Chronic Consumptive Coagulopathy With A Uterine Leiomyoma

Summary: A case of benign uterine leiomyoma presenting as chronic consumptive coagulopathy and microangiopathic haemolytic anaemia (MAHA) is reported. Resection of the tumour was followed by recovery of normal haemostatic function. The histological finding of extensive thrombosis of sinusoidal blood vessels within the large tumour suggested interesting pathogenetic relationships.

DIFFUSE HEMANGIOMATOSIS, COAGULOPATHY AND MICROANGIOPATHIC HEMOLYTIC ANEMIA

Described here is an autopsy case of a 30-year-old woman with systemic hemangiomatosis accompanied by coagulopathy and microangiopathic hemolytic anemia. She had hepato-splenomegaly, anemia, thrombocytopenia, prolonged prothrombin time and partial thromboplastin time, and fibrinogenopenia. A splenectomy was performed and a diffuse angioma of the spleen was found. Postmortem examination revealed cavernous hemangiomas and hemangioendotheliomatous lesions in the liver, bone marrow, intestine, and lymph nodes. Coagulation studies suggested that exacerbation of coagulopathy occurred due to Liniac (10MV X-ray) irradiation. Our observation raised the possibility that the irradiation might lead to chronic localized consumption coagulopathy, which was confined to the hemangioma, to acute disseminated types of intravascular coagulation.

Activation of the coagulation system in connective tissue diseases. A study of [125I] fibrinogen clearance.

Fifty-one patients with different connective tissue diseases in either active or clinically inactive phase were studied to detect the incidence of chronic or subclinical consumption coagulopathy in these forms. Every patient received intravenously 20 mu Ci of [125I] fibrinogen and the clearance of the labeled fibrinogen was evaluated and expressed by the T/2 method. The data confirm the activation of the coagulation system in connective tissue diseases. In patients with S.L.E., enhanced fibrinogen consumption was significantly correlated with disease activity, DNA-binding activity test, serum complement level and an aspecific inflammatory index (serum mucoprotein level). These preliminary data suggest the usefulness of complementary treatment in connective tissue diseases to counteract this specific pathogenetic mechanism.

Coagulopathy in non-cirrhotic portal fibrosis.

SummaryA 12‐year‐old girl suffering from non‐cirrhotic portal fibrosis is presented. The clinical picture is characterized by splenomegaly without hematemesis and ascites. Portal hypertension was found without evidence of intra‐ or extra‐hepatic obstruction. Liver function test results were mildly disturbed. Open liver biopsy revealed fairly well preserved structure of the liver except for increased fibrosis confined to the portal spaces but without characteristic cirrhotic changes.Coagulation studies revealed disturbance of various coagulation factors, the most important being thrombocytopenia, hypofibrinogenemia and low levels of factors 11, VII and X. It is suggested that the primary underlying cause fcr these findings is a chronic consumptive coagulopathy and not liver disease.Appreciation of the coagulopathy accompanying this syndrome may contribute to our understanding of the pathogenesis of this entity.
