Comparison of Imaging Findings between Granulomatosis with Polyangiitis and Eosinophilic Granulomatosis with Polyangiitis on Sinus CT: Importance of High-Density Opacification of the Paranasal Sinuses.

Granulomatosis with polyangiitis (GPA) and eosinophilic granulomatosis with polyangiitis (EGPA) are the most common causes of chronic sinusitis from systemic granulomatous diseases. While both are small- to medium-sized vasculitis with necrotizing granulomas, they have different clinical courses and prognoses. High-density sinus opacification has been reported in allergic fungal sinusitis with eosinophilic infiltrates. Given that EGPA also has eosinophilic tissue infiltrates, we evaluated the differences in sinus CT findings, focusing on the sinus secretion attenuation between patients with GPA and EGPA, along with other previously described findings. This study included 31 patients with GPA and 22 patients with EGPA who underwent sinus CT. The attenuation of secretions within the paranasal sinuses was visually assessed, and the Hounsfield unit (HU) of the highest-density portions within each sinus was measured. Lund-Mackay scores (LMS), bony destruction, sclerotic wall changes, adjacent organ involvement, and nasal polyps were evaluated and compared between patients with GPA and EGPA. Multiple logistic regression analyses were conducted to determine which factors independently discriminated GPA from EGPA, and the diagnostic ability to differentiate between these 2 diseases was evaluated by using a receiver operating characteristic curve analysis. More patients in the GPA group showed bony destructions, bone sclerosis, and involvement of organs adjacent to paranasal sinuses than in the EGPA group (P = .006, 0.048, and 0.035, respectively). The EGPA group had higher LMS and more nasal polyps than the GPA group (P = .078 and 0.333, respectively). More patients in the EGPA group showed internal high-density opacification than in the GPA group, and patients with EGPA had higher mean HUs (both P < .0001). The presence of high-density opacification or mean HUs independently distinguished GPA from EGPA (OR, 53.67 and 1.07; 95% CI, 4.07-708.03 and 1.02-1.13, respectively) and showed a greater ability to discriminate between these diseases compared with other findings. Patients with EGPA had more high-density sinus opacification and higher mean HU on sinus CT than the patients with GPA. In addition to the previously reported CT findings, such as bony destruction, bone sclerosis, and adjacent organ involvement, evaluating secretion attenuation can assist in distinguishing between GPA and EGPA.

THE ROLE OF BREAST IMAGING MODALITIES IN THE DIAGNOSIS AND MANAGEMENT OF BENIGN BREAST DISEASES

Objective: Accurate diagnosis and management of benign breast diseases are crucial aspects of breast healthcare. Breast imaging modalities such as mammography, ultrasound, and magnetic resonance imaging (MRI) play a pivotal role in differentiating benign from malignant lesions and guiding appropriate management strategies. Methods: This retrospective study was conducted at [Institution Name] from January 2018 to December 2022. It involved a review of medical records, imaging results, and management outcomes of patients diagnosed with benign breast diseases. Imaging modalities included mammography, ultrasound, and MRI. A comprehensive exploration of gallbladder health: from common to rare imaging findings.

This comprehensive review explores a wide range of imaging findings associated with the gallbladder (GB), from anatomic variants to rare diseases. Through an in-depth review of diagnostic modalities including ultrasound, magnetic resonance cholangiopancreatography, CT, and MRI, we aim to highlight the crucial role of imaging techniques in diagnosing GB disorders, as congenital anomalies, inflammatory diseases, neoplasms, and surgical complications. Employing a detailed analysis and comparison of imaging findings across various modalities, this review seeks to improve diagnostic accuracy for GB-related pathologies, facilitating optimal patient management.

Evaluating the Efficacy of Perfusion MRI and Conventional MRI in Distinguishing Recurrent Cerebral Metastasis from Brain Radiation Necrosis.

Differentiating recurrent cerebral metastasis (CM) from brain radiation necrosis (BRN) is pivotal for guiding appropriate treatment and prognostication. Despite advances in imaging techniques, however, accurately distinguishing these conditions non-invasively is still challenging. This single-center retrospective study reviewed 32 cases (28 patients) with confirmed cerebral metastases who underwent surgical excision of lesions initially diagnosed by MRI and/or MR perfusion scans from 1 January 2015 to 30 September 2020. Diagnostic accuracy was assessed by comparing imaging findings with postoperative histopathology. Conventional MRI accurately identified recurrent CM in 75% of cases. MR perfusion scans showed significantly higher mean maximum relative cerebral blood volume (max. rCBV) in metastasis cases, indicating its potential as a discriminative biomarker. No single imaging modality could definitively distinguish CM from BRN. Survival analysis revealed gender as the only significant factor affecting overall survival, with no significant survival difference observed between patients with CM and BRN after controlling for confounding factors. This study underscores the limitations of both conventional MRI and MR perfusion scans in differentiating recurrent CM from BRN. Histopathological examination remains essential for accurate diagnosis. Further research is needed to improve the reliability of non-invasive imaging and to guide the management of patients with these post-radiation events.

Quantitative susceptibility mapping for detection of kidney stones, hemorrhage differentiation, and cyst classification in ADPKD.

The objective is to demonstrate feasibility of quantitative susceptibility mapping (QSM) in autosomal dominant polycystic kidney disease (ADPKD) patients and to compare imaging findings with traditional T1/T2w magnetic resonance imaging (MRI). Thirty-three consecutive patients (11 male, 22 female) diagnosed with ADPKD were initially selected. QSM images were reconstructed from the multiecho gradient echo data and compared to co-registered T2w, T1w, and CT images. Complex cysts were identified and classified into distinct subclasses based on their imaging features. Prevalence of each subclass was estimated. QSM visualized two renal calcifications measuring 9 and 10mm and three pelvic phleboliths measuring 2mm but missed 24 calcifications measuring 1mm or less and 1 larger calcification at the edge of the field of view. A total of 121 complex T1 hyperintense/T2 hypointense renal cysts were detected. 52 (43%) Cysts appeared hyperintense on QSM consistent with hemorrhage; 60 (49%) cysts were isointense with respect to simple cysts and normal kidney parenchyma, while the remaining 9 (7%) were hypointense. The presentation of the latter two complex cyst subtypes is likely indicative of proteinaceous composition without hemorrhage. Our results indicate that QSM of ADPKD kidneys is possible and uniquely suited to detect large renal calculi without ionizing radiation and able to identify properties of complex cysts unattainable with traditional approaches.

Relationship between Treatment Choices according to the Modified Osteoporotic Fracture Score and Posttreatment Radiographic Outcomes.

Retrospective observational study. To evaluate the modified osteoporotic fracture (mOF) scores in three treatment groups and compare imaging findings in patients treated and not treated according to the mOF score-based treatment recommendation. The osteoporotic fracture (OF) score was established by the AO Spine to guide therapeutic decisions. To enhance its applicability, a mOF score was recently introduced. Consecutive patients diagnosed with OFs at Fujieda Heisei Memorial Hospital were divided into three groups: nonsurgical therapy, balloon kyphoplasty (BKP), and open surgery groups. The mOF score was calculated, and the levels of independence and posttreatment imaging data were compared between patients treated and not treated according to the mOF score-based treatment recommendation. In total, 118 patients were included (nonsurgical therapy, n=57; BKP, n=48; open surgery, n=13), of whom 100 (85%) received treatment consistent with the mOF score-based treatment recommendation. In the BKP and open surgery groups, the mOF scorebased treatment recommendations were consistent with the actual treatment in 93% of the patients. However, in the nonsurgical group, the mOF score-based treatment recommendation was not consistent with the actual treatment in 25% of the patients. In this group, patients not treated according to the mOF score had significantly shorter vertebral body height, greater local kyphosis, and smaller sacral slope after treatment than patients treated according to the mOF score-based treatment recommendation. In the BKP and open surgery groups, the mOF scores were consistent with actual clinical selection. In the nonsurgical therapy group, patients not treated according to the mOF score-based treatment recommendation exhibited severe vertebral body deformity and a less well-balanced spine shape after treatment. The mOF score may help in selecting suitable treatments for OFs.

Role of Gene Xpert MTB/RIF assay for detecting tuberculous spondylitis in specimens obtained by CT-guided aspiration/biopsy and spectrum of imaging findings in spinal tuberculosis

ObjectiveTo evaluate the utility of the GeneXpert MTB/RIF assay for the diagnosis of tuberculous spondylitis in samples obtained by CT-guided aspiration/biopsy. The effect of anti-tubercular treatment, specimen type, and imaging findings on laboratory results was also assessed along with a comparison of imaging findings in tubercular versus non-tubercular cases. MethodsThis prospective observational study, planned for CT-guided aspiration/ biopsy, included 70 adults with suspected tuberculous spondylitis. Specimens were sent for microscopy, histopathology, TB culture, and Gene Xpert MTB/RIF assay. The correlation of sampling location, specimen type, anti-tubercular treatment, and imaging findings on microbiology results was documented. ResultsA definite diagnosis of tuberculous spondylitis was obtained in 50 cases. The assay had a 100 % agreement for smear-positive, culture-positive, specimens while the overall agreement was 41.3 %. Rifampicin resistance was detected in 8 cases. Seventy-one percent of patients were on empiric ATT. Imaging findings such as end plate destruction, paravertebral soft tissue thickening and abscess, disc involvement, sequestrum, and bony fragments were significantly associated with a final diagnosis of spinal tuberculosis. ConclusionsThe Gene Xpert MTB/RIF assay performed on specimens obtained by CT-guided aspiration/biopsy is a sensitive and rapid method of establishing a diagnosis of tuberculous spondylitis even in cases on ATT.

The importance of the imaging methods in patients with COVID-19 infection

Background Echocardiography, pulmonary CT and cardiac magnetic resonance (CMR) were have been used in detecting organ damage in COVID-19 patients. The aim of this study is to compare imaging findings and clinical symptoms patients with COVID-19. Methods The study was conducted in our university clinical center. All pts underwent clinical examination, laboratory analysis, echocardiography, pulmonary CT and CMR. Results 126 pts with COVID-19 infection, 61 (48%) male, mean age 45±15years with symptoms of palpitation, fatigue, dyspnea and chest pain. Meantime from the onset of infection to observation was 107 days. In pts with palpitation, brain natriuretic peptide (NTproBNP) was not higher (39.4±9.1 vs. 19.2±9.4 pg/ml, p = 0.075). In case of pts with dyspnea ejection fraction of the left ventricle (LVEF) as founded by echocardiography, was significantly lower (62±6 vs. 67±6%, p = 0.025). The CT severity score index was not significantly higher in patients with fatigue after acute infection (21.0±8.1 vs. 10.1±5.1, p = 0.060). Late gadolinium enhancement (LGE) was seen in 61/126 (48%) pts, distribution was predominantly in mid and epicardial layers 41/61 pts (67%), &amp;gt;3 segments were identified in 37/61 pts (61%), myocardial edema was seen in 50/126 pts (40%). The extracellular volume (ECV) quantification by CMR were significantly higher in patients with fatigue (22.6±3.8 vs. 19.7±5.2%, p = 0.010). There is no significant correlation between clinical symptoms and myocardial oedema by using T2FS. Conclusions Echocardiography and CMR are the most important imaging methods in risk stratification and optimal therapeutic strategies in pts with myocardial injury caused by COVID-19 infection.

Longitudinal analyses using 18F-Fluorodeoxyglucose positron emission tomography with computed tomography as a measure of COVID-19 severity in the aged, young, and humanized ACE2 SARS-CoV-2 hamster models

This study compared disease progression of severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) in three different models of golden hamsters: aged (≈60 weeks old) wild-type (WT), young (6 weeks old) WT, and adult (14–22 weeks old) hamsters expressing the human-angiotensin-converting enzyme 2 (hACE2) receptor. After intranasal (IN) exposure to the SARS-CoV-2 Washington isolate (WA01/2020), 2-deoxy-2-[fluorine-18]fluoro-D-glucose positron emission tomography with computed tomography (18F-FDG PET/CT) was used to monitor disease progression in near real time and animals were euthanized at pre-determined time points to directly compare imaging findings with other disease parameters associated with coronavirus disease 2019 (COVID-19). Consistent with histopathology, 18F-FDG-PET/CT demonstrated that aged WT hamsters exposed to 105 plaque forming units (PFU) developed more severe and protracted pneumonia than young WT hamsters exposed to the same (or lower) dose or hACE2 hamsters exposed to a uniformly lethal dose of virus. Specifically, aged WT hamsters presented with a severe interstitial pneumonia through 8 d post-exposure (PE), while pulmonary regeneration was observed in young WT hamsters at that time. hACE2 hamsters exposed to 100 or 10 PFU virus presented with a minimal to mild hemorrhagic pneumonia but succumbed to SARS-CoV-2-related meningoencephalitis by 6 d PE, suggesting that this model might allow assessment of SARS-CoV-2 infection on the central nervous system (CNS). Our group is the first to use (18F-FDG) PET/CT to differentiate respiratory disease severity ranging from mild to severe in three COVID-19 hamster models. The non-invasive, serial measure of disease progression provided by PET/CT makes it a valuable tool for animal model characterization.

Diagnostic performance of MRI for the assessment of extraocular muscle invasion in malignant sinonasal tumors.

Preoperative assessment of extraocular muscle invasion is essential for therapeutic strategies and prognostic evaluation. The aim of this study was to assess the diagnostic accuracy of MRI for evaluation of extraocular muscle (EM) invasion by malignant sinonasal tumors. Consecutively, 76 patients of sinonasal malignant tumors with orbital invasion were included in the present study. Preoperative MRI imaging features were analyzed by two radiologists independently. The diagnostic performances of MR imaging features for detecting EM involvement were evaluated by comparing imaging findings to histopathology data. A total of 31 extraocular muscles were involved by sinonasal malignant tumors in 22 patients, including 10 medial rectus muscles (32.2%), 10 inferior rectus muscles (32.2%), 9 superior oblique muscles (29.1%), and 2 external rectus muscles (6.5%). The EM involved by sinonasal malignant tumors usually showed relatively high signal intensity on T2-weighted images, indistinguishable from the tumor, nodular enlargement and abnormal enhancement (p = 0.001, < 0.001, < 0.001 and < 0.001, respectively). Using a combination of EM abnormal enhancement and indistinguishable from the tumor in multivariate logistic regression analysis, sensitivity, specificity, positive predictive value, negative predictive value and diagnostic accuracy for detecting orbital EM invasion by sinonasal tumors were 93.5, 85.2, 76.3, 96.3and 88%, respectively. MRI imaging features show high diagnostic performance for the diagnosis of extraocular muscle invasion by malignant sinonasal tumors.

Comparison of imaging findings on three-dimensional black-blood enhanced MR imaging between intracranial atherosclerotic occlusion and thrombotic occlusion

PurposeThe purpose of this study was to compare the imaging findings on three-dimensional (3D) black-blood (BB) contrast-enhanced MR imaging between intracranial atherosclerotic occlusion (IAO) and thrombotic occlusion (TO) of the middle cerebral artery (MCA) territory. Materials and methodsFrom August 2020 to September 2021, we retrospectively reviewed the BB contrast-enhanced MR imaging of patients visiting the emergency room for evaluation of acute ischemic stroke. In total, 77 patients with complete occlusion of the MCA territory on 3D BB contrast-enhanced MR imaging and cerebral angiography were enrolled in this study. We divided the IAO and TO groups according to occlusion causes based on angiography findings. ResultsOf 77 patients, 44 (57.1%) had an IAO in the M1 and M2 and 33 had a TO. Lesion length contrast enhancement (CE) in patients with a TO was significantly longer than that in patients with an IAO (18.95 mm [IQR: 20.91] vs. 7.1 mm [8.92], p <0.001). Overall, 38 (39.4%) patients showed a disconnection of CE on 3D BB contrast-enhanced MR imaging, and 35 showed CE before and after the stenotic or thrombotic lesion. Symptomatic lesions on diffusion-weighted imaging in the TO group were significantly higher than that of the IAO group (97.0% vs, 70.5%, p = 0.003). ConclusionThe long segment CE on 3D BB contrast-enhanced MR imaging was related to TO of MCA. CE before and after a stenotic or thrombotic lesion is a common finding on 3D BB contrast-enhanced MR imaging.

Neuroimaging of human and non-human animal emotion and affect in the context of social relationships.

Long-term relationships are essential for the psychological wellbeing of humans and many animals. Positive emotions and affective experiences (e.g., romantic or platonic love) seem to be closely related to the creation and maintenance of social bonds. When relationships are threatened or terminated, other emotions generally considered to be negative can arise (e.g., jealousy or loneliness). Because humans and animals share (to varying degrees) common evolutionary histories, researchers have attempted to explain the evolution of affect and emotion through the comparative approach. Now brain imaging techniques allow the comparison of the neurobiological substrates of affective states and emotion in human and animal brains using a common methodology. Here, we review brain imaging studies that feature emotions characterized by the context of social bonding. We compare imaging findings associated with affective and emotional states elicited by similar social situations between humans and animal models. We also highlight the role of key neurohormones (i.e., oxytocin, vasopressin, and dopamine) that jointly support the occurrence of socially contextualized emotions and affect across species. In doing so, we seek to explore and clarify if and how humans and animals might similarly experience social emotion and affect in the context of social relationships.

Role Of Ultrasound For The Diagnosis Of Complications Of Dengue Fever In Gujrat

Background : The purpose of this study is to evaluate if ultrasonography may be used as an useful addition to clinical and radiology profiles in diagnosing dengue fever and predicting virus severity by comparing imaging findings. The difference in sonographic characteristics observed in patients of various ages was also investigated. &#x0D; Objective : The goal of this study is to determine role of ultrasound for diagnosis of complications of dengue fever in Gujrat &#x0D; Materials and Methods: This is a retrospective study. From October and December 2021, 79 patients who were serologically confirmed with dengue fever were referred for ultrasound scanning of the abdomen at Aziz bhatti shaheed hospital Gujrat, and the imaging data were evaluated. The statistical kit for social science (spss) is used for data analysis.&#x0D; Results: out of 76 positive dengue patients there are 71(89.9%) male and 8(10%) are female .48 (60.8%) patients showed splenomegaly, 24(30%) patients showed ascites in which 8 (10%) have pelvic ascites and 16 (20.3%) abdominal ascites, 63 (79.7%) patients showed edematous GB wall thickening, 21(26.6%) showed pleural effusion while 18 (22.8 %) have bilateral pleural effusion and 3 (3.8%) have right sided. &#x0D; Conclusion: In patients presenting with fever and concomitant symptoms, sonographic signs of thickening GB wall, pleural effusion (bilateral or right side), ascites, and splenomegaly should strongly help in early diagnosis of dengue fever, especially during an outbreak. &#x0D;

A Case of Odontogenic Myxoma of the Mandible with Expansion to the Alveolar Crest – Comparison of Imaging Findings and Pathological Findings: A Case Report

Background: An odontogenic myxoma is an intraosseous tumor characterized by stellate and spindle-shaped cells embedded in an abundant myxoid or mucoid extracellular matrix. We herein describe an odontogenic myxoma that expanded not only to the bone marrow but also to the outside of the alveolar bone. Diagnosis of an odontogenic myxoma in a tooth-deficient region by imaging findings alone was difficult because the positional relationship between the tumor and the tooth is unknown. Furthermore, some of these odontogenic myxomas reportedly show rapid growth. Case Report: Here, we present the case of a patient, a 44-year-old man, who had a hard, bone-like swelling on his right mandible molar region and mild paresthesia on his right cheek. An odontogenic myxoma and ameloblastoma were suspected based on the imaging findings; however, pathological examination of the biopsy led to a diagnosis of odontogenic myxoma. Right segmental mandibulectomy was performed, and there was no recurrence observed after surgery. Conclusion: To improve the accuracy of imaging diagnosis, it is important to compare the imaging findings with the pathological findings of the surgical specimen. This comparison in the present case revealed differences in the magnetic resonance imaging signal intensity in regions with different types of cell components.

Diagnostic accuracy of imaging approaches for early tumor detection in children with Li-Fraumeni syndrome.

The Toronto protocol for cancer surveillance in children with Li-Fraumeni syndrome has been adopted worldwide. To assess the diagnostic accuracy of the imaging used in this protocol. We conducted a blinded retrospective review of imaging modalities in 31 pediatric patients. We compared imaging findings with the reference standards, which consisted of (1) histopathological diagnosis, (2) corresponding dedicated imaging or subsequent surveillance imaging or (3) clinical outcomes. We individually analyzed each modality's diagnostic performance for cancer detection and assessed it on a per-study basis for chest and abdominal regional whole-body MRI (n=115 each), brain MRI (n=101) and abdominal/pelvic US (n=292), and on a per-lesion basis for skeleton/soft tissues on whole-body MRI (n=140). Of 763 studies/lesions, approximately 80% had reference standards that identified 4 (0.7%) true-positive, 523 (85.3%) true-negative, 5 (0.8%) false-positive, 3 (0.5%) false-negative and 78 (12.7%) indeterminate results. There were 3 true-positives on whole-body MRI and 1 true-positive on brain MRI as well as 3 false-negatives on whole-body MRI. Sensitivities and specificities of tumor diagnosis using a worst-case scenario analysis were, respectively, 40.0% (95% confidence interval [CI]: 7.3%, 83.0%) and 38.2% (95% CI: 29.2%, 48.0%) for skeleton/soft tissues on whole-body MRI; sensitivity non-available and 97.8% (95% CI: 91.4%, 99.6%) for chest regional whole-body MRI; 100.0% (95% CI: 5.5%, 100.0%) and 96.8% (95% CI: 90.2%, 99.2%) for abdominal regional whole-body MRI; sensitivity non-available and 98.3% (95% CI: 95.3, 99.4) for abdominal/pelvic US; and 50.0% (95% CI: 2.7%, 97.3%) and 93.8% (95% CI: 85.6%, 97.7%) for brain MRI. Considerations for optimizing imaging protocol, defining criteria for abnormalities, developing a structured reporting system, and practicing consensus double-reading may enhance the diagnostic accuracy for tumor surveillance.

Sensitivity of pre-operative imaging and radiologist inter-rater reliability in detecting lesions in Crohn's disease.

Computed tomography (CT), computed tomography enterography (CTE) and magnetic resonance enterography (MRE) are commonly used pre-operatively in surgical planning in Crohn's Disease (CD). The findings on CT, CTE and MRE may not, however, correlate with operative findings. This study aims to establish the sensitivity of these imaging modalities and analyse radiologist inter-rater reliability by comparing imaging findings of strictures, fistulas and abscesses with intra-operative findings. A retrospective review of CD patients who had either CT, CTE and/or MRE imaging and CD related surgical intervention at a public health service from 2010 to 2019 inclusive. The number and locations of strictures, fistulas and abscesses on pre-operative original radiology reports (OR) were recorded. Subsequently, all scans were re-read by two specialist abdominal radiologists and consensus recorded (SR). Lesions recorded from both OR and SR were compared to those found intra-operatively. Eighty-three patients were included. For strictures, sensitivity was 67%, 74% and 79% for OR and 88%, 71% and 87% for SR for CT, CTE and MRE respectively. The frequency of fistulas and abscesses were small hence a conclusion could not be drawn. The level of agreement between radiologists ranged from 44% to 82% for strictures and 64 to 100% for fistulas and abscesses across all three imaging modalities. CT and MRE have similarly high sensitivities for the identification of strictures pre-operatively when read by specialist radiologists. Inter-rater reliability calculations found similar agreement levels between specialist radiologists and between OR and SR for strictures, fistulas and abscesses across CT, CTE and MRE.

We aim to summarize common imaging findings of 3 betacoronaviruses (b-CoVs) and the common clinical manifestation, to provide a better understanding of the courses of the disease. The Pubmed and Google Scholar databases were searched for the terms "SARS-CoV" OR "COVID-19" OR "MERS-CoV". Imaging-specific searches included keyword searches for "CT" AND "imaging". Clinical presentation-specific searches included keyword searches for "clinical" AND "manifestation" AND "cardio-vascular" OR "neurology" OR "gastrointestinal" OR "hematology". In total, 77 articles were selected for discussion in the current literature review. Human b-CoVs infection presented consistent indications of ground-glass opacities (GGO), consolidation, and interlobular septal thickening. Pleural effusion was also common in all 3 b-CoVs, but it was least present in SARS-CoV-2 infection. Bilateral lung involvement was common to both MERS-CoV and SARS-CoV-2 infection. Cardiovascular, neurological, haematological, and gastrointestinal were common clinical presentations found in patients infected with b-CoVs. The comparison of imaging findings can be applied in clinical practice to distinguish the 3 CoV through different imaging modalities. It is crucial to understand the possible imaging findings and clinical presentations to better understand the course of the disease as well as prepare for future variants.

Imaging of Skull Base and Orbital Invasion in Sinonasal Cancer: Correlation with Histopathology.

The aim of this study was to assess the diagnostic performance of two common imaging techniques (CT and MRI) for the diagnosis of skull base and orbital invasion by comparing imaging findings to histopathological data. Methods: This was a retrospective two-center study including patients with sinonasal cancer involving the skull base and/or the orbit operated on between 2000 and 2019. Patients were included only if pre-operative CT and/or MRI, operative and histopathologic reports were available. A double prospective blinded imaging review was conducted according to predefined radiological parameters. Radiologic tumor extension was compared to histopathological reports, which were considered the gold standard. The predictive positive value (PPV) for the diagnosis of skull base/orbital invasion was calculated for each parameter. Results: A total of 176 patients were included. Ethmoidal intestinal-type adenocarcinoma was the most common type of cancer (41%). The PPV for major modification of the bony skull base was 78% on the CT scan, and 89% on MRI. MRI signs of dural invasion with the highest PPVs were: contact angle over 45° between tumor and dura (86%), irregular deformation of dura adjacent to tumor (87%) and nodular dural enhancement over 2 mm in thickness (87%). Signs of orbital invasion had low PPVs (<50%). Conclusions: This retrospective study provides objective data about the diagnostic value of pretreatment imaging in patients with sinonasal cancer.

Chestultrasonography versus computed tomography in the diagnosis of pleural effusion (Menoufia experience)

Background Pleural effusion is a common clinical problem and can arise from many diseases. Several imaging methods such as chest radiograph, ultrasonography (US), and computed tomography (CT) scan are being used to diagnose and detect the etiology of pleural effusion. Objective The aim of this work was to compare imaging findings of chest US and CT and the role of chest US in diagnosis of pleural effusion. Patients and methods This observational prospective cohort study was done on 70 patients with pleural effusion of determined etiology admitted to the Chest Department, Menoufia University Hospital, in the period between August 2018 and June 2019. We compared between chest US and CT in different types of pleural effusion. Results This study showed that pleural septations were detected by US more than CT, and the difference was statistically significant (P&lt;0.05). There was a significant statistical difference in detection of pleural thickening by US over CT (P&lt;0.005). It showed a highly significant statistical difference in the detection of the site of pleural thickening and nodules by US, especially on diaphragmatic pleura (P&lt;0.001). There was also a highly significant statistical difference in the detection of pleural nodules by US over CT (P&lt;0.001). There was a significant difference in the detection of parenchymal lesions by US and CT, where US could see collapse more than CT (P&lt;0.05), but CT detected consolidation patches more than US. Conclusion Chest US finding has higher diagnostic accuracy than chest CT finding in the diagnosis of pleural effusion and more sensitive in detection of pleural septations, diaphragmatic pleural nodules, and thickening.
