Corneal Endothelial Morphological Alterations in Type 2 Diabetes: Associations with Glycemic Control, Disease Duration, and Diabetic Retinopathy Severity—A Cross-Sectional Study in Benghazi, Libya

Background The corneal endothelium is essential for maintaining corneal transparency and fluid regulation, and its dysfunction can lead to corneal edema and decreased visual acuity. Corneal specular microscopy is used to detect corneal disorders early. Aim This study aimed to evaluate morphological changes in the corneal endothelium of patients with type 2 diabetes mellitus (DM) using specular microscopy. Methods This study enrolled 50 individuals with type 2 diabetes and 50 nondiabetic individuals as control subjects. Patients with certain ocular and systemic conditions were excluded. The collected data included demographic information, medical history, recent hemoglobin A1c (HbA1c) values, visual acuity, ophthalmological examination, and diabetic retinopathy (DR) staging. The corneal endothelial morphology was evaluated using a noncontact specular microscope, which measured corneal endothelial cell density (ECD), central corneal thickness (CCT), coefficient of variation of cell size (CV), and hexagonality. The data were analyzed using SPSS software, and the results were presented as mean ± standard deviation, numbers, and percentages. An unpaired t-test was used to compare different means, and a p-value less than 0.05 was considered statistically significant. Results A majority of diabetic patients (62%) had diabetes for more than 10 years, and 58% of them had higher than 7.5% HbA1c levels. In terms of DR severity, 46% of patients had mild nonproliferative diabetic retinopathy (NPDR), 36% had moderate NPDR, and 10% had severe NPDR. There were significant differences between diabetic and nondiabetic groups in corneal ECD (2480 ± 223 cells/mm2 for DM group vs. 2652 ± 234 cells/mm2 for non-DM), hexagonality (39.6 ± 2.8% for DM group vs. 47 ± 2.1% for non-DM), CV (42 ± 2.9% for DM group vs. 35.5 ± 2.3% for non-DM), and CCT (550 ± 14.8 µm for DM group vs. 530 ± 9.6 µm for non-DM). Patients who had diabetes for more than 10 years had significantly lower ECD (2356 cells/mm2 vs. 2689 cells/mm2), lower hexagonality (39 vs. 41%), and higher CV (43 vs. 41%) and higher CCT (553.9 ± 4.6 vs. 545.5 ± 4.0) than those with less than 10 years of diabetes. As the severity of DR increased, there was a significant decrease in ECD (from 2641 ± 194 cells/mm2 for mild NPDR to 2310 ± 82 cells/mm2 for severe PDR), a decrease in hexagonality (from 40.9 ± 3.2% for mild PDR to 37.4 ± 1.1% for severe PDR), an increase in CV (from 40.1 ± 2.3% for mild PDR to 44 ± 2.9% for severe PDR), and an increase in CCT (from 543.7 ± 13.8 for mild PDR 563.8 ± 2.9 for severe PDR). Conclusion This study revealed that type 2 diabetes is associated with significant reductions in ECD and hexagonality, as well as an increase in CV and CCT. The severity of DR and the duration of diabetes were correlated with changes in these parameters. These findings underscore the need for a comprehensive evaluation of corneal health in diabetic patients.

Evaluation of clinical and histological effects of KGF-2 and NGF on corneal wound healing in an experimental alkali burn rabbit model

Endogenously produced peptide growth factors such as keratinocyte growth factor-2 (KGF-2) and nerve growth factor (NGF) play a key role in the natural corneal wound healing process. However, this self-healing ability of the corneal tissue is often impaired in cases of severe corneal damage, as in corneal alkali injuries. In the present study, we investigated the clinical and histopathological effects of topical recombinant human keratinocyte growth factor-2 and nerve growth factor treatments in a rabbit model of corneal alkali burn. After induction of an alkali burn, 24 rabbits were divided equally into three groups: control group, KGF-2 group, and NGF group. Clinical parameters including epithelial healing, opacification, neovascularization and central corneal thickness were evaluated on the first (D1), seventh (D7) and fourteenth (D14) days after injury. Corneal histology was performed using hematoxylin/eosin (H&E) and Masson's Trichrome stains. Immunohistochemical staining for matrix metalloproteinase-2 (MMP-2), MMP-9 and transforming growth factor-β (TGF-β) was performed. On D14, the percentage of epithelial defect and opacity were significantly less in the KGF-2 and NGF groups compared to the control group (p < 0.05). There was no significant difference between the groups in central corneal thickness. In the evaluation of neovascularization on D14, the NGF group was significantly less vascularized than the control group (p = 0.011). Histological examination showed a significant increase in stromal edema and inflammation in the control group compared to both treatment groups (p < 0.05). There was also a significant difference between the NGF and control groups in histological evaluation of epithelial repair and vascularization (p < 0.05). When immunoreactivity of MMP-2, MMP-9 and TGF-β was examined, there was a significant increase in the control group compared to the NGF group (p < 0.05). Taken together, both NGF and KGF-2 treatments were effective for early re-epithelialization and decrease in inflammation, opacity and neovascularization after corneal alkali burn. The inhibitory effect of NGF treatment on chemical-induced neovascularization was found to be superior to KGF-2 treatment.

Assessment of corneal biomechanics, tonometry and pachymetry with the Corvis ST in myopia

To evaluate the repeatability of Corvis ST corneal biomechanical, tonometry and pachymetry measurements, and agreement of pachymetry measures with the Pentacam HR and RTVue OCT. Three consecutive measurements of the right eye of 238 myopic subjects were acquired with the Corvis ST, Pentacam HR, and RTVue OCT. Repeatability of Corvis ST was evaluated by within-subject standard deviation [Sw] and repeatability limit [r]. The agreement of central corneal thickness (CCT) measurements were compared among the three instruments using the Bland–Altman limits of agreement. Comparisons were further stratified by CCT (Corneathin ≤ 500 µm; Corneanormal = 500–550 µm; Corneathick > 550 µm). Sw was below 1 mmHg in Corneathin, Corneanormal, and Corneathick groups for IOP and bIOP. Sw for SP-A1 were 4.880, 6.128, 7.719 mmHg/mm respectively. Sw for CBI were 0.228, 0.157, 0.076, and correspondingly Sw for TBI and SSI were 0.094 and 0.056, 0.079 and 0.053, 0.070 and 0.053. The Bland–Altman plots for CCT implied poor agreement with mean differences of 29.49 µm between Corvis and OCT, 9.33 µm between Pentacam and OCT, and 20.16 µm between Corvis and Pentacam. The Corvis ST showed good repeatability with the exception of CBI in the various CCT groups. The CCT measured by Corvis ST was not interchangeable with Pentacam HR and RTVue OCT.

Evaluation of ocular findings in patients with lichen planus.

IntroductionLichen planus (LP) is a chronic, inflammatory disease of unknown etiology.AimTo evaluate the tear functions and the retinal and choroidal thickness (CT) in patients with LP.Material and methodsIn total, 33 patients and 30 healthy controls were enrolled. All participants were evaluated using the Ocular Surface Disease Index (OSDI) questionnaire, Schirmer 1 test (with anesthesia), tear break-up time (TBUT) test, intraocular pressure (IOP), axial length (AL), and central corneal thickness (CCT). The measurements of retinal thickness (RT) and CT were obtained by spectral domain optical coherence tomography.ResultsNo significant difference was observed between the groups in IOP, AL, CCT, or RT (p > 0.05 in each group). There was a significant difference in the Schirmer 1 test, TBUT, and OSDI questionnaire (p < 0.05 in each group). Choroidal thickness in LP patients was thinner than that in the controls (p = 0.001 in each location). There was a moderate negative correlation between the disease duration and Schirmer 1 test, TBUT, and subfoveal CT (r = –0.426, r = –0.555, r = –0.637; p = 0.001, p = 0.001, p = 0.001, respectively). There was a moderate negative correlation between the oral mucosal involvement and Schirmer 1 test, TBUT, and subfoveal CT (r = –0.345, r = –0.392, r = –0.467; p = 0.006, p = 0.001, p = 0.001, respectively). There was a positive correlation between disease duration, oral mucosal involvement and OSDI score (r = 0.717, r = 0.345; p = 0.001, p = 0.006, respectively).ConclusionsLichen planus may influence tear function tests and may cause dry eye. Patients with LP had lower CT values than healthy controls. Further studies are needed to clarify the effect of LP on the eyes.

Corneal Measurements in patients with Diabetes Mellitus

Purpose: To estimate the corneal measurements using Scheimpflug camera in patients with diabetes mellitus. Methods: Twenty five diabetic patients were prospectively recruited. Two groups were stratified, diabetic group and control group. Central corneal thickness (CCT), keratometry values (Kmean and Kmax), corneal volume (CV), anterior chamber depth (ACD), anterior chamber volume (ACV), Qvalue, frontal and back elevation, and the parameters of corneal variance indices, including minimum radius (Rmin) were measured using Pentacam Scheimpflug camera. Endothelial cell density (ECD) was also recorded. Findings were evaluated and compared between the 2 groups. Results: Two groups were found to have different Kmax (P = 0.03, one-tailed) and Rmin (P = 0.04, one-tailed) parameters. There was no statistical significant difference between the 2 groups in CCT, Kmean, CV, ACD, ACV, frontal/back elevation, Qvalue, ECD, and parameters of corneal variance indices. Conclusions: Diabetes mellitus affects keratometry and radius values of the human cornea based on the corneal measurements from Scheimpflug camera.

Age-dependency of ocular parameters: a cross sectional study of young and elderly healthy subjects.

To investigate the effect of aging on ocular parameters, including intraocular pressure (IOP), measured with different tonometry methods in healthy young (HY) and healthy elderly (HE) subjects and to study the effect of corneal parameters on tonometry methods. In this prospective, cross-sectional study, fifty eyes of 50 HY subjects (28 females, 22-31 years of age) and 43 eyes of 43 HE subjects (22 females, 64-79) were included. IOP was measured with four tonometry methods in a standardized order: ocular response analyser (ORA), dynamic contour tonometry (DCT), applanation resonance tonometry (ART) and Goldmann applanation tonometry (GAT). Other measurements included axial length (AL), central corneal thickness (CCT), corneal curvature (CC), anterior chamber volume (ACV), corneal hysteresis (CH) and corneal resistance factor (CRF). The mean IOP (HY/HE; mmHg ± standard deviation (SD)) was 12.2 ± 2.2/14.1 ± 3.5 with GAT. IOP was significantly higher (difference ± standard error) in HE compared to HY measured with an ORA (+3.1 mmHg ± 0.6), GAT (+1.9 ± 0.6) and DCT (+1.6 ± 0.6). No significant difference was found in IOP measured with ART. CH and ACV were significantly lower in HE compared to HY. There was no difference between the groups in CCT, CC, AL or CRF. No tonometry method was dependant on CCT or CC. IOP measured with an ORA and via DCT and GAT was higher in HE compared to HY Swedish subjects, while IOP measured with ART did not differ between the groups. In these homogeneous groups, tonometry methods were independent of CCT and CC.

A Study on the Central Corneal Thickness of Primary Angle Closure and Primary Angle Closure Glaucoma and Its Effect on Visual Field Progression.

To determine the mean central corneal thickness (CCT) and the relationship between the CCT and visual field progression in primary angle closure (PAC) and primary angle closure glaucoma (PACG). A combined cross-sectional and prospective study on PAC and PACG. A total of 35 eyes were included in the study for each group of normal control, PAC, and PACG patients from eye clinics in Kota Bharu, state of Kelantan, Malaysia, from January 2007 to November 2009. The PAC and PACG patients were divided into thin and thick CCT groups. They were followed up for 12 to 18 months for visual field progression assessment with their mean Advanced Glaucoma Intervention Study (AGIS) score. The CCT was 516.8 ± 26.0 µm for PAC and 509.7 ± 27.4 µm for PACG. Both were significantly thinner compared with the control group with CCT of 540 ± 27.8 µm (P < 0.001). There was a statistically significant increase in the mean AGIS score after 12.9 ± 1.7 months of follow-up in the thin CCT group for PACG (P = 0.002). However, no significant increase in the mean AGIS score was found for the thick CCT group in PACG and for both thin and thick CCT in PAC. The PAC and PACG had statistically significant thinner CCT compared with the controls. Thin CCT was associated with visual field progression based on the mean AGIS score in PACG.

Corneal Biomechanics as a Function of Intraocular Pressure and Pachymetry by Dynamic Infrared Signal and Scheimpflug Imaging Analysis in Normal Eyes

To evaluate corneal biomechanical deformation response using Ocular Response Analyzer (ORA) and Corvis ST data. Prospective observational case-control study. A total of 1262 eyes of 795 patients were enrolled. Three groups were established, according to the corneal compensated intraocular pressure (IOPcc): Group I (10-13 mm Hg), Group II (14-17 mm Hg), and Group III (18-21 mm Hg). Each group included 3 subgroups, based on central corneal thickness (CCT): Subgroups 1 (465-510 μm), 2 (510-555 μm), and 3 (555-600 μm). In addition, similar groups of CCT were divided into subgroups of IOPcc. Corneal hysteresis (CH) and corneal resistance factor (CRF) were derived from ORA. The parameters of highest concavity with the parameters of first and second applanation were recorded from Corvis ST. CH and CRF, applanation 1 time, and radius of curvature at highest concavity showed significant differences between CCT subgroups for each IOPcc group (P < .0001). CH, applanation 1 and 2 time, and applanation 2 velocity, as well as deformation amplitude (DA), showed significant differences by IOP subgroups for all CCT groups. IOPcc is correlated negatively with CH (r = -0.38, P < .0001). There are positive correlations of IOPcc with applanation 1 time, applanation 2 velocity, and radius and negative correlations with applanation 2 time (r = -0.54, P < .0001), applanation 1 velocity (r = -0.118, P < .0001), and DA (r = -0.362, P < .0001). ORA and Corvis ST parameters are informative in the evaluation of corneal biomechanics. IOP is important in deformation response evaluation and must be taken into consideration.

The Effect of Thin, Thick, and Normal Corneas on Goldmann Intraocular Pressure Measurements and Correction Formulae in Individual Eyes

To evaluate the usefulness of the central corneal thickness (CCT)-based correction formulae for stratified CCT groups, with intraocular pressure (IOP) from the Pascal dynamic contour tonometer (PDCT) as the reference standard. Retrospective case series. Two hundred eighty-nine patients attending a specialist glaucoma practice and a mixture of normal subjects and subjects with confirmed glaucomatous optic neuropathy. Intraocular pressure was measured using PDCT, Goldmann applanation tonometry (GAT), and the Ocular Response Analyzer (ORA; Reichert Corp, Buffalo, NY). The GAT readings were obtained before automated readings and were adjusted for CCT using 4 different correction formulae. Discrepancies between GAT and CCT-corrected GAT readings were evaluated after stratification into thin, intermediate, and thick CCT groups. The IOP measurements from GAT, the ORA, and CCT-adjusted IOP were compared against PDCT IOP measurements using Bland-Altman analysis. Mean, 95% limits of agreement, and proportion of patients with IOP difference of 20% or more between PDCT IOP and each of GAT IOP, Goldmann-correlated IOP (IOPg), corneal-compensated IOP (IOPcc), and adjusted IOP using CCT-based correction formulae. Average PDCT IOP values were higher than GAT, IOPg, IOPcc, and CCT-adjusted IOP. The GAT IOP readings demonstrated poor agreement with PDCT IOP (95% limits of agreement, ± 4.7 mmHg); however, IOPg, IOPcc, and adjustment of GAT IOP with CCT-based formulae resulted in even poorer agreement (range of 95% limits of agreement, ± 5.1 to 6.7 mmHg). If PDCT was used as the reference standard, there was a 26% to 39% risk of making an erroneous IOP adjustment of magnitude of 20% or more at all levels of CCT. This risk was greatest in the patients with thicker corneas (CCT, ≥568 μm). Adjusting IOP using CCT-based formulae resulted in poorer agreement with PDCT IOP when compared with unadjusted G AT IOP. If PDCT is the closest measure we have to intracameral IOP, there is a risk of creating clinically significant error after adjustment of GAT IOP with CCT-based correction formulae, especially in thicker corneas. This study suggests that although CCT may be useful in population analyses, CCT-based correction formulae should not be applied to individuals.

Relationship between central corneal thickness and visual field defect in open-angle glaucoma

To explore the relationship between central corneal thickness (CCT) and visual field defect in open-angle glaucoma (OAG). In this cross-sectional study, we tested 344 eyes in 344 eligible patients, including 233 with normal-tension glaucoma (NTG) and 111 with primary open-angle glaucoma (POAG). The association among variables, especially that between visual field defect and CCT, was probed by multivariate regression in eyes with NTG or POAG, and in all eyes. All eyes were divided into early, moderate, or severe visual field defect groups according to Anderson's classification. Statistical analysis was performed for all cases, and for the three CCT groups. Multivariate regression analysis revealed an association between CCT and visual field defect in eyes with NTG but not in eyes with POAG or in all eyes. The eyes with early visual field defect had greater CCT than did those with severe visual field defect (533.2 versus 519.0 microm). The eyes with greater CCT had better visual field indices than did those with thinner CCT (-6.91 versus -9.17 dB). Central corneal thickness is a factor associated with the status of the visual field defect: a greater CCT is associated with a better visual field index. Other factors such as the glaucoma subtype play a role in the effect of CCT on visual field defect.

Effects of Central Corneal Thickness on the Efficacy of Topical Ocular Hypotensive Medications

To determine the effect of central corneal thickness (CCT) on the efficacy of intraocular pressure (IOP)-reducing drugs in patients with ocular hypertension (OHT). This retrospective study analyzed research records of 115 OHT patients and 97 ocular normotensive (ONT) volunteers. CCT was measured by slit-lamp pachymetry and IOP by pneumatonometry. The OHT patients were divided into Thick (>540 microm, n=52) and Thin (<or=540 microm, n=63) Cornea groups. Measurements in the OHT group were made after washout of all IOP-lowering drugs and at 1 week of treatment with latanoprost 0.005%, dorzolamide 2%, brimonidine 0.2%, apraclonidine 0.5%, pilocarpine 2%, or unoprostone 0.15% to 1 eye and vehicle contralaterally. ONT volunteers also were divided into Thick (n=34) and Thin (n=63) Cornea groups. Results were compared between groups using unpaired t tests or nonparametric Wilcoxon tests and within groups using linear regression analyses. Baseline IOPs were not different between CCT groups of OHT patients or of ONT volunteers. After 1 week of drug treatment, IOP was significantly (P=0.02) lower in the OHT Thin Cornea group (16.0+/-3.0 mm Hg, mean+/-SD) than the OHT Thick Cornea group (17.4+/-2.8 mm Hg). There was a positive correlation between IOP and CCT (R=0.06, P=0.007) in OHT drug-treated eyes, but not OHT vehicle-treated or ONT untreated eyes. The final IOP was significantly lower in the Thin than the Thick Cornea group treated with brimonidine (P=0.02) but not with latanoprost (P=0.91). When dosed with IOP lowering drugs, eyes with thinner corneas had lower IOPs than eyes with thicker corneas. This suggests a reduced efficacy of some glaucoma medications in ocular hypertensive patients with thick corneas.

Relationship between central corneal thickness and severity of glaucomatous visual field loss in a primary care population

Background Although the ability of central corneal thickness (CCT) to predict development of primary open-angle glaucoma has become increasingly well recognized, the ability of CCT to predict severity of glaucoma remains uncertain. This study was designed to expand the available knowledge about the relationship between CCT and glaucoma severity. Methods Retrospective identification of all patients with a clinical diagnosis of either primary open angle glaucoma (POAG) or ocular hypertension who were seen from September 2002 through May 2003 at the Albuquerque VA Medical Center eye clinic was completed. Eligible subjects were segregated into no, mild, moderate, or advanced visual field loss groups based on Advanced Glaucoma Intervention Study (AGIS) visual field scoring criteria. Following statistical analyses comparing the visual field groups, the sample was divided into thin, intermediate, and thick CCT groups, and further analysis was performed. Results Mean CCT was significantly higher in the no field loss group compared with all 3 groups with glaucomatous visual field loss. Mean CCT was not statistically different, however, between the mild, moderate, and advanced visual field loss groups. In linear regression analyses, no significant relationship was found between CCT and severity of visual field loss. Conclusions Although CCT was associated strongly with development of POAG-related visual field loss, CCT was not associated with severity of visual field loss in this study. These findings suggest that glaucoma patients with thinner corneas are just as likely to have advanced levels of field loss as glaucoma patients with thicker corneas. Prospective studies are needed to validate these findings.

Corneal edema and intraocular pressure after cataract surgery*1Randomized comparison of Healon5 and Amvisc Plus

Purpose: To compare the protective effect of Healon®5 (sodium hyaluronate 2.3%) and Amvisc Plus® (sodium hyaluronate 1.6%) against corneal edema and their association with postoperative intraocular pressure (IOP) spikes in patients having phacoemulsification and intraocular lens (IOL) implantation. Setting: Ophthalmology department of a general hospital. Methods: One hundred forty patients were randomly assigned to have surgery with Healon5 (n = 70) or Amvisc Plus (n = 70). One eye of each patient was analyzed. Data collected preoperatively included best corrected visual acuity (BCVA) and IOP. Central ultrasonic pachymetry was performed in all patients. The same ophthalmologist performed all surgeries. The IOP and central corneal thickness (CCT) were measured 1 and 4 days and 1 month after surgery. The BCVA was also assessed at 1 month. Results: There were no significant preoperative differences between the Healon5 and Amvisc Plus groups in sex, age, ocular pathology, BCVA, IOP, or CCT. Intraoperative variables were similar between groups, but it took significantly longer to remove the Healon5. Postoperatively, there were no differences between groups in the evolution of CCT or of IOP. Intraocular pressure spikes over 30 mm Hg were detected at 1 day in 7 patients in the Healon5 group and 2 patients in the Amvisc Plus group (10.0% versus 2.9%; P = .165). Conclusions: Both OVDs were beneficial in a wide range of cataract patients. However, the results suggest a tendency toward a higher complication rate with Healon5.
