Schwannoma- A Rare Cause of Proptosis

Schwannoma  a tumor of cranial and peripheral nerves is rarely found in the orbit and if present causes proptosis. We report a rare cause of unilateral proptosis . A 15 year old boy reported with a 10 months history of unilateral proptosis. Ophthalmic and slit lamp examination revealed no other abnormality except for a dystopia, 3mm inferiorly and 1.5mm medially of left eye. On palpation a small, non-tender rubbery nodular swelling in the left supero-temporal region was found. MRI was suggestive of a lacrimal gland mass with a hyper intense area in the region of left lacrimal gland. Complete excision biopsy was done which reported a Schwannoma of lacrimal nerve. This case emphasis on two important points.First how to approach and manage a rare cause of unilateral proptosis and also that Schwannoma of lacrimal nerve has a very good prognosis after complete surgical excision and a good follow up.

A Rare Case of Mucinous Cystadenoma Presenting as Unilateral Proptosis at Birth

Mucinous cystadenoma is a rare cause of unilateral proptosis at birth. A two day old female baby presented with non-pulsatile, transiliuminantaxial proptosis right eye. On CT scan cystic lesion was seen behind the globe in retrobulbar space. Gross total excision of cyst was done through trans-frontal craniotomy approach. Histopathological examination revealed mucinous cystadenoma. This case has been reported because of rarity of this condition.

Optic nerve glioma – a case study

AbstractPurposeTo evaluate a 5 year old boy with painless progressive enlargement of the left eye since the past 1 year. He also complained of a loss of vision in the left eye.MethodsStudy Design: Case Study.Duration of Study: 1 year.The patient was evaluated over 1 year and investigations were carried out as deemed necessary.InvestigationsSlit lamp biomicroscopy with 90 D Volk lens was done for anterior and posterior segment examination. Gonioscopy, Applanation Tonometry, Automated Perimetry and Indirect Ophthalmoscopy were done. CT and MRI of the orbit was also carried out.ResultsPresenting visual acuity was absence of light perception in the left eye and the proptosis was measured of 25 mm which was in the downward and outward direction. A relative afferent pupillary defect was noticed in the left eye and extraocular movements were restricted superiorly and medially. Funduscopy of the left eye revealed a pale disc, blurring of the disc margins and presence of optociliary shunt vessels. CT scan of the left eye showed a fusiform optic nerve swelling. MRI showed a fairly large fusiform swelling in left optic nerve, pressure effect on ocular bulb with proptosis, no extension of the lesion in optic canal. This was consistent with the diagnosis of optic nerve glioma.ConclusionOptic nerve glioma is an important cause of unilateral proptosis in children. Early diagnosis is mandatory and management should involve an integrated approach of neurosurgeons, plastic surgeons, ophthalmologists and radiologists.

Mass lesions constitute a common cause of unilateral proptosis in elderly individuals in which lymphoproliferativelesions account for 11% and metastasis up to 2 -9% of cases. Literature review showed various tumors metastasizing to the orbit but simultaneous presence of two unrelated tumors in a person are extremely uncommon. A study of tumours giving rise to unilateral proptosis.

Study of distribution, clinical features, radiological features, histopathological aspects, management and outcomes of tumours giving rise to unilateral proptosis. This is a prospective study including 40 patients with benign and malignant tumours of orbital, paraorbital and other sites giving rise to unilateral proptosis. Final diagnosis was made following detailed radiological and histopathological examination. Then, treatment modality was decided, which included surgery, radiotherapy, chemotherapy or required combination. Proptosis was measured by simple/plastic ruler exophthalmometry method. The maximum number of cases 8(20%) were in age group 31-40years. The youngest patient was 4year old and the oldest was 80years. M:F ratio was 1.85:1. Malignant tumours were 32(80%) and benign tumours were 8(20%). 12 cases were having 3mm proptosis (minimum) and one case having 18mm (maximum). Only 4(10%) orbital tumours were responsible for unilateral proptosis. Paraorbital tumours 34(85%) was the major cause for unilateral proptosis and 2(5%) were from distant sites. Surgery was the mainstay of treatment 14(35%), followed by combined (surgery+radiotherapy) 5(12.5%), chemotherapy 4(10%), There was 4(10%) mortality, 16(40%) improved 3(7.5%) had same status and 17(42.5%) deteriorated. Thus nasal obstruction is the commonest ENT presenting complains in these patients. The major cause of unilateral proptosis was found to be paraorbital tumours (34%). Thus, a case of proptosis should never be ignored but a thorough ENT and ophthalmic examination is necessary. As malignancy is one of the major cause of unilateral proptosis catching and treating them early will reduce mortality.

Ewing′s sarcoma of the orbit with intracranial extension: A rare cause of unilateral proptosis

Ewing's sarcoma causing unilateral proptosis along with bifrontal extradural infiltration in a child is an unusual presentation. A female patient presented with features of painless proptosis of the left eye with visual deterioration. Her radiology revealed an infiltrating intraorbital, extraconal tumor with intracranial bifrontal extradural extension causing mass effect. Total excision of the intraorbital and intracranial part of the tumor along with postoperative chemo- and radiotherapy brought about a substantial relief. The clinicoradiological presentation and management of this rare entity are discussed.

Periorbital (preseptal) cellulitis in children

Infections of the eye can be periorbital (preseptal) or orbital in origin. The management of children with periorbital cellulitis varies with the clinical presentation. Some children have trivial or self-limited disorders, while others can have sight- or life-threatening problems. Thorough physical examination helps the clinician to differentiate among the causes of periorbital cellulitis and between periorbital and orbital cellulitis. The differential diagnosis of periorbital cellulitis includes conjunctivitis, hordeolum, dacryocystitis, dacryoadenitis, bacteremic cellulitis, traumatic cellulitis and inflammatory edema secondary to sinusitis. Orbital cellulitis, although very much less common than periorbital cellulitis, is the most common cause of unilateral proptosis in children.

Surgical approaches to cavernous haemangiomas of the orbit

Cavernous haemangiomas are the most common orbital masses and the second most common cause of unilateral proptosis after thyroid ophthalmopathy. We retrospectively analysed 19 patients with retrobulbar cavernous haemangiomas, 9 of whom had lateral orbitotomy to remove retrobulbar cavernous haemangiomas located superior ( n = 4), inferior ( n = 2) or lateral ( n = 3) to the optic nerve. Seven patients had lateral orbitotomy together with an anterior medial approach to gain access to retrobulbar cavernous haemangiomas located medially to the optic nerve in the posterior half of the orbit. An anterior approach was used in 3 patient with an anteriorly located cavernous haemangioma. We describe here the planning of surgical treatment based on the site of the lesion.

Pleomorphic adenoma of the lacrimal gland in a nine‐year‐old child

A case of a 9-year-old child with pleomorphic adenoma of the lacrimal gland is described. This tumour is usually found in adults; cases younger than 10 years of age have proved to be extremely rare. Because of its age distribution, pleomorphic adenoma of the lacrimal gland is usually overlooked as a possible cause of unilateral proptosis in children. Also, the history of disease provided by the family may be incorrect, possibly leading to misdiagnosis by the ophthalmologist.

An Unusual Cause of Proptosis: Orbital Solitary Fibrous Tumor: Case Report

Although solitary fibrous tumors are well-recognized tumors, they have only recently been described in the orbit. Although histopathological awareness of the lesion has been heightened recently because of the discovery of CD-34 immunoreactivity in solitary fibrous tumors, including orbital lesions, it remains unrecognized clinically as a cause of unilateral proptosis. Clinical awareness of the lesion is important, because benign and malignant forms of the tumor occur. The malignant forms pursue an aggressive course manifested by local invasion, recurrent growth, or metastases. The benign lesions are histologically banal and are cured by surgical excision. This case report describes unilateral proptosis caused by an orbital solitary fibrous tumor, which occurred in a 43-year-old woman. The tumor had a benign histomorphology and clinical course, with surgical excision being curative. In reviewing the literature on orbital solitary fibrous tumors, clinical awareness of the tumor as a cause of unilateral proptosis is emphasized.

Computed tomography (CT) is the primary imaging modality for evaluation of an orbital mass presenting as proptosis. Axial myopia is a benign cause of unilateral proptosis frequently encountered by the ophthalmologist. Identification of this condition by CT avoids additional investigation of the cause of the proptosis.

Aspergilloma of paranasal sinuses — A common cause of unilateral proptosis in Sudan

The radiological appearances of six cases of primary fungal granuloma of the paranasal sinuses with involvement of the orbit are described. All were caused by Aspergillus flavus. Bone destruction is common and can be extensive, but is characteristically well defined, especially in the lateral wall of the antrum. The discrepancy between the gross radiological changes and the mild symptoms is a feature very suggestive of the condition. Associated displaced "floating" teeth, expansion of ethmoids, and intracranial calcification are described and discussed. It is expected that, at least in certain localities where the incidence is significant, the radiologist would be able to help in recognising the earlier cases.
