Future structure of clinical tropical medicine in the United Kingdom.

A century ago, and less, the practice of clinical tropical medicine was largely dominated by physicians and surgeons from the United Kingdom. Latterly, the position has changed dramatic? ally.1 The Empire and the Raj have faded into past history, most former colonial territories have gained independence, and most developing tropical countries now undertake the training of physicians and surgeons in their own medical schools. Despite these developments, the structure of clinical tropical medicine in the United Kingdom has remained essentially unchanged, and it is timely to review the current position and to attempt to decide what should best be done for the future. Changes have been made in the non-clinical areas that have long dominated the tropical schools of the United Kingdom; in the clinical area there has been stagnation with a resulting loss of a sense of direction.

Symposia in Clinical Tropical Medicine

In 1967, The Tropical Disease Center, St. Clare's Hospital, New York City, began the sponsorship of a series of symposia on clinical tropical medicine. Three symposia have been given: on 27 May 1967 (on schistosomiasis); on 17 October 1967 (on American contributions in the history of tropical medicine); and on May 1968 (on hepatitis in the tropics). The papers presented by invited speakers at these symposia were published in the Bulletin of the New York Academy of Medicine in the numbers for March and June 1968, and for February 1969. This book collates and reprints these papers; its preface states that the symposia will continue and that other volumes will follow.Each symposium has an introduction by Dr. Kenneth M. Cahill, Director of the Tropical Disease Center, organizer of the symposia, and Editor of the volume. There follow short review articles pertinent to the subject of each symposium.Four of the six articles in that on schistosomiasis, and three of the six articles in that on hepatitis are followed by brief, formal discussions by invited speakers.

Clinical Tropical Diseases

That this handbook of clinical tropical medicine has progressed through four editions since 1953 indicates continued acceptance of the authors' original objective, namely, to supply “essential facts as dogmatically and concisely” as possible. The general format remains unchanged with subject material arranged alphabetically and arbitrarily in 47 short chapters. Thus, amoebiasis follows the initial chapter on ainhum, and worm infections and yaws conclude the list. Although new material has been introduced, the authors have managed to limit the length of the text, which has been increased by only 74 pages in the course of the four editions. Chapters in the first edition devoted to subjects such as dracontiasis and yellow fever have by now been integrated with other material, and new chapters have been introduced on subjects such as kuru and lymphomatous syndrome of children. Parenthetically, the term “Burkitt's tumor” appears nowhere in the latter chapter, although the eponym is listed in the index.

Synopsis of Clinical Tropical Medicine

Clinical tropical medicine is so broad in scope that adequate coverage by any of the standard texts involves huge and expensive publishing ventures. For example, the 15th edition of Manson's book contains 1191 pages and costs $15.00. Craig and Faust's classic in tropical parasitology (with Paul Russell) contains 1099 pages and costs $16.50; it does not include those diseases of the tropics not caused by parasites. Dr. Oscar Felsenfeld has succinctly and helpfully compressed an enormous body of information into a compact, 378-page book. The publisher has provided good paper; the illustrations are, on the whole, new and good; the cover is suitable for the tropics; and the book fits easily into a pocket. The reviewer is especially grateful that, despite the demands of space, the author has provided at the end of each chapter a list of recommended readings, although not of specific references. Unfortunately, I have never been able to determine for whom a synopsis is useful.

Clinical Tropical Diseases

In the introduction to the first edition of Clinical Tropical Diseases, the authors express their belief in the need for a handbook giving that information essential for the clinical diagnosis and treatment of those diseases which occur primarily in the tropics. Their aim was to supply essential facts as dogmatically and concisely as they could. The aim in the second edition was to modernize the book without increasing its bulk.The result is 540 pages of concise information covering forty-four major disease or disease categories which will be of great value to all interested in tropical diseases, but especially those having clinical responsibilities.The clinical entities are presented alphabetically, each subject usually being described in respect to definition, geographical distribution, etiology, pathology, clinical picture, diagnosis, treatment, and prophylaxis.The book is both clearly written and pertinent, especially in regard to treatment. It is unquestionably a valuable contribution to clinical tropical medicine.
