V. Treatment of Tuberculous Laryngitis. Considering Especially the Use of Chaulmoogra Oil

Many of the fixed oils are used in medicine, and some of them, such as cod liver, castor and chaulmoogra oils, are of great value in therapy. The discussions in this volume include the general properties of fixed oils and fats, their sources, methods of extraction, constitution and chemical properties, classification and identification, color tests, and methods of analysis. The substances are considered by groups, such as the olive oil group, the castor oil group and the tallow and butter group. Each oil is then considered separately, and the information needed by the analyst and technologist given with completeness. In the chapter on cod liver oil, no mention is made of the fact that this oil may be standardized for its value in deficiency diseases with considerable precision, or that several brands of oil so standardized are now on the market for medicinal use. CHAULMOOGRA OIL IN LEPROSY

To the Editor: —So much has been written of the results of the ethyl esters of chaulmoogra oil that I feel that I should report on our experience with the oil itself. We used our first chaulmoogra oil here about fourteen years ago by mouth, but about 1911 we began to use it by intramuscular injections, and have continued to do so since. Several preparations and combinations were used until finally we used the oil with 1 per cent. camphor intramuscularly. This month (July, 1922) we are discharging ten cases as arrested or probable cures with instructions that the patients return every six months for examination. From all appearance and as far as we can tell they are cured, though time only will tell us whether they are permanently cured. EXPERIENCE WITH CHAULMOOGRA OIL DERIVATIVES IN TREATMENT OF LEPROSY

The recent encouraging reports¹on the treatment of leprosy with the esters of chaulmoogra acid in the Hawaiian Islands induced us to give the leper patients at the San Francisco Hospital the benefits of this remedy. The extensive chemotherapeutic studies of Walker and Sweeney²at the Hooper Foundation of the University of California furthered our interest in the subject. Only by their active cooperation has this work been made possible. There have been from twenty to twenty-five patients with leprosy at the San Francisco Hospital for many years. When we began treating these cases, we did not anticipate the publication of our findings. Since the publication of Lukens' paper1on the use of chaulmoogra oil in tuberculous laryngitis, we have introduced this treatment in the Throat Clinic of the U. S. Veterans' Hospital, Maywood, Ill. Of the patients in this hospital, from 300 to 400 are suffering from pulmonary tuberculosis in some of its various stages. In the last four months, forty cases of tuberculous laryngitis were selected for treatment with chaulmoogra oil. Our technic differs from that of Dr. Lukens only in these respects: A special laryngeal syringe is used; the patients are treated daily, and only the 20 per cent, solution of chaulmoogra oil in liquid petrolatum is employed. The cases handled fall into four groups: GROUP 1 The largest number treated were those with distinct but not extreme laryngeal lesions. Recently Lukens¹called attention to the use of chaulmoogra oil in the treatment of tuberculous laryngitis, stressing the relief of pain and dysphagia which followed its use. Certainly, any agent which is capable of relieving the distress caused by an actively tuberculous larynx is worthy of the utmost consideration, and should be given a trial. Although the use of chaulmoogra oil has been attended by quite markedly beneficial results in the treatment of leprosy and, by some, is regarded as a specific in this disease, its use in tuberculosis is still in the experimental stage. Kolmer, Davis and Jager²have shown that the oil has no effect on virulent tubercle bacilli in their experiments on guinea-pigs, using an in vitro-vivo method. Since my original publication,1much interest has arisen as to the value of these fatty acids in leprosy and other diseases, especially tuberculosis. On reviewing the literature on the use of the oil by subcutaneous injections, I find that it was probably first used by Blanc of New Orleans as early as 1888, according to Dyer.2 Sandes3has also used it hypodermically at Robbins Island. Black4gave a mixture of olive oil and chaulmoogra oil. Ohmann-Dumesnil5gives preference to the hypodermic administration of the oil. Jeanselme,6in 1911, published the formula which was probably the basis of the Mercardo formula employed later in the Philippines. It was composed of chaulmoogra oil, guaiacol and camphor. To the Editor:— I would like very much, as a matter of record, to put before your readers the possibility of the fatty acid derivatives of the oil of Aleurites triloba or A. moluccana , being of great value in the treatment of leprosy and other skin diseases. The tree is more commonly known as the kukui tree and is distributed throughout Polynesia, the Philippines, India and especially Hawaii. The fatty acid group of this tree, to my mind, will prove to have equally as good a therapeutic action as that obtained by the fatty acid derivatives of chaulmoogra oil. I have been able to combine this group of fatty acids with iodin, thus producing an ideal iodin compound which, I believe, may be given subcutaneously or by intramuscular injections. Chaulmoogra oil has been advocated as a remedy for leprosy for many years, and interest in the subject has been greatly renewed by the investigations of Heiser,1 who has reported a series of cases either apparently cured or greatly improved by intramuscular injections of mixtures of chaulmoogra oil, camphorated oil and resorcin. Subsequent reports by Hopkins,2 McCoy and Hollmann,3 Coghill,4 Hall,5 Cadbury 6 and Hollmann and Dean,7 have indicated that these chaulmoogra mixtures have a definite curative effect in leprosy. As the B. leprae is an acid-fast micro-organism, these results naturally suggest that chaulmoogra oil may be destructive for other acid-fast bacilli, notably the tubercle bacillus, and at the suggestion of Dr. Jay F. THE HYPODERMIC USE OF CHAULMOOGRA OIL IN LEPROSY

Of the remedies used in the treatment of leprosy, chaulmoogra oil has given the most constant results. Its use by mouth, however, has been very difficult, since it induces such nausea that patients cannot bear prolonged treatment by that method. Those patients who can take the oil over a long period of time have invariably shown some improvement. In 1914 Heiser 1 reported exceptionally good results in a series of cases treated at the Leper Colony at Culion, P. I., with hypodermic injections of a chaulmoogra oil formula. Camphorated olive oil and resorcin were added to the chaulmoogra oil. No bad effects on the patients thus treated were noted, the oil being absorbed into the system without even local irritation. In May, 1916, this treatment was begun in a series of fourteen cases in a leper settlement outside the Hoi How city wall. Later seven more cases were added. These
