Letter from the Editor

Dear Reader, In 2023, the Columbia University Journal of Global Health continued to impact the Columbia campus community and beyond. Through leading discussions in public health and engaging with faculty across institutions, we strive to share research perspectives from around the world that confront the challenges of achieving equitable and accessible healthcare. We continue to celebrate the importance of research and the underlying rigor of the scientific process. We are excited to share our newest Spring 2023 issue with our readers. Our community engagement efforts in New York resulted in multiple collaborations with Columbia University Mailman School of Public Health faculty and the New York City Fire Department, which has engaged the Columbia University student body in relevant and timely global health topics. We distributed fentanyl testing kits and partnered with the New York City Fire Department for a CPR training event to educate students with useful skills in addressing health emergencies. In February, we hosted a panel discussion titled “Black Professionals in Public Health”, which featured guest speakers Dr. Paris Adkins-Jackson–Assistant Professor at the Columbia Department of Epidemiology and Sociomedical Sciences–and Dr. Kiros Berhane–Chairman of Biostatistics at the Columbia Mailman School of Public Health. Adkins-Jackson and Berhane addressed how they designed research studies to investigate racial disparities in access to care and navigated public health and academia from a personal perspective. In recognizing the importance of having accessible and open scientific communication between the scientific community and the general public, our “What is Global Health?” podcast and blog has served as an active online platform to foster discussions with Columbia faculty. Our recent blog posts share perspectives on topics from women’s menstrual health to global hunger. The newest episode of the podcast features a discussion on utilizing data science to inform public health with Dr. Mary Beth Terry from Columbia University’s Mailman School of Public Health. This Spring 2023 issue explores public health topics including a novel community participatory approach to violence intervention programs for Latino youth, challenges in accessing maternity care for immigrants and medical interpretation. We would like to express our gratitude towards our journal team, faculty advisors Esther Jackson and Professor James Colgrove, authors and peer reviewers for their submissions and contributions without which our journal could not exist. Natural Property Rights: An Introduction

This Article introduces a symposium hosted by the Texas A&M University Journal of Property Law. The symposium is on a forthcoming book, and in that book the author introduces and defends a theory of property relying on labor, natural rights, and mine-run principles of natural law. Parts I and II of the Article preview the main claims of the book, summarizing part by part and chapter by chapter.
 The rest of the Article illustrates how the theory introduced in the book applies to a contemporary resource dispute. The Article studies an ongoing lawsuit styled Campo v. United States, now pending in federal court. In Campo, oyster producers are suing the United States for inverse condemnation. The class plaintiffs seek $1.6 billion in just compensation for the U.S. Army Corps of Engineers having (allegedly) killed oysters they were raising when it diverted water from the Mississippi River through a spillway into the Gulf Coast.
 The Campo case repays study for two reasons. Labor and natural rights are already at play in the Campo litigation. The U.S. government moved to dismiss the plaintiffs’ claims to property in oysters, and when the presiding judge denied that motion he relied in part on natural rights and the labor theory John Locke introduced in his Second Treatise of Government. Separately, the underlying dispute fairly tests any general theory of property. The dispute raises questions about: whether oysters should be private property; whether people should be allowed to claim private property in coastal water bottoms; how property in oysters and water bottoms should be reconciled with public interests in shoreline protection; why have an institution like eminent domain; and how nuisance law should apply to a government-sponsored water diversion into coastal areas. If a theory of property can shed helpful light on all of those issues, it applies broadly enough to constitute a general theory of property, and the theory introduced in this Article satisfies that standard. Along the way, this Article also shows how a labor- and rights-based property theory differs from justifications for property and regulation influential in contemporary law, policy, and scholarship.

Natural Property Rights: A Reply

This Reply concludes the symposium hosted by the Texas A&M University Journal of Property Law on the author’s forthcoming book Natural Property Rights. The Reply shows how natural law and rights apply to a wide range of doctrinal examples raised in this symposium—including business associations, correlative oil rights, timber extraction, sinking coastlands, water law, nuisance law, property rights in subsurface minerals, and the issues about sovereignty and property disposition associated with Johnson v. M’Intosh (1823). The Reply also addresses a wide range of skeptical objections to natural law—especially the arguments that it relies too much on intuitions and not enough on hard empirical data. The Reply responds to objections to natural rights familiar from law and economic scholarship—and rehearses important but often-neglected reasons why economic analysis of law needs support from moral and political theory. And the Reply responds to criticisms of rights theories typical from Progressive property scholarship—and argues that a Lockean theory of rights is more sober and tougher-minded than Progressive theories are about whether and how much law can secure justice in practice.

Post-operative sonological evaluation of pelvi-ureteric drainage of unilateral A-H pyeloplasty in children

Background: Pelvi-ureteric junction obstruction is the most common cause of pediatric hydronephrosis where A-H pyeloplasty with D-J stent is the established treatment option with a high success rate. We observed the predictive value of sonological parameters for early detection of postoperative obstruction at the new pelvi-ureteric junction in children following unilateral A-H pyeloplasty. Methods: This cross-sectional study included 12 children who underwent unilateral A-H pyeloplasty. Post-operative follow-up was done after D-J stent removal and completed within six months. Maximum antero-posterior pelvic diameter (APPD), cortical thickness (CT), and pelvi-cortical (P/C) ratio were compared. Results: Mean APPD was 32.7 mm at the day after removal of the stent, 24.4 mm at one month, and 19.7 mm at four months. Mean CT was 5.2 mm at the day after removal of the stent, 6.1 mm at one month and 8.0 mm at four months. P/C ratio was 8.3 mm at the day after removal of the stent, 5.2 mm at one month, and 3.4 mm at four months. Increased CT and reduced P/C ratio were significant at four months (P= 0.05). Conclusion: CT value and P/C ratio can be used as an early marker of success for pelvi-ureteric drainage following A-H pyeloplasty.

Utility of multiplex real-time polymerase chain reaction for the detection of bacteria from sputum samples of community acquired pneumonia patients: A study from Dhaka, Bangladesh

Background: This study was carried out to evaluate the utility of multiplex real-time polymerase chain reaction (PCR) to identify the common bacterial agents of community acquired pneumonia (CAP). Methods: Sputum and blood samples were collected from 80 clinically suspected CAP patients in three tertiary-level hospitals in Dhaka city. Multiplex real-time PCR assay was carried out to simultaneously detect five common bacterial agents of CAP; Streptococcus pneumoniae, Haemophilus influenzae, Mycoplasma pneumoniae, Chlamydophila pneumoniae, and Legionella pneumophila. Routine microbiological methods and serology were carried out. The results of PCR were compared with culture, Gram stain and serology. Results: Among the 80 patients, sputum samples of 35 (43.7%) patients were positive by PCR, of which the most commonly detected bacteria were S. pneumoniae (25/35, 71.4%), followed by H. influenzae (9/35, 25.7%) and L. pneumophila (1/35, 2.9%). All 80 sputum samples were negative for both M. pneumoniae and C. pneumoniae by PCR. Out of the 26 culture positive sputum samples, 8 (30.7%) were positive for S. pneumoniae and 1 (3.8%) was positive for H. influenzae. Among the 52 Gram stain valid sputum samples, 24 (46.1%) were S. pneumoniae and 7 (13.5%) were H. influenzae. By serology, out of the 80 cases, M. pneumoniae was detected in 32 (40%) and C. pneumoniae in 24 (30%) of cases. Mixed infections comprised of 38.8% (31/80) cases. Conclusion: Multiplex real-time PCR is useful for the rapid and simultaneous detection of bacterial pathogens of CAP in sputum and can help support traditional laboratory methods for the accurate diagnosis of CAP patients.

Hemophilia in a newborn without family history: A case report

Haemophilia is the most frequent diagnosed inborn clotting factor deficiency in newborn. Studies reveal that majority of hemophilia A cases are due to an inversion of the long arm of X-chromosome. Newborns with hemophilia are at risk of intracranial and extracranial hemorrhage, and other bleeding complications. Diagnostic criteria for haemophilia include confirmation of a factor activity level below 40% of normal (below 0.40 IU/mL), or a hemophilia gene mutation. Haemophilia typically result in an isolated prolongation of the activated partial thromboplastin time. However, the definitive diagnosis requires measurement of factors VIII and IX levels. Here, we describe a newborn with cephalhematoma gastrointestinal bleeding that is the manifestation of neonatal haemophilia with antenatally diagnosed duodenal atresia and there is no positive family history. This case shows a rare hemophilia presentation reflecting the importance of coagulation studies when faced with unexplained bleeding.

Vincristine induced peripheral neuropathy in children undergoing chemotherapy for acute lymphoblastic leukaemia during induction

Background: Vincristine is an anticancer agent administered to all children with acute lymphoblastic leukemia (ALL), and peripheral neuropathy is the major dose-limiting toxicity of this therapy. As cure rates of childhood ALL exceeds 80%, therefore treatment-related toxicities need to be reduced. Thus, the aim of this study was to determine the prevalence and risk factors of vincristine-induced peripheral neuropathy (VIPN) in children with ALL undergoing induction chemotherapy. Methods: A case-control study was conducted from September 2017 to August 2018 in the Department of Paediatric Haematology and Oncology at Bangabandhu Sheikh Mujib Medical University, Dhaka, Bangladesh. Eighty newly diagnosed ALL and 35 acute myeloid leukemia (AML) cases aged 5 to 17 years with no pre-existing neurological abnormality were recruited. To assess the peripheral neuropathy, we used pediatric-modified total neuropathy score and National Cancer Institute- Common Terminology Criteria for Adverse Events (NCI-CTCAE), version-04 grade. Results: Among ALL patients, 29.2% developed peripheral neuropathy compared to 10% in AML control group (P=0.04). Higher proportion (57.1%) of peripheral neuropathy was found in age below 10 years (P<0.001). There was no significant association of peripheral neuropathy with sex and body mass index of the patients. Conclusion: Almost 3 in 10 patients developed VIPN during the induction therapy which is significantly higher in age below 10 years compared to ≥ 10 years.

Polya partial gastrectomy for megaduodenum: A case report

Megaduodenum is an extremely rare condition in children presenting with features of duodenal obstruction. It may be associated with bands and adhesions. However, when the obstruction is chronic and functional, mere excision of the band and adhesionolysis is not curative. Some form of bypass procedure is required for its successful treatment. Here we are describing polya partial gastrectomy in a 13-year-old girl suffering from megadoudenum for its successful treatment.

Management of perforated corneal ulcer with iris prolapse by cyanoacrylate glue: A case report

We report a rare case of perforated corneal ulcer due to recurrent viral keratitis with stromal necrosis and iris prolapse. The patient was 75 years old farmer, normotensive, diabetic, and presented with complaints of pain, watering, photophobia, redness and dimness of vision in the left eye. The left eye's visual acuity was perception of light and projection of rays in all four quadrants. Slit lamp examination found swollen eyelids, matted eyelashes, congested conjunctiva, seidel test positive, an inferonasal corneal perforation, shallow anterior chamber, irregular pupil, and prolapse of the iris through the perforated cornea. The patient was diagnosed with a left-sided perforated corneal ulcer (recurrent viral keratitis with stromal necrotizing variety) having iris prolapse. The perforation was sealed by cyanoacrylate glue and a soft bandage contact lens. This improved the patient’s condition. Early medical and surgical interventions thus can save vision as well as the eyeball.

Fasting with diabetes during Ramadan: An updated review

Introduction: In reality, many Muslims have an intense spiritual urge to participate in fasting, even those who could get an exemption. There are many cultural variations in the Muslim population in different parts of the world, even in the same country with different social and family backgrounds. Thus, persons with diabetes should seek medical advice before planning to fast or proceed to fast during Ramadan. Materials: This was a narrative, non-systematic review of the international literature from the major medical online database (PubMed and Google Scholar) in 2023. The term “Ramadan fasting” and “Diabetes” was used to search, and the relevant literature was captured and narrated in a concise thematic account. Results: Practically, there are certain risks associated with fasting for patients with diabetes, which might exacerbate their existing illnesses. Along with the globally increasing prevalence of diabetes, there is an increased number of the population who intend to or fast. The article captured a wide array of topics related to Ramadan fasting by patients with diabetes, including its epidemiology, risk stratification, lifestyle modification, assessment of safety and efficacy profiles of pharmacotherapies, hypo or hyperglycemia, and the impact of diabetes education. Conclusion: Fasting with diabetes poses a range of risks and thus pre-Ramadan risk stratification is essential. Particular attention should focus on the patient’s current risk factors, complications, comorbidities, and the roles of newer antihyperglycemic medicines and advanced technology for safer fasting practices to mitigate the risks of hypoglycemia and or hyperglycemic crises.

High prevalence of musculoskeletal disorders among Bangladeshi women deserves attention

The high prevalence of MSDs among Bangladeshi adults especially women are a significant public health issue that needs urgent attention. Change of lipid profile in children with acute lymphoblastic leukemia due to induction chemotherapy in a tertiary care hospital of Bangladesh

Background: Acute lymphoblastic leukemia (ALL) is the most common childhood malignancy. In the Department of Paediatric Haematology and Oncology of Bangabandhu Sheikh Mujib Medical University (BSMMU), 58% of ALL cases were recorded among 455 newly diagnosed malignancy patients in a single year. Studies found that remarkable hypertriglyceridemia occurs with L-asparaginase therapy and steroid. This study was done to evaluate the changes of serum total cholesterol, triglycerides (TG), high density lipoprotein (HDL), and low-density lipoprotein (LDL) during and after induction chemotherapy in children with ALL. Methods: This prospective observational study was performed in the Department of Pediatric Hematology and Oncology of BSMMU from March-November 2013. Newly diagnosed acute lymphoblastic leukemia patients aged 3-15 years were included in this study after having written consent from the parents of the participants to participate in the study and enrolled for the treatment of ALL (according to modified UKALL 2003 protocol). Results: Total cholesterol, TG, HDL, and LDL changed significantly due to induction therapy. Serum total cholesterol and LDL decreased after completion of L-asparaginse in comparison to before induction, increased significantly after completion of induction in comparison to after completion of L-asparaginase (P=0.001), and increased significantly after induction in relation to before induction therapy (P=0.003). TG decreased significantly (P=0.033) after completion of L-asparaginase than before induction but increased after completion of induction. HDL increased after completion of L-asparaginase and after induction significantly (P=0.001). LDL decreased after completion of L asparaginase which was significant (P=0.005). Conclusion: After induction chemotherapy, total cholesterol, HDL and LDL level increased and TG level decreased among ALL patients.

Degree of fibrosis and its association with angiogenesis in the myelofibrotic bone marrow

Background: Primary and secondary myelofibrosis has become a global burden due to its increased mortality and morbidity. Angiogenesis is a significant driving force in the development of fibrogenesis in the bone marrow, which leads to myelofibrosis. The microvascular density (MVD) with immunomarker CD34 can be used to assess the degree of angiogenesis. The objective of this study was to examine the association between degree of myelofibrosis and angiogenesis in hematological malignancies. Methods: Forty-six trephine biopsy specimens of various hematological malignancies with myelofibrosis were studied at the Department of Pathology of Bangabandhu Sheikh Mujib Medical University. Extent of myelofibrosis in each case was assessed by examining the reticulin and Masson’s trichrome stained sections using a semiquantitative grading system of bone marrow fibrosis (MF) within a scale of MF-0 to MF-3. Angiogenesis was measured by counting MVD in the ‘hotspots’ after immunostaining with CD34 antibody. Results: The trephine biopsy cases were grouped into early fibrotic (MF-1) and advanced fibrotic (MF-2,3) consisting of 16 (34.8%) and 30 (65.2%) patients, respectively. Angiogenesis was estimated as mean MVD count which revealed 16.7 ± 5.4 and 32.0 ± 11.5 in these groups, respectively. Significant difference of mean MVD values (P<0.001) between the early and advanced fibrotic groups revealed the association of angiogenesis and degree of myelofibrosis. Conclusion: MVD may be used to measure angiogenesis in myelofibrotic marrow along with other clinical and laboratory indices as a marker of disease activity in hematological malignancies, thus aiding disease prognosis.

Data and reproducibility. Interpretation of international guidelines and journals’ best policies and practices

Relevance. The 2022 Update of the COPE, DOAJ, OASPA, and WAME joint guidelines on The Principles of Transparency and Best Practice in Scholarly Publishing encourages journals to establish their own policy in “data sharing and reproducibility” (DS&R). However, this document neither provides detailed recommendations / templates nor explains the reproducibility crisis phenomenon.Objectives. To analyze and interpret the international guidelines, the best practices of global publishers and journals, as well as typical mistakes and experience of selected Russian journals, to help a journal develop its own DS&R policy and its implementation.Materials and methods. The analysis of various sources (literature, reporting guidelines, data repositories), policies of 83 Russian university journals, as well as policies of the top 5 international publishers and their journals. Interviews with 6 editors-in-chief of Russian journals regarding DS&R.Results. All the top 5 global publishers in their DS&R policy adapt the TOP Guidelines and offer their own data sharing statement templates. Discussion and interpretations. The author suggests Russian translation of the TOP Guidelines and the main templates (e.g., data sharing statement). He also discusses 9 best journal policies and practices (including pre-registration studies).Conclusions. Numerous international sources, as well as the experience of selected Russian journals, demonstrate that the implementation of the DS&R policy increases articles citation (averagely by 25.3%), the growth of journal’s bibliometric and altmetric indicators, and also contributes to the trust of the target audience. As a result, it strengthes the journal portfolio to enable publishing articles well ahead of schedule. However, only the declarative statement of DS&R policies by journals without proper implementation does not bring tangible benefits to the journals.

In September 2023, the Dalat University Journal of Science (DLU-JOS) became aware that the article had previously been published elsewhere by the same author. This matter may have arisen from the practice of submitting identical studies nearly simultaneously, either in the same or another language, to two journals. We have reason to believe that this issue was due to the author’s inadvertent error rather than research misconduct. In accordance with the international guidelines established by the Committee on Publication Ethics, of which the DLU-JOS is a member, we have determined that these are grounds for retraction. The author has agreed to the retraction of the paper. The DLU-JOS regrets this incident and extends its apologies to its readers.

Use of scientific journals in Spanish universities: analysis of the relationship between citations and downloads in two university library consortia

We analyse the relationship between downloads of electronic journals included in four big deal bundles subscribed to by public university libraries affiliated to two library consortia in Spain (Castile and León and Galicia) and citations of the same journals by researchers at these universities. Download data on the big deals analysed (Emerald, ScienceDirect, Springer and Wiley) were obtained from COUNTER Journal Reports 1, and citation data were obtained from the bibliographic references given in articles indexed in Scopus between 2010 and 2017. The results show that only a low percentage of the subscribed journals was used in the scientific output of the universities’ researchers, with values ranging from 15 to 50%, and that there was a strong correlation between the universities’ volume of scientific production and the percentage of cited journals. We also found a strong correlation between downloads and citations, which was higher in the case of universities with a higher scientific output.
