Calcified Intracranial Hydatid Cyst: Case Report

We present a 26-year-old patient who did farming related work had an asymptomatic intracranial hydatid cyst. A calcified intracranial cystic mass was found on radiological investigations and the patient underwent surgery. The mass was totally excised. The histopathology result reported a hydatid cyst. There were no postoperative complications. Calcification is quite rare in cerebral hydatid cyst. The computed tomography and magnetic resonance imaging provide information that is especially useful for preoperative diagnosis. Observing membrane detachment and daughter cysts during these investigations is pathognomonic. The presence of calcification in the cyst wall and intracystic membranes indicated hydatid cyst.

Computed tomography findings in cerebral hydatid disease

To describe imaging findings of cerebral hydatid cysts on computed tomography of brain. We retrospectively reviewed CT scans of brain in 5 patients with pathologically confirmed hydatid cysts in cerebral hemispheres. The patients were scanned either on a spiral (single slice) CT or on multidetector-row CT before and after intravenous injection of iodinated contrast material. All the patients were children aged 8 to 13 years with 3 boys and 2 girls. Features of raised intracranial tension were present in all the cases at presentation. CT findings of a large intracerebral cystic lesion with significant mass effect and without any calcification or enhancement were common in all 5 cases. Perilesional edema was present in 1 case. Cerebral hydatid was seen as either a homogenous fluid attenuation unilocular cyst (3 cases) or a unilocular cyst with few peripheral daughter cysts (1 case) or cyst filled by multiple daughter cysts inside (1 case). All the cases were operated and cyst was removed completely after craniotomy. Accurate preoperative diagnosis of cerebral hydatid by CT followed by surgery with care to avoid cyst rupture can result in favorable outcome.

Multidetector-row computed tomography in cerebral hydatid cyst

Intracranial localization is a rare manifestation of hydatid cyst disease (Echinococcosis). It comprises only 2% of cases of Echinococcosis infection even in endemic areas and is predominantly seen in children. Clinical manifestations resulting from raised intracranial tension are nonspecific. Imaging with computed tomography (CT) may suggest the diagnosis preoperatively with reasonable accuracy. Multidetector-row CT (MDCT) with its high resolution multiplanar reformations can demonstrate the relationship of the cyst with adjacent brain structures and thus help in planning surgery. This has a practical utility in places where magnetic resonance imaging is not available. We describe a case of cerebral hydatid cyst in a 13-year-old boy who was diagnosed with MDCT, which helped in planning its surgical removal.

Multiple cerebral hydatid cysts

Contrast enhanced CT (Figures 1 to4) showed multiple well-defined bilat-eral, rounded, cystic lesions in the supra-tentorial neuroparenchyma. One of thelesions showed daughter cysts (Figure2). There was no perilesional edema. Oninjection of contrast, the lesions did notshow significant enhancement. In addi-tion to these lesions, there was cerebraledema. With these findings, a diagnosisof multiple cerebral hydatid cysts wassuggested. Abdominal sonography didnot reveal any cysts in other organs. Thepatient did not improve with conserva-tive management and surgery was per-formed. The accessible cystic masseswere removed and were confirmed to behydatid cysts upon histologic examina-tion. The patient improved with addeddrug treatment.

Surgery of cerebral hydatid cysts:102 cases experience

Objective To discuss effect and clinical classification of 102 patients with cerebral hydatid cysts retrospectively. Method According to hallmark of clinical manifestation and imaging,hydatid cysts were divided into two types,one is echinococcosis granulosa,the other is echinococcsis multilocularies. Echinococcosis granulosa were totally resected by dowling method and echinococcsis muhilocularies were also totally ablated. Results Of 102 patients,94 patients were totally removed without cysts rupture,8 cases were totally with cysts rupture, and 3 cases were died of allergic shock with infection. Five cases relapsed and reoperated. Conclusions Despite any kinds, patients can be cured when the hydatid cysts were totally removed. But suegeons should avoid allergic shock by en block resection without cysts rupture. Key words: Echinococcosis; Microsurery

The complications of cerebral hydatid cyst surgery in children

Complications related to cerebral hydatid cyst surgery are not uncommon but require prompt diagnosis and treatment. The aim of this study is to demonstrate the radiological findings that would indicate complications after cerebral hydatid cyst surgery in children and to report our results. The data of 25 pediatric patients who underwent surgery for cerebral hydatid cysts over a 16-year period were analyzed retrospectively. The complications related to surgical technique and cyst location were recorded. Postoperative radiological findings of the patients were documented. Intraoperative cyst rupture occured in three patients. Subdural effusion developed in five patients, porencephalic cyst in four, subdural effusion associated with porencephalic cyst in two, hemorrhage in two, epidural hematoma in one, and pneumocephalus in three patients. Subdural-peritoneal shunt was placed in two patients. Recurrence of cerebral hydatid cyst was observed in only one patient who experienced intraoperative cyst rupture. There was no surgery-related death. The complications following cerebral hydatid cyst surgery in children are various and usually not fatal. When appropriate surgical procedures are planned and carefully applied, the results will be excellent with no mortality.

Cranial epidural hydatid cysts: Clinical report and review of the literature

Cerebral hydatidosis accounts for approximately 1-3% of all cases of hydatid disease. In this report, 32-year-old female with epidural multilocular hydatid cysts in the midline posterior parietal region is reported. Most commonly, cerebral hydatid cysts are single lesions and locate in the watershed of the middle cerebral artery. However, cerebral epidural hydatid cysts seem to have a tendency occurring around in the highly vascularized venous sinus.

Cerebral hydatid cysts: technique and pitfalls of surgical management

Hydatid cysts are rare, but most often they occur in the liver and lungs. Cerebral manifestation is very rare, and surgery is the main treatment. The goal of surgery is to remove the cysts in toto without rupture. The aim of this study was to investigate the surgical technique of removing cerebral hydatid cysts and to show the possible pitfalls of surgery. This study included 17 patients who underwent surgery for an intracranial hydatid cyst. The Dowling technique was used in all patients. This technique is based on the large cranial opening, careful handling, meticulous cortical dissection, and removal of the cyst by hydrostatic assistance. The hydatid cysts were removed unruptured in 88% of the cases. The cysts ruptured intraoperatively in 2 patients, and the ventricular system was opened in 1 of them. Anaphylactic reaction or chemical meningitis did not occur, but recurrence and spinal seeding were observed in the follow-up of these patients. The thin cyst wall, periventricular locations, and microadhesions to the surrounding brain tissue were the main surgical problems. None of the patients died after the surgery. Although this technique seems safe and easy, there are some pitfalls concerning the cyst location and surgical approach. Successful management requires a flexible therapeutic strategy and meticulous dissection by an experienced surgeon.

Primary soliter and multiple intracranial cyst hydatid disease: Report of five cases

Echinococcosis is a parasitic disease caused by infestation of various body tissues by the encysted larvae of Echinococcosis granulosa, the tapeworm. The disease because of the inefficiency of environmental health and protective care and dealing with animals is prevalent in South America, Africa, Australia, the Mediterranean countries and the Middle East. Cerebral hydatid cysts are rare and comprise only 0.5–3% of all reported hydatid cysts. Brain involvement is seen rare and primary form is very rare. We report five cases of primary hydatid disease of the brain. In three cases (60%), brain involvement was solitary, while two cases (40%) had primary multiple intracranial hydatid cysts. This study confirms that hydatid disease is a difficult health problem in Turkey.

Multiple infected cerebral hydatid cysts

We report an unusual patient with multiple infected cerebral hydatid cysts. A 20-year-old man presented with a 2-month history of headache and progressive left-sided hempiparesis. A cerebral CT scan showed a large and heterogeneous parieto-occipital lesion. During surgery an infected hydatid cyst was discovered with multiple daughter vesicles. Post-operatively the patient was treated with albendazol, cefotaxime and metronidazole. The clinical course was good with total recovery of the hemiparesis. A follow-up CT scan showed persistence of some small deep-seated cysts. Multiple infected cerebral hydatid cyst is uncommon and can be confused with other cystic brain lesions. The aim of surgery is to remove the cyst unruptured and this should be followed by antihelminthic and antibiotic treatment in order to avoid recurrences.

Hydatid cyst is rarely observed in the brain (0.5–4.5%). The frequency of calcified cyst is less than 1%. We present a case of a 15-year-old girl with a 5-year follow-up for grand mal seizures that became resistant to three-drug therapy. The CT scan revealed a calcified parieto-occipital lesion. MRI disclosed a suggestive detached membrane. At surgery, the cyst wall was calcified with typical hydatid sand contents. Since surgery, seizure control has been achieved with one drug. Calcification of a cerebral hydatid cyst is exceptional. MRI enabled the diagnosis in this patient.

Multiple cerebral hydatid cysts: Report of a case with 24 pieces

A case of 19 years old female patient with 24 large primary cerebral hydatid cysts, in different localisations is presented. Cysts were removed unruptured by five separate operations in 6 months. The patient was observed to be in good health 1 year after the last operation. This case is the first case presented that contains such high number of cysts to our knowledge. The importance of MRI examination in localizing the cysts and designing the operative plan is emphasized. Interestingly, albendazole, which is an antiparasitic drug, was reported to be ineffective by infectious disease department of our hospital, because of large diameter of the cysts, in this case.

Hydatid cysts of the internal acoustic canal and jugular foramen

Brain involvement with hydatid disease occurs in 1–2% of all Echinococcus granulosus infections. Cerebral hydatid cysts are usually supratentorial, whereas infratentorial lesions are quite rare. Here we report a 19-year-old man with hydatid cysts in the right cerebellopontine cistern with the involvement of internal acoustic canal and jugular foramen. The patient presented with signs of increased intracranial pressure and multiple cranial nerve palsies. Surgery was performed in the semi-sitting position using a lateral suboccipital approach for a right-sided craniotomy. Magnetic resonance imaging clearly demonstrated cisternal, neural and vascular relationships which aided in intact surgical removal of the lesion using microsurgical techniques. Total removal without rupture should be the surgical goal in all hydatid cysts.

Microsurgery in the Removal of a Large Cerebral Hydatid Cyst: Technical Case Report

Surgery is presently the mainstay in the treatment of cerebral hydatid cysts, but removal without perforation of the cyst wall is critical. The Dowling-Orlando technique has been the main surgical choice for the intact removal of intracranial hydatid cysts. Dowling described his technique in 1929, well before the introduction of the microscope in neurosurgery. Using the operating microscope has not been reported in the removal of a large hydatid cyst. The case of a 16-year-old male with a large, right parieto-occipital hydatid cyst is presented. In this case, surgery was greatly aided by the use of the microscope in the initial stage of the removal of the cyst. Cyst extraction during the delivery process was continued without the aid of the microscope. The cyst was successfully removed intact. The use of the magnification during the early part of the surgery of a hydatid cyst is extremely helpful because, at a crucial stage of the procedure, it prevents inadvertent damage of the very thin cyst wall and allows development of the critical surgical plane.

Co-occurrence of Cardiac and Cerebral Hydatid Cysts: A Case Report

Cardiac and cerebral hydatid cysts are rarely encountered. In this case, we report a male patient admitted to our hospital with hemiparesis, headache and dysphasia which occurred as a result of complications of both 3,1x3,5 cm single hydatid cyst in the left cardiac ventricle and multiple cerebral hydatid cysts (approximately 18 particles, the largest being 3× 2,2cm). He had undergone surgery 17 month earlier due to a multiple brain hydatid cysts. Although he had been treated with albendazole, multiple cerebral hydatid cysts re-appeared 17 months after operation. This was a rare case in which left ventricular intracavitary hydatid cyst occurred together with brain multiple cysts at the same time. Physicians should be alert about the probability of cardiac involvement when a cerebral hydatid cyst is diagnosed and appropriate investigations should always be conducted.
