Comparison of objective visual quality between SMILE and FS-LASIK in moderate-to-high myopia.

This study aims to compare the changes in the corneal wavefront aberrations and the objective visual quality resulting from two types of eye surgery-small incision lenticule extraction (SMILE) and femtosecond laser-assisted in situ keratomileusis (FS-LASIK)-in patients with moderate-to-high myopia. A prospective analysis was performed on 98 eyes of 51 patients who underwent SMILE. Additionally, 88 eyes of 45 patients who underwent FS-LASIK were analyzed. All patients underwent ocular examination preoperatively and at 1 day, 1 week, 1 month, and 3 months postoperatively. Corneal aberrations and objective visual quality were measured using the Optical Quality Analysis System II (OQAS II) and Optical Path Difference Scan III (OPD-Scan III). At postoperative 1 day and 1 week, there was a statistically significant difference in uncorrected distance visual acuity (UDVA) between SMILE and FS-LASIK (P < 0.05). Postoperative spherical (S), cylinder (C) and spherical equivalent refraction (SE) were similar between the two groups (P > 0.05). In both groups, the absolute magnitude of total higher-aberrations (tHOA), piston, vertical tilt, vertical coma, and spherical aberration (SA) increased after surgery compared to preoperative values (P < 0.05). There was no significant difference in Δhorizontal tHOA, Δhorizontal tilt, Δhorizontal coma, and Δhorizontal trefoil between the two groups (P > 0.05), and the FS-LASIK had higher Δvertical trefoil and ΔSA (P < 0.05) but lower Δpiston, Δvertical tilt, and Δvertical coma than the SMILE group (P < 0.05). There was a rise in objective scattering index (OSI) and a decline in both modulation transfer function (MTF) cutoff and Strehl ratio (SR) after surgery compared to preoperative values in both groups (P < 0.05). There was a statistically significant difference in the OSI at 1 day and 3 months between the two groups (P < 0.05). Postoperative MTF cutoff and SR were similar between the two groups (P > 0.05). Postoperative OSI was positively correlated with corneal tHOA (0.261 ≤ R ≤ 0.483, P < 0.05) and was negatively correlated with vertical tilt and vertical coma (-0.315 ≤ R ≤ -0.209, P < 0.05) in both groups. While both SMILE and FS-LASIK can effectively correct moderate-to-high myopia, there is an increase in corneal aberrations and a postoperative delay in objective visual quality. The cornea may require a longer recovery period in the SMILE. OPD-Scan III combined with OQAS II is a useful supplementary inspection for assessing the optical quality following refractive surgery.

Comparison of long-term changes in the effective optical zone following SMILE and FS-LASIK for moderate and high myopia

BackgroundVisual quality after corneal refractive surgery is linked to the postoperative effective optical zone (EOZ). This study aims to compare long-term changes in the EOZ following small incision lenticule extraction (SMILE) and femtosecond laser-assisted in-situ keratomileusis (FS-LASIK) for moderate and high myopia.MethodsThis study included 42 patients (72 eyes) who underwent either SMILE (36 eyes) or FS-LASIK (36 eyes). A custom software program based on the tangential curvature difference map of the Pentacam HR (Oculus Optikgeräte GmbH) was used to define the EOZ at 3 and 7 years postoperatively. The EOZ, its chronological changes compared to the programmed optical zone (POZ), and the corneal wavefront aberrations following SMILE and FS-LASIK were analyzed. Correlations between the EOZ changes and relevant parameters were evaluated.ResultsThree years postoperatively, EOZ following SMILE and FS-LASIK were 5.13 ± 0.27 mm and 4.70 ± 0.24 mm (P < 0.001), respectively. Seven years postoperatively, EOZ following SMILE and FS-LASIK decreased to 5.03 ± 0.28 mm and 4.63 ± 0.23 mm (P < 0.001), respectively. At postoperative 7 years, the percentages of EOZ/POZ were negatively correlated with Q-value changes (β = -5.120, P = 0.009) following SMILE and positively correlated with the cylinder correction (β = 1.184, P = 0.004) following FS-LASIK. The induced spherical aberrations in the SMILE group were less than those in the FS-LASIK group (P < 0.05) and were negatively correlated with the EOZ/POZ (β = -16.653, P < 0.001).ConclusionsThe EOZ following SMILE was larger than that following FS-LASIK in the long postoperative term for moderate and high myopia. Furthermore, a continual reduction in the EOZ was noted after both surgical modalities.

Randomized clinical trial comparing customized corneal crosslinking: epi-on in high oxygen and epi-off in room air for keratoconus.

To compare clinical outcomes of customized transepithelial (epi-on) corneal crosslinking (CXL) in high oxygen and customized CXL with epithelial removal (epi-off) in room air for keratoconus (KC). Umeå University Hospital, Umeå, Sweden. Prospective, randomized, single-masked, intraindividually comparing study. 32 participants with bilateral progressive KC were treated with bilateral customized topography-guided CXL, 30 mW/cm 2 ; 7.2 to 15 J/cm 2 and were randomized to epi-on in one eye (32 eyes) and epi-off in the fellow eye (32 eyes). Uncorrected distance visual acuity (UDVA) and corrected distance visual acuity (CDVA), maximal keratometry (Kmax), subjective ocular discomfort, low-contrast visual acuities (LCVAs) at 10% and 2.5% contrast, ocular and anterior corneal wavefront aberrations, manifest refractive spherical equivalent, endothelial cell count (ECC), and adverse events were assessed through 24 months. Both treatments showed improvements at 24 months in UDVA; -0.16 ± 0.24 ( P < .001) and -0.13 ± 0.20 logMAR ( P = .006), respectively, CDVA; -0.10 ± 0.11 ( P < .001) and -0.10 ± 0.12 ( P = .001), Kmax; -1.74 ± 1.31 ( P < .001) and -1.72 ± 1.36 D ( P < .001). LCVA 10% improved for both protocols ( P < .001), but LCVA 2.5% improved for epi-on CXL only ( P = .001). ECC was unaltered, and no adverse events occurred. The epi-on eyes had significantly less discomfort symptoms during the whole first week posttreatment ( P < .05). High-oxygen customized epi-on CXL is a viable alternative to room air customized epi-off CXL, with faster improvements in CDVA and LCVA and less early ocular discomfort.

Purpose. To improve the efficiency and safety of SMILE-type lenticular surgery procedure in correction of different degrees myopia based on the use of low-energy laser parameters. Material and methods. This study includes a comparative analysis of data from 193 patients (288 eyes) operated for different degrees myopia including a number of cases with myopic astigmatism using SMILE technology with different FSL energy levels from 130 to 150 nJ. The parameters were analyzed on the 1st day and after 1 week, 1 month, 3, 6 and 12 months after surgery with assessment of NCVA, quality of life (QIRC questionnaire), general corneal wavefront aberrations (RMS), densitometry and pachymetry. Results. According to visometry, quality of life assessment, RMS, densitometry and pachymetry, the best functional results were obtained in subgroups with mild and moderate myopia on the 1st day after SMILE using an energy level in the range of 130 nJ. And in subgroups with a high degree of myopia, when using an energy level in the range of 140 nJ, no direct effect of energy on other indicators was found. Conclusion. According to the results of the study, it was found that the correction of myopic refractive error using the SMILE technology with a FSL energy level in the range of 130–140 nJ in its effectiveness on the 1st day after surgery exceeds the previously used energy levels (150 nJ) in terms of the quality of restoration of visual functions and does not have a negative effect on other parameters of the cornea. Relevance. Until recently, visual acuity was the main criterion for determining the timing of surgery, but already in the early stages of cataract development, patients may suffer from reduced image quality, which affects the quality of life (“night” myopia, light scattering, blurring). Deterioration in visual function in these patients may be explained by refractive errors. Analysis of internal higher-order aberrations resulting from slight opacification of the lens is important for determining the effect of media opacity on wavefront aberrations (WF) and for determining the indications and timing of surgical treatment, especially in patients with high visual acuity. Purpose. Evaluate the optical system of the eye with initial cataracts and high visual acuity. Evaluate the wavefront and its aberrations, contrast sensitivity and Shtrel coefficient. Material and methods. The study included 111 eyes with cataracts and 42 eyes without signs of cataracts (control group). Patients aged on average 57±5.3 years. Wavefront analysis was performed using an iTrace ray tracing aberrometer (Tracey Tech., USA) in a dark room under maximum drug-induced mydriasis. In each group, total, corneal, and internal wavefront aberrations were measured at a pupil diameter of 6.0 mm. Higher order aberrations were analyzed using Zernike polynomials (3rd to 5th order). The modulation transfer function (MTF) at different spatial frequencies (0–30 c/deg) and the Strehl coefficient in patients in the study groups were also assessed. Results. Total and internal higher-order wavefront aberrations were significant in different cataract groups compared with control patients, while higher-order corneal aberrations were not statistically different. We noted a higher level of total and internal aberrations in the group of patients with posterior subcapsular cataract. In this regard, patients with posterior subcapsular cataract require surgical treatment at an earlier stage, despite high visual acuity. Conclusion. We recommend assessing the optical quality of vision and wavefront aberrations when observing early cataracts in order to decide whether to perform surgery at an earlier date. Effect of Chalazion on Corneal Astigmatism

A common eyelid disease chalazion causes eye morbidity due to inflammation and cosmetic disfigurement. Important factors in corneal refractive surgery, intraocular lens power calculations for cataract surgery and visual acuity assessments are corneal topographic changes. For better outcome of ocular surgery corneal astigmatism should be corrected earlier. The aim of this study is to evaluate changes in corneal astigmatism according to chalazion size and location. Methods: This cross-sectional study was conducted in saveetha medical college and hospitals, Thandalam , Department of Ophthalmology Outpatient department. A total of 44 eyes from 33 patients were included in the chalazion group and 70 eyes from 46 patients comprised the control group. Chalazia were classified according to location and size. An autokeratorefractometer and a dual-Scheimpflug analyzer were utilized to evaluate corneal changes. Result: Oblique astigmatism was greater in the chalazion group compared with the control group (p &lt; 0.05). Astigmatism by simulated keratometry (simK), steep K by simK, total root mean square, second order aberration, oblique astigmatism, and vertical astigmatism were significantly greater in the upper eyelid group (p &lt; 0.05). Astigmatism by simK, second order aberration, oblique astigmatism, and vertical astigmatism were significantly greater in the large-sized chalazion group (p &lt; 0.05). Corneal wavefront aberration was the greatest in the upper eyelid chalazion group, whole area group, and large-sized chalazion group (p &lt; 0.05). Conclusions: Large-sized chalazion in the upper eyelid should be treated in the early phase as great changes in corneal topography were noticed. Corneal topography should be performed preoperatively before chalazion is treated.

A Comprehensive Wavefront Assessment of Keratoconus Using an Integrated Scheimpflug Corneal Tomographer/Hartmann-Shack Wavefront Aberrometer.

To characterize higher-order aberrations (HOAs) in different severities of keratoconus (KC) from the anterior and posterior corneal surfaces and whole eye using an integrated Scheimpflug corneal tomographer/Hartmann-Shack wavefront aberrometer. This study included eyes with clinical KC, topographic KC (no clinical signs), fellow eyes with very asymmetric ectasia with normal topography and no clinical signs (VAE-NT), and control eyes. Corneal and ocular wavefront aberrations were obtained using an integrated Scheimpflug tomographer/Hartmann-Shack wavefront aberrometer. The diagnostic capability of distinguishing VAE-NT from the control was also tested. This study included 68 eyes with clinical KC, 44 with topographic KC, 26 with VAE-NT, and 45 controls. Clinical KC had significantly greater total HOAs and coma from the anterior and posterior corneal surfaces and whole eye than the other groups ( P <0.05). Although topographic KC had significantly greater values in all wavefront parameters than the control ( P <0.05), ocular and corneal HOAs did not differ between the VAE-NT and control groups. The coma from the anterior cornea in topographic KC was significantly greater than that in VAE-NT ( P <0.05); the coma from the posterior cornea and whole eye did not differ. Total HOAs from the anterior corneal surface exhibited the highest area under the receiver operating characteristic curve value of 0.774 (sensitivity, 73%; specificity, 78%). A comprehensive wavefront assessment can be used to quantitatively evaluate corneal and ocular HOAs across various severity of KC. Total HOAs from the anterior corneal surface exhibited the potential ability in distinguishing VAE-NT from the control eyes.

Comparison of corneal aberrations from anterior segment swept source OCT versus Placido-topography combined spectral domain OCT in cataract patients

BackgroundTo comprehensively evaluate the agreement of component corneal aberrations from the newly updated wavefront analysis software of a swept-source optical coherence tomographer (SS-OCT) and a referential Placido-topography combined OCT device in elderly cataract patients.MethodsRetrospective study including 103 eyes from 103 elderly patients scheduled for cataract surgery that were measured on the same day with a SS-OCT (Heidelberg Engineering, Germany) device and a Placido-topography combined OCT device (CSO, Italy). Anterior, total, and posterior corneal wavefront aberrations were evaluated for their mean differences and limits of agreement (LoA) via Bland-Altman plots. Vector analysis was additionally employed to compare corneal astigmatism measurements in dioptric vector space.ResultsMean differences of all corneal aberrometric parameters did not exceed 0.05 μm. Total corneal aberrations were not significantly different from 0 except for vertical coma (− 0.04 μm; P = 0.003), spherical aberration (− 0.01 μm, P < 0.001), and root mean square (RMS) higher-order aberration (HOA) (0.03 μm, P = 0.04). The 95% LoA for total corneal aberration parameters between both devices were − 0.46 to 0.42 μm for horizontal astigmatism, − 0.37 to 0.41 μm for oblique astigmatism, − 0.19 to 0.17 μm for oblique trefoil, − 0.33 to 0.25 μm for vertical coma, − 0.20 to 0.22 μm for horizontal coma, − 0.22 to 0.20 μm for horizontal trefoil, − 0.11 to 0.08 μm for spherical aberration, and − 0.22 to 0.28 μm for RMS HOA. Vector analysis revealed no statistically significant mean differences for anterior, total, and posterior corneal astigmatism in dioptric vector space.ConclusionIn eyes undergoing cataract surgery with a regular elderly cornea, corneal wavefront analysis from the SS-OCT device showed functional equivalency to the reference device. Nevertheless, clinically relevant higher order aberration parameters should be interpreted with caution for surgical decision-making.

First European results of a new refractive lenticular extraction procedure-SmartSight by SCHWIND eye-tech-solutions.

To evaluate vision 3 months after SmartSight lenticule extraction treatments. Case series. This case series of patients were treated at Specialty Eye Hospital Svjetlost in Zagreb, Croatia. Sixty eyes of 31 patients consecutively treated with SmartSight lenticule extraction were assessed. The mean age of the patients was 33 ± 6 years (range 23-45 years) at the time of treatment with a mean spherical equivalent refraction of -5.10 ± 1.35 D and mean astigmatism of 0.46 ± 0.36 D. Monocular corrected distance visual acuity (CDVA), uncorrected distance visual acuity (UDVA) were assessed pre- and post-operatively. Ocular and corneal wavefront aberrations have been postoperatively compared to the preoperative baseline values. Changes in ocular wavefront refraction, as well as changes in keratometric readings are reported. At 3 months post-operatively, mean UDVA was 20/20 ± 2. Spherical equivalent showed a low myopic residual refraction of -0.37 ± 0.58 D with refractive astigmatism of 0.46 ± 0.26 D postoperatively. There was a slight improvement of 0.1 Snellen lines at 3-months follow-up. Compared to the preoperative status, ocular aberrations (at 6 mm diameter) did not change at 3 months follow-up; whereas corneal aberrations increased (+0.22 ± 0.21 µm for coma; +0.17 ± 0.19 µm for spherical aberration; and +0.32 ± 0.26 µm for HOA-RMS). The same correction was determined using changes in ocular wavefront refraction, as well as changes in keratometric readings. Lenticule extraction after SmartSight is safe and efficacious in the first 3 months postoperatively. The post-operative outcomes indicate improvements in vision.

Combined simultaneous photorefractive keratectomy and collagen cross-linking in keratoconus suspect patients

To evaluate the safety and efficacy of combined simultaneous photorefractive keratectomy (PRK) with collagen cross-linking (CXL) in keratoconus suspects (KCS). This was a retrospective, non-randomized study of KCS patients who underwent combined simultaneous PRK with CXL. The efficacy, safety, refractive outcomes, and corneal wavefront aberration changes were assessed after the surgery and compared with existing preoperative data. Fifty-six eyes of 28 patients, including 20 females (71.4%), with a mean age of 30.92±4.09 years, were enrolled. The mean follow-up was 19.46±8.48 months (range: 7-35). At the conclusion of the study, mean uncorrected distance visual acuity LogMAR improved from 0.89±0.44 preoperatively to 0.04±0.09 postoperatively (P<0.001). In addition, a statistically significant corneal flattening was observed postoperatively, with a decrease in manifest refraction. A statistically significant increase was found in higher-order aberrations (P<0.001), horizontal coma (P<0.001), and spherical aberration (P<0.001) compared with preoperatively. Postoperatively, 41% exhibited refractive astigmatism of 0.50 diopter (D) or less; 83.8% showed 1.00 D or less. The results of our study indicate that combined simultaneous PRK with CXL can be a safe and effective method for treating refractive instability in KCS patients.

Biomechanical Responses of Different Cap Thicknesses of Corneas After Small Incision Lenticule Extraction: Finite Element Analysis.

This study analyzed the biomechanical responses of different corneal cap thicknesses after small incision lenticule extraction (SMILE). Individual finite element models of myopic eyes were constructed based on the clinical data. Then, four types of corneal cap thicknesses after SMILE were included for each model. The biomechanical effects of material parameters and intraocular pressure on corneas with different cap thicknesses were analyzed. When the cap thickness increased, the vertex displacements of the anterior and posterior corneal surfaces decreased slightly. The corneal stress distributions demonstrated little change. Regarding wave-front aberrations caused by the displacements of the anterior surface, the absolute defocus value decreased slightly, but the magnitude of primary spherical aberration increased slightly. The horizontal coma increased, and the levels of other low-order and high-order aberrations were small and demonstrated little change. The corneal vertex displacement and wave-front aberration were significantly affected by elastic modulus and intraocular pressure, whereas the corneal stress distribution was greatly affected by intraocular pressure. There were obvious individual differences in the biomechanical responses of human eyes. The biomechanical difference of different corneal cap thicknesses after SMILE was small. The effect of corneal cap thickness was significantly less than that resulting from material parameters and intraocular pressure. Individual models were constructed based on the clinical data. The elastic modulus was controlled by programming to simulate its heterogeneous distribution in the actual human eye. The simulation was improved to bridge the gap between basic research and clinical care.

Comparison of Refractive Outcomes and Optical Zone Decentration After SMILE Between Patients With Symmetrical and Asymmetrical High Astigmatism.

To compare the refractive outcomes and optical zone decentration between patients with symmetrical and asymmetrical high astigmatism after small incision lenticule extraction (SMILE). This was a prospective analysis of 89 patients (152 eyes) with myopia and astigmatism of more than 2.00 diopters (D) treated with the SMILE procedure. There were 69 eyes with asymmetrical topographies (asymmetrical astigmatism group) and 83 eyes with symmetrical topographies (symmetrical astigmatism group). Decentralization values were assessed using the tangential curvature difference map preoperatively and 6 months after surgery. Decentration, visual refractive outcomes, and induced changes in corneal wavefront aberrations were compared between the two groups 6 months postoperatively. Both groups achieved favorable visual and refractive outcomes, with a mean postoperative cylinder of -0.22 ± 0.23 and -0.20 ± 0.21 D in the asymmetrical and symmetrical astigmatism groups, respectively. In addition, visual and refractive outcomes and induced changes in corneal aberrations were comparable between the asymmetrical and symmetrical astigmatism groups (all P > .05). However, the total and vertical decentration in the asymmetrical astigmatism group was greater than that in the symmetrical astigmatism group (all P < .05), whereas no significant differences were found in the values of horizontal decentration between the two groups (P > .05). There was a weak positive correlation between induced total corneal higher order aberrations and total decentration (r = 0.267, P = .026) in the asymmetrical astigmatism group but not in the symmetrical astigmatism group (r = 0.210, P = .056). An asymmetrical corneal surface might affect treatment centration after SMILE. Subclinical decentration may be associated with the induction of total higher order aberrations, but it did not affect high astigmatic correction or induced corneal aberrations. Comparison of Corneal Densitometry and Visual Quality after Small Incision Lenticule Extraction (SMILE) and Laser Epithelial Keratomileusis (LASEK): One-Year Comparative Study.

To investigate changes in corneal densitometry (CD) and visual quality following small incision lenticule extraction (SMILE) and laser epithelial keratomileusis (LASEK) in patients with mild-to-moderate myopia. A retrospective analysis was performed on 24 and 25 patients (46 eyes each) who underwent SMILE and LASEK, respectively, for mild-to-moderate myopia. The visual quality and CD values were recorded. Using the Pentacam Scheimpflug system, CD values were collected in three concentric optical zones at the depths of the anterior, central, and posterior layers. Efficacy, safety, predictability, corneal wavefront aberrations, and QoV scores were measured to evaluate visual quality. A correlation analysis was performed between changes in CD and clinical characteristics. There were no statistical differences in efficacy and safety indices between the two groups. At 3 months postoperatively, a pronounced reduction in several zones was observed in the LASEK group (p < 0.05), whereas no obvious change was observed in the SMILE group. There were obvious changes in the CD values in several zones in the SMILE and LASEK groups (p < 0.05) after 1 year. The magnitude of the CD changes in the anterior and central corneal layers was smaller in the SMILE group than in the LASEK group (all p < 0.05). Lower HOAs, spherical aberration, and horizontal comas of the anterior and whole corneal surfaces were observed in the SMILE group. QoV scores were similar between the two groups. CD decreased in the SMILE and LASEK groups after 1 year; there was a smaller reduction in SMILE than in LASEK. SMILE and LASEK did not differ significantly in terms of safety and effectiveness in correcting mild-to-moderate myopia.

Clinical outcomes of the immediate reapplication of small-incision lenticule extraction without adjusting the surgical parameters after suction loss

This study was to analyze the clinical outcomes of immediate reapplication of small-incision lenticule extraction (SMILE) without adjusting the surgical parameters after suction loss and to compare the outcomes with contralateral eyes that underwent uneventful SMILE. A total of 74 patients who underwent uneventful SMILE in one eye (Uneventful group) and immediate reapplication of SMILE without adjusting the surgical parameters after suction loss in the contralateral eye (Suction loss group) were included. Suction loss occurred during the posterior lenticule surface cut in 39 eyes (53%) and the cap cut in 35 eyes (47%). Surgical outcomes, including visual acuity, manifest refraction, keratometry, and corneal wavefront aberrations, were evaluated at 6 months postoperatively. The mean uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), and spherical equivalent were − 0.02 ± 0.07, − 0.04 ± 0.04, and − 0.10 ± 0.46 diopters (D), respectively, in the Suction loss group and − 0.02 ± 0.07, − 0.04 ± 0.05, and − 0.19 ± 0.53 D, respectively (P = 0.965, 0.519, and 0.265, respectively), in the Uneventful group. Changes between the preoperative and 6-month postoperative total corneal aberrations, spherical aberrations, and horizontal and vertical coma did not significantly differ between the Suction loss and Uneventful groups. Immediate reapplication of SMILE without adjusting the surgical parameters after suction loss resulted in good surgical outcomes that were comparable with those of uneventful SMILE.

Long-term visual quality after small incision lenticule extraction (SMILE) and laser assisted subepithelial keratomileusis (LASEK) for low myopia

BackgroundFew studies have reported the visual outcomes of small-incision lenticule extraction (SMILE) and laser-assisted subepithelial keratomileusis (LASEK) for myopia correction. This study aims to compare the visual quality and corneal wavefront aberrations after SMILE and LASEK for low-myopia correction.MethodsIn this prospective study, we included 29 eyes of 29 patients who received SMILE and 23 eyes of 23 patients who received LASEK between June 2018 and January 2019. The following measurements were assessed: uncorrected (UDVA) and corrected (CDVA) distance visual acuity, manifest refraction, corneal wavefront aberrations, and subjective visual quality. All patients were followed up for two years.ResultsAll procedures were uneventful. An efficacy index of 1.19 ± 0.17 was established in the SMILE group and 1.23 ± 0.20 in the LASEK group. No eyes lost more than two lines of CDVA. We found that 93% (27/29) of the treated eyes in the SMILE group and 91% (21/23) in the LASEK group had spherical equivalent (SE) within ± 0.25D. The increases in the total corneal spherical aberration and the corneal front spherical aberration were lower in the SMILE group than in the LASEK group (P < 0.01). In contrast, the increases in the total corneal vertical coma and the corneal front vertical coma in the SMILE group were greater than those in the LASEK group (P < 0.01).ConclusionBoth SMILE and LASEK have good safety, stability, and patient-reported satisfaction for low myopia. SMILE induced less corneal spherical aberration but greater vertical coma than LASEK.

Changes of high order aberrations after photorefractive keratectomy (PRK) and FemtoLASIK

Purpose: a comparative study of corneal and total wavefront aberration changes after traditional PRK and FemtoLASIK.Material and methods. Corneal and total wavefront aberrations were studied on OPD-Scan III aberrometer (Nidek) in 63 patients (126 eyes) with moderate and high myopia before and after FemtoLASIK (Ziemer + Nidek-Quest, Japan) (44 patients, 88 eyes) and PRK (Nidek-Quest, Japan) (19 patients, 38 eyes). Spherical aberration (SA) was considered as the sum Z4 + Z8 + Z12.Results. The corneal asphericity coefficient Q changed to positive values after both types of intervention. Corneal RMS HOA increased in both Femto and PRK groups. Corneal Tilt 1 increased after FemtoLASIK and moved to negative values after PRK, corneal tilt 2 moved to negative values after Femto and to positive values after PRK. The vertical trefoil showed no significant change in the Femto group and dropped in the PRK group. The vertical and horizontal coma (coma 7, coma 8) changed synchronously. Corneal SA increased after Femtolasic, and became negative after PRK. The total aberrations showed a lesser change and the changes were not synchronized with the corneal ones: in particular, the RMS HOA fell from 0.28 ± 0.05 to 0.19 ± 0.05 in the Femto group, while the corneal RMS increased significantly. In the PRK group, the corneal RMS HOA showed some increase, but it was much less than corneal RMS: from 0.25 ± 0.07 to 0.62 ± 0.02 μm. The total SA increased from 0.11±0.03 to 0.27±0.02 in the Femto group and showed an insignificant drop from 0.09±0.03 to 0.03 ± 0.01, while remaining positive in the PRK group.Conclusions. The changes of the wavefront after FemtoLASIK and PRK are both significant and different. The revealed differences in the changes of corneal aberrations correspond to different profiles of the anterior surface of the cornea formed after FemtoLASIC and PRK. The internal optics of the eye partially compensates the corneal aberrations induced by excimer laser surgery.

Comparison of Anterior Corneal Aberrometry, Keratometry and Pupil Size with Scheimpflug Tomography and Ray Tracing Aberrometer.

This study aimed to assess the anterior corneal wavefront aberrations, keratometry, astigmatism vectors and pupil size between Pentacam HR® (Oculus Optikgeraete GmbH, Wetzlar, Germany) and iTrace® (Tracey Technologies Corp., Houston, TX, USA). In this observational study, 100 eyes (50 healthy volunteers) were scanned in mesopic light condition with a Pentacam HR® and iTrace®. Anterior corneal aberrations (spherical aberration (Z40), vertical coma (Z3 − 1), horizontal coma (Z3 + 1)), keratometry in the flattest (K1) and steepest meridian (K2), mean astigmatism, astigmatic vectors (J0 and J45), and pupil size were measured. We found a significant difference in Z40 (Pentacam®: +0.30 ± 0.11 µm and iTrace®: −0.03 µm ± 0.05 µm; p < 0.01) with no correlation between the devices (r = −0.12, p = 0.22). The devices were in complete agreement for Z3 − 1 (p = 0.78) and Z3 + 1 (p = 0.39), with significant correlation between the machines (r = −0.38, p < 0.01 and r = −0.6, p < 0.01). There was no difference in K1, K2 and mean astigmatism. J0 was negative with both devices (against-the-rule astigmatism), but there was no correlation. J45 was negative with the Pentacam HR® (more myopic oblique astigmatism) but significantly correlated between the devices. Pupil size was smaller with Pentacam HR® (p < 0.01). In summary, these devices cannot be used interchangeably. Corneal Z40 was significantly different with more negative Z40 with iTrace® compared to Pentacam HR®. iTrace® operates with lower illumination, giving larger pupil size than Pentacam HR®, which uses intense blue light during measurement. No correlation was found for J0. Pentacam HR® had a trend to record more negative J45 (myopic oblique astigmatism).

The effect of cross-linking procedure on corneal wavefront aberrations in patients with keratoconus

Background/Aim. Corneal cross-linking (CXL) treatment shows the best results in stabilizing the cornea and stopping the progress of the ectatic process. The aim of the study was to assess the impact of CXL on the keratoconus regarding higher-order aberrations (HOAs) and potential improvement of visual function. Methods. In 19 patients, a standard epithelium-off CXL was performed with an energy density of three mW/cm2 for half an hour. The cornea was examined by Pentacam topography before CXL and one and six months after CXL. Best-corrected visual acuity (BCVA), topographic data, and aberrations were collected. Results. A significant reduction in vertical coma was observed from preoperative -1.03 ? 1.89 to -0.74 ? 1.73 (p = 0.004) six months after CXL, and in spherical aberration from preoperative -0.22 ? 1.05 to -0.08 ? 1.13 (p = 0.002) six months after CXL. Root mean square (RMS) HOAs six months after CXL also significantly reduced from 2.31 ? 1.82 to 2.26 ? 1.84 six months after CXL (p = 0.001). BCVA improved from preoperative 0.43 ? 0.15 to 0.71 ? 0.19 six months after surgery (p &lt; 0.001). Conclusion. CXL is a very potent treatment method for keratoconus, which significantly reduces certain corneal wavefront aberrations, especially vertical coma, spherical aberration, and RMS, and leads to a significant improvement in visual acuity.

Analysis of Corneal Scheimpflug Densitometry and Ocular Wavefront Aberrations Post Descemet Stripping Automated Endothelial Keratoplasty.

To assess the role of corneal densitometry and wavefront aberrations post Descemet stripping automated endothelial keratoplasty (DSAEK) and their correlation with visual acuity. Twenty-seven eyes of 25 patients with nonresolving corneal edema for more than 3 months as a result of Fuchs endothelial corneal dystrophy, pseudophakic bullous keratopathy, or secondary endothelial dysfunction were enrolled in a prospective interventional study and underwent DSAEK. Postoperative evaluation for corneal aberrations, including Higher order aberrations (HOAs) and corneal densitometry (CD), was performed using anterior-segment optical coherence tomography (Avanti RTvue XR; Optovue) and Scheimpflug imaging (Pentacam, Oculus Optikgeräte GmbH, Wetzlar, Germany) at 6 months. A correlation analysis of corneal aberrations, including HOAs and CD, with postoperative best-corrected visual acuity at 6 months was performed. Mean best-corrected visual acuity (BCVA) improved from 1.67±0.53 log of minimum angle of resolution (logMAR) to 0.2±0.22 logMAR post DSAEK at 6 months (P≤0.0001). At 6 months, mean root-mean-square (RMS) total corneal aberrations (includes HOA and low-order aberration [LOA]), RMS total HOA, and RMS LOA of the central 6-mm zone were 4.99±2.64, 1.80±0.9, and 4.55±2.64, respectively. The mean corneal densitometry from anterior, central, and posterior zones were 39.12±12.77, 23.9±7.2, and 13.54±2.04 gray scale units, respectively. Total anterior aberrations (r=0.051; P=0.006), anterior LOA (r=0.049; P=0.009), total corneal aberrations (r=0.051; P=0.001), total HOAs (r=0.095; P=0.057) and LOAs (r=0.050; P=0.002), and total CD from 0 to 2 mm (r=0.010; P=0.038) and 2 to 6 mm (r=0.014; P=0.018) showed a significant inverse correlation with postoperative BCVA. There was an inverse relationship between post DSAEK BCVA and total corneal HOAs and full-thickness densitometry at 6 months. Scheimpflug imaging with added tools for corneal aberration and densitometry analysis gives further insight into the suboptimal vision achieved despite transparent corneas post DSAEK.

Selective transepithelial ablation with simultaneous accelerated corneal crosslinking for corneal regularization of keratoconus: STARE-X protocol.

To evaluate the changes in refractive outcomes and corneal aberrations in central and paracentral keratoconus after selective transepithelial topography-guided photorefractive keratectomy combined with accelerated corneal crosslinking (STARE-X). Centro Polispecialistico Mediterraneo, Siena Crosslinking Center, and University of Messina, Italy. Prospective, interventional, multicentric study. Patients were subdivided into 2 groups: Group 1 with cone located within the central 3 mm zone (50 eyes) and Group 2 (50 eyes) with cone located outside the central 3 mm zone. Follow-up was 2 years at least for all eyes. Outcome parameters included uncorrected distance visual acuity (UDVA) and corrected distance visual acuity (CDVA). Corneal tomography and corneal wavefront aberrations were assessed and compared before and 2 years after the treatment. 100 eyes of 100 patients underwent STARE-X protocol. At 2 years, UDVA and CDVA improved, and sphere, cylinder, and Kmax reduced after treatment in both groups (P < .001, respectively). Moreover, a statistically significant reduction was observed of total higher-order aberrations root main square (RMS), coma RMS, and spherical aberration RMS in both groups (P < .001, respectively). However, CDVA improved more in Group 1 than in Group 2 (P < .02). The STARE-X protocol demonstrated effective results in halting keratoconus progression and improving corneal regularity with a safe and effective profile. STARE-X improved both visual acuity and corneal aberration at 2 years. Longer follow-up studies are warranted to observe further long-term CXL flattening effect on the cone.
