Treatment of bruising after injection to reduce cellulite.

Cellulite 80%-98% of post-pubertal women and results in cutaneous dimples in the affected area. Therapies that target collagenous fibrous septae often provide successful outcomes. Collagenase clostridium histolyticum-aaes (CCH) for injection consists of two collagenases which, under physiologic conditions, disrupt collagen structures by hydrolyzing Type I and III collagen. Subcutaneous CCH was approved in the United States in 2020 for the treatment of moderate-to-severe cellulite in the buttocks of adult women. Although bruising is primarily an aesthetic concern, a conspicuous procedure-induced bruise may limit a patient's social activities and result in hesitation to undergo a similar cosmetic procedure in the future. The purpose of this review is to describe the available techniques for hastening bruise resolution and how they apply to resolving bruises associated with a CCH injectable.

Relationship between aesthetic concern and self-esteem in adolescents with severe malocclusion

Objective This study aimed to evaluate the relationship between aesthetic concern and self-esteem in adolescents with severe malocclusion. Methods A cross-sectional study developed with 543 adolescents from 11 to 13 years of age and severe malocclusion was selected. Dental aesthetic index (DAI) diagnosed the malocclusion and selected the groups with greater severity (DAI 3 and 4). Aesthetic concern was assessed using the oral aesthetic subjective impact scale (OASIS) and considered an outcome variable. The global negative self-evaluation (GSE) evaluated self-esteem. The variables were analysed using Poisson multiple regression and the model adjustment determined by the Akaike information criterion (AICC). The effect of each variable on OASIS was expressed as a prevalence ratio (PR) with 95% confidence intervals. The analyses were performed using the R program with a significance level of 5%. Results A significant association was observed between aesthetic concern scores and self-esteem (p<.05). Individuals with low self-esteem had OASIS scores 14% higher (PR: 1.14; 95% CI: 1.08–1.21). There was no significant association between OASIS and gender and age (p>.05). Conclusions Self-esteem modulates how the individual perceives malocclusion. Adolescents with severe and very malocclusion and low self-esteem have greater aesthetic concerns.

MINIMALLY INVASIVE TECHNIQUE FOR ANTERIOR TEETH REPLACEMENT- CASE SERIES

Patients are very much concerned nowadays about the replacement of their missing teeth. Missing anterior teeth are a serious concern for patients, be it an aesthetic concern or functional loss. Aesthetics is the harmony between the teeth and the entire stomatognathic system. Aesthetic is the art that the dentist must procure, with a subtle touch of artistry, appreciation, and scientific knowledge, touch to recreate the beauty of a smile that suits the face and the personality of an individual. Beauty is in the eye of the beholder hence beauty is perspective. Science and dental treatment have progressed over the years, bringing us various options from removable partial dentures to fixed partial dentures and implants. Treatment options are delineated based on the patients concern as well as anatomic, functional, and scientific norms. Every treatment being fixed or removable has its own merits and demerits. An appropriate treatment plan should look into not only the aesthetic needs of the patient but also try to restore and maintain the harmony of the entire orofacial complex. Anterior teeth replacement can be done using a short-span cantilever bridge, as it is conservative and has a significant survival rate. Conventional cantilever fixed partial dentures have a survival rate of 82% over 10 years [1]. Maryland bridge is amongst other options which necessitate minimal tooth reduction with optimum bonding using adhesive cement.

The Efficacy and Safety of a 755-nm Picosecond Laser in the Treatment of Physiologic Lip Hyperpigmentation in Thai Patients.

Physiologic lip hyperpigmentation is a common aesthetic concern, especially in Southeast Asia. There is limited data on the application of the picosecond laser for this condition. To evaluate the efficacy and safety of using a 755-nm picosecond laser in the treatment of physiologic lip hyperpigmentation. Twenty healthy patients with physiologic lip hyperpigmentation received 5 bi-weekly treatments with a 755-nm picosecond laser using a 6-mm spot size, fluence of 0.71 J/cm 2 at 5 Hz. Subjective and objective evaluation on the improvement of lip hyperpigmentation were obtained at baseline, 2 weeks after each treatment and at 1, 3, and 6 months after the final treatment. Patient self-assessment, pain score, and adverse reactions were also recorded. All patients completed the study and attended all follow-ups. Most (52.6%) patients presented with moderate clinical improvement at 6-month follow-up. The average melanin index decreased significantly after the fourth treatment ( p = .048) and at 1-month follow-up ( p = .026). More than half the patients (70%) reported moderate-to-marked improvement at 6-month follow-up. Only 1 patient presented with a transient adverse reaction of lip edema. The 755-nm picosecond laser is safe and effective for the treatment of physiologic lip hyperpigmentation in Thai patients.

Cross-Cultural Adaptation and Validation of the Portuguese Version of the Psychosocial Impact of Dental Aesthetics Questionnaire.

The aim of this study was to cross-culturally adapt and validate the psychosocial impact of dental esthetics questionnaire (PIDAQ) to the Portuguese language. The PIDAQ was culturally translated and adapted according to international guidelines. In this cross-sectional study, we enrolled 501 subjects in a population-based epidemiological survey conducted at the Egas Moniz Dental Clinic (Almada, Portugal) in June 2022. The participants answered the Portuguese version of the PIDAQ (PIDAQ-PT), which was a 23-item scale with four conceptual domains (self-confidence, social impact, psychological impact and esthetic concern factor). Psychometric properties were estimated using content validity, construct validity, internal consistency and test–retest reliability. The PIDAQ-PT presented an intraclass correlation coefficient (ICC) of 0.84, and a 95% confidence interval (0.73–0.90, p < 0.001), with values for the Cronbach’s alpha coefficient of the subconstructs ranging from 0.93 to 0.98. In the confirmatory factor analysis (CFA), the final models presented a good fit, with the comparative fit indices (CFIs) ranging from 0.905 to 0.921 and the root mean squared error of approximation (RMSEA) ranging between 0.088 and 0.090. The PIDAQ-PT was shown to be a valid and reliable tool to assess oral health values in a Portuguese population. Further studies should evaluate the psychometric properties of the oral personal representation on dental specialties and its impact on dental appointments and procedures.

Preservation of historical document or aesthetic value? Mualla Eyüboğlu’s Topkapı Palace Harem restorations in the 1960s

ABSTRACT This article focuses on the Chamber of Crown Princes (Veliaht Dairesi) located in the residential part (Harem) of the Topkapı Palace, the Ottoman imperial seat in Istanbul (Turkey), where the removal of later additions was discussed during its restoration in the 1960s. Based on the archival documents of Mualla Eyüboğlu, the architect responsible for the restoration of the Topkapı Palace between 1961 and 1969, and the resolutions of the High Board of Antiquities and Monuments, this study reveals a stimulating debate on the balancing of the preservation of historic document value versus aesthetic value. Ambitious to present the building in its original function and use of the space as a part of the imperial residence, Eyüboğlu decided to liberate the original building of all the later additions and tried to reinstate the building to its initial seventeenth-century state. The removal of historical layers was disapproved by some of her contemporaries, while others appreciated her aesthetic concern. The case is reviewed at a broader scale, opening up to and integrating similar discussions in European preservation theory in the 1960s.

The efficacy of polycaprolactone filler injection on enlarged facial pores

Facial pores, visible topographic change of skin representing enlarged openings of pilosebaceous follicles, are common aesthetic concern. The aim of this study was to examine the efficacy of polycaprolactone (PCL) filler on enlarged facial pores and skin texture improvement. A total of seven participants with enlarged facial pores were included in the study and underwent deep dermal injection of PCL based filler (Ellanse M, Sinclair Pharma; London, United Kingdom). Skin quality measurements including skin evenness, red areas, UV spots, wrinkles and pore numbers evaluated with an automated aesthetic camera (FotoFinder®, Teach Screen Systems software GmbH; Bad Birnbach, Germany) before and 3 months after the injection session. Adverse events during procedure were monitored and recorded. Subjects were asked to report any injection related side effects. The average size of enlarged pores was 33.51 ± 41 before treatment and 20.51 ± 35 after treatment. The difference was statistically significant (P< 0.05). The skin evenness, red areas, UV spots, and wrinkles showed no significant improvement after filler injection. No serious adverse events were reported at follow-up visit. The PCL based filler seems to be notably effective in improvement of enlarged facial pores especially in moderate to severe cases. However, further structured studies with larger sample size and control group could be more beneficial to elucidate the possible efficacy of PCL filler injection on the skin quality characteristics.

Parental perception of malocclusion, its severity and aesthetic concerns in children with mixed dentition: A cross-sectional study

This study aimed to associate the parents' perception of the need for treatment and the children's aesthetic concern, considering the influence of malocclusion severity levels. A population-based cross-sectional study was conducted with 770 children aged 8 to 10 years in the mixed dentition stage. A closed question sent to parents assessed the subjective orthodontic treatment need (outcome). Orthodontic Aesthetic Subjective Impact Score (OASIS) evaluated children's aesthetic concerns, and the Dental Aesthetic Index (DAI) diagnosed the anterior characteristics and malocclusion severity levels. Individual analyses were performed relating the outcome as independent variables. The variables with P<0.20 in the individual analyses were tested in multiple logistic regression models, and those with P<0.10 remained in the model. The adjusted odds ratio was estimated with a 95% confidence interval. Parents of children with higher aesthetic concerns are 1.82 times more likely to believe that their child needs orthodontic treatment (P<0.05). Parents of children with very severe malocclusion are 1.91 times more likely to perceive their child's need for orthodontic treatment (P<0.05). Parents of children with greater aesthetic concerns are more likely to perceive the need for orthodontic treatment, and very severe malocclusion also influences parents' perception of the need for orthodontic treatment.

Effect of personality traits on the association between clinically assessed malocclusion and the psychosocial impact of dental aesthetics.

We aimed to assess whether personality traits affect the relationship between malocclusion and the psychosocial impact of dental aesthetics and whether they act as its mediators or moderators. The cross-sectional study included 252 participants with permanent dentition (62% female) aged 12-39 years. The Big Five Inventory, the Psychosocial Impact of Dental Aesthetics Questionnaire and the Index of Complexity, Outcome and Need were used. Moderation and mediation were tested by hierarchical stepwise multiple linear regression. Malocclusion was the most significant predictor of dental self-confidence (DSC), aesthetic concern (AC), social impact (SI) and psychological impact (PI), accounting for 12%-28% of respective variability (P< .05). Adult age, male sex and higher neuroticism and agreeableness were additional predictors of higher DSC (P < .05). Adolescent age was a predictor of higher AC, SI and PI, and female sex of higher AC and PI (P < .05). Mediation by personality traits was not proven, however, openness, agreeableness and neuroticism were moderators. A tendency of decrease in AC was associated with an increase in malocclusion complexity in adolescents lower in openness and decrease in SI in adolescents with lower agreeableness. There was also a tendency of higher increase of AC associated with an increase of malocclusion degree in adults higher in neuroticism. In adolescents, openness and agreeableness moderate the relationship between malocclusion and AC and SI, respectively. Neuroticism is a moderator of the association between malocclusion and AC in adults.

BOTULINUM TOXIN FOR CORRECTION OF GINGIVAL SMILE

The gingival smile is an aesthetic concern that affects a large portion of the population, an aesthetically acceptable smile must be harmonious as to the set of teeth, lips and gingival tissue. When the etiological factor is muscle hyperfunction, a treatment option is the application of botulinum toxin type A in the muscles responsible for lifting the upper lip, so the objective of this case report is to demonstrate the use of botulinum toxin to correct gummy smile. . Female patient, age 32, Phototype II, sought care at the Specialization Clinic in Orofacial Harmonization of the IOA-IOP, Campina Grande-PB, due to excessive gingival exposure when smiling. After clinical evaluation, it was observed that the patient had hyperactivity of the levator labii superioris muscles and a gingival exposure of 4 mm. It was proposed to the patient the application of botulinum toxin for the treatment of gummy smile and then the Free and Informed Consent Term was signed specifying all possible complications that could happen, as well as the benefits and results. After the photographs and demarcations, 2 units were applied to the levator labii superioris muscle bilaterally and 2 units to the nasal septum. After 15 days of application, the patient was reassessed, demonstrating an excellent result without excessive exposure of the gingiva and there was no need for a new application. It was concluded that TBA brought a very satisfactory result for the correction of the gummy smile and constitutes a safe and effective treatment.

Comparative study of the effect of BlueM active oxygen gel and coe-pack dressing on postoperative surgical depigmentation healing

IntroductionBlack colored gingiva is an esthetic concern, especially when accompanied by a high lip line or gummy smile.Surgical depigmentation with a scalpel is still considered the golden standard in gingival pigmentation management although it causes an area of open wound, which needs a special management with dressing.This study aimed to comparatively evaluate the effectiveness of reactive oxygen gel species (BlueM gel) and the traditional Coe-Pack dressing on gingival healing and pain after surgical depigmentation. Materials and MethodsThis split-mouth randomized clinical trial was conducted on 20 nonsmoking individuals aged 20–38 years with maxillary physiologic gingival pigmentation classes (III) and (IV) according to the Dummett–Gupta Oral Pigmentation Index (40 treated sites) who had requested an esthetic treatment for gingival hyperpigmentation of the maxillary gingiva.The maxilla was randomly divided into two symmetrical parts—from the right second premolar to the midline and from the midline to the left second premolar—to receive either BlueM gel or Coe-Pack as a dressing after surgical depigmentation with a scalpel. Various indices were assessed, such as pain and reepithelization index with toluidine blue, and the follow-up period was 1 month. ResultsA total of 20 patients were included in this study. There were statistically significant differences in the pain index after 1, 2, 3, 4, and 5 days. The BlueM gel group showed a higher significant difference after 1, 2, and 3 weeks in the reepithelization index. ConclusionHence, BlueM gel can be considered as a good alternative for the Coe-Pack dressing after gingival depigmentation owing to its pain reduction properties, acceleration of wound healing, and postoperative reepithelization.

Age and Gender Are Associated with the Component of Psychosocial Impact of Dental Aesthetics Questionnaire in Young People: A Cross-Sectional Study.

(1) The aim of the study was to investigate the association between age, gender, and the component of psychosocial impact of dental aesthetics (PIDAQ) in Malaysian young people. (2) Cross-sectional data on the PIDAQ (comprised of dental self-confidence, social impact, psychological impact, and aesthetic concern variables) of Malaysian youth (n = 1425) recruited through multi-stage sampling were analyzed for mediation and moderated mediation analyses using the PROCESS macro on SPSS software. (3) Participants (mean age 16.0 ± 2.8) represented 54.8% of girls and 45.2% of boys. In the mediation model, psychological impact and aesthetic concern completely mediated the effects of social impact on dental self-confidence. In the moderated mediation model, social impact directly influenced dental self-confidence amongst participants at one standard deviation below the sample mean age and among boys. However, psychological impact completely mediated the influence of social impact on dental self-confidence amongst participants at the sample mean age and at one standard deviation above the sample mean ages, and among girls. Neither age nor gender moderated the effect of aesthetic concern on dental self-confidence. (4) Age and gender moderated the influence of social impact and psychological impact on dental self-confidence.

Comparative Evaluation of Depigmentation Technique with Diode Laser and Electrosurgery – A Case Report

Excessive gingival pigmentation is a major esthetic concern for many people. The gingiva is an important intraoral tissue which when affected particularly by pigmentation is mainly responsible for the unpleasant appearance. A harmonious smile accounts for a perfect balance between the pink and white component of oral cavity. It mainly affects the confidence, self-esteem, and esthetics of an individual, which can also have a psychological impact on them. Melanin pigmentation is known to be caused by melanin granules within the gingival epithelium. In this case report, a split-mouth depigmentation was done using electrosurgery and diode laser with melanin hyperpigmentation in the anterior maxilla and mandible which provides increased satisfaction in patients.

LIP REPOSITIONING SURGERY ALONG WITH ESTHETIC CROWN LENGTHENING FOR THE TREATMENT OF GUMMY SMILE:A CASE REPORT

Excessive gingival display is an aesthetic concern that may be addressed with a variety of methods. Lip repositioning is a treatment option for people who are concerned about their appearance. Excessive gingival display, commonly known as gummy smile, is characterised by a gingival display of more over 3mm when smiling. A patient with a gummy smile was treated with lip repositioning surgery in the current case report. Our goal was to present lip repositioning as a viable treatment technique for reducing gingival display and asymmetry with a simple and conservative surgical procedure, strong long-term durability, and little recurrence.

Impact of Single Eyelid on Superior Visual Field.

A puffy eyelid without a crease, also known as single eyelid, is a common characteristic in East Asians. Existence of a lid crease is generally considered an aesthetic concern for both patients and surgeons, and postoperative evaluations have mainly focused on the appearance of the eye. The aim of this study was to clarify the functionality of the superior visual field (SVF) with single eyelid.This prospective cohort study compared edge of the upper eyelid to central corneal light reflex distance (edge reflex distance [ERD]) preoperatively and postoperatively and examined the SVF, as measured by Goldmann perimetry, in single-eyelid patients who underwent blepharoplasty.Twenty patients (40 eyelids) with a median age of 21 years were examined. The median preoperative and postoperative ERDs were 1.45 and 3.4 mm, respectively. The median areas of the SVF preoperatively and postoperatively were 34,443 and 50,796 degrees2, respectively. Although a positive correlation existed between preoperative ERD and SVF, no correlation was observed between postoperative ERD and SVF.The SVF of the single eyelid is narrower than that of the double eyelid, even under circumstances of equivalent palpebral fissure height. The single eyelid commonly found in East Asian populations is associated with both cosmetic and functional issues.

PERSPECTIVE OF DENTAL PROFESSIONALS TOWARDS USE OF CAST PARTIAL DENTURE IN NEPAL

Background: With increase in treatment options for rehabilitation of partially edentulous patients, the use of cast partial denture in clinical practice as well as teaching curriculum has been decreased. The objective of this study was to evaluate the perspective of dental professionals towards use of cast partial denture in Nepal. Methods: The study consisted of nine close-ended questions regarding the use of cast partial denture by dental professionals in clinical practice throughout Nepal. The weblink of the questionnaire were shared via E-mail to dentists practicing in Nepal, to obtain a response of 350. Statistical analysis was done using SPSS version 26. Results: The study showed that the most preferred choice of rehabilitation was fixed partial denture by all dental professionals around 52.6% of , 60% of prosthodontists and 48.9% by other specialist dentists.The least preferred choice was Removable partial denture about 16.6% by general dental practitioners, 18.5% by prosthodontists and 13.3% by other specialist dentists.Among the different types of removable partial dentures, acrylic partial denture was the most preferred one and the primary reason for not recommending cast partial dentures by the dental professionals was too complicated procedure to be carried out. Nevertheless, majority of the dentists were in favor of giving more importance for teaching cast partial dentures in graduation curriculum. Conclusions: The study concluded that the use of cast partial denture is declining in clinical practice in Nepal due to the complicated procedure of fabrication and esthetic concern. Therefore, there is a need to emphasize the significance of teaching of cast partial dentures in graduation curriculum.

Minimally invasive modified tunnel approach for the treatment of gingival recession: A case report

Gingival recession is a common incidence and its occurrence will increase with age. It can lead to clinical problems, reduced cosmetic appeal and therefore esthetic concern. There are numerous strategies for root coverage. The preference for improved esthetics has elevated distinctly over the years. The idea of minimal invasive approach has received significance within the latest years and is changing the open surgical approaches. The treatment of gingival recession has continually been technique sensitive along with the related patient morbidity due to the invasive nature. This article presents a report of a case treated with a minimally invasive modified tunnel approach.
