Combined corneal biomechanical and tomographical indices in subclinical and forme fruste keratoconus.

Evaluation of combined corneal tomographic and biomechanical parameters in subclinical/forme fruste keratoconus (ScKC/FFKC). Cross-sectional observational case-control study. Thirty-one eyes with ScKC (fellow eye of KC with any one sign keratometry >47 diopters, cylinder >1.5 D, central corneal thickness <500 μm, with/without abnormal topography) or FFKC (fellow eye of KC with normal topography and slit lamp examination) >13 years (cases) and 44 eyes of age-matched 22 healthy subjects (controls). Clinically diagnosed KC, presence of corneal scars, and prior ocular surgery eyes. Sixteen Pentacam, 15 Corvis ST, and five Sirius parameters were analyzed using paired sample t-test, and a subsample found to be significantly different was used in receiver operating characteristic curve analysis. The Youden index was calculated, and Pearson's correlation analysis was done. Five Pentacam, three Corvis ST, and two Sirius parameters had an area under curve (AUC) >0.75. Tomographic and biomechanical index (TBI) (cutoff 0.59, 95% specificity, 77% sensitivity), Belin Ambrosio enhanced ecstasia display (cutoff 1.8, 81% specificity, 80% sensitivity), and symmetry index of posterior corneal curvature (cutoff 0.16, 97% specificity, 67% sensitivity) best identified early KC. TBI strongly correlated with maximum Pentacam parameters in both cases and controls. Corvis biomechanical index strongly correlated only in cases, and SP-A1-SD weakly correlated in cases. Upon combined analysis, the average sensitivity and specificity, respectively, of top three parameters (according to AUC) from Pentacam and Corvis ST were 74.1% and 95.4% for posterior elevation and TBI. A Corneal Biomechanical Study Measured with a Scheimpflug Dynamic Analyser in Soft Contact Lens Wearers.

The aim of this study was to evaluate the biomechanical changes in the cornea after wearing soft contact lenses (CLs) in healthy myopic patients measured with a Corvis ST® (CST, Oculus Optikgeräte GmbH, Wetzlar, Germany) analyser. This prospective, cross-sectional, single-centre study was performed on twenty-two Caucasian patients aged between 19 and 24 years (20.64 ± 1.21 years) range. Five device-specific biomechanical parameters, the central corneal thickness (CCT), and biomechanically corrected intraocular pressure (bIOP) were measured prior to fitting and one month after CL wear. Differences between the means of the deflection amplitude ratio (DA Ratio) and the standard deviation of the DA Ratio (SD DA Ratio) pre- and post-CL wear were found to be significant (p value = 0.002 in both cases). Significant differences were found between pre- and post-CL wear values in CCT (p value = 0.013). For all other biomechanical measures, no significant differences were observed before and after treatment. A significant association was found between changes in bIOP and classification according to changes in Int. Radius (p value = 0.047) and SSI (p value = 0.026) standard deviations. The corneal biomechanical indices provided by CST demonstrate that the fitting of soft CLs is a safe optical compensation method for the stability of corneal stiffness. No significant differences were found pre- and post-CL wear in the assessment of bIOP.

Screening of sensitive in vivo characteristics for early keratoconus diagnosis: a multicenter study.

Purpose: To analyze and compare sensitive in vivo characteristics for screening early keratoconus. Methods: This multicenter, case-control study included 712 eyes, after matching for age and biomechanically corrected intraocular pressure, from three clinics in different cities. The keratoconus (n = 288), early keratoconus (n = 91), and normal cornea (n = 333) groups included eyes diagnosed with bilateral keratoconus, fellow eyes with relatively normal topography with unilateral keratoconus, and normal eyes before refractive surgery, respectively. After adjusting for central corneal thickness, differences in vivo characteristics were analyzed among the three groups. The in vivo characteristics were measured by Pentacam and Corvis ST. Fifty-four indices were evaluated to screen for a sensitive index for the detection of early keratoconus. Results: Significant differences were observed in 26 of the 36 corneal biomechanical indeces between the early keratoconus and normal corneas. The area under the receiver operating characteristic curve of tomographic and biomechanical index, Belin/Ambrósio deviation, and Da in differentiating keratoconus from normal cornea was 1.000. Among the top five indeces of the area under the receiver operating characteristic curve for detecting early keratoconus, the corneal biomechanical-related index accounted for 80% (4/5), including A1 dArc length, highest concavity radius, A2 time, and tomographic and biomechanical index, of which the area under the receiver operating characteristic curve of A1 dArc length was 0.901. Conclusion: A1 dArc length and several corneal biomechanical indices are highly sensitive for the detection of early keratoconus, even in the absence of topographic abnormalities. Ophthalmologists should focus on the clinical application of corneal biomechanics and combine corneal tomography for the timely and accurate detection of early keratoconus.

Comparison of Ectasia Detection in Early Keratoconus Using Scheimpflug-Based Corneal Tomography and Biomechanical Assessments.

The aim of this study was to determine the detection of keratoconus using corneal biomechanical parameters only, a corneal tomographic parameter only, and a parameter that combines corneal biomechanical and tomographic indices. The discriminatory power of the Pentacam Random Forest Index (PRFI), Belin/Ambrósio Enhanced Ectasia Display (BAD-D) index, Corvis Biomechanical Index (CBI), and Tomographic and Biomechanical Index (TBI) to differentiate between normal eyes (n = 84), eyes with very asymmetric corneal ectasia (VAE-E, n = 21), and the fellow eyes without apparent ectasia based on normal tomography (VAE-NT, n = 21) was assessed. Statistical analyses were completed with R software using t-tests, Wilcoxon rank sum tests, and receiver operating characteristic (ROC) curves. The DeLong test was used to compare the area under the ROC curve (AUROC). The TBI and PRFI had the highest AUROC when distinguishing between normal and VAE-E corneas (AUROC = 1.00, 95% CI = 1.00-1.00); however, they were not statistically superior to the CBI (AUROC = 0.97, P = 0.27) or BAD-D (AUROC = 1.00, P = 0.34). The TBI (AUROC = 0.92, 95% CI = 0.86-0.98) was superior to CBI (AUROC = 0.78, P = 0.02) and BAD-D (AUROC = 0.81, P = 0.02) when distinguishing between healthy and VAE-NT corneas. At a threshold of 0.72, the TBI had 99% sensitivity, 67% specificity, and 92% accuracy in distinguishing normal and VAE-NT corneas. The TBI is a useful parameter for the screening of subclinical and frank keratoconus in tomographically normal eyes.

Assessment of the corneal biomechanical features of granular corneal dystrophy type 2 using dynamic ultra-high-speed Scheimpflug imaging.

To evaluate the corneal biomechanical features of eyes with granular corneal dystrophy type 2 (GCD2) by analyzing corneal biomechanical indices obtained using a Corvis ST (CST) dynamic ultra-high-speed Scheimpflug imaging device. In this retrospective case-control study, 35 CST parameters were compared in normal eyes (control) and eyes of patients with GCD2 treated at Osaka University Hospital, Osaka, Japan. The parameters included the Corvis Biomechanical Index (CBI), which is important in differentiating eyes with keratoconus from normal eyes. We measured the deposition rates of lesions in the central 7-mm region of the eye and assessed the correlation between the deposition rate and the CBI. Twenty-one eyes with GCD2 and 23 control eyes were analyzed. Eyes with GCD2 showed significantly less corneal stiffness in 15 CST parameters than did control eyes. In particular, the CBI was remarkably higher in eyes with GCD2 than in control eyes (P = 0.000006). Additionally, the deposition rate and the CBI were positively correlated. GCD2 eyes had softer corneas than did control eyes in most biomechanical CST parameters, and one of the parameters (the CBI) was linked to the rate of deposited lesions. Since IOP may be underestimated in GCD2 eyes, management should be especially careful in GCD2 cases complicated by glaucoma.

Corneal-compensated intraocular pressure, Goldmann-correlated intraocular pressure and their associated factors in the geriatric population, a population-based study.

To determine the distribution of corneal-compensated intraocular pressure (IOPcc), Goldmann-correlated intraocular pressure (IOPg) and their associated factors in a geriatric population. The present cross-sectional study was performed in individuals above 60years of age in Tehran, the capital of Iran. The sampling was performed using multi-stage random cluster sampling method. All participants underwent preliminary ocular examinations, and then imaging by Pentacam HR and IOL master 500. Measurement of IOPg, IOPcc, and corneal biomechanical indices including corneal hysteresis (CH) and corneal resistant factor (CRF) was performed in a random sub-sample using ocular response analyzer (ORA). The mean IOPg and IOPcc were 16.76 ± 4.71mmHg and 19.05 ± 4.67mmHg, respectively. There were no statistically significant differences in both IOPg (p = 0.891) and IOPcc (p = 0.248) between males and females. Based on the multiple linear regression models, both IOPg and IOPcc showed a statistically significant direct relationship with CRF (P < 0.001) and a significant inverse relationship with CH (P < 0.001 for IOPg and IOPcc), anterior chamber angle (ACA) (p = 0.006 for IOPg and p = 0.017 for IOPcc), and spherical equivalent refractive error (p = 0.032 for IOPg and p = 0.046 for IOPcc). Mean IOPg and IOPcc in the present study were higher compared to most previous studies. Corneal biomechanical indices including CH and CRF, refractive error and anterior chamber angle were independent associated factors of IOPg and IOPcc in the present study. There was no significant relationship between ORA-derived IOP values and CCT.

Evaluation of corneal topographic, tomographic and biomechanical indices for detecting clinical and subclinical keratoconus: a comprehensive three-device study.

To evaluate the diagnostic ability of topographic and tomographic indices with Pentacam and Sirius as well as biomechanical parameters with Corvis ST for the detection of clinical and subclinical forms of keratoconus (KCN). In this prospective diagnostic test study, 70 patients with clinical KCN, 79 patients with abnormal findings in topography and tomography maps with no evidence on clinical examination (subclinical KCN), and 68 normal control subjects were enrolled. The accuracy of topographic, tomographic, and biomechanical parameters was evaluated using the area under the receiver operating characteristic curve (AUC) and cross-validation analysis. The Delong method was used for comparing AUCs. In distinguishing KCN from normal, all parameters showed statistically significant differences between the two groups (P<0.001). Indices with the perfect diagnostic ability (AUC≥0.999) were Sirius KCN vertex of back (KVb), Pentacam random forest index (PRFI), Pentacam index of height decentration (IHD), and Corvis integrated tomographic/biomechanical index (TBI). In distinguishing subclinical KCN from normal, Sirius symmetry index of back (SIb; AUC=0.908), Pentacam inferior-superior difference (IS) value (AUC=0.862), PRFI (AUC=0.847), and Corvis TBI (AUC=0.820) performed best. There were no significant differences between the highest AUCs within keratoconic groups (DeLong, P>0.05). In clinical KCN, all topographic, tomographic, and biomechanical indices have acceptable outcomes in terms of sensitivity and specificity. However, in differentiating subclinical forms of KCN from normal corneas, curvature-based parameters (SIb and IS value) followed by integrated indices (PRFI and TBI) are the most powerful tools for early detection of KCN.

Optical Quality in Keratoconus Is Associated With Corneal Biomechanics.

To evaluate the correlations between corneal biomechanical indices from dynamic Scheimpflug assessment and optical quality assessed as higher-order aberrations (HOAs) using a Hartmann-Shack ocular wavefront sensor in patients with keratoconus (KC). In this prospective, observational case series, the eyes with KC or KC suspect (KCS) from Osaka University Hospital, Osaka, Japan, were analyzed. Corneal biomechanical assessment was performed using Corvis ST (Oculus Optikgeräte GmbH, Wetzlar, Germany), and ocular wavefront aberrations were measured using the KR-1W (Topcon Corp, Tokyo, Japan). Correlations between the biomechanical indices and ocular HOAs were assessed. Corneal biomechanical indices included the deformation amplitude ratio within 2 mm, integrated radius, stiffness parameter at the first applanation, and the linear Corvis Biomechanical Index. Wavefront data of the central 4-mm region were expanded up to the sixth order of Zernike polynomials. The magnitudes of trefoil, coma, tetrafoil, secondary astigmatism, and spherical aberration were calculated by Zernike vector analysis and then used as ocular HOA parameters along with total HOAs. Thirty-four KC eyes and 37 KCS eyes were included. KC eyes showed significant correlations between ocular HOAs and biomechanics, whereas there were few significant correlations in KCS eyes. In KC eyes, deformation amplitude ratio within 2 mm, integrated radius, and Corvis Biomechanical Index beta showed stronger correlations with coma among the wavefront parameters. Corneal biomechanical indices correlated with ocular HOAs in patients with KC. In particular, there was a strong association with the increase in coma caused by inferosuperior asymmetry of the shape of the cornea in patients with KC.

Correlation Between Corneal Biomechanical Indices and the Severity of Keratoconus.

To investigate the correlations between the biomechanical indices determined in Scheimpflug-based corneal biomechanical assessments and the severity of keratoconus (KC) based on corneal tomographic assessments in patients with different stages of KC. Fifty-three patients who presented with clinical KC in 1 eye and KC suspect in the fellow eye were included. Corneal tomographic and biomechanical assessments were performed using the Pentacam HR and Corvis ST (Oculus Optikgeräte GmbH, Wetzlar, Germany). Correlations between the tomographic indices and biomechanical indices were assessed, including the anterior radius of curvature (ARC) and posterior radius of curvature (PRC) at a 3.0-mm optical zone and the thinnest pachymetry (Tmin), deformation amplitude ratio max 2 mm (DAR2mm), integrated radius, stiffness parameter at the first applanation, and linear Corvis Biomechanical Index (beta). DAR2mm correlated negatively with ARC (R = -0.722), PRC (R = -0.677), and Tmin (R= -0.650) (P < 0.001 for all). Integrated radius correlated negatively with ARC (R = -0.700), PRC (R = -0.668), and Tmin (R= -0.648) (P < 0.001 for all). Stiffness parameter at the first applanation correlated positively with ARC (R = 0.622), PRC (R = 0.601), and Tmin (R = 0.703) (P < 0.001 for all). The Corvis Biomechanical Index beta correlated negatively with ARC (R = -0.754), PRC (R = -0.755), and Tmin (R= -0.765) (P < 0.001 for all). Corneal biomechanical indices correlated with corneal tomographic parameters in patients with KC. These findings support the possibility of developing biomechanical-based staging classification for KC in combination with topographic or tomographic indices.

Accuracy of Scheimpflug-derived corneal biomechanical and tomographic indices for detecting subclinical and mild keratectasia in a South Asian population

To test the predictive accuracy of the Belin-Ambrósio deviation index (BAD-D), the stiffness parameter A1 (SPA1), the Corvis biomechanical index (CBI), and the tomographic and biomechanical index (TBI) assessments for distinguishing subclinical and mild keratoconic eyes from normal eyes. Medical Research Foundation, Sankara Nethralaya, Chennai, India. Retrospective case series. In this cross-sectional clinical study, the following 3 groups of eyes were analyzed: very asymmetric ectasia with normal topography (very asymmetric-normal topography), mild keratoconus, and normal control. All eyes had comprehensive assessment with corneal topography (TMS-IV), Scheimpflug tomography (Pentacam HR), and dynamic Scheimpflug biomechanical analysis (Corvis ST). The outcomemeasures were the BAD-D, SPA1, CBI, and TBI. Receiver operating characteristic (ROC) curve analysis was performed to determine each parameter's predictive accuracy in distinguishing between eyes with subclinical or mild keratoconus and control eyes. The area under the curve (AUC) ROC values for the very asymmetric-normal topography and normal control comparison were 0.81 (BAD-D), 0.76 (SPA1), 0.78 (CBI), and 0.90 (TBI). The TBI (using cutoff value 0.16) showed the highest diagnostic accuracy (85%), with 84% sensitivity and 86% specificity. The AUC ROC values for the mild keratoconus and normal control comparison were 0.998 (BAD-D), 0.91 (SPA1), 0.97 (CBI), and 0.999 (TBI). The TBI (with a 0.63 cutoff) showed the highest accuracy (99.5%), with 99% sensitivity and 100% specificity. The TBI also showed the weakest correlation with mean keratometry, biomechanically corrected intraocular pressure, and central corneal thickness in normal eyes and keratoconic eyes. The TBI best distinguished eyes with mild ectasia from normal eyes and had the weakest correlation with biomechanical confounding factors, reinforcing the hypothesis that the TBI represents corneal biomechanical susceptibility.

Ocular response analyzer parameters in healthy, keratoconus suspect and manifest keratoconus eyes.

Background:To evaluate and compare corneal biomechanical indices and their specificity among keratoconus (KC), keratoconus suspect (KCS), and normal eyes (NL) before and after controlling potential confounders.Materials and Methods:A total of 160 eyes in three groups were included prospectively: NL, KC, and KCS groups based on clinical examination and topography. Corneal hysteresis (CH) and corneal resistance factor (CRF) were measured by the ocular response analyzer. CH and CRF were compared between the three groups by analysis of variances test.Results:The three groups consisted of 80 NL, 48 KC, and 32 KCS eyes. The mean CH measured was 10.4 ± 1.25, 7.83 ± 1.28 and 10.17 ± 1.80 mm Hg in NL, KC and KCS eyes, respectively. The mean CRF was 10.23 ± 1.75, 6.5 ± 1.63 and 9.98 ± 2.00 mm Hg in NL, KC and KCS eyes, respectively. Mean CH and CRF were significantly different between the NL and KC (P < 0.05); however after controlling for central corneal thickness and sex; there was no significant difference between NL and KCS (P > 0.05).Conclusion:CH and CRF can be helpful in differentiating KC from NL eyes; however, they are not valuable for detecting KCS that is the main concern for refractive surgery. Future studies focusing on more accurate tests for identifying KCS, using a consistent grading scale for defining KC and KCS are still warranted.

Descriptive study of topographic patterns in clinically diagnosed keratoconus

Abstract Purpose This study was undertaken with the objective of responding to some questions: how are our patients´s keratoconus? and could we establish unified topographic patterns to describe them? Methods Descriptive analysis of different parameters evaluated by Orbscan corneal topography system (BFS, RCA/RCP, anterior and posterior elevation, Simk, mean pwr, thinnest) and biomechanical indices measured with Ocular Response Analyzer (CRF and CH) as well as topographic patterns that can be found in the clinical keratoconus diagnosed between 2006 and 2009 in ophthalmology service at Hospital Clinico "Lozano Blesa" in Zaragoza. Results We have studied 40 eyes of 24 patients diagnosed with keratoconus.Subclinical keratoconus and those who had received treatment,either rigid contact lenses or implanted intracorneal ring segments were previously excluded. The average age of patients was 33.7+/‐11.1 years. Quantitative topographic descriptors were: BFS = 55.99+/‐3.35 D; RCA/RCP = 1.25+/‐0.04, anterior surface elevation = 48.08+/‐26.66 µ; back surface elevation = 107.18+/‐43.11 µ; simk = ‐5.4+/‐2.6 D, mean pwr = 48.5+/‐3.7 D; thinnest = 423+/‐67 µ; biomechanical values: CRF = 6.9+/‐1.7 and CH = 7.8+/‐1.4. Four topographic patterns are also described depending on the situation of the apex of the cone: 30% central, 35% lower, 30% inferior temporal, 5% inferior nasal. Conclusion There is great morphological variation in keratoconus, it is therefore important to unify criteria for classification. The apex location criteria allows us to make a simple classification of keratoconus and is useful for planning intracorneal ring segments implantation.
