0.001, R2 = 28.3%). Young age group<25 years are more prone to corneal remodelling and CCT changes after vitrectomy.

Correlation of central corneal thickness and associated factors in premature infants with and without retinopathy of prematurity

ObjectiveCentral corneal thickness (CCT) in premature infants is described in racially homogenous populations, and factors affecting CCT in infants are relatively unknown. This study describes CCT in premature infants and its association of steroid and oxygen requirements, gestational age (GA), birth weight (BW), race, and their relationship with CCT and corneal haze. MethodsCCT measurements of 87 infants/174 eyes screened for retinopathy of prematurity were taken between 30 and 44 weeks of GA. CCT was analyzed using a mixed model for its relationship with BW, GA, race, corneal clarity, steroid, and oxygen use. ResultsAverage CCT decreased at a rate of 12.3 μm/week. Caucasians had the thickest corneas and Hispanics the thinnest (p < 0.01) at baseline, but the rate of CCT decline varied based on racial/ethnic group (p = 0.079). Infants with BW <1000 g had a higher CCT at baseline, but CCT decreased at a faster rate than infants with higher BW (−13. 4 μm/week vs −9.9 μm/week, p = 0.020). Infants born <27 weeks of GA had higher CCT at baseline, but CCT decreased at faster rate compared with patients born later (−13.3 μm/week vs −10.1 μm/week, p = 0.029). Steroid and oxygen use were not statistically significantly associated with CCT or corneal haze (p > 0.05) ConclusionsCCT varies by racial group in premature infants. Lower BW and GA are associated with increased CCT at baseline but thin at a faster rate. Average, CCT decreases at a rate of 12.3 μm/week between 30 and 44 weeks GA and averages to 550 μm by 44 weeks GA.

571-P: A Cross-Sectional Observational Study on Correlation between the Central Corneal Thickness and Diabetic Retinopathy

Introduction: In the recent years, several papers have evaluated the changes in central corneal thickness (CCT) associated with diabetes mellitus (DM) with conflicting results. Aims: To compare the CCT of subjects with DM with age and sex matched healthy subjects and to determine the correlation of CCT with duration of DM and severity of diabetic retinopathy (DR). Methods: In this cross-sectional, observational study 124 subjects with DM and 65 age matched healthy controls were studied. CCT was measured by pulsair non- contact tonometer. According to retinopathy status based on the ETDRS subjects with diabetes were categorized as PDR (Gr III), NPDR (Gr II) and NO DR (Gr I). Nondiabetic healthy control subjects were categorized as Group IV. Results: The mean CCT for the study group was 547.02±2.34 and for the control group was 504.6±3.14(p=0.02).Mean CCT in subjects with diabetes with duration &amp;gt;10 years was higher (557.71±19.44) as compared to those having it for less than 10 years (534.05±27.30) (p=0.178). Patients with PDR had higher mean CCT (572.13±17.50) compared to NPDR (553.78±8.80). Patients with DM but no DR had lesser mean CCT than NPDR thus implying positive correlation of CCT with severity of DR. Conclusion: CCT is significantly greater in subjects with diabetes compared to healthy control. It is positively correlated with severity of DR and duration of DM. Disclosure A. Baidya: None. Q. Correlation of Central Corneal Thickness and Keratometry with Refraction and Axial Length: A Prospective Analytic Study.

Introduction The correlation between corneal curvature and central corneal thickness (CCT), with ocular parameters like axial length and refraction, remains elusive. The various ocular biometric parameters are interdependent and their correlation varies with the population studied. A comprehensive study is required for a better understanding of the ocular biometric properties of Indian eyes.MethodologyThis is a prospective study done in 156 subjects. Subjects with clear lens and clear cornea were included in the study. Those with corneal opacity, cataract, pregnancy, and diabetes were excluded. Cycloplegic refraction, autokeratometry (Potex, ultrasonic auto keratometer), central corneal thickness (ultrasonic pachymeter), and axial length (Sonomed) were done in all subjects. Subjects were divided into two groups based on refraction, for an analysis of parameters. Group one included subjects with refraction of +3 diopters (D) to -2.99D and group two with subjects with > -3D refraction.ResultsWith an increasing axial length and myopic refraction, the corneal curvature tends to be steeper. The average CCT of subjects in group one and group two were 530.34 microns and 542.63 microns, respectively. Subjects with refraction more than 10 diopters or axial length more than 25 mm had a mean CCT of 525.25 microns. Subjects with myopic refraction between 3 diopters and 10 diopters had a mean CCT of 551.32 microns.ConclusionIncrease in corneal power is associated with increasing myopic refraction. Steeper corneal curvature is correlated with increasing axial length and thinner corneas. The mean CCT was 533.87 microns with a standard deviation (SD) of 40.02. Axial myopia is associated with an increase in corneal thickness. These ocular biometric findings have crucial implications in refractive surgeries.

A study for the assessment of central corneal thickness and visual field defects in patients of primary open-angle glaucoma

Purpose: The aim of this study is to assess the central corneal thickness (CCT) and visual field (VF) changes in patients of primary open-angle glaucoma (POAG) and the influence of CCT on glaucomatous damage in glaucoma clinic of Dr. R. P. G. M. C Kangra at Tanda. Materials and Methods: This cross-sectional study conducted on 103 eyes of 58 patients of POAG- >40 years. All patients underwent a complete eye examination including Goldmann applanation tonometry, CCT measurement through ultrasonic pachymeter, stereoscopic disc photography, retinal nerve fiber layer (RNFL) photography, and automated perimetry. Correlation of CCT with age, gender, intraocular pressure (IOP), advanced glaucoma intervention study (AGIS) score, VF parameters (mean deviation, pattern standard deviation [PSD]), vertical cup-disc ratio, average RNFL, and number of antiglaucoma medications was analyzed. The patients were divided into two groups – thin and thick CCT taking a reference range of 529 μm derived from a previous study done in same area with larger sample size. Pearson correlation was used for correlation coefficient and a value of P < 0.05 was considered statistically significant. Results: Thin CCT was significantly correlated with IOP (r = 0.28, P = 0.004). Thin CCT was also significantly associated with worsened PSD of VF (r = 0.25, P = 0.02). The mean AGIS score was more (8.07 ± 5.52) in patients with thin CCT (<529 μm) in comparison (5.96 ± 6.10) to patients with thick CCT (>529 μm) which was statistically significant (P < 0.05). Conclusion: CCT is a significant risk factor for glaucomatous VF loss. Patients with thinner CCT are more likely to have advanced VF defects in comparison to patients with thick CCT.

Correlation between central corneal thickness and visual field defects, cup to disc ratio and retinal nerve fiber layer thickness in primary open angle glaucoma patients.

Objective:To evaluate the correlation between Central Corneal Thickness (CCT) and Visual Field (VF) defect parameters like Mean Deviation (MD) and Pattern Standard Deviation (PSD), Cup-to-Disc Ratio (CDR) and Retinal Nerve Fibre Layer Thickness (RNFL-T) in Primary Open-Angle Glaucoma (POAG) patients.Methods:This cross sectional study was conducted at Armed Forces Institute of Ophthalmology (AFIO), Rawalpindi from September 2015 to September 2016. Sixty eyes of 30 patients with diagnosed POAG were analysed. Correlation of CCT with other variables was studied.Results:Mean age of study population was 43.13±7.54 years. Out of 30 patients, 19 (63.33%) were males and 11 (36.67%) were females. Mean CCT, MD, PSD, CDR and RNFL-T of study population was 528.57±25.47µm, -9.11±3.07, 6.93±2.73, 0.63±0.13 and 77.79±10.44µm respectively. There was significant correlation of CCT with MD, PSD and CDR (r=-0.52, p<0.001; r=-0.59, p<0.001;r=-0.41, p=0.001 respectively). The correlation of CCT with RNFL-T was not statistically significant (r=-0.14, p=0.284).Conclusion:Central corneal thickness had significant correlation with visual field parameters like mean deviation and pattern standard deviation, as well as with cup-to-disc ratio. However, central corneal thickness had no significant relationship with retinal nerve fibre layer thickness.

Repeatability and Reproducibility of Measurements Using a NT-530P Noncontact Tono/Pachymeter and Correlation of Central Corneal Thickness with Intraocular Pressure

Purpose. To investigate the repeatability and reproducibility of intraocular pressure (IOP) and central corneal thickness (CCT) measurements using a noncontact tono/pachymeter (NT-530P) and to assess the correlation of CCT with IOP. Methods. Forty-six eyes of healthy volunteers were measured by two examiners. Three consecutive measurements per eye were performed. Repeatability was assessed using the coefficient of variation, and reproducibility was assessed using Bland-Altman plots. Linear correlations were used to determine agreement between CCT and noncorrected IOP and CCT and corrected IOP, which was calculated using a formula built into the NT-530P. Results. The coefficient of variation for IOP was 6.4% and for CCT was 0.4%. The 95% limits of agreement between examiners were −0.17 ± 1.42 mmHg (range: −2.95 to 2.61 mmHg) for IOP, −0.93 ± 4.37 μm (range: −9.50 to 7.64 μm) for CCT. The corrected IOP was significantly higher than the noncorrected IOP (P = 0.010.3). The noncorrected IOP significantly correlated with CCT (r = −0.4883, P = 0.0006). The corrected IOP showed no significant correlation with CCT (r = −0.0285, P = 0.8509). Conclusions. NT-530P offered repeatability and reproducibility in both IOP and CCT measurements. The corrected IOP calculated using the NT-530P was independent of the CCT, suggesting that this IOP may be less influenced by the central corneal thickness.

Correlation of central corneal thickness and optic nerve head topography in patients with primary open-angle glaucoma

Purpose:To evaluate whether changes in optic nerve head topography and visual field in patients with primary open-angle (POAG) are related to central corneal thickness (CCT).Materials and Methods:Eighty eyes of 50 patients with POAG underwent ophthalmic examination; optic nerve head imaging with the Heidelberg Retina Tomography II (HRT II), ultrasound corneal pachymetry, and visual field evaluation with the Humphrey visual field analyser (program 24–2). Correlation between CCT, age, gender, family history of glaucoma, visual acuity, intraocular pressure (IOP), optic disc surface area, vertical and horizontal cup: disc ratios, neuroretinal rim area, mean deviation of visual field, and number of glaucoma medications was analyzed. Patients were divided into a thin CCT group <540 μm or a thick CCT group >540 μm. Pearson correlation was used for correlation coefficient and a P value of <0.05 was considered statistically significant.Results:Thin CCT was significantly correlated with vertical and horizontal cup: disc ratios, neuroretinal rim area loss, and smaller optic disc surface area (r=0.043, r=0.021, r=0.036, and 0.031 respectively). Thin CCT was also significantly associated with worsened mean deviation of visual field, and increased number of glaucoma medications (r=0.065 and r=0.423). Patients with positive family history correlated with with greater vertical cup: disc ratio, and more glaucoma medications but this was not statistically significant.Conclusions:In patients with POAG those with thinner CCT are likely to develop greater glaucomatous optic nerve and visual field damages than those with a thicker CCT.

Correlation of central corneal thickness and axial length to the optic disc and peripapillary atrophy among healthy individuals, glaucoma and ocular hypertension patients

Clinical observational study for the correlation between the central corneal thickness (CCT), axial length (AXL), optic disc area (D), cupping area (C) and peripapillary atrophy (PPA), in healthy eyes (group A), eyes with primary open angle glaucoma (POAG) (group B) and eyes with ocular hypertension (OH) (group C), in order to assess the possible contribution of this correlation to the diagnosis or prognosis of glaucoma. Ninety eyes of 90 persons participated in the study (30 eyes of 30 persons in each group). All eyes underwent intraocular pressure measurement, visual field testing, endothelial cell count (ECC), measurement of the CCT and AXL, and gonioscopy. After dilation of the pupil, a digital photograph of each optic disc was performed, followed by PPA, D area and C area measurements. We found a statistically significant difference in the C/D area ratio between healthy and POAG eyes and a moderate negative relationship between AXL and D in healthy eyes. We also found a moderate negative relationship between CCT and C/D area ratio in POAG eyes and a strong negative relationship between PPA and D in OH eyes. CCT was not correlated with PPA and AXL in any one of the three groups. OH eyes seem to have more tolerant and stable disc and peripapillary area compared to healthy and POAG eyes.

Central corneal thickness and its relationship to intraocular pressure in mucopolysaccararidoses-1 following bone marrow transplantation

To examine the ocular findings in mucopolysaccaridoses-1 (MPS-1) and the relationship between intraocular pressure (IOP) and central corneal thickness following bone marrow transplantation. clinical practice. 23 subjects with MPS-1 following bone marrow transplantation were examined. age, sex, visual acuity, presence of strabismus, refractive error, fundus examination, intraocular pressure, and central corneal thickness were assessed for each individual. correlation of central corneal thickness with intraocular pressure. Only the right eye was used in correlations. Forty-six eyes of 23 subjects were examined. All subjects had been treated with successful bone marrow transplantation with mean follow-up of 8.5 years. Ages ranged from 17 months to 19 years (SD 5.03). Of the subjects, 60.8% were female; 85.5% were hyperopic; 8.6% had best-corrected visual acuity of >/=6/12; 34.5% had best-corrected visual acuity of <6/12 and >6/36 with 56.9% <6/36; 30% had strabismus. IOP ranged from 13 to 41 mm Hg with four children on antiglaucoma therapy. Central corneal thickness ranged from 484 microm to 705 microm and was moderately correlated with measured IOP in the right eye (r = 0.56). Corneal opacification moderately correlated with central corneal thickness (r = 0.57). Seventy percent had a normal disk evaluation; 30% had abnormal disk cupping. This is the first large case series of MPS-1 subjects demonstrating a correlation between measured IOP and central corneal thickness. All subjects had corneal opacification moderately correlating with corneal pachymetry.
