Postnatal Effects of Sex Hormones on Click-Evoked Otoacoustic Emissions: A Study of Adolescents with Gender Dysphoria

Click-evoked otoacoustic emissions (CEOAEs) are echo-like sounds, generated by the inner ear in response to click-stimuli. A sex difference in emission strength is observed in neonates and adults, with weaker CEOAE amplitudes in males. These differences are assumed to originate from testosterone influences during prenatal male sexual differentiation and to remain stable throughout life. However, recent studies suggested activational, postnatal effects of sex hormones on CEOAEs. Adolescents diagnosed with gender dysphoria (GD) may receive gonadotropin-releasing hormone analogs (GnRHa) in order to suppress endogenous sex hormones and, therefore, pubertal maturation, followed by cross-sex hormone (CSH) treatment. Using a cross-sectional design, we examined whether hormonal interventions in adolescents diagnosed with GD (62 trans boys, assigned female at birth, self-identifying as male; 43 trans girls, assigned male at birth, self-identifying as female), affected their CEOAEs compared to age- and sex-matched controls (44 boys, 37 girls). Sex-typical differences in CEOAE amplitude were observed among cisgender controls and treatment-naïve trans boys but not in other groups with GD. Treatment-naïve trans girls tended to have more female-typical CEOAEs, suggesting hypomasculinized early sexual differentiation, in support of a prominent hypothesis on the etiology of GD. In line with the predicted suppressive effects of androgens, trans boys receiving CSH treatment, i.e., testosterone plus GnRHa, showed significantly weaker right-ear CEOAEs compared with control girls. A similar trend was seen in trans boys treated with GnRHa only. Unexpectedly, trans girls showed CEOAE masculinization with addition of estradiol. Our findings show that CEOAEs may not be used as an unequivocal measure of prenatal androgen exposure as they can be modulated postnatally by sex hormones, in the form of hormonal treatment.

Prenatal masculinization of the auditory system in infants: The MIREC-ID study

Sex differences in inner-ear function are detectable in infants, notably through the measurement of otoacoustic emissions (OAEs). Prevailing theories posit that prenatal exposure to high levels of androgens in boys may weaken OAEs, and that this phenomenon may predominantly affect the right ear/left hemisphere (Geschwind-Galaburda (GG) hypothesis). Yet, actual tests of these models have been difficult to implement in humans. Here we examined the relationship between markers of fetal androgen exposure collected at birth (anogenital distances (AGD); penile length/width, areolar/scrotal/vulvar pigmentation) and at 6 months of age (2nd to 4th digit ratio (2D:4D)) with two types of OAEs, click-evoked OAEs (CEOAEs) and distortion-product OAEs (DPOAEs) (n = 49; 25 boys; 24 girls). We found that, in boys, scrotal pigmentation was inversely associated with the amplitude and reproducibility of CEOAEs in the right ear at 4 kHz, with trends also present in the same ear for mean CEOAE amplitude and CEOAE amplitude at 2 kHz. Penile length was inversely associated with the mean amplitude of DPOAEs in both the right and left ears, as well as with DPOAE amplitude in the right ear at 2 kHz and the reproducibility of CEOAEs in the left ear at 2.8 kHz. Finally, AGD-scrotum in boys was positively associated in boys with the amplitude of DPOAEs in the left ear at 2.8 kHz. Unexpectedly, there were no sex differences in the amplitude or reproducibility of OAEs, nor, in girls, any associations between androgenic markers and auditory function. Nonetheless, these findings, reported for the first time in a sample of human infants, support both the prenatal-androgen-exposure and GG models as explanations for the masculinization of auditory function in male infants.

Increased medial olivocochlear reflex strength in normal-hearing, noise-exposed humans.

Research suggests that college-aged adults are vulnerable to tinnitus and hearing loss due to exposure to traumatic levels of noise on a regular basis. Recent human studies have associated exposure to high noise exposure background (NEB, i.e., routine noise exposure) with the reduced cochlear output and impaired speech processing ability in subjects with clinically normal hearing sensitivity. While the relationship between NEB and the functions of the auditory afferent neurons are studied in the literature, little is known about the effects of NEB on functioning of the auditory efferent system. The objective of the present study was to investigate the relationship between medial olivocochlear reflex (MOCR) strength and NEB in subjects with clinically normal hearing sensitivity. It was hypothesized that subjects with high NEB would exhibit reduced afferent input to the MOCR circuit which would subsequently lead to reduced strength of the MOCR. In normal-hearing listeners, the study examined (1) the association between NEB and baseline click-evoked otoacoustic emissions (CEOAEs) and (2) the association between NEB and MOCR strength. The MOCR was measured using CEOAEs evoked by 60 dB pSPL linear clicks in a contralateral acoustic stimulation (CAS)-off and CAS-on (a broadband noise at 60 dB SPL) condition. Participants with at least 6 dB signal-to-noise ratio (SNR) in the CAS-off and CAS-on conditions were included for analysis. A normalized CEOAE inhibition index was calculated to express MOCR strength in a percentage value. NEB was estimated using a validated questionnaire. The results showed that NEB was not associated with the baseline CEOAE amplitude (r = -0.112, p = 0.586). Contrary to the hypothesis, MOCR strength was positively correlated with NEB (r = 0.557, p = 0.003). NEB remained a significant predictor of MOCR strength (β = 2.98, t(19) = 3.474, p = 0.003) after the unstandardized coefficient was adjusted to control for effects of smoking, sound level tolerance (SLT) and tinnitus. These data provide evidence that MOCR strength is associated with NEB. The functional significance of increased MOCR strength is discussed.

Click-Evoked Otoacoustic Emissions in Children and Adolescents with Gender Identity Disorder

Click-evoked otoacoustic emissions (CEOAEs) are echo-like sounds that are produced by the inner ear in response to click-stimuli. CEOAEs generally have a higher amplitude in women compared to men and neonates already show a similar sex difference in CEOAEs. Weaker responses in males are proposed to originate from elevated levels of testosterone during perinatal sexual differentiation. Therefore, CEOAEs may be used as a retrospective indicator of someone's perinatal androgen environment. Individuals diagnosed with Gender Identity Disorder (GID), according to DSM-IV-TR, are characterized by a strong identification with the other gender and discomfort about their natal sex. Although the etiology of GID is far from established, it is hypothesized that atypical levels of sex steroids during a critical period of sexual differentiation of the brain might play a role. In the present study, we compared CEOAEs in treatment-naïve children and adolescents with early-onset GID (24 natal boys, 23 natal girls) and control subjects (65 boys, 62 girls). We replicated the sex difference in CEOAE response amplitude in the control group. This sex difference, however, was not present in the GID groups. Boys with GID showed stronger, more female-typical CEOAEs whereas girls with GID did not differ in emission strength compared to control girls. Based on the assumption that CEOAE amplitude can be seen as an index of relative androgen exposure, our results provide some evidence for the idea that boys with GID may have been exposed to lower amounts of androgen during early development in comparison to control boys.

Nonlinear properties of otoacoustic emissions in normal and impaired hearing

Click-evoked otoacoustic emissions (CEOAEs) exhibit nonlinearities in amplitude and time domains. The first objective of this study was to investigate whether there is any correlation between the temporal and amplitude nonlinearities of CEOAEs in normals. Additionally there is evidence that pathology affects the normal cochlear nonlinearity. The second objective was to investigate whether pathology affects the temporal nonlinear components. Conventional and maximum length sequence (MLS) CEOAEs were recorded in normal subjects and in patients with mild hearing loss. The slope of the input–output (I/O) function of the conventional CEOAE measured the amplitude nonlinearity. Two measures of temporal nonlinearity were the magnitude of the suppression that occurs with increase in stimulus rate and the amplitudes of the second and third order temporal interaction components (Volterra slices). The amplitude nonlinearity is well correlated with both the magnitude of the rate suppression and the amplitudes of the Volterra slices. The ‘linear’ CEOAE amplitude showed no differences between the normal and patient groups but the differences in the Volterra slices were substantial. This suggests that the first sign of damage to the cochlea is that the system becomes more linear. Hence the Volterra slices may provide a sensitive measure of cochlear damage.

Study of auditory function in patients with chronic obstructive pulmonary diseases

This study was designed to measure auditory function in patients with chronic obstructive pulmonary diseases (COPDs) who generally suffer from chronic hypoxemia. Control and COPD subject groups received a battery of tests to assess overall hearing sensitivity and peripheral (end-organ and eighth-nerve) and brain-stem auditory function, as well as blood-gas analysis. Results showed a statistically significant difference for all audiological measures between the control group and a COPD subgroup – the presumptive hypoxic subjects (partial oxygen tensions, PO 2, <75 mm Hg). Correlation analyses of results from all subjects (regardless of PO 2) also revealed significant covariance with PO 2 for overall, RMS, amplitude of click-evoked otoacoustic emissions (RA), hearing threshold level, and auditory brain-stem response (ABR, I-V inter-peak latency). χ 2 or Fisher’s exact tests were statistically significant for frequencies of cases classified according to a criterion PO 2 of 70 mm Hg (putative critical O 2 for completely normal auditory function) and either hearing thresholds falling below or RA values above 1.5 standard deviations of the control-group means, respectively. However, χ 2 was not significant for a comparable criterion of ABR I-V IPL. In general, clinically significant hearing loss was uncommon in COPD patients, and the observed effects represented relatively small changes in the auditory measures examined. Still, overall, changes were in the direction of poorer function, and these results suggest physiologically significant impact of chronic hypoxemia and the need for further study to evaluate thoroughly this medical condition as a potential risk factor for audio-vestibular dysfunction.

Otoacoustic emissions in patients with Meniere's disease

Introduction. The analysis of the properties of otoacoustic emissions (OAEs) in patients with Meniere's disease was undertaken. Methods. Within a comprehensive project, click-evoked as well as three-distortion-product OAEs were measured in 100 patients with Meniere's disease. The measurements were performed using the standard ILO equipment. Results. The OAE incidence in affected ears (56%) was lower than in unaffected ears (85%). The mean emission amplitude was also significantly lower (2.6 dB). No differences between uni- and bilaterally affected ears were present. Ears with OAEs clearly showed smaller hearing losses than ears without OAEs (24 dB difference). The average hearing loss showed correlations with the emission amplitudes, although not very strongly; when plotted against the smallest hearing loss, a clearer upper limit for the emission amplitude was present. The amplitude of click-evoked OAEs showed a considerable correlation with the largest of the three-distortion-product OAEs. Conclusion. Otoacoustic emissions in patients with Meniere's disease show no deviant properties compared with other hearing impaired patients: an audiogram with partly normal thresholds possibly yields OAEs.

Tones disappear faster in the right ear than in the left.

In order to gain further information on the characteristics and physiological correlates of tone decay in humans, the tone decay test was administered to 58 normal-hearing subjects, successively in the left and right ears and in absence and presence of a contralateral noise. The results revealed that tone decay was greater in the right than in the left ear and was increased by contralateral noise. The contralateral effect of this noise on cochlear biomechanisms was then estimated by measuring contralaterally induced variations in the amplitude of click-evoked otoacoustic emissions in the same subjects. In the right ear, the increase in tone decay and the decrease in otoacoustic emission amplitude--both induced by contralateral noise--were positively correlated (r = .315, p = .016). Furthermore, the contralateral changes in otoacoustic emission amplitude were found to be on average larger in the right than in the left ear, this asymmetry being correlated with that observed for the tone decay. These findings are discussed in relation to previous results on simple and induced loudness adaptation in the vicinity of threshold, on contralateral attenuation of otoacoustic emissions and on the influence of the auditory efferents on cochlear biomechanisms.

Otoacoustic emissions in preterm infants: indications for cochlear development?

A longitudinal study of click-evoked otoacoustic emissions (CEOAEs) in 19 ears of 11 preterm infants--post-conceptional age (PCA): 30 to 39 weeks--resulted in a total of 90 CEOAE recordings. All but one of the 19 ears showed an increase of CEOAE amplitude at increasing PCA. The mean increase rate was 1.36 dB per week (dB/wk) for the left ears (n = 11, SD = 1.04 dB/wk), and 1.17 dB/wk for the right ears (n = 8, SD = 0,87 dB/wk). In six ears of three infants we were able to follow a total of 15 frequencies of spontaneous otoacoustic emissions (SOAEs). All of the monitored SOAE frequencies showed a positive shift in frequency with time, ranging from about 10 Hz/wk around 2000 Hz to about 50 Hz/wk around 5000 Hz. This increase of CEOAE amplitude and SOAE frequency indicates that OAE properties are not fully developed in preterm infants. Although the influence of middle ear properties cannot be excluded or proved, the observed SOAE frequency shift suggests development of the fine structures in the cochlea itself.
