Investigation of the accuracy of dynamic condylar position: A model study

ObjectivesTo evaluate dynamic condylar positions by integrating mandibular movement recording data and cone-beam computed tomography (CBCT) and to investigate its accuracy via dynamic model experiments. MethodsA polyvinyl chloride skull model was utilized. A robot arm was used to operate the mandible to perform mouth opening, closing, protrusion, and lateral movements. A recording device, worn on the skull, was used to record the dynamic process and an optical position tracking (OPT) system was used to simultaneously trace the movements. A self-developed software module was used to evaluate the dynamic condylar position by integrating the dynamic tracing data and a virtual skull model derived from CBCT images. Errors were defined as differences between the dynamic coordinates of six landmarks around the condylar area derived from the software module (test) and OPT system (gold standard). ResultsThe condylar position errors were 0.76 ± 0.31, 0.55 ± 0.15, and 0.68 ± 0.23 mm for mouth opening, bilateral, and protrusion movements, respectively. Furthermore, the errors for small, moderate, and large mouth opening movements were 0.62 ± 0.19, 0.69 ± 0.29, and 0.94 ± 0.31 mm, respectively. The errors for all movements, except for large mouth opening, were significantly less than 1 mm (P < 0.05). The error was not different from 1 mm in the large mouth opening movement (P > 0.05). ConclusionsOur developed method of achieving dynamic condylar position by integrating mandibular movement recording data and CBCT images is clinically reliable. Clinical significanceThis study proved the reliability of evaluating dynamic condylar position using a commercial dynamic recording instrument and CBCT images.

Evaluation of the clinical feasibility of cone-beam computed tomography guided online adaption for simulation-free palliative radiotherapy

Background and purposeSimulation-free radiotherapy, where diagnostic imaging is used for treatment planning, improves accessibility of radiotherapy for eligible palliative patients. Combining this pathway with online adaptive radiotherapy (oART) may improve accuracy of treatment, expanding the number of eligible patients. This study evaluated the adaptive process duration, plan dose volume histogram (DVH) metrics and geometric accuracy of a commercial cone-beam computed tomography (CBCT)-guided oART system for simulation-free, palliative radiotherapy. Materials and methodsTen previously treated palliative cases were used to compare system-generated contours against clinician contours in a test environment with Dice Similarity Coefficient (DSC). Twenty simulation-free palliative patients were treated clinically using CBCT-guided oART. Analysis of oART clinical treatment data included; evaluation of the geometric accuracy of system-generated synthetic CT relative to session CBCT anatomy using a Likert scale, comparison of adaptive plan dose distributions to unadapted, using DVH metrics and recording the duration of key steps in the oART workflow. ResultsAuto-generated contours achieved a DSC of higher than 0.85, excluding the stomach which was attributed to CBCT image quality issues. Synthetic CT was locally aligned to CBCT anatomy for approximately 80% of fractions, with the remaining suboptimal yet clinically acceptable. Adaptive plans achieved a median CTV V95% of 99.5%, compared to 95.6% for unadapted. The median overall oART process duration was found to be 13.2 mins, with contour editing being the most time-intensive adaptive step. ConclusionsThe CBCT-guided oART system utilising a simulation-free planning approach was found to be sufficiently accurate for clinical implementation, this may further streamline and improve care for palliative patients.

Geometry calibration for a dental cone-beam CT system with an offset detector

In this work, we propose a robust offline geometry calibration method for an offset-detector-based dental cone-beam computed tomography (CBCT) system. Although a host of geometric calibration methods for CT system have been developed, the calibration of dental CBCT systems remains to be one of the challenging cases due to the presence of an offset detector and due to the smaller system dimensions. We highlight the technical difficulties encountered in such dental CBCT geometric calibration, and provide an efficient and robust recipe for the accurate calibration. Through a particular design of the phantom, a heuristic estimation of the isocenter, and an enforcement of smooth change of the extracted geometric parameters, we show that the aforementioned challenges can be overcome. Fabrication errors that may exist in the geometry calibration phantom are also considered. We scanned two physical phantoms with a commercial dental CBCT for evaluating the proposed geometry calibration method. The results confirm that the proposed method can achieve an accurate and efficient geometry calibration in the offset-detector-based CBCT systems.

Clinical Assessment of a Novel Ring Gantry Linear Accelerator-Mounted Helical Fan-Beam kVCT System

PurposeTo assess clinically relevant image quality metrics (IQMs) of helical fan beam kilovoltage (kV) fan beam computed tomography (CT). Methods and MaterialskVCT IQMs were evaluated on an Accuray Radixact unit equipped with helical fan beam kVCT to assess the capabilities of this newly available modality. kVCT IQMs were evaluated and compared to a kVCT simulator and linear accelerator-based cone beam CTs (CBCT) using a commercial CBCT image quality phantom. kVCTs were acquired on the Accuray Radixact for all combinations of kVp and mAs in fine mode using a 440-mm field of view (FOV). Evaluated IQMs were spatial resolution, overall uniformity, subject contrast, contrast-to-noise ratio (CNR), and effective slice thickness. Imaging dose was assessed for planar kV imaging. ResultsOn this kVCT system spatial resolution and contrast were consistent across all settings with 0.28 ± 0.03 lp/mm and 9.8% ± 0.7% (both 95% confidence interval). CNR strongly depended on selected mode (views per rotation) and body size (mA per view) and ranged between 7.9 and 34.9. Overall uniformity was greater than 97% for all settings. Large FOV was not found to substantially affect the IQMs whereas small FOV affected IQMs due to its effect on pitch. Technique-matched CT simulator scans were comparable for uniformity and contrast, while spatial resolution was higher (0.43 ± 0.06 lp/mm), and CNR was between 4% (140 kVp) and 51% (100 kVp) lower. For kV-CBCT, spatial resolutions ranging from 0.37 to 0.44 lp/mm were achieved with comparable contrast, CNR, and uniformity to kVCT. All kVCT scans exhibit imaging artifacts due to helical acquisition. Clinical acquisitions of megavoltage (MV) CT, kV-CBCT, and kVCT on the same patient showed improved and comparable image quality of kVCT compared to MVCT and kV-CBCT, respectively. ConclusionsHelical fan beam kVCT allows for daily image guidance for localization and setup verification with comparable performance to existing kV-CBCT systems. Scan parameters must be selected carefully to maximize image quality for the desired tasks. Due to the large effective slice thicknesses for all parameter combinations, kVCT scans should not be used for simulation or planning of stereotactic procedures. Finally, improved image quality over MVCT has the potential to greatly improve manual and automated adaptive monitoring and planning.

Prototype Small-Animal PET-CT Imaging System for Image-guided Radiation Therapy.

Molecular imaging is becoming essential for precision targeted radiation therapy, yet progress is hindered from a lack of integrated imaging and treatment systems. We report the development of a prototype positron emission tomography (PET) scanner integrated into a commercial cone beam computed tomography (CBCT) based small animal irradiation system for molecular-image-guided, targeted external beam radiation therapy. The PET component consists of two rotating Hamamatsu time-of-flight PET modules positioned with a bore diameter of 101.6 mm and a radial field-of-view of 53.1 mm. The measured energy resolution after linearity correction at 511 KeV was 12.9% and the timing resolution was 283.6 ps. The measured spatial resolutions at the field-of-view center and 5 mm off the radial center were 2.6 mm × 2.6 mm × 1.6 mm and 2.6 mm × 2.6 mm × 2.7 mm respectively. 18F-Fluorodeoxyglucose-based PET imaging of a NEMA NU 4-2008 phantom resolved cylindrical volumes with diameters as small as 3 mm. To validate the system in-vivo, we performed 64Cu-DOTA-M5A PET and computed tomography (CT) imaging of carcinoembryonic antigen (CEA)-positive colorectal cancer in athymic nude mice and compared the results with a commercially available Siemens Inveon PET/CT system. The prototype PET system performed comparably to the Siemens system for identifying the location, size, and shape of tumors. Regions of heterogeneous 64Cu-DOTA-M5A uptake were observed. Using 64Cu-DOTA-M5A PET and CT images, a Monte Carlo-based radiation treatment plan was created to escalate the dose to the 64Cu-DOTA-M5A-based, highly active, biological target volume while largely sparing the normal tissue. Results demonstrate the feasibility of molecular-image-guided treatment plans using the prototype theranostic system.

Cone Beam Computed Tomography Image Quality Improvement Using a Deep Convolutional Neural Network.

IntroductionCone beam computed tomography (CBCT) plays an important role in image-guided radiation therapy (IGRT), while having disadvantages of severe shading artifact caused by the reconstruction using scatter contaminated and truncated projections. The purpose of this study is to develop a deep convolutional neural network (DCNN) method for improving CBCT image quality.MethodsCBCT and planning computed tomography (pCT) image pairs from 20 prostate cancer patients were selected. Subsequently, each pCT volume was pre-aligned to the corresponding CBCT volume by image registration, thereby leading to registered pCT data (pCTr). Next, a 39-layer DCNN model was trained to learn a direct mapping from the CBCT to the corresponding pCTr images. The trained model was applied to a new CBCT data set to obtain improved CBCT (i-CBCT) images. The resulting i-CBCT images were compared to pCTr using the spatial non-uniformity (SNU), the peak-signal-to-noise ratio (PSNR) and the structural similarity index measure (SSIM).ResultsThe image quality of the i-CBCT has shown a substantial improvement on spatial uniformity compared to that of the original CBCT, and a significant improvement on the PSNR and the SSIM compared to that of the original CBCT and the enhanced CBCT by the existing pCT-based correction method.ConclusionWe have developed a DCNN method for improving CBCT image quality. The proposed method may be directly applicable to CBCT images acquired by any commercial CBCT scanner.

Influence of a Commercial Lead Apron on Patient Skin Dose Delivered During Oral and Maxillofacial Examinations under Cone Beam Computed Tomography (CBCT).

The purpose of this paper is to investigate the impact of a commercial lead apron on patient skin dose delivered during maxillofacial CBCT in five critical regions by means of solid-state-dosimetry. Five anatomical regions (thyroid gland, left and right breast, gonads, back of the phantom torso) in an adult female anthropomorphic phantom were selected for dose measurement by means of the highly sensitive solid-state dosimeter QUART didoSVM. Ten repeated single exposures were assessed for each patient body region for a total of five commercial CBCT devices with and without a lead apron present. Shielded and non-shielded exposures were compared under the paired Wilcoxon test, with absolute and relative differences computed. Reproducibility was expressed as the coefficient of variation (CV) between the 10 repeated assessments. The highest doses observed at skin level were found at the thyroid (mean shielded ± SD: 450.5 ± 346.7 μGy; non-shielded: 339.2 ± 348.8 μGy, p = 0.4922). Shielding resulted in a highly significant (p < 0.001) 93% dose reduction in skin dose in the female breast region with a mean non-shielded dose of approximately 35 μGy. Dose reduction was also significantly lower for the back-region (mean: -65%, p < 0.0001) as well as for the gonad-region (mean: -98%, p < 0.0001) in the shielded situation. Reproducibility was inversely correlated to skin dose (Rspearman = -0.748, p < 0.0001) with a mean CV of 10.45% (SD: 24.53 %). Skin dose in the thyroid region of the simulated patient was relatively high and not influenced by the lead apron, which did not shield this region. Dose reduction by means of a commercial lead apron was significant in all other regions, particularly in the region of the female breast.

Fast shading correction for cone beam CT in radiation therapy via sparse sampling on planning CT.

The image quality of cone beam computed tomography (CBCT) is limited by severe shading artifacts, hindering its quantitative applications in radiation therapy. In this work, we propose an image-domain shading correction method using planning CT (pCT) as prior information which is highly adaptive to clinical environment. We propose to perform shading correction via sparse sampling on pCT. The method starts with a coarse mapping between the first-pass CBCT images obtained from the Varian TrueBeam system and the pCT. The scatter correction method embedded in the Varian commercial software removes some image errors but the CBCT images still contain severe shading artifacts. The difference images between the mapped pCT and the CBCT are considered as shading errors, but only sparse shading samples are selected for correction using empirical constraints to avoid carrying over false information from pCT. A Fourier-Transform-based technique, referred to as local filtration, is proposed to efficiently process the sparse data for effective shading correction. The performance of the proposed method is evaluated on one anthropomorphic pelvis phantom and 17 patients, who were scheduled for radiation therapy. (The codes of the proposed method and sample data can be downloaded from https://sites.google.com/view/linxicbct) RESULTS: The proposed shading correction substantially improves the CBCT image quality on both the phantom and the patients to a level close to that of the pCT images. On the phantom, the spatial nonuniformity (SNU) difference between CBCT and pCT is reduced from 74 to 1HU. The root of mean square difference of SNU between CBCT and pCT is reduced from 83 to 10HU on the pelvis patients, and from 101 to 12HU on the thorax patients. The robustness of the proposed shading correction is fully investigated with simulated registration errors between CBCT and pCT on the phantom and mis-registration on patients. The sparse sampling scheme of our method successfully avoids false structures in the corrected CBCT even when the maximum registration error is as high as 8mm. We develop an effective shading correction algorithm for CBCT readily implementable on clinical data as a software plug-in without modifications of current imaging hardware and protocol. The algorithm is directly applied on the output images from a commercial CBCT scanner with high computational efficiency and negligible memory burden.

A comparison between various radiological techniques in the localization and analysis of impacted and supernumerary teeth

An increasing number of different types of commercial cone-beam computed tomography (CBCT) devices are available for three-dimensional (3D) imaging in the field of dental and maxillofacial radiology. When removing impacted or supernumerary teeth, surgical teams often operate adjacent significant anatomical structures such as nerves, vessels, adjacent teeth roots, and paranasal sinuses. It is therefore important to choose the appropriate surgical approach to avoid iatrogenic damage to the essential anatomical neighbouring structures. CBCT, also called digital volume tomography (DVT), can visualize impacted and supernumerary teeth in all standard planes, as well as multisectional 3D views. These devices have shown to be highly beneficial in the assessment of small bony lesions and maxillofacial injuries. However, it is still necessary to determine the effectiveness of such devices in the assessment of impacted and supernumerary teeth, in comparison to the conventional radiological methods of intraoral X-rays and panoramic X-rays. During a period of 2 years, a total of 61 patients of whom majority had impacted teeth or supernumerary elements in the frontal maxillary region were studied with CBCT and treated at the St. Olavs University Hospital. Patients were referred to our Department of Oral and Maxillofacial Surgery with both conventional and digital intraoral X-rays and/or panoramic X-rays. None had any acute infections or odontogenic abscesses, and most presented with asymptomatic impacted tooth. A comparison between the preoperative conventional and the CBCT images, the resulting diagnoses, and the intraoperative findings as "gold standard" were made and recorded in a compiled scoring sheet. The objects of interest were researched with the magnification method. Each patient was identified only with a patient number. In contrast to the conventional X-rays, the pre-surgical evaluation with the CBCT revealed detailed imaging of significant anatomical structures and objects of interest, with highly accurate anatomical and morphologic imaging, when compared to the intraoperative findings. Furthermore, no diagnostic problems, in relation to the anatomical localization, occurred preoperatively. The CBCT provides true and precise anatomical information with high surgical predictability without distortion or artefacts, and is superior to conventional radiography. It enables more time-efficient surgeries and reduces costs and surgical complications.

Ultra-Fast Digital Tomosynthesis Reconstruction Using General-Purpose GPU Programming for Image-Guided Radiation Therapy

The purpose of this work is to demonstrate an ultra-fast reconstruction technique for digital tomosynthesis (DTS) imaging based on the algorithm proposed by Feldkamp, Davis, and Kress (FDK) using standard general-purpose graphics processing unit (GPGPU) programming interface. To this end, the FDK-based DTS algorithm was programmed "in-house" with C language with utilization of 1) GPU and 2) central processing unit (CPU) cards. The GPU card consisted of 480 processing cores (2 x 240 dual chip) with 1,242 MHz processing clock speed and 1,792 MB memory space. In terms of CPU hardware, we used 2.68 GHz clock speed, 12.0 GB DDR3 RAM, on a 64-bit OS. The performance of proposed algorithm was tested on twenty-five patient cases (5 lung, 5 liver, 10 prostate, and 5 head-and-neck) scanned either with a full-fan or half-fan mode on our cone-beam computed tomography (CBCT) system. For the full-fan scans, the projections from 157.5°-202.5° (45°-scan) were used to reconstruct coronal DTS slices, whereas for the half-fan scans, the projections from both 157.5°-202.5° and 337.5°-22.5° (2 x 45°-scan) were used to reconstruct larger FOV coronal DTS slices. For this study, we chose 45°-scan angle that contained ~80 projections for the full-fan and ~160 projections with 2 x 45°-scan angle for the half-fan mode, each with 1024 x 768 pixels with 32-bit precision. Absolute pixel value differences, profiles, and contrast-to-noise ratio (CNR) calculations were performed to compare and evaluate the images reconstructed using GPU- and CPU-based implementations. The time dependence on the reconstruction volume was also tested with (512 x 512) x 16, 32, 64, 128, and 256 slices. In the end, the GPU-based implementation achieved, at most, 1.3 and 2.5 seconds to complete full reconstruction of 512 x 512 x 256 volume, for the full-fan and half-fan modes, respectively. In turn, this meant that our implementation can process > 13 projections-per-second (pps) and > 18 pps for the full-fan and half-fan modes, respectively. Since commercial CBCT system nominally acquires 11 pps (with 1 gantry-revolution-per-minute), our GPU-based implementation is sufficient to handle the incoming projections data as they are acquired and reconstruct the entire volume immediately after completing the scan. In addition, on increasing the number of slices (hence volume) to be reconstructed from 16 to 256, only minimal increases in reconstruction time were observed for the GPU-based implementation where from 0.73 to 1.27 seconds and 1.42 to 2.47 seconds increase were observed for the full-fan and half-fan modes, respectively. This resulted in speed improvement of up to 87 times compared with the CPU-based implementation (for 256 slices case), with visually identical images and small pixel-value discrepancies (< 6.3%), and CNR differences (< 2.3%). With this achievement, we have shown that time allocation for DTS image reconstruction is virtually eliminated and that clinical implementation of this approach has become quite appealing. In addition, with the speed achievement, further image processing and real-time applications that was prohibited prior due to time restrictions can now be tempered with.

Computed tomography dose index and dose length product for cone‐beam CT: Monte Carlo simulations of a commercial system

Dosimetry in kilovoltage cone beam computed tomography (CBCT) is a challenge due to the limitation of physical measurements. To address this, we used a Monte Carlo (MC) method to estimate the CT dose index (CTDI) and the dose length product (DLP) for a commercial CBCT system. As Dixon and Boone (1) showed that CTDI concept can be applicable to both CBCT and conventional CT, we evaluated weighted CT dose index (CTDIw) and DLP for a commercial CBCT system. Two extended CT phantoms were created in our BEAMnrc/EGSnrc MC system. Before the simulations, the beam collimation of a Varian On‐Board Imager (OBI) system was measured with radiochromic films (model: XR‐QA). The MC model of the OBI X‐ray tube, validated in a previous study, was used to acquire the phase space files of the full‐fan and half‐fan cone beams. Then, DOSXYZnrc user code simulated a total of 20 CBCT scans for the nominal beam widths from 1 cm to 10 cm. After the simulations, CBCT dose profiles at center and peripheral locations were extracted and integrated (dose profile integral, DPI) to calculate the CTDI per each beam width. The weighted cone‐beam CTDI (CTDIw,l) was calculated from DPI values and mean CTDIw,l(CTDIw,l)¯ and DLP were derived. We also evaluated the differences of CTDIw values between MC simulations and point dose measurements using standard CT phantoms. In results, it was found that CTDIw,600¯ was 8.74±0.01 cGy for head and CTDIw,900¯ was 4.26±0.01 cGy for body scan. The DLP was found to be proportional to the beam collimation. We also found that the point dose measurements with standard CT phantoms can estimate the CTDI within 3% difference compared to the full integrated CTDI from the MC method. This study showed the usability of CTDI as a dose index and DLP as a total dose descriptor in CBCT scans.PACS number: 87.57.uq

Preliminary Studies for a CBCT Imaging Protocol for Offline Organ Motion Analysis: Registration Software Validation and CTDI Measurements

Cone-beam X-ray volumetric imaging in the treatment room, allows online correction of set-up errors and offline assessment of residual set-up errors and organ motion. In this study the registration algorithm of the X-ray volume imaging software (XVI, Elekta, Crawley, United Kingdom), which manages a commercial cone-beam computed tomography (CBCT)–based positioning system, has been tested using a homemade and an anthropomorphic phantom to: (1) assess its performance in detecting known translational and rotational set-up errors and (2) transfer the transformation matrix of its registrations into a commercial treatment planning system (TPS) for offline organ motion analysis. Furthermore, CBCT dose index has been measured for a particular site (prostate: 120 kV, 1028.8 mAs, approximately 640 frames) using a standard Perspex cylindrical body phantom (diameter 32 cm, length 15 cm) and a 10-cm-long pencil ionization chamber. We have found that known displacements were correctly calculated by the registration software to within 1.3 mm and 0.4°. For the anthropomorphic phantom, only translational displacements have been considered. Both studies have shown errors within the intrinsic uncertainty of our system for translational displacements (estimated as 0.87 mm) and rotational displacements (estimated as 0.22°). The resulting table translations proposed by the system to correct the displacements were also checked with portal images and found to place the isocenter of the plan on the linac isocenter within an error of 1 mm, which is the dimension of the spherical lead marker inserted at the center of the homemade phantom. The registration matrix translated into the TPS image fusion module correctly reproduced the alignment between planning CT scans and CBCT scans. Finally, measurements on the CBCT dose index indicate that CBCT acquisition delivers less dose than conventional CT scans and electronic portal imaging device portals. The registration software was found to be accurate, and its registration matrix can be easily translated into the TPS and a low dose is delivered to the patient during image acquisition. These results can help in designing imaging protocols for offline evaluations.

Comparison of Five Cone Beam Computed Tomography Systems for the Detection of Vertical Root Fractures

Introduction This study compared the accuracy of cone beam computed tomography (CBCT) scans made by five different systems in detecting vertical root fractures (VRFs). It also assessed the influence of the presence of root canal filling (RCF), CBCT slice orientation selection, and the type of tooth (premolar/molar) on detection accuracy. Methods Eighty endodontically prepared teeth were divided into four groups and placed in dry mandibles. The teeth in groups Fr-F and Fr-NF were artificially fractured; those in groups control-F and control-NF were not. Groups Fr-F and control-F were root filled. CBCT scans were made using five different commercial CBCT systems. Two observers evaluated images in axial, coronal, and sagittal reconstruction planes. Results There was a significant difference in detection accuracy among the five systems ( p = 0.00001). The presence of RCF did not influence sensitivity ( p = 0.16), but it reduced specificity ( p = 0.003). Axial slices were significantly more accurate than sagittal and coronal slices ( p = 0.0001) in detecting VRF in all systems. Significantly more VRFs were detected among molars than premolars ( p = 0.0001). Conclusions RCF presence reduced specificity in all systems ( p = 0.003) but did not influence accuracy ( p = 0.79) except in one system ( p = 0.012). Axial slices were the most accurate in detecting VRFs ( p = 0.0001).

Simple methods to reduce patient dose in a Varian cone beam CT system for delivery verification in pelvic radiotherapy

Cone-beam computed tomography (CBCT) is a three-dimensional imaging modality that has recently become available on linear accelerators for radiotherapy patient position verification. It was the aim of the present study to implement simple strategies for reduction of the dose delivered in a commercial CBCT system. The dose delivered in a CBCT procedure (Varian, half-fan acquisition, 650 projections, 125 kVp) was assessed using a cylindrical Perspex phantom (diameter, 32 cm) with a calibrated Farmer type ionisation chamber. A copper filter (thickness, 0.15 mm) was introduced increasing the half value layer of the beam from 5.5 mm Al to 8 mm Al. Image quality and noise were assessed using an image quality phantom (CatPhan) while the exposure settings per projection were varied from 25 ms/80 mA to 2 ms/2 mA per projection. Using the copper filter reduced the dose to the phantom from approximately 45 mGy to 30 mGy at standard settings (centre/periphery weighting 1/3 to 2/3). Multiple CBCT images were acquired for six patients with pelvic malignancies to compare CBCTs with and without a copper filter. Although the reconstructed image is somewhat noisier with the filter, it features similar contrast in the centre of the patient and was often preferred by the radiation oncologist because of greater image uniformity. The X-ray shutters were adjusted to the minimum size required to obtain the desired image volume for a given patient diameter. The simple methods described here reduce the effective dose to patients undergoing daily CBCT and are easy to implement, and initial evidence suggests that they do not affect the ability to identify soft tissue for the purpose of treatment verification.

Cone Beam Computed Tomography(CBCT) is emerging as an alternative modality for radiotherapy treatment simulation and verification. Evaluation of CBCT performance for radiotherapy applications has been confined to in‐house built or modified CBCT devices. In this work, we investigated the imaging performance of a commercial (Varian Acuity) simulator having CBCT capability. The objective of this study was to compare its performance to that of a conventional CT simulator (Picker PQ5000) used for acquiring 3D data for generating radiotherapy treatment plans. Specifically the tests conducted include: density resolution, temporal stability, scan uniformity and spatial linearity, effect of varying scan widths, resolution (high and low contrast), and noise measurements, performed with a Catphan phantom. The variation in density resolution that is representative of the linearity in CT numbers was negligible for both modalities except for high density materials. Operational stability monitored by routinely tracking the CT numbers showed variations <14%, which we predict based on other studies, will result in only <2% uncertainty in dose calculations. Spatial resolution measurements showed better resolution capability for CBCT unlike for low contrast resolution, which was better for conventional CT. These results indicate that CBCT is comparable to CT and demonstrates its potential for treatment planning applications. We will explore this possibility further in future work. Currently, neither AAPM nor CAPCA, have standards for evaluating CBCT performance tolerances. Hence, we also anticipate that this study will provide guidelines for these documents when establishing tolerances for using commercial CBCT simulators.
