Combatting Diseases and addressing Antimicrobial Resistance are critical in safeguarding public health globally, and discussions on Livestock Improvement are integral to the sustainability of agricultural systems. OMICS: The Future heralds a new era of scientific exploration and discovery, powered by advanced technologies. Navigating Sensitive Conversations: Patient-Centered Communication and Politeness Markers in Chinese Online Medical Consultations.

Background/Objectives: In China, discussing sexual and reproductive health remains taboo, often preventing patients from seeking care or advice on sensitive topics. Online medical consultations (OMCs) offer a unique platform for patients to discuss these concerns more openly. This study investigates how patient-centered communication (PCC) practices, including conversational themes and the use of politeness markers, influence patient satisfaction in Chinese OMCs, with a focus on sensitive gynecology and andrology topics. Methods: This study used a mixed-methods approach, including theme-oriented discourse analysis (TODA) and content analysis on 328 OMCs (179 in andrology, 149 in gynecology) collected from Dr. Chunyu, a popular Chinese online healthcare platform that provides medical consultations, from 19 to 22 March 2022. Logistic regressions were conducted to assess the influence of politeness markers on patient satisfaction, while TODA examined PCC practices in sensitive conversations. Results: TODA identified two key themes in PCC that enhanced patient satisfaction: normalizing sensitive health concerns and fostering collaborative decision-making. Politeness markers, specifically the use of polite words and expressions of best wishes, were positively associated with patient satisfaction. However, downtoners, emojis, and sentence-final particles showed no significant effect. There were no significant differences in the impact of politeness markers between gynecology and andrology consultations. Conclusions: This study highlights the importance of PCC and politeness markers in improving patient satisfaction in OMCs, especially when addressing sensitive sexual health topics.

"I Been Taking Adderall Mixing it With Lean, Hope I Don't Wake Up Out My Sleep": Harnessing Twitter to Understand Nonmedical Prescription Stimulant Use among Black Women and Men Subscribers.

Black women and men outpace other races for stimulant-involved overdose mortality despite lower lifetime use. Growth in mortality from prescription stimulant medications is increasing in tandem with prescribing patterns for these medications. We used Twitter to explore nonmedical prescription stimulant use (NMPSU) among Black women and men using emotion and sentiment analysis, and topic modeling. We applied the NRC Lexicon and VADER dictionary, and LDA topic modeling to examine feelings and themes in conversations about NMPSU by gender. We paid attention to the ability of natural language processing techniques to detect differences in emotion and sentiment among Black Twitter subscribers given increased mortality from stimulants. We found that, although emotion and sentiment outcomes match the directionality of emotions and sentiment observed (i.e., Black Twitter subscribers use more positive language in tweets), this belies limitations of NRC and VADER dictionaries to distinguish feelings for Black people. Even still, LDA topic models showcased the relevance of hip-hop, dependence on NMPSU, and recreational use as consequential to Black Twitter subscribers' discussions. However, gender shaped the relevance of these topics for each group. Greater attention needs to be paid to how Black women and men use social media to discuss important topics like drug use. Natural language processing methods and social media research should include larger proportions of Black, Hispanic/Latinx, and American Indian populations in development of emotion and sentiment lexicons, otherwise outcomes regarding NMPSU will not be generalizable to populations writ large due to cultural differences in communication about drug use online.

Identification of cultural conversations in therapy using natural language processing models.

Researchers have historically focused on understanding therapist multicultural competency and orientation through client self-report measures and behavioral coding. While client perceptions of therapist cultural competency and multicultural orientation and behavioral coding are important, reliance on these methods limits therapists receiving systematic, scalable feedback on cultural opportunities within sessions. Prior research demonstrating the feasibility of automatically identifying topics of conversation in psychotherapy suggests that natural language processing (NLP) models could be trained to automatically identify when clients and therapists are talking about cultural concerns and could inform training and provision of rapid feedback to therapists. Utilizing 103,170 labeled talk turns from 188 psychotherapy sessions, we developed NLP models that recognized the discussion of cultural topics in psychotherapy (F-1 = 70.0; Spearman's ρ = 0.78, p < .001). Exploring communication challenges in dementia care: Insights from home healthcare nurses

ObjectivesCare for people living with dementia predominantly takes place at home and is often characterized by multiple hospitalizations throughout the lengthy disease trajectory. Care transitions from the hospital often involve home health care. The purpose of this study was to explore communication challenges experienced by home health nurses to inform communication training needs for home-based dementia care. MethodsSeven audio-recorded focus group discussions were held with home health nurses (n = 31) from a large home care agency serving rural and urban counties. Focus group transcripts were thematically analyzed. ResultsWe identified the following themes highlighting communication challenges for home health nurses: addressing potential risk for harm or injury, patient vulnerability, care partner needs, shifting decision-making to surrogate, and discrepancy in home health goals and family needs. These themes identified specific care transition conversation topics and communication content imperative for dementia-specific staff training. ConclusionsTopics for communication skills training to improve person-centered dementia care were identified. These findings inform the future development of a home health staff communication curriculum for dementia care. Advanced communication skills are necessary to ensure these conversations take place and are engaged with sensitivity. Practice implicationsOur study informs the development of communication training on key communication topics in nurse-delivered and home healthcare interventions aimed at improving dementia care.

A research brief investigating educational television and U.S. children’s interest in science and world culture

ABSTRACT Research on educational television has overwhelmingly investigated the impact of viewing on children’s knowledge acquisition. However, this content might influence other important outcomes, such as children’s interest in learning about new topics. To investigate this possibility, we invited parents of 3- to 8-year-old children (N = 83) to answer three surveys over 1 month. The first survey included a video encouraging families to view a popular science program (Hero Elementary) or cultural program (Let’s Go Luna). The remaining surveys asked about children’s exposure to science and cultural programs, as well as their engagement with science and cultural topics in everyday conversation. Although our encouragement intervention had no impact on children’s exposure to science or cultural programs, viewing that content was associated with increased science and cultural talk over time. These findings suggest that educational television might motivate children to explore new topics of interest in non-mediated contexts.

Identifying Topics Around Nicotine Gum: A Machine Learning Approach with Twitter Data

Background Nicotine gum products from brands like Lucy and Rogue are relatively new arrivals to the tobacco marketplace. While studies of correlates of nicotine gum use are in their nascent stage, data from social media can be used to stay abreast of user experiences with novel tobacco products. This study leveraged machine learning to identify topics of Twitter posts about nicotine gum from the year 2022. Methods Twitter data was collected using the Twitter Application Programming Interface (API), with search terms “nicotine” AND “gum” OR “nic” AND “gum”. 16,940 tweets from 10,353 unique users were included in the analysis. Topic modeling with Top2Vec was used to identify topics and a string search of popular brands and flavors was also conducted. Results Eight distinct topics were identified. Smoking and vaping cessation was the most common topic, followed by promotion, pricing, and marketing; appeal; product comparisons; perceived benefits; distrust in institutions; health concerns; and COVID-19 misinformation. The most mentioned brand was Nicorette. A tenth of the tweets contained a reference to a flavor. Conclusion The goal of identifying topics in nicotine gum-related conversations is to better understand the public’s perceptions and experiences with nicotine gum. These findings may be used to inform survey-based research, policy targets, and health communication campaigns.

The role of objects in negotiations in convoys of care: Addressing fundamental concerns of informal caregivers

Due to an increased policy focus on informal care in many Western countries, professionals and informal caregivers increasingly grow more interdependent. Increased involvement of informal caregivers in professional care has consequences for the work of professionals, the care that is received by care recipients, and the role of informal caregivers. Care needs to be negotiated between them within the dynamic networks of care recipients and caregivers (i.e., “care convoys”). Scant attention has been given to objects as part of these convoys and negotiations. Therefore, in this paper, we answer the question: How do objects become part of and what is their role in the negotiations between healthcare professionals and informal caregivers? We use interview data from 48 participants (care recipients, professionals, informal caregivers, and persons in managerial positions). In the results, we discuss how objects, in terms of their affordances and the values they embody, become important in the relationship between professionals and informal caregivers and how they become part of negotiations on quality of care. We find that seemingly mundane objects become topics of conversation to address more fundamental concerns in how healthcare is organized for and provided to individual care recipients. Our study helps to open the care convoys model to objects as important actors and further understand the politics within care convoys.

Characterization of Telecare Conversations on Lifestyle Management and Their Relation to Health Care Utilization for Patients with Heart Failure: Mixed Methods Study.

Telehealth interventions where providers offer support and coaching to patients with chronic conditions such as heart failure (HF) and type 2 diabetes mellitus (T2DM) are effective in improving health outcomes. However, the understanding of the content and structure of these interactions and how they relate to health care utilization remains incomplete. This study aimed to characterize the content and structure of telecare conversations on lifestyle management for patients with HF and investigate how these conversations relate to health care utilization. We leveraged real-world data from 50 patients with HF enrolled in a postdischarge telehealth program, with the primary intervention comprising a series of telephone calls from nurse telecarers over a 12-month period. For the full cohort, we transcribed 729 English-language calls and annotated conversation topics. For a subcohort (25 patients with both HF and T2DM), we annotated lifestyle management content with fine-grained dialogue acts describing typical conversational structures. For each patient, we identified calls with unusually high ratios of utterances on lifestyle management as lifestyle-focused calls. We further extracted structured data for inpatient admissions from 6 months before to 6 months after the intervention period. First, to understand conversational structures and content of lifestyle-focused calls, we compared the number of utterances, dialogue acts, and symptom attributes in lifestyle-focused calls to those in calls containing but not focused on lifestyle management. Second, to understand the perspectives of nurse telecarers on these calls, we conducted an expert evaluation where 2 nurse telecarers judged levels of concern and follow-up actions for lifestyle-focused and other calls (not focused on lifestyle management content). Finally, we assessed how the number of lifestyle-focused calls relates to the number of admissions, and to the average length of stay per admission. In comparative analyses, lifestyle-focused calls had significantly fewer utterances (P=.01) and more dialogue acts (Padj=.005) than calls containing but not focused on lifestyle management. Lifestyle-focused calls did not contain deeper discussions on clinical symptoms. These findings indicate that lifestyle-focused calls entail short, intense discussions with greater emphasis on understanding patient experience and coaching than on clinical content. In the expert evaluation, nurse telecarers identified 24.2% (29/120) of calls assessed as concerning enough for follow-up. For these 29 calls, nurse telecarers were more attuned to concerns about symptoms and vitals (19/29, 65.5%) than lifestyle management concerns (4/29, 13.8%). The number of lifestyle-focused calls a patient had was modestly (but not significantly) associated with a lower average length of stay for inpatient admissions (Spearman ρ=-0.30; Padj=.06), but not with the number of admissions (Spearman ρ=-0.03; Padj=.84). Our approach and findings offer novel perspectives on the content, structure, and clinical associations of telehealth conversations on lifestyle management for patients with HF. Hence, our study could inform ways to enhance telehealth programs for self-care management in chronic conditions.

Detecting climate anxiety in therapy through natural language processing

A well-documented consequence of global warming is increased psychological distress and climate anxiety, but data gaps limit action. While climate anxiety garners attention, its expression in therapy remains unexplored. Natural language processing (NLP) models can identify climate discussions in therapy, aiding therapists and informing training. This study analyzed 32,542 therapy sessions provided by 849 therapists to 7,916 clients in U.S. behavioral health programs between July 2020 and December 2022, yielding 1,722,273 labeled therapist-client micro-dialogues. Climate- and weather-related topics constituted a mere 0.3% of the sessions. Clients exhibiting higher levels of depressive or anxiety symptoms were less likely to discuss weather and climate compared to those with mild or no symptoms. Findings suggest that although global warming is known to impact mental health, these issues are not yet adequately addressed in psychotherapy. This study suggests a potential gap between the documented mental health concerns associated with climate change and their representation in psychotherapy. NLP models can provide valuable feedback to therapists and assist in identifying key moments and conversational topics to inform training and improve the effectiveness of therapy sessions.

Revisiting Four Conversations in Technical and Professional Writing Scholarship to Frame Conversations About Artificial Intelligence

This article explores four different topics of conversation in technical and professional communication (TPC) scholarship that overlap and connect with contemporary issues in generative artificial intelligence (AI): process and iteration, theory and power, actors and activity, and the social justice turn. The authors offer four nonexhaustive reviews of these conversations, offering insight into key issues and texts that have animated discourse in the field and can directly or indirectly address the complex relationship between TPC work and generative AI.

Sentiment Analysis of Google Maps User Reviews on the Play Store Using Support Vector Machine and Latent Dirichlet Allocation Topic Modeling

These days, traveling is made easier by utilizing easily accessible online directions such as Google Maps. Google Maps provides real-time routes by displaying and presenting the closest routes that users can take. However, lately, the routes provided by Google Maps services often get users lost by presenting routes such as forests, narrow roads, and even dead ends. Therefore, this study aims to determine the level of user satisfaction and sentiment into two categories, namely positive and negative, based on reviews on the Google Play Store platform using the Support Vector Machine (SVM) algorithm and topic modeling using Latent Dirichlet Allocation (LDA) to find out the collection of topics that are the main topics of conversation by users regarding Google Maps services. The results of this study show that the SVM algorithm is feasible to use in sentiment analysis classification with an accuracy value of 86%, precision of 93%, recall of 53%, and f1-score of 52%. In addition, topic modeling is applied to generate coherence values for each topic, which shows that the higher the coherence value, the more specific the topic is. The highest coherence value generated in this study was two topic models with a coherence value of 35.15%, but this study took five with a coherence value of 33.39%. The five topic models to be applied in this study are selected because they have a good enough coherence value to identify the main topics and hidden topics in Google Maps user reviews with the Latent Dirichlet Allocation model. The topic model shows five aspects users often discuss: Google Maps route accuracy, system and service errors, navigation application directions, lost time history, and convoluted route provision.

9New Materialism

Abstract This review essay focuses on how the year’s work in new materialism builds on a vibrant discourse in literary studies and nascent, growing trends in postcolonial and indigenous new materialisms. A variety of new materialist works published in 2023, from the monographs Earthly Things: Immanence, New Materialisms, and Planetary Thinking and The Garden Politic: Global Plants and Botanical Nationalism in Nineteenth-Century America, to the journal article ‘New Materialism and Posthumanism in Roman Archaeology’, explore the ways in which the pillars of new materialist thought, such as the agentic capacity of vibrant matter, ‘speaking objects’, and entanglement, can lead to decolonial revelations about human culture and history. The essay is grouped according to common themes addressed in this year’s body of work: 1. Introduction; 2. Literature and Beyond; 3. Colonial Plants and Enslaved Objects; 4. New Materialism and Eco-Marxism; 5. Immanence and Planetary Thinking; 6. New (Materialist) Topics of Conversation in Science; and 7. Conclusion. The varied scholarship surveyed points to the conclusion that what was once seen as new materialism’s ethical weakness—its focus on matter over human beings—is now becoming a decolonial strength.

The first AI-based Chatbot to promote HIV self-management: A mixed methods usability study.

We developed MARVIN, an artificial intelligence (AI)-based chatbot that provides 24/7 expert-validated information on self-management-related topics for people with HIV. This study assessed (1) the feasibility of using MARVIN, (2) its usability and acceptability, and (3) four usability subconstructs (perceived ease of use, perceived usefulness, attitude towards use, and behavioural intention to use). In a mixed-methods study conducted at the McGill University Health Centre, enrolled participants were asked to have 20 conversations within 3 weeks with MARVIN on predetermined topics and to complete a usability questionnaire. Feasibility, usability, acceptability, and usability subconstructs were examined against predetermined success thresholds. Qualitatively, randomly selected participants were invited to semi-structured focus groups/interviews to discuss their experiences with MARVIN. Barriers and facilitators were identified according to the four usability subconstructs. From March 2021 to April 2022, 28 participants were surveyed after a 3-week testing period, and nine were interviewed. Study retention was 70% (28/40). Mean usability exceeded the threshold (69.9/68), whereas mean acceptability was very close to target (23.8/24). Ratings of attitude towards MARVIN's use were positive (+14%), with the remaining subconstructs exceeding the target (5/7). Facilitators included MARVIN's reliable and useful real-time information support, its easy accessibility, provision of convivial conversations, confidentiality, and perception as being emotionally safe. However, MARVIN's limited comprehension and the use of Facebook as an implementation platform were identified as barriers, along with the need for more conversation topics and new features (e.g., memorization). The study demonstrated MARVIN's global usability. Our findings show its potential for HIV self-management and provide direction for further development.

A review of walk-and-talk conversations with children as a research method

ABSTRACT This review of research publications in the period 1989–2021 examines both advantages and limitations of walking methodologies used as a research method, aiming to serve education researchers contemplating using the approach with children. Walking methodologies combine walking with verbal and non-verbal communication, such as body language or photography, and are particularly useful to study how children and young people relate to their environment, as well as for research on experiential and outdoor education. Advantages mentioned in the reviewed studies included improved communication, enhanced memory and reducing power imbalances. Challenges included group dynamics, possible exclusion based on disability or language, practical issues such as distractions, and power imbalances or limited relationship capital. Child-led walks, distractions, effects of the weather, and influence of place or the children’s interest on conversation topics reduced the researcher’s control over the research process, while at the same time offering greater agency for the children.

Perspectives of Nursing Home Staff in Advance Care Planning Conversations: Experiences from the APPROACHES Project

ObjectivesAdvance care planning (ACP) is considered a best practice in the nursing home setting; however, there is a lack of consistency in the training of nursing home staff and implementation of structured ACP programs. A qualitative study interviewing ACP specialists in nursing homes was conducted to understand the experience of staff engaged in Aligning Patient Preferences – a Role offering Alzheimer's patients, Caregivers, and Healthcare providers Education and Support (APPROACHES), an embedded pragmatic clinical trial to improve ACP. DesignQualitative interviews regarding ACP specialists’ experiences and perceived intervention impact. Setting and ParticipantsStaff of intervention-assigned nursing home facilities who completed a minimum of 10 ACP conversations with residents during APPROACHES program implementation. MethodsFourteen staff were interviewed. Interviews were transcribed and coded by the research team. ResultsThere were 21 codes identified that were then distilled into the following 5 themes: (1) experiences with the ACP specialist program, (2) engaging in ACP conversations, (3) considerations related to dementia, (4) benefits and challenges of ACP, and (5) involvement of multiple people in the ACP process. Participant responses suggested variability in experiences with the ACP specialist program and highlighted many aspects relevant to engaging in conversations with families and residents, particularly those living with dementia. Benefits of ACP, including relationship building and increased preparedness for changes in health status, were balanced with challenges related to sensitive conversation topics and logistical difficulties in scheduling ACP discussions. ACP specialists discussed the multiple roles that others played in the ACP process. Conclusions and ImplicationsFindings from this analysis provide insights into tailoring APPROACHES and other ACP programs for full-scale implementation in the nursing home setting. Nursing home staff experiences tailoring the program to fit their environments were reflective of the pragmatic nature of the ACP specialist program.

Embracing Endings: Unveiling the Connection Between Final Conversations, the Opportunity Model for Presence, and Coping With the Death of Our Parents.

This study examines adult children's loss of a parent through final conversations and communal coping. By looking at survivors' experiences through the lens of the opportunity model for presence (OMP) during the end-of-life (EOL) process, a model that provides a path of engagement leading to a good or bad death and consequent bereavement processes, we quantitatively test the relationships between two communication constructs before and after death of a parent. While final conversations as one construct did not lead to appraisal or action coping, results illustrate that final conversation topics of identity, instrumental, love, and everyday talk, were significantly related to shared appraisal. Instrumental talk and love were significantly related to joint action. Joint action and shared appraisal together led to participants engaging in all three types of coping. Final conversations as a whole, was important for bereavement outcomes, whereas communal coping's role in these relationships was convoluted.

Endometriosis Online Communities: How Machine Learning Can Help Physicians Understand What Patients Are Discussing Online

Study ObjectiveUse machine learning to characterize the content of endometriosis online community posts and comments. DesignRetrospective Descriptive Study. SettingEndometriosis online health communities (OHCs) on the platform Reddit. ParticipantsUsers of the endometriosis OHCs r/Endo and r/endometriosis. InterventionsMachine learning was used to analyze thousands of posts made to endometriosis OHCs. Content of posts and comments was interpreted using topic modeling, persona identification, and intent labeling. Measurements included baseline characteristics of users, posts, and comments to the OHCs. Machine-learning techniques; topic modeling, intent labeling, and persona identification were used to identify the most common topics of conversation, the intents behind the posts, and the subjects of people discussed in posts. System performance was assessed via accuracy at F1-score. ResultsA total of 34 715 posts and 353 162 comments responding to posts were evaluated. The topics most likely to be a subject of a post were menstruation (8%), sharing symptoms (8%), medical appointments (8%), medical story (9%), and empathy (7%). The majority of posts were written with the intent of seeking information about endometriosis (49%) or seeking the experiences of others with endometriosis (29%). Users expressed a strong preference for surgeons performing excision rather than ablation of endometriosis. ConclusionEndometriosis OHCs are mostly used to learn about symptoms of endometriosis and share one's medical experiences. Posts and comments from users highlight the need for more empathy in the clinical care of endometriosis and easier access for patients to high-quality information about endometriosis.

2254 Improving the quality and breadth of advance care planning discussions on gerontology wards in an acute London trust

Abstract Introduction Advance care planning (ACP) offers people the opportunity to plan their future care whilst they have capacity to do so and is supported by national guidance. Decisions regarding future care are more likely to be individualised and holistic when patients and their significant others are involved. This QI project aimed to address this by increasing the frequency of ACP discussions being offered and recorded on gerontology wards in an acute London Trust. Method A multi-professional steering group was established to improve ACP using PDSA methodology. A new ACP toolkit, training programme and electronic flowsheet (within the hospital’s patient record system) were implemented. ACP documentation quality was audited on gerontology wards pre and post implementation (over one to four months respectively). Data was compared using Pearson’s Chi-squared test. Results ACP flowsheets were completed by junior and senior doctors, and clinical nurse specialists in frailty and palliative medicine. The initial audit found disparity between documented topics of ACP conversations, with cardiopulmonary resuscitation recommendations being most discussed. Post implementation, 24 ACP flowsheets were reviewed, showing that more ACP topics were documented where these conversations were had; preferred place of death increased from 24% to 60% (p 0.011); treatment escalation plan increased from 41% to 75% (p 0.014); preferred place of care increased from 59% to 71% (p 0.066). Topics not showing significant improvement in documentation (despite inclusion in the flowsheet) were spiritual needs, information needs and prognostic discussion, broader social needs and what was most important to the patient. Conclusion The implementation of an electronic ACP flowsheet improved documented ACP conversations in some topics, guiding healthcare professionals to deliver care that aligns with peoples’ wishes and preferences. Documented conversations became easier to access, review and audit. Work is still needed to promote ACP conversations being centralised around what matters most to patients.

Boredom and English language conversational topics: an idiodynamic approach

ABSTRACT Purpose: The study aims to determine the effect of various conversational topics on the dynamics of boredom within and between individuals in second language (L2) learning. It explores the moment-to-moment changes in boredom levels using an idiodynamic method. Design/Methodology/Approach: The study involved four intermediate-level university students engaging in conversations on four different topics. Participants self-rated their boredom levels after each conversation and participated in follow-up interviews to explain changes in their boredom. The idiodynamic method was applied to capture the dynamic changes in boredom. Findings: Results indicated that boredom levels fluctuate both within and between individuals based on conversational topics. Factors contributing to these fluctuations included interest in the topic, familiarity with the subject matter, and personal experiences. The study provided insights into how different conversational topics could influence learners' boredom and engagement levels dynamically. Originality/Value: This research contributes to the understanding of the dynamic nature of boredom in L2 learning by employing an innovative idiodynamic approach. It highlights the importance of topic selection in language teaching and offers practical recommendations for educators to enhance student engagement and reduce boredom in L2 classrooms.
