Clinical Experience with Quantitative Analysis of Superficial Digital Flexor Tendon Injuries in Thoroughbred and Standardbred Racehorses

A method to quantitate as well as to document SDFT pathology has been described. This report indicates that computer-assisted evaluation of SDFT damage can improve the accuracy of prognosis in clinical practice. It would appear that a 3.8 severity rating is a limit past which prognosis for successful racing starts to worsen rapidly.

A Concurrent Validity Study of the APTCOM's General Intelligence Scale: A Pilot Investigation

The general intelligence scale of the APT/COM computer-assisted vocational evaluation system was investigated for concurrent validity, using the Slosson Intelligence Test as the criterion. Fifteen university students with disabilities served as subjects in this pilot study. It was found that the APTICOM's intelligence scale is significantly correlated to the Slosson Intelligence Test. Implications for rehabilitation research and practice are discussed.

Thioctic Acid Induces Dose-Dependent Sprouting of Neurites in Cultured Rat Neuroblastoma Cells

Quantitative computer-assisted evaluation of thioctic acid-induced sprouting in Neuro-2a cells reveals a clear dose dependence with respect to several parameters of sprouting. At first predominantly the length of main sprouts per cell (neurites from cell soma to first branching) and the number of branchings per cell are increased. With the higher dosages, the length of branches per cell rises considerably, too. In contrast, the degree of branching (mean length of neurites per branching) seems to be independent of the concentration of thioctic acid. The capacity of thioctic acid to induce sprouting is discussed in terms of the involvement of SH groups and with regard to regeneration phenomena as they may occur in diabetogenic neuropathy.

Computer-assisted evaluation of respiratory data in ventilated critically ill patients.

In intensive care unit, a lot of data are currently available but remain unused by nurses and residents because of complexity of analysis. We have developed a system for interpretation of respiratory data (RESPAID) in order to improve monitoring of patients under respiratory support and also to provide a high level of information. RESPAID is a real-time system which interprets quantitative and qualitative aspects of the usual respiratory data at different levels of information. Initial knowledge base was built from data given by four specialists in intensive care. Major attention was paid to different aspects of the system: monitor interface, user interface and time representation. Data are issued from standard respirators and/or monitors used in the intensive care unit. Informations provided by RESPAID are alarm identification, ventilator settings modification and proposal for physiological evolution of the patient or suspected complication. RESPAID runs on IBM PCAT3 with 1st class shell. It is currently in clinical validation procedure.

Book reviews

The 5th issue of Frontiers in European Radiology contains four chapters. The first (by J. Buck) is on “Angiocinedensitometry of renal blood circulation by electrocardiogram-controlled injection and computer assisted evaluation”. This is an experimental technique that very few radiologists will be familiar with. Angiocinedensitometry has so far not been applied widely to study circulation in various organs in a clinical setting and is not likely to in the future.

Local muscular strain dependent on the direction of horizontal arm movements.

Manual material handling still plays an important role in assembly work as well as in the service industry. By means of an ergonomic layout of working places, the effects of disadvantageous working conditions, especially static load resulting from unfavourable postures of the body as well as unnecessary physiological cost arising from body movements may be avoided. In this context knowledge about the optimum directions of horizontal arm movements is fundamental. Therefore by means of advanced methods of multi-channel electromyography (EMG) and applying computer assisted evaluation methods, physiological responses to one-handed horizontal material handling in a sitting position were recorded. In order to obtain myoelectric signals from the most important task-relevant muscles out of more than 40 muscle groups of the hand-arm-shoulder-system, preceding research was done also aiming at standardized lead positions for surface EMG. Groups of five young females each participated in a series of successive experiments, in which the direction of movements of the left arm was varied. The subjects worked without and with an external load imposed by weights of 0, 1, 2 and 4 kg. The movements between 20 degrees and 230 degrees--measured from the frontal plane of the subjects--included working from ahead and from behind. The lifting distance was 38 cm starting from different outward points within the reach to a fixed point near the body. Also in one test series the handling frequency was varied between 12, 24 and 48 lifts per minute. Assessment of the physiological cost by electromyographic activities (EA), especially the determination of the most stressful and least expensive directions of repetitive horizontal movements, was more effective when differentiating static and dynamic portions of muscular strain than common means of EA.

Electronic data processing of the patient record for computer-assisted evaluation of pregnancy and labor

This article describes an EDP-adequate data collection system which can also be used in the conventional way. The relational data base system used is PROFESSIONAL ORACLE, Revision 5.1.; the requisite hardware consists of 3 IBM-AT-compatible personal computers with a main storage comprising 4 MB, system MS/PC-DOS, and a hard disk of at least 20 MB. The standardized case history compiles the user to collect the data carefully and completely. Besides formulating research problems the system automatically prepares the discharge report. By means of statistical analysis a continuous internal quality assessment was realized.

Automated corneal topography: computerized analysis of photokeratoscope images.

The analysis of corneal topography has recently become popular, mainly as a consequence of the rapid development of refractive surgery techniques. To date, computer-assisted evaluation of photokeratoscope pictures has been the most popular method used to assess corneal topography. The processing system described previously by other authors requires hand-digitizing of the photokeratoscope pictures and therefore the constant presence of an operator for a considerable amount of time. We have introduced the use of a video camera to digitize the photokeratoscope images, making the processing automatic and quicker. Moreover, precision glass spheres have been used to calibrate the system in order to minimize the intrinsic errors even further. The data obtained from the computerized analysis are presented in three different ways. Besides having the numerical values in diopters, corneal maps with various shades of grey are used, together with the quantitative, three-dimensional representation of corneal astigmatism. An automated system for the analysis of corneal topography was tested for both experimental models and clinical conditions. It was easy to use and showed high precision (less than 1% error when processing photographs of precision glass spheres).

Structural studies on the fucosamine-containing O-specific polysaccharide of Proteus vulgaris O19.

The polysaccharide chain of Proteus vulgaris O19 lipopolysaccharide contains D-galactose, N-acetyl-D-glucosamine N-acetyl-D-galactosamine and N-acetyl-L-fucosamine in the ratio 1:1:1:1. The structure of the polysaccharide was established by full acid hydrolysis and methylation analysis, as well as by non-destructive methods, i.e. the computer-assisted evaluation of the 13C-NMR spectrum and computer-assisted evaluation of the specific optical rotation by Klyne's rule. The polysaccharide is regular and built up of tetrasaccharide repeating units of the following structure: ----3)-alpha-L-FucNAcp-(1----3)-beta-D-GlcNAcp-(1----3)-alph a-D-Galp- (1----4)-alpha-D-GalNAcp-(1---- The O19-antiserum cross-reacts with lipopolysaccharide from P. vulgaris O42, the structure of which is still unknown. No cross-reactions were observed with O-polysaccharides Pseudomonas aeruginosa O7 and Salmonella arizonae O59 in spite of some structural similarities.

Gravity and vertical magnetic gradient investigations of a localised area of the Benue Trough, Nigeria

Gravity and vertical magnetic gradient studies were carried out in Lokpanta-Lekwesi area in the Lower Benue Trough to investigate the local structures which might be associated with the Lead-Zinc mineralisation as evident in the adjacent area (Ishiagu Pb-Zn mineralisation). For this reason 134 gravity and vertical magnetic gradient readings were taken at intervals of 250 m along major roads and track routes that connect widely separated parts of the 25 km 2 area. The instruments used are the Canadian Scintrex CG-2 gravimeter (No. 318), the American Paulin altimeter and the Proton Precession Magnetometer. The geology of the areacomprises the Ezeaku Formation, and the Awgu Formation, their ages ranging from Middle to Upper Cretaceous. The study was used to delineate two igneous intrusions in the Ezeaku Formation. The intrusive bodies show high positive Bouguer anomalies which ranged from 0.5 mgal to 3.3 mgal in contrast to the negative Bouguer anomaly of the country rock that ranged from 0 to − 3.0 mgal. The vertical magnetic gradient method identified the two intrusive bodies as negative anomalies having vertical magnetic gradient values ranging from −5.0 to −25.0 gammas per meter in contrast to 5.0 to 10.0 gammas per meter values of the country rock. The Profiles were interpreted using computer assisted evaluation procedure of non-linear optimization and interactive techniques. Based on two dimensional model, the minimum depth computations to the upper surfaces of the anomalies A and B by gravity method are 60mm and 160 m respectively while the vertical magnetic gradient method gave a minimum depth estimates of the anomaly A as 75 m and that of B as 180 m.

Computer-assisted evaluation of polydisperse two-dimensional gel patterns of polysaccharide-protein conjugate preparations with regard to size and net charge.

Native Hemophilus influenzae polysaccharide-protein conjugate particles were analyzed by a two-dimensional agarose electrophoresis procedure. In view of their preparation by random chemical crosslinking, the conjugates necessarily exhibit a polydisperse two-dimensional gel pattern which varies depending on the conditions of the particular preparation. The polydisperse patterns were interpreted with regard to the size and surface net charge density of the conjugate on the basis of the extended Ogston model. Data processing was performed by a new program, designated ZWEIDI.DO, written in the language of M-LAB (modeling laboratory). The program computes particle and gel fiber specific parameters from the positions of standards and unknown(s) on the two-dimensional gel using a simultaneous linear least-square curve fitting routine. Based on these calculations, the program serves to compute a nomogram of iso-size and iso-free-mobility profiles. Superimposing these profiles on the gel patterns, the size and free mobility range of the polydisperse conjugate mixtures is obtained. Potentially, the procedure could serve as a tool for quality control in the production of conjugates as vaccines and for the physical characterization of polydisperse subcellular particles and vesicles.

Digital radiography of the gastrointestinal tract.

The gastrointestinal tract lends itself quite well to digital imaging. Since fluoroscopy is already employed, the images can easily be obtained in digital format and several manufacturers have now developed systems for commercial use. Because of the type of pathology as well as the inherent subject contrast, it would appear the resolution requirements for digital imaging of the gastrointestinal tract are less than in other organ systems. The necessary resolution level is already technically available and feasible without significant cost. Digital imaging also holds promise of at least a modest reduction in radiation dose to the patient, as well as a reduction in costs, although these factors are operator dependent. However, digital imaging provides a latitude in performing the examination that is not available with screen-film systems. Finally, there is promise that by acquiring gastrointestinal images in digital format, manipulation of the images may help increase diagnostic accuracy by improving both technical and perceptive components of diagnosis. Not only will simple image manipulation be helpful but there is even potential for computer-assisted evaluation of gastrointestinal images.

Division 40: Task force report on computer-assisted neuropsychological evaluation

Abstract Early in 1984, the Executive Committe of Divison 40 of the American Psychological Association established a task force on the use of computer technology in testing and remediation/retraining programs in neuropsychology. Composed of a nucleus of Division 40 members suggested by the Executive Committee, plus a larger group of Division 40 members who responded to an open invitation to participate in the development of these guidelines, the task force assembled, reviewed, and discussed a large number of pertinent documents and the broad range of opinion that characterizes this controversial subject. Fortunately, during the same time period in which the Division 40 Task Force was addressing this topic, a set of guidelines for computer-based tests and interpretations was under development by APA's Committee on Professional Standards and Committee on Psychological Tests and Measurements. Computer-assisted general medicine clerkship evaluation.

This report describes the development and analysis of a computer-assisted evaluation system designed to standardize and simplify student evaluations on a general medicine clerkship. Three standard clinical evaluation components were employed: written examination; oral examination; and clinical performance evaluation. Computer spread sheet technology was used to weight each component separately and calculate a final numerical grade for the clerkship. The system provides a consistent, well-documented and well-defined method for justifying individual grades of honours, pass or fail. It has been very helpful in identifying evaluation problems occurring in particular hospital sites or with particular evaluators.

Computer support in a rural setting: A comparison of computer-assisted and handwritten evaluation reports

The present blind study compared the quality of psychoeducational reports generated under two conditions. On the average, reports written with computer assistance were rated to be of higher quality than those prepared by hand. Also presented are data validating the large expenditure of staff time on report preparation, along with estimates of the time saving that may be realized with computer assistance. The relevance of computer support to relatively isolated, rural-based clinical staff is discussed.

The Giessen model of a cement-free implantable total endoprosthesis for the hip joint

Complete endoprostheses of the bitrochanteric type were implanted in Giessen in 111 patients the average age of whom was 74 years. Computer-assisted evaluation showed that more than 70% of these patients developed autonomous mobility. The rest had not been able to walk before surgery, but could be nursed or became able to sit after the operation or were still followed up. At the end of follow-up treatment, a rate of 90% of mobile patients with or without stick is to be expected. Initially, the implantation technique is certainly more difficult to learn with this model than with other allo-endoprostheses of the hip joint. This is due to the fact that the implants are an exact replica of the pertrochanteric region, thus permitting a broad contact face to the cortical substance instead of the punctiform contact of some other models. Once learned, the technique can be applied without considerable problems. At our Department it has proved to be successful.
