Temporal Bone Radiology: Beyond the Basics

Program Description: Through this interactive presentation, the audience is exposed to challenging temporal bone radiology cases and offered strategies to improve his or her interpretation. We will cover key findings for chronic ear disease and conductive hearing loss, lateral skull base lesions, and the petrous apex. The emphasis is placed on subtle but pertinent findings on computed tomography (CT) and MRI examinations and their importance in creating and narrowing a differential diagnosis. We will present guidelines to aid in the efficient acquisition and interpretation of imaging for patients with common otologic symptoms such as vertigo, sudden hearing loss, and pulsatile tinnitus. Educational Objectives: 1) Interpret key radiographic findings for common pathologic conditions of the temporal bone. 2) Order appropriate radiographic tests to evaluate common conditions and understand the strengths and limitations of various imaging modalities.

Contemporary Management of Chronic Ear Disease

Program Description: This miniseminar will discuss various aspects of the management of chronic ear disease, including ossicular chain reconstruction, cartilage and pediatric tympanoplasty, the role of endoscopes, and the management of chronic otitis media without cholesteatoma. Time permitting, a discussion on the management of labyrinthine fistulas will also be presented. A case based approach to these topics will be presented by the moderators to the panel members. Educational Objectives: 1) Describe ossicular chain reconstruction, including approaches, staging, and prosthesis selection (including autologous materials). 2) Recognize indications and methods of cartilage tympanoplasty and pediatric tympanoplasty. 3) Manage chronic otitis media without cholesteatoma and the role of endoscopes in chronic ear surgery.

Temporal bone encephalocele and cerebrospinal fluid fistula repair utilizing the middle cranial fossa or combined mastoid–middle cranial fossa approach

The goals of this study were to report the clinical presentation, radiographic findings, operative strategy, and outcomes among patients with temporal bone encephaloceles and cerebrospinal fluid fistulas (CSFFs) and to identify clinical variables associated with surgical outcome. A retrospective case series including all patients who underwent a middle fossa craniotomy or combined mastoid-middle cranial fossa repair of encephalocele and/or CSFF between 2000 and 2012 was accrued from 2 tertiary academic referral centers. Eighty-nine consecutive surgeries (86 patients, 59.3% women) were included. The mean age at time of surgery was 52.3 years, and the left side was affected in 53.9% of cases. The mean delay between symptom onset and diagnosis was 35.4 months, and the most common presenting symptoms were hearing loss (92.1%) and persistent ipsilateral otorrhea (73.0%). Few reported a history of intracranial infection (6.7%) or seizures (2.2%). Thirteen (14.6%) of 89 cases had a history of major head trauma, 23 (25.8%) were associated with chronic ear disease without prior operation, 17 (19.1%) occurred following tympanomastoidectomy, and 1 (1.1%) developed in a patient with a cerebral aqueduct cyst resulting in obstructive hydrocephalus. The remaining 35 cases (39.3%) were considered spontaneous. Among all patients, the mean body mass index (BMI) was 35.3 kg/m(2), and 46.4% exhibited empty sella syndrome. Patients with spontaneous lesions were statistically significantly older (p = 0.007) and were more commonly female (p = 0.048) compared with those with nonspontaneous pathology. Additionally, those with spontaneous lesions had a greater BMI than those with nonspontaneous disease (p = 0.102), although this difference did not achieve statistical significance. Thirty-two surgeries (36.0%) involved a middle fossa craniotomy alone, whereas 57 (64.0%) involved a combined mastoid-middle fossa repair. There were 7 recurrences (7.9%); 2 patients with recurrence developed meningitis. The use of artificial titanium mesh was statistically associated with the development of recurrent CSFF (p = 0.004), postoperative wound infection (p = 0.039), and meningitis (p = 0.014). Also notable, 6 of the 7 cases with recurrence had evidence of intracranial hypertension. When the 11 cases that involved using titanium mesh were excluded, 96.2% of patients whose lesions were reconstructed with an autologous multilayer repair had neither recurrent CSFF nor meningitis at the last follow-up. Patients with temporal bone encephalocele and CSFF commonly present with persistent otorrhea and conductive hearing loss mimicking chronic middle ear disease, which likely contributes to a delay in diagnosis. There is a high prevalence of obesity among this patient population, which may play a role in the pathogenesis of primary and recurrent disease. A middle fossa craniotomy or a combined mastoid-middle fossa approach incorporating a multilayer autologous tissue technique is a safe and reliable method of repair that may be particularly useful for large or multifocal defects. Defect reconstruction using artificial titanium mesh should generally be avoided given increased risks of recurrence and postoperative meningitis.

Active middle ear implants: Vibroplasty™ in children and adolescents with acquired or congenital middle ear disorders

Conclusion: Active middle ear implant (AMEI) implantation in children and adolescents is safe and provides improved hearing results. No statistical difference in hearing outcomes was shown in the group of patients affected by chronic middle ear diseases versus aural atresia. Also, the transducer location (round window versus oval window placement) did not lead to different outcomes in hearing abilities. Objectives: (1) To assess the hearing outcomes with the active implant Vibrant Soundbridge (VSB) in children and adolescents. (2) To evaluate whether functional results of the subjects in the study could depend on the hearing loss etiology (chronic middle ear diseases versus aural atresia) or on transducer location (round window versus oval window placement). Methods: The study was carried out with a retrospective, single-subject, repeated measures design, and included 22 children and adolescents with conductive or mixed hearing loss due to aural atresia or chronic middle ear diseases. Preoperative and postoperative pure tone air conduction (AC) and bone conduction (BC) thresholds were measured to demonstrate implantation safety. Free-field warble tone and speech audiometry were performed to assess postoperative hearing abilities with and without the VSB. Results: No significant changes in mean BC or AC thresholds between preoperative and postoperative conditions were seen in the 22 patients. Mean PTA4 functional gain was 30.7 dB. Averaged over all 22 patients, word recognition at 65 dB SPL changed from an average of 19% in the unaided postoperative condition to 97% in the VSB-aided condition. Functional results were independent of hearing loss etiology and transducer location.

Effects of Pillar implants for sleep-related breathing disorders on middle ear function

Pillar implants provide a reasonable outcome with minimal post-operative morbidity and complications in treating patients with sleep-disordered breathing (SDB) who had obvious palatal obstruction. The palatal structure is responsible for a normal functioning Eustachian tube; however, little is known if there is any potential otologic implication of minimally invasive palatal stiffening surgery for SDB. The aim of this study is to evaluate the effects of Pillar implantation on middle ear function. We performed a prospective study in a tertiary referral center. Thirty SDB patients (25 men, 5 women; mean age, 44.3 years) who underwent Pillar implants for treating palatal obstruction were enrolled. The subjects had normal otologic exam and no previous history of chronic ear disease. Pure-tone audiometry and tympanometry were performed pre-operatively, and post-operative days 1 and 7, and months 1 and 3. Baseline and post-operative middle ear pressures (MEPs) in decipascals were compared. Statistical analysis was performed by repeated measures of ANOVA. Eight patients (8/30, 26.7%) reported otologic complaints such as ear pressure and/or otalgia within 1 week post-operatively. No permanent otologic discomfort occurred. A trend toward reduced MEP was noted in this study. The decrease in MEP became apparent on post-operative day 1 after surgery. However, mean pressure changes were no longer significantly different from pre-operative values by 1 week after surgery. Pillar implantation for SDB induces changes in middle ear function. However, the changes were temporary and not significant 1 week after surgery.

Alterations in salivary IgA levels in infectious and inflammatory disorders of upper respiratory tract

Abstract: BACKGROUND AND AIM: The airway surfaces are one of the most common ways of entry of infectious agents. The impact of upper respiratory tract diseases on salivary IgA production has not been fully understood. Therefore, in this study, we investigated the salivary IgA levels in patients suffered from upper respiratory tract diseases to indicate the effect of these diseases on salivary IgA production. METHODS: In this study, salivary IgA level of 156 patients with inflammatory diseases of the upper respiratory tract including chronic rhinosinusitis, ear and pharynx diseases have been evaluated by direct immunoenzymatic determination. RESULTS: In pharynx disorders 11.8 % of patients were IgA deficient , 76.2 % were normal and11.8 % had elevated level of IgA .In patients with chronic rhinosinusitis IgA deficiency was observed in 9.2 % , 75.9 % were normal and there was an elevation in 14.8 % of patients .In ear disorders 11.6% were IgA deficient ,76.7 % normal and 11.6 % had elevated IgA level. CONCLUSION: This study provided evidence for the first time that changes in salivary IgA level are almost the same in different sites of infectious and inflammatory diseases of upper respiratory tract. Our investigation revealed that local up regulation of salivary IgA is not particular interest in majority of patients with upper respiratory tract infections.

Inner Ear Tissue Remodeling and Ion Homeostasis Gene Alteration in Murine Chronic Otitis Media

Studies were designed to ascertain the impact of chronic middle ear infection on the numerous ion and water channels, transporters, and tissue remodeling genes in the inner and middle ear. Permanent sensorineural hearing loss is a significant problem resulting from chronic middle ear disease, although the inner ear processes involved are poorly defined. Maintaining a balanced ionic composition of endolymph in the inner ear is crucial for hearing; thus, it was hypothesized that this may be at risk with inflammation. Inner and middle ear RNA collected separately from 6-month-old C3H/HeJ mice with prolonged middle ear disease were subjected to qRT-PCR for 8 common inflammatory cytokine genes, 24 genes for channels controlling ion (sodium, potassium, and chloride) and water (aquaporin) transport, tight junction claudins, and gap junction connexins, and 32 tissue remodeling genes. Uninfected Balb/c mice were used as controls. Significant increase in inner ear inflammatory and ion homeostasis (claudin, aquaporin, and gap junction) gene expression, and both upregulation and downregulation of tissue remodeling gene expression occurred. Alteration in middle ear ion homeostasis and tissue remodeling gene expression was noted in the setting of uniform upregulation of cytokine genes. Chronic inflammatory middle ear disease can impact inner ear ion and water transport functions and induce tissue remodeling. Recognizing these inner ear mechanisms at risk may identify potential therapeutic targets to maintain hearing during prolonged otitis media.

Health professionals’ views on Indigenous Health and the delivery of healthcare services in the Pilbara

To explore health professionals' views about Australian Indigenous people's health and the delivery of healthcare to them in the Pilbara region of Western Australia. An open-ended questionnaire was used to gather information from health professionals located across diverse regions in the Pilbara. The responses were analysed with the use of thematic analysis. In the first stage, codes were developed by assigning names to small sections of the interview transcripts. Next, the most salient incisive codes were identified and developed into themes that captured the most important issues. Twenty-eight health professionals indicated that the most important health issues were chronic diseases, substance abuse and ear disease. These health issues were often attributed to a cycle of poor health perpetuated throughout generations. Educational initiatives were thought to be integral to intervening in this cycle. Of particular importance in improving the effectiveness of educational initiatives was facilitating the participation of Australian Indigenous peoples to determine the content of such initiatives. The other main issues the health professionals identified were lifting the standard of Australian Indigenous housing and implementing strategies to improve the continuity of healthcare. Educational initiatives need to be prioritised to improve the health of Australian Indigenous people in the Pilbara and the initiatives should be delivered with the involvement of the local community in order to increase the likelihood of sustained behavioural change. Innovative solutions are required to improve the continuity of healthcare in the Pilbara, including increased use of mobile services.

Clinical Study of Influence of Prognostic Factors on the Outcome of Tympanoplasty Surgery

Asbtract: Myringoplasty is the operation specifically designed to close tympanic membrane defects the main aim of surgery of chronic ear disease is to eliminate disease process and reconstruct middle ear to give the patient a dry safe and functioning ear. In this study a clinical study of myringoplasty underlay technique in the management of central perforations done. Main emphasis is laid on evaluation of prognostic factors like Eustachian tube function, middle ear mucosa at operation and audiometric evaluation pre and post operatively. A series of patients treated in the Department of ENT, Government General Hospital, Kurnool during the period of two years is taken for the study. Relevant literature regarding anatomy, physiology, pathophysiology, clinical features and various methods of medical and surgical management are reviewed.

Long-term hearing result using Kurz titanium ossicular implants

Titanium implants in middle ear surgery were introduced in the late 90s and are now frequently used in middle ear surgery. However, long-term studies of patient outcome are few and have only been published in subgroups of patients. We report the long-term effect of titanium middle ear implants for ossicular reconstruction in chronic ear disease investigated in a Norwegian tertiary otological referral centre. Retrospective chart reviews were performed for procedures involving 76 titanium implants between 2000 and 2007. All patients who underwent surgery using the Kurz Vario titanium implant were included in the study. Audiological parameters using four frequencies, 0.5, 1, 2, and 3 kHz, according to AAO-HNS guidelines, was assessed pre and postoperatively. Otosurgical procedures, complications, revisions, and extrusion rates were analyzed. The study had no dropouts. The partial ossicular replacement prosthesis (PORP) was used in 44 procedures and the total ossicular replacement prosthesis (TORP) in 32 procedures, respectively. Mean follow-up was 5.2 years (62 months). The ossiculoplasties were performed as staging procedures or in combination with other chronic ear surgery. The same surgeon performed all the procedures. A postoperative air-bone gap of ≤ 20 dB was obtained in 74 % of the patients, 82 % for the Bell (PORP) prosthesis, and 63 % for the Arial (TORP) prosthesis. The extrusion rate was 5 %. We conclude that titanium ossicular implants give stable and excellent long-term hearing results.

Mastoid cavity obliteration: Our experience

Introduction: A chronic discharging ear has been a challenge for the otologists for centuries. The mainstay of treatment for cholesteatoma is surgery and the surgical treatment of choice is modified radical mastoidectomy. The open mastoidectomy technique has been the mainstay of management of chronic ear disease; however this type of surgery is not without its problem. It results in the formation of a mastoid cavity which has various problems. Mastoid obliteration is done to eliminate the cavity related problems. Aims and objectives: To study surgical results of mastoid obliteration, efficacy of different methods of mastoid obliteration, cavity problems and the need of cavity care. Materials and methods: This study was conducted on 30 subjects having evidence of attico-antral type of disease in middle ear cleft. The mastoid cavity was obliterated with either cartilage/bone dust/flaps Scientific design-prospective study. Results and conclusions: Incidence of discharge, debris, giddiness, pain was reduced in obliterated cavities. Healing of cavity was better in obliterated ears. Cavity obliterated with bone dust and flap had better and early epithelialisation as compared to cartilage.

Predictors for Sensorineural Hearing Loss in Patients With Tubotympanic Otitis, Cholesteatoma, and Tympanic Membrane Retractions

To determine predicting value of otitis type, age, gender, ear suppuration, disease duration, mucosal changes, cholesteatoma spreading, labyrinthine fistula, size, localization of tympanic membrane perforation, type and stage of its retraction and ossicular chain disruption with sensorineural hearing loss in patients with tubotympanic otitis, cholesteatoma, and tympanic membrane retractions. Retrospective case review study. Tertiary referral center. Approximately 264 adult patients with unilateral chronic ear disease, 60 adult patients with tympanic membrane retractions, 78 with cholesteatoma, and 126 with tubotympanic otitis. Otomicroscopy, pure tone audiometry, impedancemetry were carried out preoperatively. Wall up, wall down tympanoplasty, or two-stage surgery was applied depending on pathology. Bone conduction thresholds for 512 to 4,096 Hz. Mean values of bone conduction thresholds for frequencies 512 to 4,096 Hz were significantly higher in otitis groups than in healthy ears (p = 0.000), without differences between the groups. Ossicular disruption correlated with sensorineural hearing loss in cholesteatoma and tubotympanic otitis for all frequencies; long incus process destruction in tubotympanic ears showed strong negative correlation with sensorineural hearing loss for 512 to 2,048 Hz (linear regression coefficient, intercept was -2.84, -2.48, and -2.41; p = 0.0024, 0.0207, and 0.0076, respectively). Perforation size correlated with sensorineural hearing loss for 512 to 2,048 Hz in tubotympanic otitis (Log regression p = 0.0008, 0.0252, and 0.0267; odds ratio, 1.13, 1.11, and 1.06). Atelectasis correlated with sensorineural hearing loss for 4,096 Hz (p = 0.022). Predictors for sensorineural hearing loss in chronic otitis are otitis itself, age, ossicular disruption, especially of long incus process, extensive labyrinthine fistula, perforation size, and type of retraction.

Indications and outcome of subtotal petrosectomy for active middle ear implants

The aim of this study was to describe the outcome and possible complications of subtotal petrosectomy (SP) for Vibrant Soundbridge (VSB) device surgery in a tertiary referral center. A secondary objective was the evaluation of hearing results in a subgroup of subjects who received the VSB device. Between 2009 and early 2011, 22 adult subjects with chronic otitis media (COM) underwent a SP, blind sac closure of the external auditory canal and abdominal fat obliteration to facilitate the application of an active middle ear implant (AMEI) in a staged procedure. Indications consisted of mixed hearing loss after previous tympanomastoplasty and failure of hearing rehabilitation with a hearing aid or bone conduction device in COM. Pre- and postoperative pure-tone audiograms were analyzed in respect to deterioration of inner ear function, unaided and aided (hearing aid, bone-anchored hearing aid and VSB) speech audiograms were compared to verify improvements in communications skills and functional gains. Incidence and type of complications were reviewed. No significant change was observed regarding mean bone conduction thresholds after the first stage procedure. Some minor wound healing problems were noted. Speech perception using the VSB (n = 16) showed a mean aided speech discrimination at 65-dB SPL of 75 % [standard deviation (SD) 28.7], at 80-dB SPL of 90 % (SD 25.1). Our results suggest that for selected patients with open mastoid cavities and chronic middle ear disease, SP with abdominal fat obliteration is an effective and safe technique to facilitate safe AMEI placement.

Persistent middle ear effusion presumably biofilm-related in a paediatric patient with common variable immunodeficiency

Bacterial biofilms play a role in upper respiratory tract diseases, including acute and chronic middle ear diseases, and are involved in chronic infections and resistance to antibiotic treatment. In particular, the nasopharynx and the surrounding tissues act as important reservoirs of resistant bacterial biofilms, which have been detected in biopsies taken from adenoid and/or middle ear mucosa of children with chronic middle ear effusion. Here we describe the management of a child with congenital immunodeficiency and a chronic middle ear effusion, resistant to traditional medical treatment and presumably due to nasopharyngeal colonization by bacterial biofilms, which has been successfully treated by means of medicated nasal douches delivering antibiotic and a biofilm-destroying compound.

Total Ossiculoplasty: Advantages of Two-Point Stabilization Technique

Objective. Evaluate a porous polyethylene prosthesis with two-point stabilization in total ossiculoplasty. This approach utilizes a lateral as well as a medial graft to stabilize a total ossicular prosthesis (TOP). Study Design. Retrospective cohort review of total ossiculoplasty. Methods. All patients who underwent total ossiculoplasty during the years 2004–2007 were included in the study group. Only five patients (10%) had primary surgery whereas 45 (90%) underwent revision surgery. Cartilage grafts covering the prosthesis (Sheehy, Xomed) laterally were used in all patients with areolar tissue being used for medial stabilization at the stapes footplate. Follow-up examination and audiometrics were performed a mean of 8.1 months following surgery. Results. The percentage of patients closing their ABG to within 10 dB was 44% with 66% closing their ABG to within 20 dB. The mean four-frequency hearing gain was 15.7 dB. The mean postoperative ABG was 15.7 dB. Conclusion. Audiometric results following total ossiculoplasty surgery using two-point stabilization exceeded results from the otologic literature. Proper two-point fixation with areolar tissue and stabilization utilizing cartilage were the keys to achieving a relatively high percentage of success in chronic ear disease in this sample.

Tympanometry, a Prognostic Indicator of Myringoplasty with Assessment of Eustachian Tube Function

Objective: To determine Eustachian tube function in tubotympanic type of chronic ear disease and to correlate Eustachian tube function with pathological change in middle ear mucosa. Materials and Methods: A prospective study of the cases of tubotympanic type of chronic suppurative otitis media was undertaken. Patients were subjected to clinical examination followed by audiometry tests and impedence audiometry to determine eustachian tube functions. Patients were then subjected to myringoplasty and eustachian tube function were then reassessed post operatively. Results: In our study, out of 30 cases who underwent myringoplasty, 21 cases had successful surgical outcome and 9 cases were failed. Out of 24 cases of good Eustachian tube function, 21 cases had successful surgical outcome intends of taking up graft, absence of otorrhea and improvement in hearing. In remaining 3 cases, there was recurrence of otorrhea with central perforation. All the 6 cases with poor Eustachian tube function had surgical failure. Conclusion: The prognosis of the middle ear reconstructive surgery has direct correlation with Eustachian tube functions and Eustachian tube function tests should be a routine investigation in all the cases planned for surgical reconstruction of the middle ear.

Time trend analysis of mastoidectomy procedures performed in Ontario, 1987–2007

Conclusions: There has been a reduction in the number of modified radical mastoidectomy and revision mastoidectomy surgeries per head of population in Ontario between 1987 and 2007, we believe that this represents a true reduction in prevalence of cholesteatoma. The increase of cases performed at the University Hospital Network, Toronto (UHN) may represent a shift to subspecialization in the treatment of chronic ear disease. Objective: To analyze the trends in mastoid operations for chronic middle ear disease in the Canadian province of Ontario between 1987 and 2007 and to determine whether an increasing proportion of these procedures are being performed in tertiary referral centres. Methods: The year on year population and number of mastoid procedures performed per year in Ontario and at the UHN between 1987 and 2007 were obtained from Statistics Canada and the Ministry of Health and Long-Term Care, Ontario, respectively. Population-adjusted rates of mastoid surgery for Ontario and the UHN. These data were collated and graphically represented for trend analysis. Results: The population-adjusted number of mastoid procedures for Ontario declined from 7.1 cases per 100 000 in 1986 to 4.1 cases per 100 000 in 2006. During this time the number of both modified radical mastoidectomies and revision mastoid surgeries at UHN increased.

Computed Tomography in Chronic Suppurative Otitis Media: Value in Surgical Planning

The present study was conducted prospectively to evaluate how accurately high resolution computed tomography scanning could define the extent and severity of the underlying disease in patients with chronic suppurative ear disease, thus, helping convert a surgical exploration into a planned procedure. Sixty adult consecutive cases of chronic suppurative otitis media underwent a detailed high resolution computed tomography by a single radiologist. The recorded radiological findings in various heads were then compared to the surgical findings during mastoid exploration of these patients by a single otologist and the two statistically compared. The presence and distribution of soft tissue in the middle ear cleft and mastoid could confidently be predicted using this modality. The malleus, body and short process of incus were well visualized, but not the long process of incus and the stapes suprastructure. Lateral semicircular canal fistulae could be demonstrated with an acceptable degree of accuracy. It was possible to detect facial nerve dehiscence and defects in tegmen tympani in significant number of cases although, statistical values were low for these structures. High resolution scanning is a modality which can accurately image the pathological anatomy in unsafe chronic suppurative otitis media. Otologists should use it more often, especially in complicated cases as an adjunct to better preoperative assessment, and thus, the surgical outcome. Its accuracy is likely to improve with larger studies and better experience, wherein its routine use may become justifiable.

Three-dimensional finite element analysis of Eustachian tube function under normal and pathological conditions

A primary etiological factor underlying chronic middle ear disease is an inability to open the collapsible Eustachian tube (ET). However, the structure–function relationships responsible for ET dysfunction in patient populations at risk for developing otitis media (OM) are not known. In this study, three-dimensional (3D) finite element (FE) modeling techniques were used to investigate how changes in biomechanical and anatomical properties influence opening phenomena in three populations: normal adults, young children and infants with cleft palate. Histological data was used to create anatomically accurate models and FE techniques were used to simulate tissue deformation and ET opening. Lumen dilation was quantified using a computational fluid dynamic (CFD) technique and a sensitivity analysis was performed to ascertain the relative importance of the different anatomical and tissue mechanical properties. Results for adults suggest that ET function is highly sensitive to tensor veli palatini muscle (TVPM) forces and to periluminal mucosal tissue (PMT) elasticity. Young children and cleft palate subjects exhibited reduced sensitivity to TVPM forces while changes in PMT stiffness continued to have a significant impact on ET function. These results suggest that reducing PMT stiffness might be an effective way to restore ET function in these populations. Varying TVPM force vector relationships via changes in hamulus location had no effect on ET opening in young children and cleft palate subjects but did alter force transmission to the ET lumen during conditions of elevated adhesion. These models have therefore provided important new insights into the biomechanical mechanisms responsible for ET dysfunction.

Intact Canal Wall Tympanomastoid Surgery: 10 Years Experience

310 cases of chronic middle ear disease were managed by Intact canal wall tympanomastoid surgery. Intact canal wall tympanomastoid surgery technique used in this study has been compared with the Jansen's classic ICW technique. A retrospective analysis of 310 cases of chronic middle ear pathology managed by Intact canal wall technique over a period of 10years (January 1996 to December 2005) was conducted. Patients information was collected from the patients record. 310 patients, aged 5-60years, were managed by Intact canal wall technique and followed for a minimum period of 5years. The male:female ratio in our study was 1.25:1 (172 males:138 females). The ratio of adult to pediatric patients was 2.7:1 (226 adults:84 children). Out of 274 (88.4%) cases having a regular follow up, 196 (71.6%) were found to be stable. Postoperative retraction of the tympanic membrane was observed in 24 (8.8%) cases and was managed by the placement of ventilation tube. Secretory otitis media was seen in 14 (5%) cases and was managed by myringotomy and placement of ventilation tube. 17(12.4%) cases were revised including recurrent disease (10 cases), residual disease (6 cases), graft failure (12 cases), stenosis of the ear canal (4 cases), meatomastoid fistula (2 case). Our technique of Intact canal wall tympanomastoid surgery takes less time than the classic Jansen's technique, is easily reproducible and taught, is a single stage procedure and can be universally applied to various chronic middle ear pathologies as the primary treatment with low recidivism rate.
