Urinary excretion of cimetropium bromide after multiple oral doses.

is a new, powerful antispasmodic compound with strong antimuscarinic properties [5, 6]. As a quater,,, 1. nary ammonium salt, it is poorly absorbed after oral ,. administration (1-4% of the dose) [7]. The aim of the present study was to examine the possibility of accumulation of the drug after multiple, oral therapeutic doses. Ten healthy volunteers (3 men and 7 women, aged 25-37 years) were given cimetropium bromide 50 mg t. d. s. before meals for 1 week. Urine fractions were collected before and up to 4 days after the last dose. 0.1 Linear regression of the log of the mean rate of urinary excretion vs time yielded a highly significant correlation coefficient ( r = 0 . 9 6 8 , p<0.01). The slope of the regression line, which represents 2z, was 0.024 h -1, which corresponds to a half-life of 29 h (Fig. 1). The excretion of cimetropium was still incomplete 4 days after the last dose, which was consistent with the long half-life calculated from the excretion rate data. The average overall cumulative excretion of cimetropium up to 96 h after the last dose was 611 ~tg (SEM _+ 221 ~tg), which reflects the very poor absorption of the drug. Discontinuous Oral Absorption of Cimetropium Bromide, a New Antispasmodic Drug

The pharmacokinetic profiles of cimetropium bromide, after either intravenous injection of 10mg or oral ingestion of 200mg, were determined in eight healthy volunteers. After intravenous administration, the plasma levels and urinary excretion indicated that the drug is distributed and eliminated at a rapid rate (terminal half-life, 50 ± 8 min) and that urinary excretion is not the exclusive route of elimination (46 ± 2%) of the administered dose). After oral administration, a low percentage of the drug is absorbed (1–4% of the administered dose), however, the amount is sufficient for therapeutic effect. The absorption is discontinuous, with two distinct phases, and ends abruptly during the second phase.
