“Resin Bonded Loop Connector Fixed Partial Denture – A Subtle Solution to Maintain Midline Diastema”

Spacing between natural teeth is considered as a normal characteristic of class 1 malocclusion which is due to its high prevalence. However, spacing in the midline of the natural dentition has always been an area of interest for a prosthodontist. The challenge is so severe that most of the times its incorporation is part of treatment plan rather than any attempt to eliminate it, because midline diastema closure with a fixed prosthesis usually results in esthetic failures. With advances in dental cements especially resin cements; there has been an increase in delivery of resin bonded prosthesis, but has not been explored for possibility of maintaining midline diastema. This article presents a case of midline diastema successfully maintained by resin bonded prosthesis through the use of a loop connector between the pontic and the retainer.

Sectional porcelain veneers for a maxillary midline diastema closure: a case report.

Diastema closure is a frequently requested, technique-sensitive cosmetic procedure. There are many techniques and materials that can be employed to close diastemas. While direct composite techniques can be economical and successful, they do present challenges in achieving satisfactory clinical and esthetic results. Traditional porcelain veneer placement may offer an excellent esthetic result, but typically requires the removal of tooth structure; as such, this is an irreversible procedure. The present article reports the case of a maxillary midline diastema closure in a healthy dentition by means of sectional porcelain veneers simply cemented onto the natural teeth and without tooth preparation. A step-bystep procedure is proposed for illustrating the proper implementation of an additive-only and noninvasive indirect technique that yields a satisfactory clinical and esthetic outcome for clinician and patient.

Closure of midline diastema through combined surgical and Removable orthodontic approach

Midline diastema is a common aesthetic problem in mixed and permanent dentition. Many innovative therapies are varying from restorative procedures such as composite build-up to surgery (frenectomy) and orthodontics are available. A high frenum attachment is often the cause of persistent diastemas. Presented herewith is a case report of a 13-year-old girl with a high frenal attachment that had caused spacing of the maxillary central incisors. This case report demonstrates the removal of the abnormal labial frenum attachment through surgery and subsequent closure of maxillary diastema following removable orthodontic treatment.

Spontaneous closure of midline diastema following frenectomy

Maxillary midline diastema is a common aesthetic problem in mixed and early permanent dentitions. The space can occur either as a transient malocclusion or created by developmental, pathological or iatrogenical factors. Many innovative therapies varying from restorative procedures such as composite build-up to surgery (frenectomies) and orthodontics are available. Although literature says every frenectomy procedure should be preceded by orthodontic treatment, we opted for frenectomy technique without any orthodontic intervention. Presented herewith is a case report of a 9-year-old girl with a high frenal attachment that had caused spacing of the maxillary central incisors. A spontaneous closure of the midline diastema was noted within 2 months following frenectomy. The patient was followed up for 4 months after which the space remained closed and there was no necessity for an orthodontic treatment at a later stage.

A postretention study of patients presenting with a maxillary median diastema.

The purpose of this study was to assess postretention stability of maxillary midline diastema closure, to search for predictors of relapse, and to test for associations between relapse and other postretention changes. The sample consisted of 35 patients with pretreatment diastemas ranging from 0.9 mm to 3.0 mm (mean 1.4, SD = 0.5) following eruption of the maxillary canines. Data were gathered from treatment charts, study models, periapical radiographs, and cephalograms taken pretreatment, posttreatment, and 1 to 26 years postretention (mean 11.4, SD = 6.4). Measurable diastema relapse was observed in only 12 cases. The majority of the relapse was 0.6 mm or less, and maximum relapse was 3.0 mm. Abnormal frenums and/or intermaxillary osseous clefts did not appear to be risk factors for relapse, and no pretreatment predictors of relapse could be established. The only posttreatment change associated with diastema relapse was proclination of the maxillary incisors (p < 0.01).
