Clinical observation on treatment of postpartum hemorrhage by xuesaitong soft capsule

To observe the effect of Xuesaitong (XST) soft capsule in treating postpartum hemorrhage. Sixty pregnant women were randomly divided into the treated group (A) and the control group (B), 30 cases in each, they were treated with XST and placebo respectively for 7 days to observe the changes of hemorrhagic conditions. After delivery, the hemoglobin, RBC count, hematocrit and cardiac index were lower at 3rd day than those before delivery in both groups (P < 0.05 or P < 0.01), reticulocyte count lowered insignificantly as compared with that before delivery in group A but significantly in control group B; the bleeding time shortened significantly in group A (P < 0.05) but unchanged in group B; significant change of blood viscosity only found in group B (P < 0.05). The amount of bleeding 30 min, 1 hr, 2 hrs and 24 hrs after delivery in group A was all lower than that in group B respectively (P < 0.01 or P < 0.05). Involution of uterus in group A at 2nd, 4th and 7th day after delivery was all better than that in group B respectively (P < 0.05). XST has definite effect in decreasing postpartum hemorrhage, it also shows effects in promoting involution of uterus, reducing lochia and accelerating recovery of parturients.

Comparisons of blood viscosity between amphibians and mammals at 3°C and 38°C

(1) The range of temperature exposure of endotherms is narrow compared to ectotherms that can experience daily and seasonal temperature fluxes. (2) Comparison of the blood viscosity of amphibians (bullfrog, Woodhouse's toad, and marine toad) and mammals (horse, dog, and rat) at 3°C and 38°C was undertaken to determine if the effect of temperature on blood viscosity was diminished in amphibians relative to mammals. (3) Mammals did not consistently show greater changes in blood viscosity, plasma viscosity, or relative viscosity with decreasing temperatures relative to the amphibians in this study. (4) These data do not support our hypothesis that blood viscosity of amphibians is less affected by temperature than mammalian blood.

Effects of temperature and haematocrit on the relationships between blood flow velocity and blood flow in a vessel of fixed diameter

To determine whether temperature and haematocrit (Hct) alter the relationship between blood flow (BF) and blood flow velocity (BFV). Using a transcranial Doppler apparatus, we measured the peak velocity of whole blood cells pumped by a cardiopulmonary bypass (CPB) circuit, through a 0.15-cm internal diameter segment of rigid tubing. BF and BFV relationships were obtained at temperatures of 19, 28, and 37 degrees C and at Hct of 0.05, 0.22, 0.39, and 0.54, by altering CPB flow over a range from 10 to 100 cc/min. Linear regression analysis was performed. The relationship between velocity and flow for the pooled Hct data was y=(0.43)x+0.86, r2=0.998 and 95% CI (0.999-1) whereas the association for the temperature data was y=(0.42)x+0.02, r2=0.9998 and 95% CI (0.999-0.9997). Changes of blood viscosity had no effect on velocity at a given flow rate. The combined effect of Hct and temperature on velocity for the relationship with flow is expressed by: y=1.3+2.4x. In fixed diameter vessels with laminar flow, the linear relationship between flow and velocity is not affected by changes in temperature and Hct in clinical ranges. These results are explained by the Fahraeus-Lindquist effect. They support the use of transcranial Doppler sonography to estimate cerebral blood flow in infants who may have large variations of Hct and/or temperature during bypass.

Changes in Blood Viscosity by Heparin and Argatroban

Anticoagulants are effective for preventing both venous and arterial thrombosis. Although antithrombotic agents have been reported to reduce thrombin formation, to our knowledge, the relation between blood viscosity (BV) and antithrombotic agents has not been examined. We examined the effects on whole BV of various dosages of the antithrombotic agents—heparin sodium and argatroban. Thirty microliters of either drug was added to 3-ml samples of blood obtained from healthy male volunteers. Whole BV was then immediately examined with an oscillation-type viscometer. When either agent was added, BV decreased and coagulation time increased dose dependently. BV was 4.5±0.3 mPa·s in untreated blood but decreased in a dose-dependent manner to a minimum of 2.5±0.3 mPa·s with heparin sodium and decreased dose dependently in a sigmoid manner with argatroban. Because thrombin generation is inhibited in all antithrombotic therapies, this inhibition might be reflected by changes in BV. Our results suggest that BV in accordance with blood coagulability are indexes of thrombotic tendency and that decreasing BV prevents life-threatening thromboembolic conditions.

Hemorheology and renal function during cardiopulmonary bypass in infants.

Acute renal failure is an occasional complication after cardiopulmonary bypass in infants. Whereas it is well known that postoperative hemodynamics inflict acute renal failure, the influence of extracorporeal circulation on the kidney is less clear. Moreover, changes in blood viscosity occur during and after surgery, which may influence renal dysfunction. For this reason, we investigated the impact of blood viscosity on renal function during cardiopulmonary bypass. In 34 patients weighting less than 10 kg, we performed repeated analysis of urine, blood, and plasma viscosity. Polyuria and proteinuria that appeared during cardiopulmonary bypass indicated an elevated transglomerular filtration gradient, which recovered within 24 hours. The appearance of N-acetyl-beta-D-glucosaminidase in the urine, and elevated excretion of sodium, were additionally indicative of mild tubular damage. Elevation of blood viscosity during hypothermic perfusion showed a statistical correlation with proteinuria and N-acetyl-beta-D-glucosaminidaseuria. With hypothermia, the relation of blood viscosity to plasma viscosity became stronger, while the relation to the hematocrit decreased compared to normothermia. During cardiopulmonary bypass perfusion, the kidney can be stressed by proteinuria and mild tubular damage. Our data provide evidence that the kidneys can be protected by improved blood viscosity during cardioplegia, but this needs confirmation in a prospective interventional study.

Lack of hypercapnic increase in cerebral blood flow at high blood viscosity in conscious blood-exchanged rats.

The hypothesis of a compensatory dilation of cerebral vessels to maintain cerebral blood flow at a high blood viscosity was tested during hypercapnia in the study after replacement of blood by hemoglobin solutions of defined viscosities. If compensatory vasodilation exists at normocapnia at a high blood viscosity, vasodilatory mechanisms may be exhausted when hypercapnia is added, resulting in a lack of increase in cerebral blood flow at hypercapnia. In conscious rats, blood was replaced by ultrapurified cross-linked hemoglobin solutions that had defined and shear rate-independent low or high viscosities (low- and high-viscosity groups). Blood viscosity differed threefold between both groups (1.2 vs. 3.6 mP x s). Thereafter, rats inhaled either a normal or an increased concentration of carbon dioxide in air. Cerebral blood flow was determined by the iodo[14C]antipyrine method. During normocapnia, global and local cerebral blood flows did not differ between both groups. With increasing degrees of hypercapnia, global and local cerebral blood flows were gradually elevated in the low-viscosity group (2.8 ml x mmHg(-1) CO2 x 100 g(-1) x min(-1)), whereas they remained unchanged in the high-viscosity group. Changes in blood viscosity do not result in changes of cerebral blood flow as long as cerebral vessels can compensate for these changes by vasodilation or vasoconstriction. However, such vascular compensatory adjustments may be exhausted in their response to further pathophysiologic conditions in blood vessels that have already been dilated or constricted as a result of changes in blood viscosity.

Haemoconcentration, shear-stress increase and carotid artery diameter regulation after furosemide administration in older hypertensives.

The aim of the present study was to determine whether changes of carotid wall shear stress induced by changes in blood viscosity after diuretic administration cause carotid arterial dilatation in elderly hypertensives, as reported in the cat. Arterial wall shear rate (ultrasound technique, profilmeter FRP III), the systo-diastolic diameter (echotracking technique) and the mean blood flow velocity and volume of the common carotid artery, the blood viscosity (rotational viscometer) and the finger arterial blood pressure (Finapress Ohmeda) were measured in 12 young volunteers (aged 25+/-2 years) and in 12 elderly hypertensives (aged 80+/-4 years) treated with short-acting calcium antagonists up to 24h before the study, both at baseline and after intravenous furosemide infusion (0.5mg/min), when the haematocrit had increased by at least two percentage points. After furosemide administration the mean arterial blood pressure decreased and blood viscosity and carotid systolic shear stress increased in both groups. However, common carotid artery diameter increased only in the young controls but not in the elderly hypertensives. These data show that an increase in carotid shear stress caused by haemoconcentration induces carotid vasodilatation only in young healthy subjects, and not in elderly hypertensives. This effect may be related to impaired endothelium function and/or arterial wall mechanics.

Properties of polyvinyl pyrrolidone/beta-chitosan hydrogel membranes and their biocompatibility evaluation by haemorheological method.

The semi-IPN hydrogel membranes of polyvinyl pyrrolidone (PVP) and beta-chitosan were synthesized by crosslinking beta-chitosan with glutaraldehyde. Hydrogel membranes were characterized by spectroscopic, swelling, thermal and mechanical properties. The in vitro biocompatibility of hydrogel membranes was studied by haemorheological method. These hydrogels have water contents in the range of 60-70% with a high fraction contributed by free water (> 45%). The gel composition, amount of cross-linking agent and swelling temperature plays an important role in swelling kinetics of these semi-IPN membranes. Melting temperature (Tm) of membranes increased with a decrease in endothermic peak with increasing beta-chitosan content. The tensile strength of membranes in the dry state was found to be high (29-43 MPa) and it increased with increasing beta-chitosan content. The in vitro haemorheological studies indicated the biocompatible nature of membranes with no significant changes in whole blood and plasma viscosity and red blood cell rigidity.

Blood viscosity and hematological changes during prolonged submergence in normoxic water of northern and southern musk turtles (Sternotherus odoratus

Musk turtles (Sternotherus odoratus) can survive at least 150 days of submergence in normoxic water at 3°C, during which time there are large increases in packed cell volume (PCV). We investigated the effects of submergence in normoxic water at 3°C on the blood viscosity of musk turtles from northern (Massachusetts) and southern (Alabama) locales. Blood was collected from air-breathing turtles and after 20, 50, 100, and 150 days of submergence in normoxic water at 3°C. Hematological responses to submergence were similar in the two groups, therefore the results were combined. Packed cell volume increased steadily above that of controls after 20, 50, 100, and 150 days of submergence. Hemoglobin concentration also progressively increased above that of controls after 20, 50, and 100 days of submergence but declined to near control values after 150 days. Blood viscosity increased with increasing PCV; however, blood viscosity of musk turtles appears less affected by PCV than is blood viscosity of mammalian species. As such, musk turtles appear to be able to maintain adequate blood flow to tissues while increasing the oxygen carrying capacity of the blood during prolonged submergence. However, after 150 days submergence, oxygen delivery should decrease due to a reduced oxygen carrying capacity of the blood and an increased resistance to blood flow, which may limit the length of time these turtles can remain viable during hibernation. The effects of sex, season, and hibernation on haematology and blood viscosity of free-ranging echidnas (Tachyglossus aculeatus)

We investigated seasonal changes in haematology and blood viscosity in free-ranging echidnas, Tachyglossus aculeatus (37 females, 28 males, and 17 juveniles), some of which had been fitted with radio transmitters, allowing them to be located and bled during hibernation. Only one haematological parameter, mean cell haemoglobin concentration, appeared to show any clear relationship with hibernation. Seasonal variation was most marked in females, with red-cell count, haematocrit, and haemoglobin levels highest in summer and lowest in spring (P &lt; 0.001). Males had higher values than females or juveniles (P &lt; 0.05). Plasma osmolality was highest in summer (307 ± 4 mosmol/kg, n = 11) and lowest in autumn (290 ± 6 mosmol/kg, n = 6). Contrary to expectation, blood from hibernating echidnas, when measured at 32°C and at a haematocrit of 45%, showed significantly higher viscosity than blood from nonhibernating echidnas. These increases in mean cell haemoglobin concentration and blood viscosity during hibernation appear to be due to a build-up of senescent red cells and a reduced rate of erythropoiesis.

Relationship between rheological and hemodynamic changes in rats with crush syndrome.

Narcotized rats with crush syndrome develop severe syndrome of increased blood viscosity. A strict correlation was found between changes in blood viscosity at different shear rates and total peripheral vascular resistance. The severity of central hemodynamic and hemorheological disturbances was different in rats with different reaction of total peripheral vascular resistance to injury accompanied by shock.

Effect of batroxobin on spontaneous echo contrast and hemorheology in left atrial appendage in atrial fibrillation assessed by transesophageal echocardiography

Controversy exists regarding the effect of defibrination on spontaneous echo contrast and flow dynamics in left atrial appendage (LAA) in atrial fibrillation. We aimed to investigate the effect of batroxobin, which decreases plasma fibrinogen level, on the echo intensity of spontaneous echo contrast in LAA. In 36 patients with atrial fibrillation (duration 7 ± 4 years), transesophageal echocardiography was performed at baseline and 24 hours after batroxobin administration (0.2 U/kg). At the orifice of the LAA, integrated backscatter of echo contrast and peak velocity of LAA emptying flow were measured. Plasma fibrinogen and whole blood viscosity were also measured. Fibrinogen and viscosity were significantly lower after batroxobin administration (96 ± 38 mg/dl and 4.35 ± 0.56 cp) than those at baseline (320 ± 61 mg/dl and 4.71 ± 0.61 cp, both p <0.001). A significant positive correlation between changes in plasma fibrinogen and whole blood viscosity (r = 0.49, p = 0.002) was shown. The integrated backscatter significantly decreased from 14 ± 3 to 12 ± 3 decibels after batroxobin (p <0.001), and the changes in integrated backscatter and plasma fibrinogen was significantly correlated. Therefore, batroxobin administration improved blood rheology and decreased blood cell aggregation, which are effective in preventing left atrial thrombus formation.

Distribution of body fluid and change of blood viscosity in broilers ( Gallus domesticus) under high temperature exposure

This study was to observe the distribution of body fluid by measuring blood volume, extracellular and intracellular fluid volumes and total body water under heat exposure, in order to clarify the mechanism of decrease in whole blood viscosity of the heat-exposed broilers. Whole blood viscosity, haematocrit, plasma protein concentration, plasma osmolality and extracellular fluid volume decreased during high temperature exposure, while plasma and blood volumes increased. No significant changes were found in both intracellular fluid volume and total body water between thermoneutral and high temperature exposure. These results indicate the decreased whole blood viscosity is induced by a plasma volume expansion, in which water may come from the interstitial space and alimentary tract, under heat exposure.

Effects of glucose in drinking water on the changes in whole blood viscosity and plasma osmolality of broiler chickens during high temperature exposure

This study was conducted to elucidate the influence of glucose in drinking water on whole blood viscosity and plasma osmolality of broilers during high temperature exposure. Two groups of birds, which had ad libitum access to either a 4% glucose-water solution (Group G) or tap water (Group W), were exposed simultaneously to 30 C from 0300 h for 12 h each day for 3 d. During the experimental period, Group G birds had greater metabolic energy intake and body weight gain than Group W. Hematocrit and whole blood viscosity decreased significantly at 30 C compared to controls at 20 C in Group W, whereas, in Group G, no changes were found for these two variables. Plasma osmolality also decreased at 30 C compared to 20 C in Group W, whereas no change was noted in this variable in Group G. However, at 20 C, plasma osmolality was significantly higher in Group W than in Group G, but no difference was observed between the two groups at 30 C. Plasma protein concentration decreased during exposure to 30 C in both groups, but the decrease tended to be greater in Group W than in Group G. These results suggest that glucose intake may alleviate the influence of heat stress on whole blood viscosity and plasma osmolality.

Hemodilution reduces clinic and ambulatory blood pressure in polycythemic patients.

Limited information is available for humans on whether blood viscosity affects total peripheral resistance and, hence, blood pressure. Our study was aimed at assessing the effects of acute changes in blood viscosity on both clinic and 24-hour ambulatory blood pressure (BP) values. In 22 normotensive and hypertensive patients with polycythemia, clinic and 24-hour ambulatory BPs were measured before and 7 to 10 days after isovolumic hemodilution; this was performed through the withdrawal of 400 to 700 mL of blood, with concomitant infusion of an equivalent volume of saline-albumin solution. Hematocrit, plasma renin activity, plasma endothelin-1, right atrial diameter (echocardiography), and blood viscosity were measured under both conditions. Plasma renin activity and right atrial diameter were used as indirect markers of blood volume changes. Plasma endothelin-1 was used to obtain information on a vasomotor substance possibly stimulated by our intervention, which could counteract vasomotor effects. Isovolumic hemodilution reduced hematocrit from 0.53+/-0.05 to 0.49+/-0.05 (P<.01). Plasma renin activity, plasma endothelin-1 and right atrial diameter were unchanged. Clinic blood pressure was reduced by hemodilution (systolic, 144.3+/-5.4 to 136.0+/-3.9 mm Hg[mean+/-SEM]; diastolic, 87.0+/-2.8 to 82.1+/-2.6 mm Hg, P<.05 for both) and a reduction was observed also for 24-hour average ABP (systolic, 133.6+/-2.9 to 129.5+/-2.7 mm Hg; diastolic, 80.0+/-2.0 to 77.3+/-1.7 mm Hg, P<.05 for both). The reduction was consistent in hypertensive patients (n = 12), whereas in normotensive patients (n = 10) it was small and not significant. Both clinic and 24-hour average heart rates were unaffected by the hemodilution. Thus, in polycythemia, reduction in blood viscosity without changing blood volume causes a significant fall in both clinic and 24-hour ambulatory BPs; this is particularly true when, as can often happen, blood pressure is elevated. This emphasizes the importance this variable may have in the determination of blood pressure and the potential therapeutic value of its correction when altered.

Venous dysfunction and the change of blood viscosity during head-up tilt.

The precise stimulus that induces vasovagal syncope is still unclear. We have previously demonstrated that the peripheral distribution of blood volume (venous pooling) is a strong predictor of tilt induced vasovagal reaction. We hypothesized that an increase in venous pooling during tilt accentuates the measured increase in blood viscosity. This hypothesis is based on the previously demonstrated increase in venous pressure and subsequent increase in transcapillary fluid transudation during tilt. The increased blood viscosity, in turn, increases vascular shear rate, which may alter the vasoconstrictive and other cardiovascular responses to decreased preload. We measured blood viscosity (supine and tilt) in 56 patients with a history of orthostatic intolerance (37 with venous pooling [VP] and 19 without venous pooling [non-VP]). VP and non-VP were separated into subgroups based on blood pressure and heart rate response to tilt. There was a positive correlation between blood viscosity and plasma aldosterone in the supine. In the group as a whole, neither supine blood viscosity nor its increase during tilt differed between VP and non-VP. However, the tilt induced increase of blood viscosity was significant only in patients with tilt provoked tachycardia plus normal blood pressure response in VP group. We suggest that the increase of blood viscosity in this group led to the normal blood pressure response. The positive correlation between supine blood viscosity and supine plasma aldosterone indicates that the normal blood pressure response in this group possibly was via stimulation of the renin-angiotensin-aldosterone system.

An analysis of hypoxia in sheep brain using a mathematical model.

Cerebral blood flow (CBF) increases as arterial oxygen content falls with hypoxic (low PO2), anemic (low hemoglobin) and carbon monoxide (CO) (high carboxyhemoglobin) hypoxia. Despite a higher arterial PO2, CO hypoxia provokes a greater increase in CBF than hypoxic hypoxia. We analyzed published data using a compartmental mathematical model to test the hypothesis that differences in PO2 in tissue, or a closely related vascular compartment, account for the greater response to CO hypoxia. Calculations showed that tissue, but not arteriolar, PO2 was lower in CO hypoxia because of the increased oxyhemoglobin affinity with CO hypoxia. Analysis of studies in which oxyhemoglobin affinity was changed independently of CO supports the conclusion that changes in tissue PO2 (or closely related capillary or venular PO2) are predictive of alterations in CBF. We then sought to determine the role of tissue PO2 in anemic hypoxia, with no change in arterial and little, if any, change in venous PO2. Calculations predict a small fall in tissue PO2 as hematocrit decreases from 55% to 20%. However, calculations show that changes in blood viscosity can account for the increase in CBF in anemic hypoxia over this range of hematocrits.

Pulmonary blood volume and its effects on pressure/flow relations and flow resistance in isolated lungs of rabbits.

Quantitative information about the effects of pulmonary blood volume (Qp) on pulmonary haemodynamics is lacking since Qp changes inevitably with flow. To separate flow-dependent from volume-dependent changes in intravascular pressures we imposed changes in Qp (measured continuously) by altering outflow pressure in seven isolated, blood-perfused rabbit lungs and studied the effects of Qp on the relations between arteriovenous pressure gradient (DeltaP) and blood flow (Q.) under two conditions: flow-dependent volume changes were either permitted or compensated. In the latter circumstances, DeltaP changed more for a given change in Q.. The DeltaP/Q. relations were shifted to smaller DeltaP when Qp was increased. Hence, the calculated flow resistance (R = DeltaP/Q.) decreased with increasing Qp at a given Q.. Assuming constant viscosity, changes in R can be predicted from changes in vessel geometry and thus Qp. We found that R increased less than expected (by a factor of 3-7.5 instead of 9) when Qp was reduced to one-third. This discrepancy may be explained by a change in blood distribution within the lung despite constant Qp and by a change in apparent blood viscosity with Q.. Regardless of these speculations we have shown that Qp determines DeltaP at each flow and thus flow resistance.

Correction of erythrocyte shape abnormalities in familial hypercholesterolemia after LDL-apheresis: does it influence cerebral hemodynamics?

It is well known that red blood cells incubated in low-density lipoprotein (LDL)-rich medium show shape abnormalities that revert to normal after reincubation in normal plasma. Patients with homozygous familial hypercholesterolemia (HFH) have an increased percentage of abnormally-shaped erythrocytes (mostly stomatocytes, knisocytes, and crenated cells) compared to normocholesterolemic controls: 7.73+/-0.96 versus 3.52+/-0.52 (mean+/-SEM; P = 0.001). To confirm the role of high LDL concentration in inducing red cell shape abnormalities we determined the percentage of abnormally shaped erythrocytes in seven HFH patients 1 day after the procedure of LDL-apheresis with a 40% cholesterol decrease. A reduction in kniscocytes, stomatocytes, and crenated cells was observed in the patients treated by LDL-apheresis (P < 0.01). To investigate the possible benefit of a reduction in erythrocyte shape abnormality on cerebral hemodynamics, cerebral flow velocity, as evaluated by transcranial Doppler, was evaluated concomitantly and found to be remarkably increased after apheresis (P < 0.01). No significant change in hematocrit, plasma viscosity, blood viscosity, mean pressure, or cardiac output was detected, 1 day after apheresis. An inverse correlation was demonstrated (r = 0.55; P = 0.04) between changes in the percentage of knisocytes+stomatocytes +crenated cells and percent changes in middle cerebral artery peak systolic velocity. The correction of erythrocyte shape abnormalities after LDL-apheresis might be related to dramatic changes in plasma phospholipid concentration and proportion occurring after this procedure in HFH patients. The reduction of erythrocyte shape abnormalities could contribute, together with other hemorheological factors, to the improvement of cerebral hemodynamics after LDL-apheresis.
