The annual sessions of the New Orleans Academy of Ophthalmology, which gather outstanding experts in a three- or four-day symposium on a major area of ophthalmology, have resulted over the years in a dozen or more bound volumes. The present book is the tenth successive annual issue still available in print. Participants in the 1973 symposium on contact lenses gave 18 formal presentations in addition to taking part in five panel discussions (accessory contact lens solutions, hard contact lens-fitting techniques, complications of contact lenses, multifocal and aphakic contact lenses and therapeutic use of contact lenses), and a series of round-table discussions. Local moderators included Oliver H. Dabezies, who contributed a formal presentation on storage and asepsis of contact lenses, and four others. This is the first of five articles on the function, characteristics and performance of solutions and systems used in contact lens care and maintenance. This article contains a discussion of the relationship between contact lenses and corneal complications, microbiological contamination of lenses and solutions and a review of some of the procedures that have been used to test the bactericidal performance of contact lens solutions.

Allergic Contact Sensitivity to Benzalkonium Chloride

Benzalkonium chloride (BAK) is a widely used quaternary ammonium cationic detergent. It has occasionally been reported as a cause of allergic dermatitis. The agent is the most commonly used preservative in ophthalmic medications, solutions for contact lenses, and "artificial tears." A physician developed a severe allergic conjunctivitis from an ophthalmic solution containing BAK. The conjunctivitis became worse with the use of another preparation containing this preservative. The patch test reaction to BAK was strongly positive. The only other instance of allergic conjunctivitis from BAK, confirmed with a positive patch test, was published in 1952. French investigators report that allergic hypersensitivity to BAK may produce systemic reactions from the administration of chemically related antihypertensive and antispasmodic compounds.

Methods for detecting the presence of polyvinyl alcohol (PVA) in contact lens solutions.

Eighty per cent of 72 women who are taking oral contraceptives have symptoms of asthenopia. About half of this number have abnormal accommodation or divergence which may be assumed to be associated with the asthenopia. The paper reports only clinical impressions, but the need for research in this area is clearly demonstrated.

Two simple methods by which an optometrist can test a contact lens solution for the presence of Polyvinyl Alcohol (PVA) are described. Eight commercially available solutions were tested by both methods; PVA was present in four of these and not present in the other four.
