IT is indeed a distinct privilege to take part in a symposium honoring Drs Paul Chandler and Peter Kronfeld. Their professional careers, spanning the last 50 years, not only dealt extensively with the diagnosis and treatment of the glaucomas, but reflected the broad interests that so often characterize the inquiring mind and superior intellect. Dr Chandler is as well known for his considerations on the treatment of congenital cataract in infancy and childhood as for his insights into the medical and surgical management of malignant glaucoma. Reflecting the complete clinician, the subject of his Jackson Lecture in 1951 was Practical Considerations Concerning Choice of Operation in Convergent Squint. Dr Kronfeld's early professional career, beginning in the mid 1920s, first reflected his primary interest in physiology with basic studies on the aqueous humor, the lens, physiologic optics, and the retina. Cataract in childhood: photograph methods in assessment.

The slit-lamp examination of the child's lens with cataract can yield useful prognostic signs, especially when this is supplemented by slit-image photography and measurement. A normal lens size for the subject's age is a good prognostic sign and is usually associated with a static cataract or with very slow deterioration in a developmental cataract. A small lens and absence of the anterior subcapsular clear zone are each associated with progressive cataract. Nuclear lamellar cataracts are shown to become smaller in diameter with time, which favours medical treatment with mydriasis.

OUTCOME OF CATARACT SURGERY IN CHILDREN

Objectives: To evaluate the management and visual outcome of cataract surgery in children.
 Study design: Descriptive.
 Material and Methods: This study included one hundred pediatric patients having cataracts over a periodof two year, from Is' January 2005 to 31 st December 2006, at the Department of Ophthalmology, SaiduTeaching Hospital, Saidu Sharif, Swat.
 Results: Age range was fourteen years and below. Sixty five (65%) patients were males and thirty five (35%) females. Sixty three (63%) patients had congenital or developmental cataracts and thirty seven (37%) had traumatic cataracts. Extracapsular cataract extraction (aspiration) was performed on 150 eyes. Postoperative visual acuity was recordable in 112 eyes. Acorrected visual acuity of 6/18 or better was obtained in50 (44.64%) eyes. Forty one (36.6%) eyes obtained visual acuity of 6/24 to 6/60, while in 21 (18.75%) eyesthe visual acuity remained below 6/60. The most common postoperative complication was development ofthick posterior capsule, which occurred in 51 (34%) eyes and vitreous loss in 10 (6.6%) eyes. Pupil blockglaucoma developed in 4 (2.66%) eyes. No case developed endophthalmitis. Posterior chamber IOL wasimplanted in 9 (6%) eyes.
 Conclusions: Management of cataracts in children is still a problem and delayed presentation leads to poorvisual outcome. The public should be educated to seek early treatment for childhood cataracts. Paramedicsand doctors should be made aware of the problem and its management. Management of unilateral cataractsis still a difficult problem. Therefore search should continue for better surgical approach and better methodsof correction of aphakia so as to achieve better visual outcome.

Hereditary nuclear cataract.

IN 1947 a young girl and her brother came to one of us (Benedict) for treatment of bilateral cataract. Both patients had had the cataracts since early childhood, and their condition had remained stationary. Although visual efficiency was impaired, especially in bright light, both children had progressed excellently in school and were not obviously handicapped. The cataracts were central only, the nucleus of the lens being clearly outlined as a gray disk and the periphery of the lens being clear. The children's mother had undergone needling for bilateral cataract in childhood, with somewhat disappointing results. The children's aunt exhibited the same gray nuclear cataracts without gross visual difficulty. Three years previously, in 1944, this aunt had brought her two sons (cousins of the children described above) to one of us (Benedict) for treatment, and their clinical records contained a description of bilateral nuclear cataract corresponding exactly with that of their
