A Coalition to Advance Treatments for Parkinson's Disease, Dementia with Lewy Bodies, and Related Disorders.

Parkinson's disease (PD) and dementia with Lewy bodies (DLB) share underlying neuropathology. Despite overlapping biology, therapeutic development has been approached separately for these clinical syndromes and there remains no treatment to slow, stop or prevent progression of clinical symptoms and development disability for people living with PD or DLB. Recent advances in biomarker tools, however, have paved new paths for biologic definition and staging of PD and DLB under a shared research framework. Patient-centered research funding organizations see the opportunity for a novel biological staging system for PD and DLB to accelerate and increase success of therapeutic development for the patient communities they serve. Amid growing momentum in the field to develop biological definitions for these neurodegenerative diseases, 7 international nonprofit organizations focused on PD and DLB came together to drive multistakeholder discussion and input on a biological staging system for research. The impact of these convenings to date can be seen in changes incorporated into a proposed biological staging system and growing alignment within the field to rapidly apply new scientific knowledge and biomarker tools to inform clinical trial design. In working together, likeminded nonprofit partners who were initially catalyzed by the significant potential for a biological staging system also realized the power of a shared voice in calling the field to action and have since worked together to establish a coalition to advance precompetitive progress and reduce hurdles to developing better treatments for PD, DLB and biologically related disorders.

An Analysis of Factors That Influence the Success of Flood Risk Management in Selected River Basins (FMSRB) Program in Banten Province and Maluku Province

FMSRB Program, under CPIU Directorate General of Regional Development Ministry of Home Affairs has successfully built 2,907 units of infrastructure, consisting of: (1) 152 units of retaining walls; (2) 24 units of reservoirs; (3) 265 units of drainages; (4) 56 units of rainfall harvesting system; (5) 46 units of retention wells; (6) 2,351 units of biopori; and (7) 13 units evacuation routes. The construction of infrastructure is in collaboration with the establishment of 4 KMSB forums in the regency, 140 KMSBs, and 140 waste banks in the community level. The success of program is primarily determined by 4 factors, including the success in the human resources management, collaboration among stakeholders, collaboration of structural and non-structural approaches, as well as the partnership development. Quantitatively, it is discovered that 55.70% of impact value indicates a very significant influence on flood control, whereas 36.80% states influential. For the construction of reservoir 57.50% of respondents state that it is high influence, whereas 33% state influential. In terms of rainfall harvesting system, 54.70% of respondents state highly influential, while 31.10% state influential. In Biopori, 60.40% of respondents state highly influential, whereas only 28.30% state influential. For retention well, 52.80% of respondents state very influential and only 38.70% states influential. In the meantime, the status of constructed infrastructure assets indicates that 65.10% are very good and 17.90% are good. Furthermore, the human resources management contributes β 0.141 with the influence of p-0,038 to the sustainability empowerment mediation. Collaboration among stakeholder is very good (59.40%) and good (25.50%). About gender and empowerment-based collaboration model, 65% of respondents fully agree and confirm, whereas only 35% agree. Meanwhile, collaboration among stakeholders is very helpful according to 65% of respondents and only 35% state helpful. As for structural and non-structural approaches collaboration, 82% of respondents state very helpful, and only 18% state helpful. For partnership, 59% of respondents fully agree with the partnership developed by FMSRB Program that uses the collaboration principle, and 41% agree. For sustainable program, advanced facilitation from the stakeholders to KMSB Forum, KMSB and Waste Bank is necessary. They will oversee utilization, maintenance, and development of the infrastructure for controlling flood.

Better Care, Same Cost - Reducing Unplanned Care for Multi-visit Patients: A Payer-Provider Model.

Many interventions implemented for multi-visit patients (MVP) have been developed to address patient-centric needs of these individuals and reduce unplanned care for ambulatory-sensitive conditions. More rigorous research is needed to better understand the impact of these interventions on changes in care utilization including unplanned care. To evaluate the impact of the Enhanced Care Program (ECP), a payer-provider collaborative model, on unplanned care use and cost of care. Using propensity methods, a comparison group was constructed using insurer membership files. Comparisons were performed using a difference-in-differences analysis. Patients enrolled in ECP through December 2019 were considered eligible for the study (n = 357). All patients had five or more ED visits in the past year or two or more inpatient hospitalizations in the past year prior to enrollment. ECP is a high-intensity outpatient intervention intended to reduce avoidable unplanned care such as ED visits and inpatient hospital stays through home visits, chronic/acute disease management, and intensive care coordination. The primary outcomes of interest were events per 100 members per year of ED use with return to home, unplanned inpatient and observational status admissions, and unplanned behavioral health inpatient admission, and cost of care per member per month. Overall total unplanned care encounters were significantly reduced with a difference-in-difference of 320 unplanned care encounters per 100 members per year in the intervention group (p < 0.05). The ECP group showed statistically significant decreases in costs of unplanned ED, unplanned observation admission, and unplanned inpatient behavioral medicine costs, but statistically significant increases in overall pharmacy costs and lab costs. Changes in total costs of care for the ECP group were not statistically different than the control group (p = 0.55). ECP showed significant reduction of unplanned care for MVP patients.

Bridging academia and industry in the era of Industry 4.0 by means of the triple helix: The PLANET4 initiative

Practical Learning of Artificial Intelligence on the Edge of Industry 4.0 (PLANET4) is a cross-disciplinary initiative funded by the European Commission under the Erasmus+ program that embodies the triple helix model of collaboration between academia, industry, and administration. It aims to bridge the gap between academic teaching and practical applications in the context of Industry 4.0. PLANET4 focuses on developing hard skills in artificial intelligence, industrial Internet of things, and cloud and edge computing, along with the soft competencies required to manage changes in the industrial ecosystem. It involves three phases: (1) training needs analysis, taxonomy development, and workshop design; (2) collection of best practices and training material design; and (3) implementation of the PLANET4 learning course. This paper presents the materialization of these phases into a blended learning course, highlighting the integration of the triple helix model and the European Commission’s support for academia-industry collaboration with the aim of improving education quality and promoting Industry 4.0 innovation in Europe.

Participation in Multidisciplinary Teams Among Healthcare Professionals: A Discrete Choice Experiment in Tertiary Public Hospitals in China.

Healthcare professionals' participation is crucial for the efficient implementation of multidisciplinary team (MDT) collaboration models. We identified the key factors influencing healthcare professionals' preference to participate in MDTs in tertiary hospitals. To clarify the attributes and levels of the discrete choice experiment (DCE), we conducted a targeted literature review and conducted in-depth interviews with MDT service providers. Following this, a DCE was designed to evaluate healthcare professionals' preferences for MDT participation, and the influence of factors such as salary subsidies, leadership attention, patient participation, quality assessment, working intensity, and case complexity. A conditional logit model estimated the utility of each attribute. Willingness-to-pay estimates were derived by taking the negative ratio of the coefficients of non-economic and economic attributes. A series of policy simulation analyses were conducted. Two hundred healthcare professionals completed the questionnaire, with 180 valid responses used for analysis. All attributes were statistically significant. Leadership attention and working intensity were the primary factors influencing staff willingness to participate in MDTs, followed by quality assessment and salary subsidies. Significant preference differences were observed between respondents; compared with mid-level staff, senior-level healthcare professionals believed patient engagement would be more helpful in boosting participation. The policy simulation showed that changing leadership attention from "neglect" to "emphasis" would increase the probability of staff choosing to participate in MDTs from 24.4% to 66.98%. Leadership attention was the primary concern for healthcare professionals in MDTs. To effectively motivate staff participation in MDTs, policymakers should adopt a holistic approach that considers work motivation and individual backgrounds, including competitive salary packages and a positive work environment. They should concurrently introduce MDT case complexity measurement tools to optimize resource allocation. Addressing staff members' unique needs and career aspirations by creating targeted training programs, pathways for advancement, and personalized career development plans are also crucial.

Essential Advance Care Planning Intervention Features in Low-Income Communities: A Qualitative Study

ContextOlder adults with low socioeconomic status (SES) participate in advance care planning (ACP) at lower rates than those with higher SES. Community feedback is an essential component of intervention design for communities with fewer social and health resources to ensure that the intervention is relevant and meaningful. ObjectivesTo understand the perspectives for potential interventions, we aimed to qualitatively explore participant priorities for ACP intervention development. MethodsUsing a qualitative descriptive design, we recruited and conducted individual and one-time, semi-structured interviews with older adults (aged 50+) with low income (< $20,000/year) (n = 20), Recruitment methods included flyers and in-person recruitment and purposive and snowball sampling methods. Following a thematic analysis plan, themes emerged from recursive transcript review by two independent coders and inductive categorization of the most robust codes. ResultsTwo themes captured participants’ perspectives regarding ACP intervention development: 1) specialist advocacy and reliability and 2) person-centered communication. Older adults with low SES prioritize ACP communication that is driven by their goals and that is led by trustworthy specialists that advocate for their needs. ConclusionOur work highlights that intervention preferences were informed by the prior strain and struggle of waiting on other kinds of health and social services. We propose an adapted model for community research collaboration to promote equity in addition to practice and policy recommendations.

Utilization of Traditional Healing Practices for Cancer Treatment: Benefits, Risks, and Integration with Modern Medicine

This review explores the utilization of traditional healing practices in cancer treatment, focusing on the benefits, risks, and integration with modern medicine. Traditional cancer treatments, including herbal remedies, spiritual interventions, and physical therapies, offer potential benefits like improved symptom management and emotional support. However, challenges like lack of standardization, scientific validation, and potential interactions with conventional treatments pose significant risks. The integration of traditional and modern medical approaches is emerging as a trend, aiming to improve treatment outcomes and patient quality of life. This requires careful consideration of safety, efficacy, and standardization. Effective integration involves developing collaborative care models, regulatory frameworks, and interdisciplinary partnerships. Future directions include advancing research to understand traditional remedies’ mechanisms, improving quality control, and exploring hybrid therapies. By fostering collaboration between traditional healers, researchers, and modern healthcare professionals, and addressing ethical and safety concerns, traditional practices can be harnessed while enhancing overall cancer care. Enhancing Patient-Centered Care in Nigeria: Challenges, Opportunities, and Future Directions

Patient-Centered Care (PCC) is a healthcare approach that prioritizes the needs, preferences, and values of patients, aiming to enhance the overall quality of care by actively involving patients in their healthcare decisions. In Nigeria, the implementation of PCC faces significant challenges due to socioeconomic disparities, cultural influences, resource limitations, and infrastructural constraints. Despite these obstacles, there are substantial opportunities to improve Nigeria’s healthcare system through the integration of PCC principles. This review provides a comprehensive analysis of the current state of PCC in Nigeria, examining the healthcare system’s structure, successful PCC models, and the barriers hindering its widespread adoption. The review also explores strategies for enhancing PCC, including community engagement, technological innovations, and collaborative care models, and highlights the impact of PCC on health outcomes, such as improved patient satisfaction and cost-effectiveness. Additionally, the review discusses the critical role of policy and governance in promoting PCC, offering insights from international experiences and presenting case studies that illustrate the successes and challenges of implementing PCC in Nigeria. By addressing these key aspects, the review outlines a roadmap for advancing PCC in Nigeria, emphasizing the need for supportive policies, capacity building, and continuous evaluation to align the healthcare system with global standards and improve patient outcomes. Patient Perspectives on Adapting Collaborative Care for Opioid Use Disorder With Depression and/or Post-traumatic Stress Disorder

Abstract: Background: Individuals with opioid use disorder and cooccurring mental health concerns experience heightened consequences and lower rates of treatment access. Engaging patients as research partners alongside health systems is critical for tailoring care for this population. Collaborative care is promising for the treatment of co-occurring disorders Objectives: We used a community-participatory partnered research1 approach to partner with patients, providers, and clinic administrators to adapt and implement a collaborative care intervention for co-occurring disorders in primary care. Methods: We conducted qualitative interviews with patients to assess their feedback on the proposed collaborative care model before intervention implementation. A first round of interviews was conducted to obtain patient feedback on our adaptation ideas ( n = 11). The team then incorporated these suggestions, beta-tested the intervention with participants ( n = 9), and assessed participant feedback. Data were analyzed using rapid content analysis and then implemented by health systems. Results: Patient feedback underscored the need for the care coordinator (CC) to be trained in patient engagement and stigma reduction and to provide assistance around socioeconomic barriers and relapse. Patients shared that it was helpful to have the CC address co-occurring disorders, emphasized the need for the CC to be flexible, and expressed that tele-health was acceptable. Patient feedback was integrated in subsequent CC training with health systems. Conclusions: The present research demonstrates the feasibility and usefulness of incorporating patient perspectives into treatment design and implementation in health systems using community-participatory partnered research .

Collaborating to heal addiction and mental health in primary care (CHAMP): A protocol for a hybrid type 2a trial

BackgroundThe gold-standard treatment for opioid use disorder (OUD) is medication for OUD (MOUD). However, less than a quarter of people with OUD initiate MOUD. Expanding the Collaborative Care Model (CoCM) to include primary care patients with OUD could improve access to and initiation of MOUD. This paper presents the methods and baseline sample characteristics of a Hybrid Type 2a trial comparing the effectiveness of CoCM for OUD and co-occurring mental health symptoms (MHS) to CoCM for MHS only. Method42 primary care clinics were cluster randomized and 254 primary care patients with OUD and elevated MHS were enrolled. Recruitment was terminated early by the Data and Safety Monitoring Board for futility. Participants completed research assessments at baseline, 3 months, and 6 months. The multiple primary outcomes were past-month number of days of nonmedical opioid use and SF12 Mental Health Component Summary (MCS) scores. ResultsMCS scores were over a standard deviation below the national mean (M = 34.5). Nearly half (47.6 %) of participants had previously overdosed in their lifetimes. Three quarters (76.0 %) were already being prescribed MOUD at baseline, only 30.4 % reported non-medical use of opioids, and only 33.9 % reported being bothered by opioid cravings. ConclusionThe unexpectedly high proportion of enrollees already prescribed MOUD at baseline indicates that most patients were in the maintenance rather than acute phase of treatment. Challenges identifying and enrolling patients in the acute phase of OUD treatment implies that intervention effectiveness will depend on its success preventing the discontinuation of MOUD rather than initiating MOUD.

Collaborative rescheduling of train timetables to relieve passenger congestions in an urban rail transit network: A rolling horizon approach

At some urban rail transit (URT) stations, large activities, emergencies, or holidays usually lead to a significant surge in passenger flow demand quickly, known as Large Passenger Flow (LPF) events. The passenger congestion will spread quickly via transfer stations and affect other stations and lines in the URT network. This study develops a timetable rescheduling and coordinating method for the URT network under LPF events. Firstly, a collaborative adjustment model of train timetables with an implement backup vehicle strategy is formulated to simultaneously consider rescheduling and coordinating problems, to reduce the congestion influence for an URT network. Then, a rolling horizon approach is developed to divide the whole adjustment problem into several decision-making stages to ensure solution efficiency. In each decision-making stage, the influence of LPF propagation within the URT network is firstly evaluated. Based on the congestion evaluation results, the proposed method determines whether it’s necessary to adjust timetables of the LPF Line or other lines. The proposed method is applied to the Xi’an Metro network in China. The results indicate that the proposed method can effectively evaluate and adjust the train timetables for large URT networks under the LPF events.

Perspectives on the Practice of Collaborative Care: Topical Themes and Future Directions From the Lens of a Collaborative Care Child Psychiatrist.

The increasing shortage of psychiatrists in the workforce is a well-documented and alarming trend. In light of this workforce shortage, psychiatrists bear a professional and ethical responsibility to identify strategies to increase access to mental health treatment and thereby reduce the impact of the psychiatrist shortage on patients and on the greater health system. One such strategy is the collaborative care model (CoCM), which has demonstrated efficacy in the mental health treatment of both pediatric and adult populations. In this article, the CoCM is presented as one effective strategy to address the behavioral workforce shortage. Current themes observed from the clinical practice of psychiatric collaborative care in a pediatric collaborative care setting are discussed with contemplation of future directions.

Abstract MP11: Evaluation of a Remote Patient Monitoring Hypertension Program with Telehealth Pharmacist Management in Nephrology Clinic Patients

Introduction: Remote patient monitoring (RPM) and physician-pharmacist collaborative models have potential in improving hypertension control. Herein, we report the effectiveness of a community based, self-measured blood pressure (SMBP) RPM program on BP control and healthcare utilization in a largely rural health system. Methods: A multidisciplinary team developed a self-measured blood pressure (SMBP) RPM program in central and northeast PA using a virtual platform with bluetooth-connected BP devices targeting patients with BP ≥140/90 mmHg seen in nephrology clinics. From 3/2022 to 9/2022, high BP notifications were transmitted to nephrologists. After 9/2022, BP notifications were transmitted first to pharmacists, who co-managed BP through collaborative practice agreements via telehealth. We captured pharmacist interventions by reviewing electronic health record documentation. Changes in SMBP over time and the effect of pharmacist interventions were analyzed using mixed effects models. All-cause hospitalizations and emergency room (ER) visits were compared in 6-month pre/post-enrollment periods. Results: A total of 205 patients (mean age 62.3y, clinic BP 146/84, 3.1 BP classes, 48% female, 53% CKD) were enrolled into the program and had ≥ 6 months follow-up data. SMBP control &lt;140/90 improved from 30% during week 1 to 67% at 6 months and 74% at 12 months. Overall, systolic SMBP improved over the first 6 months with stronger effect on patients with higher baseline BP (baseline BP ≥150/100 mmHg: -3.3 mmHg/month (95% CI: -3.5, -3.1); baseline 140-149/90-99: -2.4 (95% CI: -2.6, -2.1) mmHg/month; baseline &lt;140/90: -0.6 (-0.9, -0.4) mmHg/month). Pharmacist telehealth encounters were documented in 65% of patients; 46% had a BP medication adjustment; 37% had new BP medication classes, including 9% mineralocorticoid receptor antagonist. Pharmacist medication intervention was associated with greater decline in systolic BP over time (-1.3 mmHg/month; 95% CI: -1.6, -1.1). Comparing the 6 months pre vs. post-enrollment, patients experienced a reduction in hospitalizations (6.0% vs. 0%; Exact Mcnemar p=0.0005) and no difference in ER visits (14.9% vs. 12.9%; p=0.6). Conclusion: Participation in a nephrologist-pharmacist SMBP RPM program was associated with improvement in BP control and reduced hospitalizations. Future efforts are underway to refine delivery of this program for the largest impact on improving BP control.

Effect of the Collaborative Model on Communication and Clinical Reasoning in DPT Students’ Clinical Experiences

Introduction: The collaborative clinical experiences pair one physical therapist (PT) clinical instructor (CI) with one or more student(s) (Wolff-Burke et al, 2022).The collaborative model has been widely accepted among nursing and other healthcare professions. Benefits of the collaborative model include improved communication and clinical reasoning, enhanced teamwork, and reduced anxiety among students. Additionally, the collaborative model facilitates peer-to-peer learning and leadership. Despite the benefits, PT education has been slow to embrace this model. This study's purpose was to determine if the collaborative model is effective in helping students obtain the required performance ratings required by the university during their clinical experiences. Subjects included student placement records were reviewed to determine inclusion and exclusion based on participation in a collaborative experience between Spring 2022 and Spring 2023. Methods: Clinical Performance Inventory (CPI) data was obtained and analyzed in communication and clinical reasoning. The professionalism and academic tracking systems were also reviewed to obtain student history related to communication and clinical reasoning (CR). Demographic information on gender and age was also included for comparison. Results: Every student passed their clinical experience. Students scored higher than the expected threshold for their respective clinical experiences, with 76.3% over performing on communication and 85.5% on clinical reasoning. Female students scored higher than male students on communication and CR for the Integrated Clinical Experience (ICE). Students referred to the professionalism tracking system scored 3% lower on communication and 2% lower on CR compared to those who were not referred to the professional tracking system. Discussion: The collaborative model appears to be an effective supervision model for clinical experiences. It may provide an effective way for students to develop their communication and clinical reasoning skills. Future studies should be designed to compare students’ performance and perceptions between those in traditional and collaborative models.

Development of LSLC-Based Collaborative Learning Model Learning Tools And Their Effects on Critical Thinking Skills

The purpose of this study is to develop collaborative learning-based learning resources and their impact on students' capacity for critical thought. This study used a combination of methods, namely development research (R&amp;D) with Thiagarajan 4D model and experimental research. Data was collected through observations, questionnaires, tests, and interviews. The validity coefficients for the Teaching Module, worksheet, and test were 3.80, 3.80, and 3.70, respectively. Observations showed that 96% of the learning tools met practical criteria. The tools were effective, with 93% of student activities rated very good, 96% of students giving positive responses, and 85% achieving learning completion. A t-test (sig = 0.007) confirmed the collaborative learning tool based on LSLC significantly enhanced students' critical thinking skills.

Managing medications for patients with cancer and chronic conditions: It's time for collaboration between primary care and oncology pharmacists.

To provide a rationale for a collaborative care model involving oncology and primary care pharmacists to improve the coordination of care of medications for cancer patients with multiple chronic conditions. A review of selected literature and the authors' own research was used. Studies illustrating the gaps in care for medications and pharmacists' roles in oncology and primary care settings from PubMed were reviewed. There has been a substantial increase in the development and utilization of oral anticancer agents (OAAs). Although OAAs offer convenience and flexibility, they also introduce challenges related to medication adherence, monitoring, and managing side effects. Up to 17.5% of patients experience moderate to severe symptoms from OAAs and about 30% report less than excellent medication adherence. Further, studies showed that 30% to 53% of adult cancer patients have at least one chronic condition that complicates their treatment plan due to the need for medications, increasing the risk of drug interactions, side effects, and non-adherence. The Primary Care Oncology Model (PCOM) incorporates both primary care and oncology pharmacists with comprehensive medication review and patient-reported outcome measure, respectively, to enhance medication appropriateness and effectiveness, and improve overall patient experience. Implementing PCOM may improve the medication management of patients taking OAAs for active cancer treatment and chronic medications for their multiple chronic conditions. This collaborative approach can transform patient care by leveraging the expertise of both primary care and oncology pharmacists.
