Analysis Of The Influence Of Clinical Pathway In Hospitals On Length Of Stay: A Scoping Review

Clinical pathway is a management tool used in healthcare to coordinate patient care efficiently, thereby reducing the length of stay required by patients in hospitals. The purpose of this study was to determine the effect of Clinical Pathway in hospitals on Length of Stay. This study was a scoping review using the PRISMA diagram. Article search was conducted based on the eligibility criteria of the PICO Model: P: Hospital Patients, I: Clinical Pathway, C: -, O: Length of Stay. The articles used were from 4 databases, namely: Google Scholar, Research Gate, PubMed, and Elsevier. With keywords including "Clinical pathway" AND "Length of Stay" AND "hospital". The implementation of clinical pathway is one of the strategies that can be used to reduce patient length of stay. Clinical pathway can help improve efficiency, quality, and compliance with healthcare standards. Based on the results of the study, the researchers recommend that effective socialization and training be conducted to improve the understanding and compliance of healthcare professionals with clinical pathway, develop clinical pathway based on strong scientific evidence, and conduct periodic monitoring and evaluation to ensure the effectiveness and suitability of the clinical pathway implemented.

Extent Of Compliance To Standard Precautions Among Healthcare Workers In Accredited Birthing Facilities In San Jose, Occidental Mindoro, Philippines

This study was conducted to determine the extent of compliance of health care professionals employed in birthing facilities in the municipality of San Jose, Occidental Mindoro. Specifically, it sought to determine the demographic data of the respondents as to age, educational attainment and years of experience; investigate their level of compliance to standard precautions and examine if there exists a significant relationship between the demographic variables of the respondents and their level of compliance to standard precautions. There were 23 health workers who participated in the study. The main instrument used in the study is the CSPS, a standardized checklist for measuring compliance to SP. Results showed that health workers in birthing facilities in the municipality are young, graduates of bachelor’s degree and are new in the service. They are also highly compliant with SP. It was also found that age and experience are significantly related with compliance to SP. The researchers recommend that this study be replicated using other health professionals as respondents such as doctors since they are the ones who are most exposed to health hazards. It is also suggested that further studies be conducted considering other variables which are not covered in this study.

Distress management in cancer patients：Guideline implementation based on CAN-IMPLEMENT

ObjectiveTo standardize the distress management of gastric cancer patients receiving chemotherapy, the adapted Cancer-related Distress Management Guidelines were implemented in nursing practice among gastric cancer patients receiving chemotherapy based on A Guideline Adaptation and Implementation Planning Resource (CAN-IMPLEMENT). MethodsBased on the theoretical framework of CAN-IMPLEMENT, A multidisciplinary team was established, barriers and facilitators obstacles of guidelines implementation in medical oncology units were assessed, corresponding solutions were formulated, the guidelines implementation process was monitored, and implementation results were evaluated. ResultsThe multidisciplinary team developed review criteria, standardized work paths, assessment tools, training manuals for healthcare professionals, education manuals for patients and their caregivers. After guidelines implementation, the completion rate of the distress management record came up to 97.9% (189/193). From September 2017 to December 2018, the compliance of medical staff on most items in the audit checklist was improved, ranging from 57.1% (100/175) to 100.0% (193/193). The positive distress rate of gastric cancer patients receiving chemotherapy was decreased from 22.7% (32/141) to 9.3% (18/193) (P < 0.05), and the Median (range) of the distress score declined from 2 (0–9) to 0 (0–7) (P < 0.001). ConclusionsThe implementation of guidelines based on CAN-IMPLEMENT promotes the establishment of a distress management system in the medical oncology units. The review standards, standardized work paths, and evaluation tools for distress in cancer patients formulated by the multidisciplinary team had clinical applicability and effectiveness. Quality control in the practice of distress management was effective. The compliance of healthcare professionals with distress management was improved. The distress of gastric cancer patients receiving chemotherapy was alleviated effectively.

Evaluation of compliance to the World Health Organization's five moments of hand hygiene: Cross-sectional observation of healthcare professionals.

Human hands are home to thousands of microorganisms, which may be transmitted to surfaces that the hands come into contact with. When in contact with people who are ailing or have weakened immune systems, some of these microorganisms can cause infections and disease. Correct hand hygiene goes a long way in eradicating these potentially infective microorganisms and forms the cornerstone of infection prevention and control (IPC) within healthcare facilities and beyond. The healthcare industry is constantly challenged by healthcare-associated infections (HAIs) and their negative effects on patient safety and clinical outcomes. Hospitals in Pretoria are facing similar challenges posed by HAIs and there is no report available on compliance of healthcare professionals (HCPs) to the World Health Organization's (WHO) 'five moments of hand hygiene'. Healthcare professional's compliance to all of the five moments of hand hygiene, particularly within the patient zone, is crucial in mitigating and reducing the spread of contact-based infections in the healthcare setting. A quantitative longitudinal design was used in a covert direct observation of HCP compliance to the WHO's five moments of hand hygiene. The observations were conducted over 4 weeks in three hospitals, covering 25 wards, inclusive of four adult critical care units using the WHO's 'five moments of hand hygiene' observation form. A total of 1906 hand hygiene opportunities were directly observed in three hospitals. Hand hygiene compliance was 17.26% (n = 329). Allied health professionals had higher compliance (23.02%) than medical (19.26%) and nursing professionals (15.76%). The moment before patient contact had the lowest compliance (8.21%) as compared with all other moments. In general, HCPs had low compliance to the five moments of hand hygiene within the patient zone. Allied health professionals had higher compliance than medical and nursing professionals. Compliance in public hospitals was lower than in private hospitals. Critical care units had higher compliance compared to general wards. Healthcare professionals better complied to the moments meant for their safety as compared to those indicated for patient safety.

Reducing central catheter-associated infections in pediatric surgery clinic: Implementation of a care bundle

Purpose: Central line care bundle comprises a few evidence-based interventions for improving patients' outcomes and recovery process. This semi-experimental study aimed to determine the effect of pediatric central line care bundle implementation on central line-associated bloodstream infections (CLABSI) rates. Materials and methods: A central line care bundle was implemented for pediatric surgery patients (n=70). Baseline observations were made to determine the central line care bundle compliance of healthcare professionals for 435 catheter days. Subsequently, physicians and nurses were educated about the central line care bundle. After the implementation period, 722 catheter days were observed to determine post-implementation compliance. Baseline CLABSI rates were compared with post-implementation CLABSI rates. Results: It was found that the entire central line care bundle compliance was 32.4% pre-implementation and 86.3% post-implementation. After education, the physicians' and nurses' central line care bundle compliance showed statistically significant improvement (p= 0.0001). There were five CLABSI events in the pre-implementation period and three CLABSI events in the post-implementation period. It was determined that the number of CLABSI decreased in the post-implementation period compared to the pre-implementation period, but this difference was not statistically significant (p= 0.207). Conclusions: Central line care bundle implementation decreased the CLABSI rates. It is recommended the Implementation of a central line care bundle on the care of pediatric surgery patients with the central venous catheter.

The impact of hand hygiene awareness programme on health care professionals' compliance with hand hygiene in a tertiary care hospital: A clinical audit

BackgroundHand hygiene is the most cost effective method to control the health care associated infections. Despite knowing the fact the compliance of health care professionals with hand hygiene is not up to the mark. MethodsAn observational, prospective study was designed to assess knowledge, and attitude of health care professionals towards hand hygiene practices. The impact of hand hygiene awareness programme (HHAP) on the compliance of 106 health care professionals working in paediatric medical and surgical wards of a tertiary care hospital was also evaluated. Clinical audit comprised of 200 observations each before and after the HHAP was done. ResultsOf total 106 health care professionals 73 (68.8%) were nurses, 33 (31.13%) doctors with mean age (years) of 32.14±7.4. Mean knowledge and attitude scores of HCP were 19.28±2.4and 39.26±3.9 respectively. Majority HCP had good knowledge (91/106, 85.8%) and favourable attitude (89/106, 83.9%) related to hand hygiene practices. Significant improvement in hand hygiene compliance was observed among the health care professionals following the hand hygiene awareness programme (p<0.001). ConclusionHand hygiene awareness programme should be continued on ongoing basis to improve the compliance of HCP with hand hygiene practices.

Health care professionals’ awareness of, knowledge about and attitude to influenza vaccination

ObjectivesInfluenza vaccination is recommended to all health care professionals (HCPs). However, vaccination rate among HCPs is low and may be due to uncertainty about the effectiveness of the vaccine and fear of its adverse effects. Therefore, this study aimed to investigate the awareness, knowledge, and attitude of HCPs towards influenza vaccination and we ascertain reasons for not getting vaccinated. MethodA cross-sectional conducted in 6 major hospitals in Saudi Arabia. 245 anonymous questionnaires were distributed to a convenient sample of staff during the 2012–2013 influenza season. The validated questionnaire consists of five sections that collect information about: demographics, attitude towards influenza vaccination, knowledge about influenza virus and vaccination, current practice and awareness of published guidelines. Results242 completed questionnaires were received, a response rate of 98%. 38% of HCPs reported getting vaccinated. The most common reasons given by HCPs for not getting vaccinated were: fear of contracting illness (16%), belief that they are not at risk from influenza because they are young and healthy (13%) and being unaware of vaccine availability (13%). Non-availability of vaccine (43%) was the highest barrier for not providing vaccine for patients and HCPs followed by safety concerns for the patients (35%) and the respondents (33%). Almost 75% of HCPs were not aware of the influenza immunization guidelines published by the Advisory Committee on Immunization Practices and Centre for Disease Control. ConclusionDespite the recommendations, only low percentage of HCPs in Saudi Arabian hospitals is vaccinated against influenza. The attention of health policy makers is needed to improve compliance of HCPs with guidelines on influenza vaccination.

Intervenções para elevar a adesão dos profissionais de saúde à higiene de mãos: revisão integrativa

The objective of this study was to identify the main strategies used to improve the compliance of health care professionals to hand washing. This is an integrative literature review, which search included journals in English, Spanish and Portuguese. Twenty—three articles were included. An electronic tool was developed on Microsoft Office Excel and the main results were submitted to descriptive analysis. Of the total studies, 87.1% had before and after designs and several methods were used to monitor compliance rate (direct observation, supply use and self-reported rates). Multimodal interventions were used in 87.0%, and the most often employed were: education, feedback, alcohol being available and posters. The largest challenge identified was not only improving the compliance rates to hand washing, but, most of all, keeping them high. It was observed there is a need to use multimodal strategies that contribute to behavior change considering the local setting. Compliance of Healthcare Professionals with Safety Measures for Control of Hepatitis Viruses in Hemodialysis Centers: An Experience from Southeast Iran

Introduction. Noncompliance with the recommended infection control measures by the healthcare professionals (HCPs) plays a major role in transmission of hepatitis B (HBV) and hepatitis C (HCV) viruses in hemodialysis (HD) wards. This study aimed to determine the compliance rate of the HCP with safety measures in the HD wards in southeast Iran. Patients and Methods. A total of 208 patients were enrolled. Adherence of HCPs with standard infection control measures was assessed. Results. Sixty-one HCPs with a mean age of 32.4 ± 11.2 years old were responsible for healthcare services. Compliance with the following items was weak: not sharing medications trolley (29.8%), disinfecting the shared instruments (46.2%), using single use materials for many patients (52.4%), carrying used materials in disposable containers (51.9%), not returning of unused materials to the clean room (55.3%), and adherence to hand washing (58.7%). Periodic monitoring for HBV and HCV was performed on 100% and 69.7% of the patients, respectively. Less than 2/3 of HCPs participated in the retraining courses. Conclusion. Compliance of HCPs with safety measures for viral hepatitis prevention was partly inadequate in HD wards. Emphasis on retraining of HCPs and official supervision would be effective steps in the reduction of viral dissemination.

The Impact of Requiring Completion of an Online Infection Control Course on Health Professionals’ Intentions to Comply with Infection Control Guidelines: A Comparative Study

Ensuring good infection control practice in health care facilities is a constant concern, yet evidence shows that the compliance of health care professionals with proper procedures is lacking, despite the existence of guidelines and training programs. An online infection control module was developed to provide ready access to training. Controversy exists about whether successfully completing such a course should be mandatory or strongly encouraged for all health care professionals. The objective of the present study was to compare the perception of safety culture and intention to comply with infection control guidelines in professionals who were required by their supervisors to take the course, and those who did so voluntarily. Survey responses on learning environment, safety climate and intention to comply with infection control guidelines in health care professionals who were required to take the course (supervisor-required group [n=143]) and those who took the same course voluntarily (voluntary group [n=105]) were compared. Because randomization was thought to be too difficult to implement in the policy context in which the study was conducted, significant differences between the two groups were taken into account in the analysis. Those required to take the course had a significantly better perception of the institutional safety climate (P<0.001), and had a higher reported intention to comply with infection control guidelines (P=0.040) than those who took the course voluntarily. Requiring that staff complete a 30 min interactive online infection control module increased their intention to comply with infection control guidelines compared with those who voluntarily accessed this material based on promotional material. Consideration should be given to making the successful completion of an online infection control module a requirement for all health care professionals.
