Background: 15% of the total cases of hypertension in West Java were found in Bandung. The prevention and management of hypertension are major public health challenges for Indonesia. The government had launched a chronic disease care program (PROLANIS) that designed to be done at primary health care with the aim to control hypertension for preventing the complication. Objective: This research aims to describe the blood pressure (BP) in PROLANIS community in Bandung, West Java. Method: A quantitative descriptive using non-analytic cross sectional study was carried out among PROLANIS community in Bandung. We performed BP examination in twenty-one primary care during March-October 2022. 853 data were collected and categorized based on the degree of hypertension by International Society of Hypertension 2020. Result: The results showed that PROLANIS community in Bandung is dominated by women (74.3%) than men. The data was categorized as normal BP (n = 274), high to normal BP (n = 191), first degree hypertension (n = 283) and second degree hypertension (n = 105). Conclusion: PROLANIS is expected to optimize the quality of life for chronic diseases patients and 75% of registered participants had a “good” result on specific examination for hypertension. But, this study showed that 45.5% of the patients did not achieve the target of BP. Most of them were in first degree hypertension (33.2%). This can be influenced by various factors, including the compliance of therapy, diet patterns, exercise, stress levels, and others. Further investigation is needed to find out the causes of BP in this population is not reaching the target.
