Towers of Kuwait Arabic Neurocognitive Assessment: A novel executive and visuospatial functions assessment tool added to the CERAD neuropsychological battery-Arabic version (CERAD-ArNB)

Assessment of executive and visuospatial neurocognitive domains is lacking in the Omani population, especially for elderly individuals with low educational levels. Therefore, the Towers of Kuwait-Arabic Neurocognitive Assessment (ToK-ArNA) was developed using similar psychometric features of Tower of London (ToL) test, and the unique architecture of Towers of Kuwait, with the potential to overcome the limitation of ToL color perception difficulties that might be encountered in subjects with hereditary or acquired color vision disorders. We enrolled 120 older Arabic-speaking Omanis from January 2022 to November 2022 and all participants underwent screening to ensure normal cognitive function before performing the ToL and ToK-ArNA tests. Validity, reliability, and non-parametric statistical tests were used for data analysis. A total of 85 participants, 51 men (60%) and 34 women (40%) met the inclusion criteria and completed the testing. Statistical analyses confirmed the validity and reliability of ToK-ArNA compared to ToL, with comparable total Time and Accuracy scores and more preference towered the ToK-ArNA among the participants. Despite the study limitations, these results indicate that the ToK-ArNA is a reliable and applicable executive and visuospatial function assessment tool and further studies are warranted to establish its validity in patients with various neurocognitive disorders

Color vision of people with headaches

The aim of the study is to obtain new data on possible changes in color vision of people with headaches. The object of the study is people who periodically experience primary headaches of different nature. The subject of the study is the specificity of perception of chromatic parameters of the environment and color sensitivity of people with headaches of different nature, quality, intensity and frequency. The data were collected using the Farnsworth-Munsell standard clinical color vision test (FM-100). The study involved 65 people aged 17 to 66 years. The results of the assessment of color vision in the experimental group were compared with the normative indicators and data obtained in the age-matched control group (N=40). Special attention was paid to discussing the possible causes of the identified disorders and analyzing the correlation between various aspects of headache with individual indicators of color sensitivity. The data analysis included calculating the total error (TES), as well as partial errors (PES) along the blue-yellow (B-Y) and red-green (R-G) axes. We also identified partial errors for individual tones. The study revealed a noticeable specificity in the perception of blue-green and blue shades of patients with severe, chronic and non-localized headaches. Since the detected color vision disorders appear to be temporary and are most likely typical of the premonitory phase of headaches, the results of this study may contribute to understanding of headache predictors. Since the identified changes relate to only one group of (blue) shades, the findings of the study can be used in the development of alternative treatment methods. In particular, in choosing the optimal color of tinted lenses, which are offered to patients to relieve headaches and reduce their frequency.

Diagnosis of color vision in children with congenital partial atrophy of the optic nerve

Background. The study of color vision is of great importance in the diagnosis and monitoring of visual functions in patients with of the partial atrophy of optic nerve (PAON). Due to the fact that PAON is one of the main causes of blindness and low vision in children, there is no doubt about the importance of effective diagnosis of color vision not in children with this pathology.Purpose: to evaluate the effectiveness of the diagnosis of color vision in children with congenital partial atrophy of the optic nerve using developed own tests in comparison with classical methods. The Rabkin and Neitz-test tables create conditions under which the examined child is given two tasks at once – color discrimination and shape identification. At the same time, the integration of information about color and shape may be difficult in children with PAON.Materials and methods. In 2020–2022 years 72 school-age children were observed, who, after a standard ophthalmological examination, were divided into two groups: 1) 37 children with congenital bilateral PAON; 2) 35 children of the control group with no pathology of the fundus and normal indicators of visual functions. To study color vision, we used our own developed test images (Patent RU 2760085 of 02.04.2021), as well as classical tests – polychromatic tables E.B. Rabkin and Neitz-test.Results. In the control group, when studying color vision according to Rabkin tables, four children had some difficulties with determining the shape of test figure in three of the 27 main tables. At the same time, the children named the colors of individual circles that make up the images correctly. In the Neitz-test, only one child did not distinguish between the shapes of brown and green tones of minimal saturation. The other children correctly identified the colored shapes in all the test images. The study with the developed tests did not cause any difficulties for any of the children of the control group. With minimal saturation, all children distinguished chromatic images from achromatic ones and correctly distinguished shades. In the group of children with PAON in the study with classical tests, 15 (40.5%) children experienced significant difficulties with determining the shape of the test figure in some Rabkin tables (while correctly naming the colors of individual circles) and 12 (32.4%) children – in Neitz-test images. Normal trichromasia was detected in 18 (48.6%) children and in 4 (10.8%) children – abnormal trichromasia according to both Rabkin’s tables and Neitz-test. With the developed tests, 6 (16.2%) children had color vision disorders. At the same time, abnormal trichromasia was detected in 4 of them according to the Rabkin and Neitz-test tables.Conclusion. The test images developed by us are easy to perform and do not pose a difficult visual task for the child to identify the chromatic shape. In this regard, they allow for effective diagnosis of color vision in children in normal and ophthalmopathology, and are also promising for use in children not only of school age, but also of younger age.

Purpose. The purpose of this study is to develop a new digital colour vision test. Material and Methods. Based on the principle of metamerism, a digital testing strategy was developed for efficient measurement of colour vision. Twentyfive subjects participated in the study, 21 of whom had normal colour vision and four of whom had a congenital colour vision disorder. Differences in colour vision were examined by monocular presentation of halfquadrants of different hues and degrees of saturation, and the algorithm calculated the colour vision defects that occurred based on the subjects’ responses. The following colour vision values were assigned from the data: a colour scale range of 2.76 – 7.18 for normal colour vision and greater than 7.18 for colour sense disorders. A second new colour scale assessed the type of colour vision disturbance: in the range of 0 – 0.3 were values for a deuteranomaly, in the range of 0.6 – 1.0 were values for a protanomaly, and a value of 1.0 corresponded to a tritanomaly. Results. The difference in colour vision between subjects with normal colour vision and those with a colour vision defect was confirmed (p < 0.001) by measuring the type and the extent of the colour vision disorder was determined. In the group of subjects with normal colour perception, a mean colour scale value of 3.41 ± 0.52 was determined. The extent of colour sense disturbance for the subjects with colour vision impairment was values of 7.18 – 14.33 according to the colour scale, indicating greater variability. Conclusion. The developed algorithm provided meaningful results regarding the colour perception of the test persons. It was possible to differentiate between normal colour vision and colour vision disorder could be shown. Furthermore, the variation between subjects with normal colour vision could be evaluated. The Effect of Physical Eye Examinations on Job Options

Purpose: To investigate physical ophthalmological parameters relevant to job choice.Methods: I analyzed the ophthalmological demands imposed on those engaged in military service and on applicants for army officer, air force officer, naval officer, railroad officer, firefighting officer, police officer, state public officer, and pilot positions; the visual requirements for a driver’s license were also examined. Visual dysfunction was classified into loss of visual acuity, visual field defects, refractive errors, color vision disorders, strabismus, and other disorders. Additional restrictions imposed by each occupation were investigated.Results: Various standards were imposed on the occupational groups. Applicants to the Air Force Academy required 1.0 visual acuity; the visual acuity for applicants to the military and naval academies were ≥0.6, and those for fire and police personnel ≥0.8. The general public official regulations do not mention visual acuity. In terms of visual field defects, such defects must be <33% for firefighters and railroad drivers; no clear standard was set for other occupations. The importance of color vision differed by job type; pilots and railroad drivers must have full color vision. In terms of refractive error and strabismus, the standards differed among occupations.Conclusions: As the ophthalmological standards vary extensively by occupation, both ophthalmologists and clients must be aware of this information. The scientific basis of the various standards requires attention.

Application of Blue Filters Increases the Usefulness of Moreland Test in Anomaloscopic Color Vision Assessment for Blue-Green Color Range.

The effect of blue light filters on the anomaloscopic examination was analyzed. Thirty subjects (18–43 y, 20 female, 10 male) without color vision disorders were examined in 4 filter conditions: no filter (F-0), Blue Control Hoya (F-BC), Med-1 JZO (F-Med1) and 450 Eschenbach (F-450). Both Rayleigh test (red–green axis) and Moreland test (blue–green axis) were performed. Application of F-BC filter shows negligible effect on color vision perception in both tests. Contrary to this, the application of strong F-450 filter causes significant shift in Moreland test towards tritanopy and the decrease in correlations of Moreland parameters with Rayleigh test parameters. The application of medium strong F-Med1 filter causes the slight shift in Moreland test towards the center of the Moreland scale and increases the Spearman correlations between Moreland and Rayleigh test parameters. This observation suggests that the about 15–40% reduction of blue diode intensity in the Moreland test may be beneficial in detecting mild changes in color vision perception in the blue-green axis and may improve its usefulness in evaluating the color vision perception disorders accompanying different illnesses, such as diabetes, glaucoma, neuritis optica, or cataract. The discussion concerning the modifications of Moreland test construction is also presented.

Variability of Rayleigh and Moreland test results using anomaloscope in young adults without color vision disorders.

AimTo validate the reference ranges proposed by the manufacturer of the Oculus HMC Anomaloscope MR for Rayleigh and Moreland tests in healthy young adults.MethodThe manual Rayleigh (red-green) and the Moreland (blue-green) anomaloscope tests were performed on 90 healthy subjects (54 female, 36 male, 178 eyes) residing in Poland, aged between 18–45 years, and without color vision disorders (assessed with HRR test). The analyzed parameters for both the Rayleigh and the Moreland tests were as follows: the lower (R1/M1) and the upper (R2/M2) limits; the center (RC/MC) and the width (RW/MW) of the matching ranges.ResultsThe results of the Rayleigh test were similar to the values proposed in the anomaloscope user’s manual, however, with a small shift of RC and R2 towards the red color. The double-peak distribution of R2 with a small second peak (approximately at R2 = 52) was mainly due to the measurements in male subjects (nmale = 8, nfemale = 2), which suggests that this group might be diagnosed with subtle protanomaly. The results of the Moreland test showed a high MW which did not correspond to the reference range described in the anomaloscope user’s manual. The observed significant correlations between R1 and M1 suggest that the M1 parameter seems to be the best indicator of blue vision quality.ConclusionsOculus HMC Anomaloscope MR is a sensitive tool for detection of prot-deuteranomalies but the reference ranges for young adults require a certain adjustment towards the red color. The parameters obtained for the Moreland test varied significantly between the subjects and therefore the test should not be used as is to diagnose color vision deficits in the green-blue area (tritanomaly).

A study on the prevalence of ocular disorders among school going children in the age group of 5 to 12 years

The aim of this study is to assess the prevalence of various ocular disorders among school going children in the age group of 5 to12 years. : Vision screening is crucial for early detection of ocular disorders among young children thereby preventing the risk of uncorrectable dimunition of vision. Hence, our programme is planned as early detection of the causative factors in the childhood period and most effectively treat as early as possible.The purpose of the screening programme is to identify various ocular diseases in children like refractive errors, amblyopia, defective color vision, lid and adnexal disorders, vitamin A deficiency, allergic conjuctivitis, glaucoma, cataract and squint. Identification of such problems and its correction in early stage is crucial for their normal growth. A school based prospective cross sectional study design was adopted to examine children aged 5–12 years in randomly selected schools of Kollam district from January 2020 to March 2020. In this cross-sectional study, two Optometrist and two Ophthalmologist and other supportive staffs screened 3100 school children aged 5 to 12 years for vision and external eye examination. An Optometrist did the visual acuity testing and refraction. Detailed ophthalmic examination was done by an Ophthalmologist. The pattern of visual acuity, colour vision and ocular disorders were studied. Children needing further assessment were referred to the hospital and analysis of the data and data entry done using Microsoft office excel 2016 with Statistical software: R software (EZR 1.32).: A total number of 3100 students from 11 different schools were examined, 1570 of whom were males, and 1530 were females. The commonest eye disorder was refractive error 16.6% (514) in which most common was myopic astigmatism 78.2% (402) of which 322 were against the rule and 80 were with the rule astigmatism, followed by simple myopia 15.2%(78) simple hypermetropia 4.7%(25) and hypermetropic astigmatism 1.7%(9). Of these spectacles usage was found to be present only in 81.3% (418) students, rest 18.7%(96) were not aware of the refractive error. Next common eye disorder was allergic conjunctivitis 1.6%(49).Squint was present in 1.4%(42) children. Corneal opacity was present in 0.26%(8), defective color vision was there in 0.2%(6); lids and adnexal disorders 0.13%(4) and cataract 0.03%(1). There were no students with congenital anomalies, congenital glaucoma or Vitamin A deficiency. Uncorrected refractive error is the most common visual impairment found in school going children. There is a need to set up a regular school vision-screening programme to detect ocular disorders in order to prevent the burden on society on long term. Since most of this morbidity is either preventable or completely treatable, school screening is an effective method to reduce this burden.

MANAJEMEN LOW VISION PADA ANAK USIA SEKOLAH

Low vision according to the World Health Organization (WHO) is a person who has impaired visual function after treatment or standard refraction correction and has visual acuity less than 6/18 (20/60) until the light perception or field of view is less than 100 from the fixation point and is still can use residual vision to perform certain activities. Low vision cases according to WHO is 3-4 times greater than cases of blindness. The prevalence of blindness in children varies from region to region or country, ranging from 0.15 to 1.5 per 1000 children. Of the 1.4 million children who are blind, 90% are in developing countries. In addition, there are 7 million low vision children. The prevalence of low vision in children is 11 times more than blindness in children. Low vision in children can be caused by eye abnormalities both congenitally and including cataracts, glaucoma, retinitis pigmentosa, clouding of the cornea, and retinopathy of prematurity (ROP). The clinical symptoms are blurred vision, limited field of view, accompanied by functional complaints such as decreased contrast sensitivity, glare, and color vision disorders so that visual aids are needed in low vision. Low vision examination consists of anamnesis, visual acuity measurement (examination of visus distance and close range), visual field examination, contrast sensitivity test, glare examination, and color blindness test. In this paper, we report cases of low vision in children where the management uses low vision aids according to their needs. Screening of Colour Vision in Paediatric Population

The descriptive cross-sectional study was done to screen the children attending paediatric OPD of Saveetha Medical College and Hospital for colour vision deficiency and to find out the overall prevalence of CVD and compare the prevalence rate among males and females. One hundred seventy-five children in the age group 4 – 12 years attending paediatric OPD of Saveetha Medical College and Hospital were taken up for the study. Visual acuity and colour vision test using the Ishihara chart was conducted for all the children. Results were tabulated and statistically analysed. It was noted that 7 (4.14%) children were found to be colour deficient. Out of which 6 (7.68%) and 1 (1.10%) were Male and Female children respectively, Considering the religion, the higher prevalence of colour vision deficiency was found among Muslim children 4(14.8%) as compared to Hindu 2 (2.1%) and Christian 1 (2.1%) children. Early diagnosis of this defect helps them in minimising the potential problems they face in their everyday life, and enable them to make adaptive strategies to improve the quality of life and most importantly helps them in choosing appropriate professional choice in their career and creating awareness through education. Medical counselling will improve to reduce the occurrence of Colour Vision disorder.

Evaluation of visual function in patients with cataract, amblyopia, optic neuritis, and retinopathies

Purpose: The purpose was to describe the changes in the visual function of patients with cataract, optic neuritis, amblyopia, and retinopathies evaluated at the Hospital de San José, Bogotá, Colombia, between August 2015 and May 2019.Materials and Methods: This is a descriptive cross-sectional study. One hundred and ninety-six patients with a diagnosis of cataract, retinopathies, optic neuritis, and amblyopia were evaluated; the tests were performed to determine the visual function (visual acuity, stereopsis, contrast sensitivity, and color tests). Qualitative and quantitative characteristics were described.Results: Of the 196 patients, 17.3% had amblyopia of any cause, 16.8% optic neuritis, 40.8% cataract, and 25% retinopathies of multiple etiologies. Amblyopic patients presented less deterioration of the stereopsis (160 arc seconds), whereas patients with retinopathy presented more significant compromise (400 arc seconds). Patients with optic neuritis showed severe deterioration in contrast sensitivity; in the other pathologies, the decline was moderate. Nearly 42.4% of the patients with optic neuritis and 38% of the amblyopic patients did not present color vision disorders, whereas 32.6% and 31.2% of the patients with retinopathy and cataract showed tritanomaly.Conclusion: Visual acuity is not equivalent to visual function. Stereopsis, contrast sensitivity, and color vision are affected by pathologies such as amblyopia, optic neuritis, cataract, and retinopathies. These assumptions of visual function could be considered for studies that take into account all the factors that may compromise the visual performance and, accordingly, define early diagnosis strategies, in addition to therapeutic behaviors such as performing early cataract surgery and visual rehabilitation.

Pattern and Prevalence of Color Vision Disorders amongst Secondary School Students in Rivers State, Nigeria

Aims: To determine the prevalence and pattern of color vision disorders (CVD) amongst secondary school students in Rivers State.
 Study Design: A community based descriptive cross sectional study.
 Place and Duration of Study: Department of Ophthalmology, University of Port Harcourt Teaching Hospital from the 20th of January to the 30th of April 2014.
 Methodology: A community based descriptive cross sectional study where study subjects were selected using multi-stage random sampling technique with inclusion criteria of consenting students with visual acuity >6/24. Ethical clearance for test was obtained from institution. Socio-demographic data obtained using an interviewer administered questionnaire. A comprehensive ocular examination was done and color vision assessed using the Ishihara 24 plate 2009 edition and the Farnsworth Munsell D 15 test for those who failed the Ishihara test. The prevalence of color vision disorder was determined by those that failed the Ishihara test.
 Data obtained was analyzed using SPSS version 21. Mean and standard deviations were determined for age. The age groups gender, other demographic distribution of the subjects amongst other was presented using frequency tables and charts.
 Statistical significance was put at p ≤ 0.05.
 Results: 1000 students were studied which consisted of four hundred and ninety five males (n=495; 49.4%) and five hundred and six females (n=506; 50.6%), with a male female ratio of 1:1.02. Mean age of subjects was 14.3±1.8 years with an age range of 9-20 years. The prevalence of color vision disorders was 2.8% (p-value 0.000) and was higher in males (2.1%) than females (0.7%) (P-value 0.000). Deutan defects were the most predominant (1.8%) compared to protan defects (0.4%) and tritan defects (0.2%).
 Conclusion: The study showed a prevalence of color vision disorders of 2.8% with male preponderance among secondary school students. Deutan color vision defects were most prevalent.

ABSTRACT Objective: The goal of the study is to analyze the color vision acuity pattern in undergraduates of health courses and to discuss the impact of these diseases in this population. Color deficiencies interfere significantly in the daily routine of professionals in the health area who need to discern different color hues in several situations of their everyday practice. Methods: Sixty-four volunteers, undergraduates of health courses of the Federal University of Alfenas (UNIFAL-MG), participated in the study. One man was excluded because he did not fit the inclusion criteria. Two groups were analyzed according to sex with the Farnsworth Munsell 100-Hue test. Results: There were no significant differences between the eyes and between the groups analyzed. The color vision acuity pattern is between 35 and 40, according to the Total Error Score. The gender issue does not influence the general pattern of the color vision acuity of the health courses undergraduates when those with color vision disorders are removed. Conclusion: Screenings and guidance should be given to undergraduates of health courses so that, aware of their condition of presenting some type of color disorder, they shall make the appropriate decision on which career to follow so that such limitation does not interfere with the quality of their daily life.

Hereditary Optic Neuropathies

Hereditary optic nerve disorders are rare. For ophthalmologists, Leber's hereditary optic neuropathy (LHON) and autosomal dominant optic atrophy (ADOA) are of particular relevance. LHON and ADOA are diseases of the retinal ganglion cells and are caused by mitchochondrial dysfunction. LHON is based on mutations of the mitochondrial, ADOA of the nuclear DNA. LHON is a disease that usually leads to severe visual impairment (visual acuity < 0.1). Since there is an approved therapy for LHON (Idebenone [Raxone]), the diagnosis has to be confirmed immediately by means of molecular genetic diagnostics. ADOA usually shows less severe visual impairment, begins in childhood, and progresses gradually, often for a long time without any noticeable worsening. Family history (dominant heredity) and blue colour vision disorder are the leading indicators for the diagnosis of ADOA, which should be confirmed by molecular genetic testing. Optic nerve diseases can also occur in the context of hereditary, syndromic disorders. Ophthalmologically relevant are Wolfram syndrome, Friedreich's ataxia and Charcot-Marie-Tooth disease. In all hereditary optic nerve disorders, foods containing cyanide and smoking should be avoided. Excessive alcohol consumption is considered to be a trigger of LHON and possibly harmful in other hereditary optic neuropathies. In the management of patients with hereditary optic nerve disorders, aspects of rehabilitation as well as social and psychological care should be considered.

Automated Diagnostic Method of Color Vision Disorders Integrated with a Hospital Information System

Visual perception disorders diagnostics is a set of complex medical procedures. Those designated to diagnose color vision, particularly Farnsworth-Munsell 100 Hue Test, are in most cases performed manually, thus not only being time-consuming but also prone to errors and inaccuracy. Therefore, such manual methods are not used as often, even though they are powerful in nature.This paper proposes a software-based approach to color vision disorders diagnostics which automates and simplifies the medical examination procedure, and allows for almost immediate results analysis. Its utilization is not only limited to hospital environment as the software application can also be deployed on mobile devices. Results of statistical analysis show that use of the designed software for color vision disorders diagnosis provides significantly better results in terms of accuracy and speed. With an user-friendly interface, the application could become a tool which would enable patients to be examined on more frequent basis so as to see trends of disorder progression, and thus being more responsive in tailoring the medical treatment.

In many circumstances, concepts, ideas and emotions are mainly conveyed by colors. Color vision disorders can heavily limit the user experience in accessing Information Society. Therefore, color vision impairments should be taken into account in order to make information and services accessible to a broader audience. The task is not easy for designers that generally are not affected by any color vision disorder. In any case, the design of accessible user interfaces should not lead to to boring color schemes. The selection of appealing and harmonic color combinations should be preserved. In past research we investigated a generative approach led by evolutionary computing in supporting interface designers to make colors accessible to impaired users. This approach has also been followed by other authors. The contribution of this paper is to provide an experimental validation to the claim that this approach is actually beneficial to designers and users.

Using 2D and 3D Computer Games to Detect Colorblindness – a Comparative Study

Abstract Computer games have accompanied the development of computer technologies since the very beginning. Despite their basic, purely entertainment-targeted appliance, games can also be used for many other purposes. Medical applications are especially interesting, as games (especially different kinds of simulations) are widely used for training personnel, e.g. to perform certain procedures or in learning to use equipment. This allows the trainees to gain knowledge and proper habits, as well as test themselves in different situations without any risk. Computer games can also be used as a diagnostic tool, although this topic is still insufficiently researched. This paper discusses the possibility of using serious games for diagnosing color vision disorders, focusing especially on two problems: differences in diagnosing colorblindness using 2D and 3D environments, and the influence of individual features, such as reflex or agility, on the diagnostic process.

Three family cases with Retinitis pigmentosa rod‐cone dystrophy with autosomal dominant inheritance

PurposeThree family cases with Retinitis pigmentosa (RP) rod‐cone dystrophy with autosomal dominant inheritance according to American Academy of Ophthalmology classification were diagnosed and studied: the grandmother G. D., 83 years old; the son A.D., 61 years old and granddaughter A.B., 27 years old.MethodsThe diagnosis was confirmed clinically, ERG, OCT, Angiography, (with colour tests) ATL, Ishihara table and Rabkin chart, All color Anomaloscope IF‐2 TOMEY, Test 28 HUE de Roth Farnsworth‐Munsell, with kinetic Kugel perimeter and automated Humphrey perimeter using the SITA‐Fast computerized perimetric threshold strategy.ResultsDuring the photopic test with ERG, there was observed retinogram with normal morphology and polarity but with drastic decline of the amplitude by 50 mV on average. In the process of dark adaptation, a decline of bioelectric activity was detected: from subnormal to non‐recording ERG during white and blue flashes at 30 Hz. In all of the three different generations’ cases, there was established reduction in residual bioelectric activity with the advancing of age of the patients with progressive degenerative RP disease. During the colour test, the grandmother and the father could read only ATL, which proves there was no acquired colour vision disorder. This is in confirmation to the autosomal dominant inheritance. In the process of anomaloscopic test and the colour tables and charts of Ishihara and Rabkin, as well as Test 28 HUE de Roth Farnsworth‐Munsell, the grandmother and the father were found to be with deuteranomaly. The granddaughter had a normal trichromacy. The perimetric tests showed concentric narrowing of the visual field to tubular vision.ConclusionsSerum for genetic tests was taken from the three family cases in order to confirm the diagnosis and administer a gene therapy.
